1.Research advances in screening methods for pancreatic lipase inhibitors

Obesity and its related metabolic diseases have become a major global public health threat, and its rising incidence significantly increases the risk of cardiovascular and cerebrovascular diseases, diabetes and other complications. Pancreatic lipase is a key enzyme that converts food-borne lipids into triglycerides and fatty acids, and the effective inhibition of its activity has become an important strategy for the treatment of obesity. This paper discusses the screening methods of pancreatic lipase inhibitors, and summarizes and reviews the basic principles, advantages and disadvantages and application status of traditional screening methods, modern new screening methods and virtual screening methods. In view of the problems faced by the screening methods of pancreatic lipase inhibitors, future research urgently needs to move towards a collaborative innovation path of multi-technology integration, intelligent screening and complex systematization of traditional Chinese medicine, so as to open up new research paradigms.
2.Evaluation of the application of a predictive model for red blood cell demand in surgical procedures

Objective: To assess the clinical application value of a prediction model for red blood cell (RBC) demand in surgical procedures. Methods: Demographic data, laboratory parameters, anesthesia and transfusion records, and model prediction data were retrospectively collected from surgical patients at the First Medical Center of Chinese PLA General Hospital between 2018 and 2024. Statistical analysis was performed using the Chi-square test, t-test, and Mann-Kendall trend test. Results: From 2018 to 2024, the predictive model for RBC demand in surgical procedures was used to evaluate a total of 112 293 surgeries. During this period, the model call rate (77.49%-98.91%, P<0.05), compliance rate (56.81%-84.92%, P<0.05), and prediction accuracy rate (66.82%-94.17%, P<0.05) all showed significant upward trends. The total blood usage across the hospital (13645.4-7723.5 units, P<0.05) and the average blood usage per surgery (0.21-0.1 units, P<0.05) exhibited overall downward trends. Postoperative average hemoglobin levels in the non-compliance group (112.1-105.3 g/L in the non-compliance group vs 106.9-92.7 g/L in the compliance group, P<0.05) and the intraoperative excessive transfusion rate (5.06%-6.05% in the non-compliance group vs 0.09%-0.04% in the compliance group, P<0.05) were significantly higher in the non-compliance group compared to the compliance group. Conclusion: The predictive model for RBC demand in surgical procedures has played a positive role in conserving blood resources, optimizing blood resource allocation, and reducing intraoperative risks.
3.Research progress on application of immune checkpoint inhibitors in neoadjuvant treatment of melanoma

As the application of immune checkpoint inhibitors(ICIs)in the perioperative treatment of melanoma is increasingly introduced at earlier stages,it presents a critical opportunity for the development and clinical translation of neoadjuvant therapy.The results of phaseⅠ/Ⅱ clinical trials on neoadjuvant ICI therapy for melanoma demonstrate that neoadjuvant ICIs effectively improve the pathologic re-sponse rate in melanoma patients.Recent studies have shown that combining ICIs with other treatment modalities,including radiotherapy,chemotherapy,and targeted therapies,can enhance antitumor efficacy of neoadjuvant treatment for patients with melanoma.Optimizing treatment regimens,managing adverse events,identifying and addressing pseudoprogression,and handling cases of oligoprogression have become key areas of research in incorporating ICI regimens into neoadjuvant treatment for patients with melanoma.The search for bio-markers to monitor immunotherapy efficacy is expected to become a major focus of future research.This article provides a review of the re-search progress,controversies,and challenges in the application of ICIs in the neoadjuvant treatment of melanoma,and discusses future re-search directions,aiming to offer insights into the clinical application and development of ICIs in melanoma neoadjuvant therapy.
4.Research progress on the relationship between HBV DNA load and immunotherapy for HCC

Immunotherapy has become a pivotal treatment regimen for hepatocellular carcinoma (HCC); however, its efficacy is influenced by various factors. Hepatitis B virus (HBV) infection is one of the primary etiological factors leading to HCC. HBV DNA replication can alter the immune microenvironment through multiple mechanisms, notably by upregulating the expression of programmed cell death protein 1 (PD-1) and its ligand (PD-L1), thereby facilitating tumor immune escape. Paradoxically, this upregulation of PD-1/PD-L1 may enhance the response rate to PD-1/PD-L1 inhibitors and potentiate the antitumor effect. This review aims to summarize current research progress on the relationship between HBV DNA load and the efficacy of PD-1/PD-L1 inhibitors, explore the underlying mechanisms, and provide a scientific basis for promoting personalized treatment strategies for patients with HBV-related HCC.
5.Individualized dosage study of vitamin D3 based on physiologically-based pharmacokinetic modeling

