1.Revascularization of Left Internal Carotid Artery for Acute Aortic Dissection Type A with Cerebral Malperfusion

Cerebral malperfusion is a serious complication of acute aortic dissection type A(AADA), and the best strategy for its management remains unclear. A 71-year-old woman was brought to our hospital because of consciousness disorder and right hemiplegia. Contrast-enhanced CT showed AADA and occlusion of the left common carotid artery. As the symptoms gradually improved and CT showed flow in the left distal carotid artery, we prioritized central repair by total arch replacement and Frozen Elephant Trunk with deep hypothermia and antegrade cerebral perfusion (ACP). Although the ACP cannula did not go into the left common carotid artery and we eventually had to do a left intra-carotid bypass, she was discharged home without any symptoms. It is acceptable that we give the priority to central repair over direct carotid artery re-perfusion when her symptoms improve. Besides we have to perform carotid bypass if the malperfusion is remains.
2.A Case of Preoperative DIC and Carotid Artery Occlusion due to Type A Acute Aortic Dissection

A 61-year-old male presented to another hospital with sudden chest and back pain, and CT revealed a diagnosis of type A acute aortic dissection with patent false lumen. The ascending aortic diameter was 45 mm and the right common carotid artery was occluded. There were no neurological abnormalities, no pericardial effusion, and only mild AR. Eight hours after onset, the patient was transferred to our hospital. The laboratory data showed severe DIC with fibrinogen <50 mg/dl, so that medical DIC treatment was given first because of the high risk of bleeding. Twenty-two hours after the onset, DIC improved and surgery was initiated. The right common carotid artery was ligated for fear of thrombus dispersion at the periphery of the occlusion site. An ascending arch replacement was then performed. Postoperative hemostasis was good, and no new neurological abnormalities were observed. Usually, type A acute aortic dissection is indicated for emergency surgery, but in this case, DIC treatment took precedence. We report here a rare experience.

Background/Aims:
Esophageal motility disorders (EMDs) contribute to the pathophysiology of gastroesophageal reflux disease. However, the causes of EMDs and their impact on gastroesophageal reflux disease-associated symptoms remain unknown. This study aims to elucidate clinical features associated with various types of EMDs in patients with heartburn symptoms.
Methods:
Of the 511 patients who underwent high-resolution manometry, 394 who were evaluated for heartburn symptoms were examined. Patients subjected to high-resolution manometry were classified into 4 groups: outflow obstruction group, hypermotility group, hypomotility group, and normal motility group. Symptoms were evaluated using 3 questionnaires. Patient characteristics and symptoms for each EMD type were compared with those of the normal motility group.
Results:
Of the 394 patients, 193 (48.9%) were diagnosed with EMDs, including 71 with outflow obstruction, 15 with hypermotility, and 107 with hypomotility. The mean dysphagia score was significantly higher in each of the 3 EMD groups compared with those with normal motility. The mean acid reflux and dyspepsia scores were significantly lower in the outflow obstruction group (P < 0.05). The mean body mass index and median Brinkman index were significantly higher in the hypermotility group (P = 0.001 and P = 0.018, respectively), whereas the mean diarrhea and constipation scores were significantly lower in the hypomotility group (P < 0.05).
Conclusions
The results of our study indicate that different EMDs have distinct characteristics. Cigarette smoking and high body mass index were associated with esophageal hypermotility. Assessment of the dysphagia symptom scores may help identify patients with EMDs.

In patients with unresectable hepatocellular carcinoma (HCC) without both macrovascular invasion and extrahepatic metastasis, the initial treatment choice recommended is transarterial chemoembolization (TACE). Before sorafenib came into wide use, TACE had been pointlessly carried out repeatedly. It was in the early 2010s that the concept of TACE refractory was advocated. Two retrospective studies from Japan indicated that conversion from TACE to sorafenib the day after patients were deemed as TACE refractory improved overall survival compared with continued TACE, according to the definition by the Japan Society of Hepatology. Nowadays, phase 3 trials have shown clinical benefits of several novel molecular target agents. Compared with the era of sorafenib, sequential treatments with these molecular target agents have gradually prolonged patients’ survival and have become major strategies in patients with HCC. Taking these together, conversion from TACE to systemic therapies at the time of TACE refractory, compared with before, may have a greater impact on survival and may be considered deeper in the decisions-making process in patients with unresectable HCC who are candidate for TACE. Up-to-date information on the concept of TACE refractory is summarized in this review. We believe that the survival of patients with unresectable HCC without both macrovascular invasion and extrahepatic metastasis may be dramatically improved by optimal timing of TACE refractory and switching to systemic therapies.
7.Changes in the recommended age for rubella-containing vaccine and rubella seroprevalence among pregnant women

