1.Study on the safety and efficacy of micro-perfusion device for preserving isolated porcine limbs

Objective To evaluate the safety and efficacy of a self-developed micro-normothermic machine perfusion (NMP) system (micro-perfusion device) for preserving isolated porcine limbs. Methods Five healthy Landrace pigs were selected, and their left and right forelimbs were randomly divided into the NMP group and static cold storage (SCS) group. The NMP group was perfused with the self-developed micro-perfusion device and polymerized hemoglobin perfusate for 32 hours at normothermia, while the SCS group was preserved at 4 ℃. Hemodynamic parameters such as perfusion pressure and flow were monitored. The pH value, partial pressure of oxygen (PO2), lactic acid (Lac), creatine kinase (CK) and lactate dehydrogenase (LDH) in the perfusate were measured. Hematoxylin-eosin staining was used to assess the muscle tissue structure, terminal deoxynucleotidyl transferase-mediated dUTP nick-end labeling was employed to evaluate muscle cell apoptosis, and immunohistochemistry staining was applied to detect the expressions of tumor necrosis factor (TNF)-α and interleukin (IL)-6. A mixed-effects model was used to analyze the effects of time and treatment methods on tissue structure, cell apoptosis and inflammatory factors. Results The device could stably maintain a perfusion pressure of (69±15) mmHg and a flow rate of (117±42) mL/min. The pH value and electrolytes of the perfusate were generally stable, with PO2 maintained at a high level. Lac was maintained at 5.38(3.81, 6.45) mmol/L, while CK and LDH increased over time. After 32 hours of perfusion in the NMP group, both the myocyte spacing and apoptosis rate were better than those in the SCS group. Mixed-effects model analysis showed that there were statistically significant differences in the effects of NMP treatment and SCS treatment on myocyte spacing and apoptosis rate per unit time (both P < 0.05). There were no statistically significant differences in TNF-α and IL-6 between the two groups, and mixed-effects model analysis showed no statistically significant differences in the effects of NMP treatment and SCS treatment on TNF-α and IL-6 per unit time (both P > 0.05). Conclusions The micro-perfusion device used in this study may achieve 32-hour normothermic preservation in a porcine limb amputation model, maintain basic metabolism and ionic homeostasis, reduce muscle structural damage and cell apoptosis without inducing additional inflammatory responses. This technology is expected to significantly extend the time window for replantation of amputated limbs in disaster rescue and long-distance transportation, providing an important technical basis for clinical translation and subsequent replantation research.

Currently, the quality control of traditional Chinese medicine (TCM) encounters substantial technical challenges, necessitating the urgent development of innovative evaluation methods for key quality attributes. This study proposes the construction of a quality assurance system centered on key quality attributes, with systems biology and chemical biology serving as its core components: utilizing multi-omics technologies to systematically analyze the core functional modules of TCM, integrating pharmacological approaches to precisely identify key bioactive components; applying chemical biology to elucidate the interaction mechanisms between components and their targets, while screening for quality markers associated with specific functions and clinical indications; establishing an integrated quality control system based on quality markers and biological potency to ensure comprehensive quality management from raw herb materials through to finished products. Additionally, it emphasizes the formulation of compatibility rules grounded in TCM theory, integrates clinical practice and multi-dimensional value assessments, and establishes a novel quality evaluation model with distinct TCM characteristics.This offers a comprehensive and systematic approach to ensuring the safety and efficacy of TCM while facilitating the high-quality development of the TCM industry.
3.Prenatal Mental Health and Its Stress-Process Mechanisms During a Pandemic Lockdown: A Moderated Parallel Mediation Model

Hundreds of countries have implemented lockdown policies to slow the spread of coronavirus disease-2019 (COVID-19), but the impact of these measures on maternal mental health is not well understood.
Methods:
This study integrated a stress-process model to examine the pathways from lockdown-related stressors to prenatal psychological outcomes, with COVID-19 coping strategies (COP) and self-efficacy in managing negative affect (NEG) as mediators and lockdown duration, hours on pandemic-related information, and number of pregnancies as moderators. Pregnant women in Shanghai completed the Regulatory Emotional Self-Efficacy Scale, COVID-19 Coping Scale, Depression, Anxiety, and Stress Scale-21. Structural equation modeling (SEM) was used to test and modify the hypothetical model, and moderated mediation and slope analyses were undertaken.
Results:
In the final SEM demonstrating satisfactory fit, three stressors—decreased household income, insufficient daily supplies, and acquired infections—showed positive direct relationships with NEG and COP. Acquired infections, NEG, and COP were identified as direct predictors of mental health outcomes. The relationship between these three stressors and mental health was mediated by NEG and COP. Additionally, the number of pregnancies moderated the mediating effect of COP; this effect was more pronounced among first-time pregnant women than those with multiple pregnancies.
Conclusion
This study provides insights into how lockdown measures impact psychological outcomes in pregnant women quarantined at home. Interventions aimed at increasing coping strategies may be more effective for primiparous women during future public health emergencies.