AIM:To establish a physiologically-based pharmacokinetic(PBPK)model for vitamin D in adults,aiming to provide guidance for the ratio-nal clinical use of vitamin D in individuals with vita-min D deficiency.METHODS:Relevant literature and databases were reviewed to obtain the physi-cochemical properties and pharmacokinetic param-eters of vitamin D3.The PBPK model for adult whole-body vitamin D was constructed,optimized,and predicted using PK-Sim? software.The model's predictive performance was evaluated using confi-dence intervals,goodness of fit,and fold error(FE).The effectiveness of commonly used clinical dosing regimens was assessed based on the final opti-mized model,and personalized dosing recommen-dations were provided.RESULTS:The established adult whole-body PBPK model for vitamin D had a goodness of fit R2 of 0.961,approaching 1,and the FE values for AUC0-∞ and Cmax were both within the range of 0.5 and 2,indicating that the constructed PBPK model possesses good data predictive capa-bility.CONCLUSION:A successful PBPK model for oral vitamin D3 in adults has been established,showing good predictive performance for single oral doses of vitamin D3.Single oral doses of vita-min D3(7 500 μg and 15 000 μg)are safe and effec-tive dosing regimens for improving vitamin D insuf-ficiency or deficiency in Asian adults.Regular moni-toring of vitamin D levels before and during treat-ment is recommended to achieve the optimal out-comes of personalized therapy.
6.Direction and clinical practice of traditional Chinese medicine in the prevention and treatment of cancer

Cancer is a major chronic disease that threatens human health, while traditional Chinese medicine (TCM) is a unique method for cancer prevention and treatment in China. After about 70 years of innovation and development, TCM has made constant progress in areas such as the clinical diagnosis, treatment, evidence-based researches, and mechanism exploration of cancer. It has special advantages in aspects such as reducing toxicity, enhancing treatment efficacy, managing symptoms, accelerating recovery, preventing recurrence and metastasis, and prolonging advanced-stage survival. However, there are still bottlenecks for TCM in cancer care. This paper cuts in the key links between TCM and western medicine in their combined application in cancer prevention and treatment, and take the original TCM theories on cancer as the lead, high-quality evidence-based researches as the drive, and analysis on the dynamic mechanism as the core, to show the advantages and effects of TCM in cancer treatment in an all-round way. It also aims to provide novel strategies for sustainable and innovative development and for formulation of comprehensive schemes that integrate TCM and western medicine for cancer prevention and treatment.
7.Application of serum microRNA-30b-5p and microRNA -141-3p levels in the diagnosis of early neonatal ventilator-associated pneumonia

Objective:To explore the application of serum microRNA (miR)-30b-5p and miR-141-3p levels in the diagnosis of early neonatal ventilator-associated pneumonia (VAP).Methods:From January 2020 to January 2023, 90 hospitalized newborns who received invasive mechanical ventilation in the Qingdao Eighth People′s Hospital were retrospectively selected as the study subjects. Among them, 49 cases with VAP were as the VAP group, and 41 cases without VAP were as the non VAP group. The serum levels of miR-30b-5p and miR-141-3p were measured before and 48 h after mechanical ventilation in the two groups, and the pulmonary infection scoring system (CPIS) was applied to evaluate the pulmonary infection status. Spearman test was applied to analyze the correlation between serum miR-30b-5p, miR-141-3p levels and CPIS scores; Logistic regression was applied to analyze the influencing factors of VAP; the efficacy of serum miR-30b-5p and miR-141-3p levels alone and in combination for the diagnosis of VAP were analyzed by the receiver operating characteristic (ROC) curve.Results:After 48 h of mechanical ventilation, compared with the non VAP group, the serum levels of miR-30b-5p and miR-141-3p in the VAP group were obviously reduced, while the CPIS scores in the VAP group was obviously increased: (0.47 ± 0.15) μg/L vs. (1.02 ± 0.24) μg/L, (0.36 ± 0.11) μg/L vs. (1.02 ± 0.28) μg/L, (5.19 ± 1.24) scores vs. (3.74 ± 1.08) scores, there were statistical differences ( P<0.05). The results of Spearman test showed that serum levels of miR-30b-5p and miR-141-3p were negatively correlated with CPIS scores ( r = - 0.572, - 0.580, P<0.05). Logistic regression analysis showed that serum miR-30b-5p, miR-141-3pand CPIS scores were risk factors that affected VAP in newborns. The results of ROC curve analysis showed that the area under the curve (AUC) of serum miR-30b-5p and miR-141-3p levels combined diagnosis was 0.891(95% CI 0.808 - 0.947), which was higher than the AUC of diagnosed separately ( Z = 1.96, 2.44, P<0.05). Conclusions:Abnormal expression of miR-30b-5p and miR-141-3p in serum has certain diagnostic value for neonatal VAP, and can provide certain reference for clinical diagnosis.
8.Analysis of risk factors for central nervous system infection after craniotomy

