Objective:To investigate the relationship between walking speed, life-space mobility (LSM), cognitive function and mild cognitive impairment (MCI) in elderly patients with type 2 diabetes mellitus in community, and compare the predictive value of walking speed and LSM alone and combined in elderly patients with type 2 diabetes mellitus. Methods:This was a cross-sectional study. Using the convenient sampling method, a total of 448 elderly patients with type 2 diabetes mellitus from three communities in Hefei City were selected as the research objects from September 2022 to May 2023. Daily walking speed was measured using the 4-Meter Walking Speed, LSM was assessed using the Chinese version of Life-Space Assessment (LSA), and cognitive function was evaluated using the Chinese version of Montreal Cognitive Assessment (MoCA). Participants were divided to the MCI group and non-MCI group. Spearman correlation analysis was used to explore the relationships between walking speed, LSM and cognitive function. Logistic regression analysis was used to analyze the relationship between walking speed, LSM and MCI. Area under the curve ( AUC) of receiver operating characteristic (ROC) curve analysis was performed to compare the effects of walking speed and Chinese version LSA score on the prediction of MCI alone and in combination. Results:The walking speed and the Chinese version LSA score were both positively correlated with the Chinese version MoCA score ( r=0.598, 0.543; P<0.05). Chinese version LSA score ( OR=0.942) and walking speed score ( OR=0.490) were influencing factors for MCI in elderly patients with type 2 diabetes mellitus ( P<0.05). The AUC for walking speed in predicting MCI was 0.875, with a cutoff value of 3 points. Chinese version LSA score in predicting MCI had an AUC of 0.887 with a cutoff value of 59 points. When used in combination, they achieved an AUC of 0.915, with cutoff values of 55 points for Chinese version LSA scores and 3 points for walking speed scores. Conclusions:The combination of walking speed and LSM can more accurately predict the occurrence of MCI in elderly patients with type 2 diabetes mellitus than the single application, which provide a reference for improving the cognitive function of elderly patients with type 2 diabetes mellitus.

Objective:To explore the chain mediating effect of self-rated health and aging attitudes on the relationship between life space and psychological distress of community-dwelling older adults. Methods:From August to November 2023, convenience sampling was used to select older adults from three communities of Baohe District in Hefei City as subjects. General Information Questionnaire, Life Space Assessment (LSA), Self-Rated Health (SRH), Attitudes to Aging Questionnaire (AAQ), and 10-item Kessler Psychological Distress Scale (K10) were used for the survey. Spearman correlation was used to analyze the correlation between life space and self-rated health, aging attitudes, and psychological distress. Process macro program of SPSS 25.0 software was used to test for mediating effects. Results:A total of 700 questionnaires were distributed and 669 valid questionnaires were collected, with a valid response rate of 95.57%. Among 669 community-dwelling older adults, the LSA, SRH, AAQ, and K10 scores were (74.35±19.39), (3.23±0.96), (80.66±13.43), and (19.29±6.26), respectively. Spearman correlation analysis showed a positive correlation between life space and attitudes to aging ( rs=0.568, P<0.01) and negative correlations between life space and self-rated health and psychological distress ( rs=-0.334 and -0.455; both P<0.01). The mediating effect test showed that self-rated health and aging attitudes had chain mediating effects on life space and psychological distress, with a mediating effect value of -0.053, accounting for 11.60% of the total effect. Conclusions:Life space can not only directly affect the psychological distress of community-dwelling older adults but also indirectly predict psychological distress through the mediating effect of self-rated health and aging attitudes.

Objective:To investigate the diagnostic value of serum matrix metalloproteinase 3 (MMP-3) combined with anti-mutant citrulline vimentin antibody (anti-MCV) and rheumatoid factor (RF) in rheumatoid arthritis (RA).Methods:A retrospective study was conducted to select 78 patients diagnosed with RA, 30 with Sjogren′s syndrome (SS), 25 with connective tissue disease (CTD), and 38 with antiphospholipid syndrome (APS) from the Nanjing Drum Tower Hospital Clinical College of Nanjing Medical University from August 2022 to April 2024. In the same period, 50 healthy controls were selected from the physical examination center of our hospital. Fasting peripheral blood RF, anti-streptolysin O (ASO), C-reactive protein (CRP), erythrocyte deposition rate (ESR), interleukin-6 (IL-6), procalcitonin (PCT), anti-keratin antibody (AKA), anti-MCV, MMP-3, anti-cyclic citrullinated peptide (anti-CCP) level. Fasting peripheral blood MMP-3 levels in SS, CTD, APS patients and healthy control group were compared, Spearman correlation analysis was used to evaluate the correlation between MMP-3 levels and various indicators, and logistic regression analysis was used to screen the related influencing factors of RA occurrence. The diagnostic value of serum MMP-3, anti-MCV, RF and combined detection for RA was evaluated by receiver operating characteristic (ROC) curve. Results:There were statistically significant differences in RF, CRP, ESR, IL6, AKA, antiMCV, MMP-3 and anti-CCP between RA group and healthy control group (all P<0.001), while there were no statistically significant differences in PCT and ASO between the RA group and the healthy control group (all P>0.05). Serum MMP-3 level in the RA group was significantly higher than that in SS, CTD, APS and healthy control group, and the difference was statistically significant (all P<0.05). MMP-3 was positively correlated with anti-CCP, CRP, ESR, anti-MCV, RF and AKA ( r=0.403, 0.532, 0.530, 0.431, 0.427, 0.391, all P<0.05). Multivariate logistic regression analysis showed that MMP-3 ( OR=1.082, P=0.02), anti-MCV ( OR=1.015, P=0.049) and RF ( OR=1.046, P=0.036) were independent risk factors for RA. ROC curve showed that when the cut-off value of MMP-3 was 53.10 ng/ml, the area under the curve for diagnosing RA was 0.889, the sensitivity was 74.5%, and the specificity was 96.0%. The area under ROC curve of MMP-3 combined with anti-MCV and RF for the diagnosis of RA was 0.975, the sensitivity was 88.2%, and the specificity was 99.7%. Conclusions:MMP-3 has a certain value in the differential diagnosis of RA, and MMP-3 combined with anti-MCV and RF is of great significance in the auxiliary diagnosis of RA.