6.Effects of 4C management mode on rehabilitation and quality of life in patients with acute heart failure in CCU

Objective:To explore the effects of 4C management mode on rehabilitation and quality of life in patients with acute heart failure(AHF)in cardiovascular intensive care unit(CCU).Methods:This randomized controlled study enrolled 110 patients with AHF admitted in CCU of Guangdong Provincial Hospital of Traditional Chinese Medicine between February 2021 and February 2023.Patients were randomly divided into intervention group(n=55)and control group(n=55).Patients in the control group received routine intervention,while those in the inter-vention group received additional 4C management mode,both groups were intervened for 3 months.The indexes of cardiac function,self-efficacy,exercise tolerance,blood oxygen saturation(SpO2),mental state and quality of life were compared between the two groups.Results:Compared to those in the control group after intervention,patients in the intervention group had significantly lower interventricular septal thickness(IVST)[(9.20±0.10)mm vs.(10.30±0.17)mm,P<0.001],left ventricular end-systolic volume index(LVESVI)[(35.21±0.74)ml/m2 vs.(39.70±1.09)ml/m2,P<0.001],left ventricular end-diastolic volume index(LVED VI)[(45.35±0.71)ml/m2 vs.(56.76±1.48)ml/m2,P<0.001],scores of self-rating anxiety scale(SAS)[(32.10±1.57)points vs.(45.36±1.94)points,P<0.001],self-rating depression scale(SDS)[(35.03±1.52)points vs.(47.70±2.12)points,P<0.001]and Minnesota living with heart failure questionnaire(MLHFQ)[(51.27±2.85)points vs.(56.46±3.53)points,P<0.001],and significantly higher score of general self-efficacy scale(GSES)[(28.71±3.84)points vs.(20.34±1.56)points,P<0.001],6 min walking distance(6MWD)[(428.53±5.50)m vs.(362.14±6.18)m,P<0.001]and SpO2[(99.11±0.87)%vs.(94.23±0.73)%,P<0.001].Conclusion:4C management mode may improve the cardiac function,self-efficacy,mental state and quality of life in patients with acute heart failure in CCU.
7.Construction and effects of evidence-based early exercise rehabilitation nursing for elderly patients undergoing total hip arthroplasty

Objective:To develop an early exercise rehabilitation nursing for elderly patients undergoing total hip arthroplasty (THA) based on the best evidence, and explore its clinical effects.Methods:This study was a randomized controlled trial. From August to December 2022, convenience sampling was used to select 126 elderly THA patients admitted to the Department of Orthopedics of the First Affiliated Hospital of Naval Medical University as participants. Patients were divided into observation group and control group using a random number table method, with 63 cases in each group. Control group was treated with a usual exercise rehabilitation nursing, while observation group was treated with an early exercise rehabilitation nursing for elderly THA patients based on the best evidence. Changhai Pain Rating Scale, Harris Hip Score, and Barthel Index were used to evaluate pain, hip function, and activities of daily living in two groups of patients. The first time off the bed, hospital stay, and incidence of complications were recorded and compared between the two groups.Results:The Harris Hip scores of observation group patients three days after surgery and on the day of discharge were (61.13±4.82) and (77.84±4.43), respectively, which were higher than those of control group (50.75±6.19) and (70.25±7.47), and the differences were statistically significant ( t=10.502, 6.937; both P<0.01). Barthel Index scores of observation group patients were higher than those of control group on the day of surgery, three days after surgery, and the day of discharge, and the differences were statistically significant ( t=3.531, 4.609, 10.112; all P<0.01). The incidence of postoperative complications in observation group was 11.11% (7/63), while in control group it was 42.86% (27/63), with a statistically significant difference (χ 2=16.113, P<0.01). There were no statistically significant differences in the resting pain score and exercise pain score between the two groups of patients on the day after surgery ( t=-1.693, -1.354; both P>0.05). The resting pain score and exercise pain score of observation group were lower than those of control group three days after surgery and on the day of discharge, and the differences were statistically significant (all P<0.05). Observation group had a shorter first time off the bed and hospital stay (1.34±0.37) and (5.48±0.66) days compared to control group (2.13±0.48) and (6.29±0.83) days, with statistically significant differences ( t=-10.346, -6.063; both P<0.01) . Conclusions:An evidence-based early exercise rehabilitation nursing for elderly THA patients can reduce the degree of pain, minimize the occurrence of complications, shorten the first time off the bed and hospital stay, and promote recovery of elderly patient.
8.Correlation of SERPINC1 and SERPINE1 gene polymorphisms with venous thromboembolism in patients with malignant tumors