Objectives : To investigate the prevalence of rubella HI antibody among pregnant women in relation to changes in the recommended age for rubella-containing vaccination.
Methods : A total of 659 primiparous women were included in this study. All subjects were ≥20 years old at the time of delivery in our hospital during the 5-year period from January 2008 to December 2012. Rubella HI antibody titers were measured. Subjects were divided into four groups according to their date of birth and retrospectively analyzed : i) single-dose mass vaccination in junior high school generation ; ii) “interim measures generation” ; iii) single-dose vaccination in infancy generation ; and iv) infancy plus catch-up MR two-dose vaccination generation. HI antibody titers ≤1 : 16 was defined as a low antibody titer.
Results : The percentage of cases with low antibody titers in the total study population was 20.2%. The percentages of cases with low antibody titers in each group were : i) 20.4%, ii) 18.2%, iii) 30.6%, and iv) 31.6%.
Conclusion : The percentage of low antibody titers among all cases was comparable to other reports. The results suggested that rubella seroprevalence was low among those in the generation including and subsequent to the single-dose vaccination in infancy generation.
8.Serum leptin concentrations, leptin mRNA expression, and food intake during the estrous cycle in rats.

The aim of this study was to investigate food intake, serum leptin levels, and leptin mRNA expression during the sexual cycle in rats. Female Wistar-Imamichi rats aged 8-10 weeks were used in this experiment. Food intake was measured during the light and dark phases (light on at 07:00 and off at 19:00) of the 4-day estrous cycle in female rats. Serum leptin levels were measured by ELISA, and leptin mRNA expression levels were analyzed using real-time PCR on diestrous- and proestrous-stage rats. Our results revealed that during the sexual cycle, food intake was significantly higher in the dark phase compared with the light phase. Food intake in proestrous females was significantly lower in the light and dark phases compared with the other groups. Serum leptin concentrations were significantly higher in both phases in proestrous rats compared with diestrous rats. There was a significant increase in leptin mRNA expression in adipose tissue during the proestrous period compared with the diestrous period. These findings suggest that increased leptin mRNA expression and serum leptin levels, which are induced by estrogen during the proestrous stage, may play a role in regulating appetitive behavior.

9.The immediate effects of various treatment styles

[Objective]In clinical practice, it is important that patients experience symptomatic improvement or at least gain a certain level of satisfaction early on during a series of treatments. Most patients decide whether or not to return to the clinic based on these factors. At our school, students are trained in protocols from three different treatment styles:modern acupuncture, meridian acupuncture (based on classic acupuncture) and traditional Chinese medicine. In this study, we investigated the characteristics of each style, such as the presence of immediate effects and the types of sensations perceived by patients.
[Methods]Questionnaires were given before and after treatment to 177 patients who came to the clinic associated with our teacher training course for acupuncture, moxibustion and massage. We received valid responses from 169 patients. The practitioners surveyed were second-year students who had treated the surveyed patients 1.2 times on average prior to answering the questionnaire. Before treatment, patients were asked about their main complaints and symptoms, and afterward about changes in their symptoms, satisfaction level and other sensations. The resulting data was subjected to statistical analysis.
[Results]The style chosen by most practitioners was modern acupuncture, followed by meridian acupuncture, and then traditional Chinese acupuncture. Most lower back, leg and joint pain was treated with modern acupuncture protocols, while internal disorders were more often treated with meridian acupuncture or traditional Chinese acupuncture. All three styles were able to alleviate symptoms and achieve patient satisfaction with no statisticallysignificant differences among the three groups.
[Discussion]The results demonstrated that training practitioners in these three styles for one year helped them achieve positive and immediate effects. While there are many different styles of acupuncture, in actual clinical practice, the style is less important than how much a patient feels the effect of the treatment.
[Conclusion]All three styles, modern acupuncture, meridian acupuncture and traditional Chinese acupuncture, were able to alleviate symptoms and achieve patient satisfaction with no statistically significant differences among them.

Until the Sui Dynasty in China, night sweat and spontaneous perspiration had been thought to be caused by same pathophysiology, that is, lowered superficial resistance by deficiency of Qi.In the Tang Dynasty, these were considered to have different pathophysiologies and a new principle indicated that pathogenic heat caused night sweat.In the Song and Jing Dynasties, deficiency of blood and pathogenic heat by deficiency of Yin was also considered to cause night sweat.In the Jing Dynasty, exogenous pathogens, such as Cold were considered to cause night sweat, which indicated the principle that not only the deficiency syndrome but also the excess syndrome caused night sweat.In the beginning of the Yuan and Ming Dynasties, it was concluded that the deficiency of Yin caused night sweat and the deficiency of Yang caused spontaneous perspiration.In the middle of the Ming Dynasty, another new theory indicated that deficiency of Yang also possibly caused night sweat; therefore we should diagnose abnormal sweat depending on the pathophysiology in each case.In the Qing Dynasty, new theories were established stating that not only exogenous pathogens but also Damp-heat, undigested food and stagnation of blood, all of which are included in excess syndrome, cause night sweat, and that based on which part of the body sweats occurred we might understand pathophysiology of night sweat. The night sweat by Warm-heat, which is different from the one by Wind-cold, was considered to be caused with deficiency of Yin.Thus we conclude that the theories of night sweat developed over time, based on Chinese medical classics.