Objective:To establish a prediction model for symptomatic radiation pneumonitis (SRP) after radiotherapy for thoracic cancer based on a longitudinal cohort and dose interval variations. Methods:Clinical data of 587 patients who received thoracic radiotherapy in Department of Radiotherapy of Yunnan Cancer Hospital from July 2022 to June 2023 were retrospectively analyzed. The National Cancer Institute common terminology criteria for adverse events (CTCAE) version 5.0 was used to grade radiation pneumonitis, and clinical factors, traditional independent dosimetric characteristics and dose interval variation characteristics were collected. Features used to predict the occurrence of SRP were screened using genetic algorithms and analyzed the correlation between the selected features and SRP occurrence. Predictive models for SRP occurrence were established using the selected features and evaluated, and the optimal predictive model was visualized using a column chart. Results:The incidence of SRP was 35.94%. Five clinical factors, seven independent dosimetric features and six dose interval variation features were screened out by genetic algorithms to effectively predict the occurrence of SRP. The area under ROC curve (AUC) of clinical factors combined with traditional independent dosimetric factors and dose interval variation factors was 76%. The AUC of clinical factors combined with traditional independent dosimetric factors and that of clinical factors combined with dose interval variation factors was 69% and 67%, respectively. The addition of the characteristics of dose interval variation factors significantly improved the effectiveness of the prediction model. Conclusions:The supplement of the characteristics of dose interval variation factors can significantly improve the performance of the SRP prediction model for thoracic tumors after radiotherapy. The SRP prediction model based on dose interval variations can effectively predict the occurrence of SRP.

Cells undergo glucose metabolism reprogramming under the influence of the inflammatory microenvironment, changing their primary mode of energy supply from oxidative phosphorylation to aerobic glycolysis. This process is involved in all stages of inflammation-related diseases development. Glucose metabolism reprogramming not only changes the metabolic pattern of individual cells, but also disrupts the metabolic homeostasis of the body microenvironment, which further promotes aerobic glycolysis and provides favourable conditions for the malignant progression of inflammation-related diseases. The metabolic enzymes, transporter proteins, and metabolites of aerobic glycolysis are all key signalling molecules, and drugs can inhibit aerobic glycolysis by targeting these specific key molecules to exert therapeutic effects. This paper reviews the impact of glucose metabolism reprogramming on the development of inflammation-related diseases such as inflammation-related tumours, rheumatoid arthritis and Alzheimer's disease, and the therapeutic effects of drugs targeting glucose metabolism reprogramming on these diseases.

Alzheimer's disease (AD) is a leading cause of dementia in the elderly. Mitogen-activated protein kinase phosphatase 1 (MKP-1) plays a neuroprotective role in AD. However, the molecular mechanisms underlying the effects of MKP-1 on AD have not been extensively studied. MicroRNAs (miRNAs) regulate gene expression at the post-transcriptional level, thereby repressing mRNA translation. Here, we reported that the microRNA-429-3p (miR-429-3p) was significantly increased in the brain of APP23/PS45 AD model mice and N2AAPP AD model cells. We further found that miR-429-3p could downregulate MKP-1 expression by directly binding to its 3'-untranslated region (3' UTR). Inhibition of miR-429-3p by its antagomir (A-miR-429) restored the expression of MKP-1 to a control level and consequently reduced the amyloidogenic processing of APP and Aβ accumulation. More importantly, intranasal administration of A-miR-429 successfully ameliorated the deficits of hippocampal CA1 long-term potentiation and spatial learning and memory in AD model mice by suppressing extracellular signal-regulated kinase (ERK1/2)-mediated GluA1 hyperphosphorylation at Ser831 site, thereby increasing the surface expression of GluA1-containing α-amino-3-hydroxy-5-methyl-4-isoxazolepropionic acid receptors (AMPARs). Together, these results demonstrate that inhibiting miR-429-3p to upregulate MKP-1 effectively improves cognitive and synaptic functions in AD model mice, suggesting that miR-429/MKP-1 pathway may be a novel therapeutic target for AD treatment.