Objective:To investigate the correlation of SERPINC1 and SERPINE1 gene polymorphisms with venous thromboembolism (VTE) in patients with malignant tumors.Methods:A retrospective case-control study was conducted. A total of 227 patients with malignant tumors in the Cancer Hospital Affiliated to Xinjiang Medical University from November 2023 to February 2024 were selected, of which 47 cases developed VTE (VTE group) and 180 cases did not develop VTE (non-VTE group). The patients' venous blood was collected, and D-dimer level was analyzed by using fully automatic coagulation analyzer; fluorescence staining was performed by using digoxin staining solution, and SERPINC1 rs2227589 locus and SERPINE1 rs1799762 locus were used as candidate genes, and fluorescence sequencing was performed by using multichannel fluorescence quantitative analyzer, and the frequencies of C, T, 4G and 5G genes were calculated.Results:There were no statistically significant differences in the age, gender, smoking, alcohol consumption, anticoagulant use, diabetes mellitus, hypertension, coronary artery disease, surgical treatment, intravenous cannulae, radiotherapy, chemotherapy, and targeted therapy between the 2 groups (all P > 0.05). The level of D-dimer in the VTE group was higher than that in the non-VTE group [(8.7±6.9) kg/m 2vs. (2.8±1.0) kg/m 2], and the difference was statistically significant ( t = 5.15, P < 0.001). The differences in C and T genotypes and gene frequencies at the SERPINC1 rs2227589 locus between the VTE group and the non-VTE group were not statistically significant (all P > 0.05). The differences in 4G and 5G genotypes and gene frequencies at the SERPINE1 rs1799762 locus between the VTE group and the non-VTE group were statistically significant (all P < 0.05), and 4G allele frequency in the VTE group was higher than that in the non-VTE group (52.13% vs. 39.72%), and the difference was statistically significant ( χ2 = 4.70, P = 0.030). Conclusions:The elevated expression of 4G allele at the SERPINE1 rs1799762 locus in patients with malignant tumors is associated with development of VTE.
9.Application of third-generation dual-source CT ultra-low tube voltage combined with advanced analog iterative reconstruction in bronchial artery CTA in patients with hemoptysis

Objective To explore the effects of ultra-low tube voltage of the third-generation dual-source CT combined with advanced modeled iterative reconstruction(ADMIRE)in bronchial artery CTA for hemoptysis patients.Methods Totally 70 hemoptysis patients admitted to some hospital from March to August 2023 had their clinical and imaging data collected retrospectively,and then divided into Group A and B with the numerical table method.In Group A image reconstruction was carried out with 120 kV tube voltage and filtered back projection(FBP)(named as Group A-FBP);In Group B image reconstruction was performed with 70 kV tube voltage respectively combined with FBP,ADMIRE Ⅲ,Ⅳ and Ⅴ(named as Group B-FBP,B-ADMIRE Ⅲ,B-ADMIRE Ⅳ and B-ADMIRE Ⅴ).All the patients were scanned with Siemens third-generation dual-source CT.The groups were compared in terms of radiation dose covering volume CT dose index,dose length product and effective dose,subjective score and objective evaluation.The subjective score agreement between 2 radiologists was evaluated with Kappa test.SPSS 26.0 software was used for statistical analysis.Results The radiation dose parameters of Group B were all lower than those of Group A,and the differences were statistically significant(P<0.001).Group B-FBP and ADMIRE Ⅳ had the lowest and highest subjective scores for bronchial artery images,res-pectively,and the five groups were significantly different in subjective score(P<0.05).The two radiologists had high agree-ment for Kappa values of the subjective scores,with the Kappa values being not lower than 0.75.Group A-FBP showed the lowest vessel CT value,Group B-FBP exhibited the highest image noise and the lowest SNR and CNR,and the five groups differed significantly in the objective evaluation of image quality(P<0.05).Group B-ADMIRE Ⅲ,B-ADMIRE Ⅳ and B-ADMIRE Ⅴ had statistically significant differences with Group A-FBP in objective evaluation of image quality(all P<0.05),with Group B-ADMIRE Ⅴ exhibiting the lowest image noise while highest SNR and CNR.Conclusion Ultra-low tube voltage of the third-generation dual-source CT combined with ADMIRE decreases the radiation dose and ensures image quality when applied to hemoptysis patient CTA,and ADMIRE Ⅳ behaves well in noise reduction and image quality enhancement.[Chinese Medical Equipment Journal,2025,46(10):60-64]