Objective To analyze the incidence and risk factors of central nervous system infection(CNSI)after craniotomy.Methods Clinical data from 1 432 patients who underwent craniotomy in the neuro-surgery department of the hospital from 2019 to 2021 were retrospectively collected,and the positive rates of CNSI after craniotomy in neurosurgery were analyzed.Univariate analysis and multivariate logistic regression analysis were adopted to identify the risk factors affecting the occurrence of CNSI.Results The positive rate of CNSI in 1 432 patients was 12.92%,and the positive rate of bacterial culture in cerebrospinal fluid samples was 2.88%.Univariate analysis showed that hypertension,length of hospital stay,preoperative length of hos-pital stay,intensive care unit(ICU)treatment,American Society of Anesthesiologists(ASA)grading,surgery time≥ 3 h,blood transfusion,cerebrospinal fluid leakage,and postoperative hormone use were influencing fac-tors for the occurrence of CNSI.Multivariate logistic regression analysis showed that hypertension(OR=1.475),length of hospital stay higher than 30 days(OR=2.498),length of ICU treatment(OR=2.381),length of surgery(OR=1.572)higher than 3 hours and cerebrospinal fluid leakage(OR=3.062)were inde-pendent risk factors for the occurrence of CNSI.Conclusion The positive rate of CNSI after craniotomy is rel-atively high,and there are many related risk factors.Medical staff should take relevant measures to address the risk factors,continuously carry out targeted monitoring,and provide a basis for clinical treatment.
9.Association of Rab37 gene polymorphism and delayed menarche:a report of 2 060 women

Objective To explore the association between single nucleotide polymorphism(SNP)of Rab37 gene and the age at menarche(AAM).Methods A case-control study design was used to conduct an epidemiologic survey on 2 060 women in a community in Jurong City,Jiangsu Province,and their venous blood samples were collected.ASA chip was used to explore the Rab37 gene genotyping of blood sample DNA to obtain the genotyping results for Rab37 SNPs rs62084865,rs10512597,rs35489971,rs1037170,rs6501732,rs77822106,and rs3178300.Based on menarche time of<16 years old or≥16 years old,the participants were divided into 2 groups.Linear and logistic regression analyses were used to explore the association between the SNP of Rab37 gene and the age at menarche based on dominant,recessive and allelic models.Results Among the 2 060 healthy women,944 women had normal menstrual periods,while 1 116 women experienced delayed menarche.Linear regression analysis showed that the gene polymorphism of rs3178300 was negatively correlated with the age at menarche in females[recessive model:CC/CT+TT,β=-0.915,95%CI(-1.692,-0.137),P=0.021;allelic model:T/C,β=-0.221,95%CI(-0.410,-0.032),P=0.022].Logistic regression analysis indicated that the gene polymorphism of rs3178300 was significantly associated with the risk of delayed menarche in females[recessive model:CC/CT+TT,OR=0.295,95%CI(0.116,0.751),P=0.010;allelic model:T/C,OR=0.796,95%CI(0.652,0.972),P=0.025].The remaining 6 SNPs showed no significant association with age at menarche.Conclusion The rs3178300 polymorphism of Rab37 gene is significantly associated with delayed menarche in Chinese women.
10.Clinical and genetic characteristics of 6 cases of congenital dyskeratosis in children

Objective:To explore the clinical and genetic characteristics of dyskeratosis congenita (DC).Methods:A retrospective analysis was conducted on the clinical, laboratory, imaging, pathological, genetic, and treatment data of 6 DC patients diagnosed at the Children′s Hospital of Zhejiang University School of Medicine from January 2010 to June 2025.Results:Among the 6 DC patients, 1 case was of Hoyeraal-Hreidarsson syndrome, 4 were male, and 2 were female. The diagnosis age 0.9-6.1 years. All 6 cases presented with bone marrow failure; 5 cases had a classic triad of skin and mucous membrane (mucosal leukoplakia, abnormal skin pigmentation, nail dystrophy); 5 cases had growth retardation, among which 2 cases had intrauterine growth retardation. Two cases had diarrhea and 1 case had abnormal liver function; 1 case had stiff and deformed limbs, accompanied by limited mobility, and dry and obstructive balanitis; 1 case had recurrent eyelid inflammation, middle ear inflammation, and nasal inflammation. All 6 cases had decreased B cell numbers, and 4 cases also had decreased natural killer cell numbers. There were 3 cases of children with cytomegalovirus (CMV) infection, of which 1 case of CMV infection led to retinal frosted branch angiitis and subsequent intracranial CMV infection resulting in death, and 1 case had CMV enteritis and died of hemophagocytic syndrome. Among 4 cases of boys, 3 cases had DKC1 gene variations and 1 case had an unknown variation gene; 2 cases of girls had TINF2 gene variations. The TINF2 c.860T>A (p.L287Q) variation site was a new mutation. Among 6 patients with DC, 2 cases died, 3 cases survived and 1 case was lost to follow-up.Conclusions:The DKC1 and TINF2 genes are common pathogenic genes in patients with DC. Bone marrow failure is a clue for the early identification of DC. The triad of skin and mucous membrane is its typical clinical manifestation. Children with DC generally have reduced B cells and natural killer killer cells, and have a high risk of fatal CMV infection. The overall prognosis is poor.
