1.Epidemiological characteristics of confirmed mpox cases in Sichuan Province in 2023

Objective To analyze the epidemiological and clinical characteristics of confirmed mpox cases in Sichuan Province in 2023 and to provide a scientific basis for the prevention and control of the mpox epidemic, and provide reference for the prevention and control of other emerging infectious diseases. Methods The data were collected from the information of confirmed mpox cases residing in Sichuan Province reported in the National Infectious Disease Reporting Information Management System until 24:00 on December 31, 2023, as well as the epidemiological investigation report of mpox cases. Excel 2020 software was used to input, sort out and plot the data, SPSS 23 statistical software was used to analyze the data, and descriptive epidemiological methods were used to analyze the basic characteristics of the cases. Results The first mpox case was reported in Sichuan Province on June 30, 2023, and the peak period of reported cases was from July to August. As of December 31, 2023, a total of 141 mpox cases had been reported. The cases were predominantly male (99.3%), aged 30-<40 years (49.6%), and MSM (men who have sex with men, accounting for 93.0% of male cases). 35.4% of cases reported HIV infection. The interval from onset of symptoms to diagnosis ranged from 2 to 90 days, with M(P25,P75) being 4 (6, 8) days. The incubation period for cases ranged from 1 to 21 days, with M(P25,P75) being 7 (4, 10) days. The majority of cases were discovered through proactive medical consultation (96.4%). The initial symptoms were rash (70.9%) and fever (31.2%). The main symptoms were rash (95.7%), fever (55.3%), and lymph node enlargement (35.5%). Conclusion Mpox outbreaks mainly occur in MSM population. It is necessary to make full use of the comprehensive intervention service system for AIDS prevention and treatment to publicize and educate key groups, timely monitor the variation of monkeypox virus, especially strengthen the surveillance of inbound cases, implement early detection and control of cases, and reduce the risk of epidemic transmission.
2.Investigation of Intestinal Absorption Characteristics of Multi-index Components of Danggui Buxuetang Based on Drug Absorption Simulating System

ObjectiveTo investigate the intestinal absorption characteristics of multi-index components in Danggui Buxuetang with drug absorption simulating system (DASS) established by everted intestinal sac model. MethodThe intestinal absorption solution at different time points after administration of Danggui Buxuetang was collected and detected by high performance liquid chromatography (HPLC), acetonitrile (A)-0.2% glacial acetic acid solution (B) was used as the mobile phase for gradient elution (0-16 min, 15%-23%A; 16-20 min, 23%-28%A; 20-25 min, 28%-30%A; 25-30 min, 30%A; 30-35 min, 30%-65%A; 35-45 min, 65%-95%A), the detection wavelength was 302 nm. HPLC fingerprint of intestinal absorption solution was established and the common peak was calibrated, and the relative cumulative absorption rate of each index component was calculated. The relative cumulative absorption curves of components were fitted with various mathematical models by DDSolver 1.0 to explore the absorption law of different components. ResultThe absorption process of C2 (calycosin-7-glucoside) and C6 in Danggui Buxuetang was in line with zero-order equation, C9 was best fitted by Weibull equation, and the remaining 7 components were in line with Makoid-Banakar equation. C1 with C2, C3, C5, C7 and C10, C2 with C5 and C7, C3 with C4, C5, C7 and C10, C4 with C6 and C10, C5 with C7, C6 with C10, C7 with C10, C8 with C9 were absorbed simultaneously during the absorption process. With the prolongation of time, the overall cumulative absorption rate of Danggui Buxuetang increased. At 120 min, the overall cumulative absorption rate of Danggui Buxuetang exceeded 38%, and reached 49.14% at 180 min. ConclusionTen ingredients in Danggui Buxuetang are absorbed in the jejunum, but absorption law of various components is different, which shows that the intestinal absorption of compound preparations of traditional Chinese medicine (TCM) has multiple characteristics. Intestinal absorption study of TCM compound preparations with chemical composition as the index can reveal some of its absorption law, but it is not complete.
3.A nomogram based on clinical factors and gadobenate dimeglumine-enhanced MRI for prediction of GPC-3 expression in hepatocellular carcinoma

Objective:To investigate the predictive value of a nomogram based on clinical factors and gadobenate dimeglumine (Gd-BOPTA)-enhanced MRI for predicting the expression of Glypican-3 (GPC-3) in hepatocellular carcinoma (HCC).Methods:The clinical and imaging data of 85 patients with HCC confirmed by pathology in the Provincial Hospital of Shandong First Medical University from July 2018 to June 2021 were retrospectively collected. All the patients underwent Gd-BOPTA-enhanced MRI scan before operation. According to the expression of GPC-3 by immunohistochemistry, the patients were divided into GPC-3 positive group (55 cases) and GPC-3 negative group (30 cases). The clinical data of patients were collected, including gender, age, hepatitis, cirrhosis, alpha-fetoprotein (AFP), alanine aminotransferase, aspartate aminotransferase, and glutamine transferase levels. The MRI qualitative signs including tumor margin, ring enhancement, intratumoral hemorrhage, enhanced capsule, and satellite nodules were reviewed. MRI quantitative parameters including the largest tumor diameter, Gd-BOPTA-enhanced tumor-to-liver parenchyma signal ratio (TLR) and tumor enhancement ratio (TER) in arterial phase (AP), portal venous phase (PP), and hepatobiliary phase (HBP) were calculated. The independent sample t-test or Mann-Whitney U test were used to compare the quantitative data between the two groups, and the χ2 test was used to compare the qualitative data between the two groups. Multivariate logistic regression analysis was used to identify the independent predictors of GPC-3 expression, and a nomogram model was established. The receiver operating characteristic (ROC) curves were used to evaluate the predictive performance of each independent factor and nomogram, and DeLong test was used to compare differences in area under the curve (AUC). Results:There were significant differences in AFP, tumor margin, intratumoral hemorrhage, and TLR-AP, TLR-PP and TLR-HBP between GPC-3 positive and negative groups (all P<0.05). Multivariate logistic regression results showed that AFP≥20 μg/L, intratumoral hemorrhage and TLR-HBP were independent predictors of GPC-3 positive expression in HCC (OR=3.816, 4.788, 0.001, all P<0.05). The preoperative clinical and Gd-BOPTA-enhanced MRI nomogram model for predicting GPC-3 expression in hepatocellular carcinoma was established. The AUC of AFP≥20 μg/L, intratumoral hemorrhage, TLR-HBP and nomogram model in predicting GPC-3 positive expression were 0.688, 0.697, 0.808, and 0.879, respectively. The AUC of nomogram model was significantly better than those of the other three single indicator ( Z=3.82, 4.13, 2.04, P<0.001,<0.001,=0.042). Conclusion:The nomogram model based on indicators of clinical and qualitative and quantitative Gd-BOPTA-enhanced MRI has better performance in predicting the expression of HCC GPC-3 before surgery, which is higher than those of each single indicator.
4.Clinical and myopathological features and genetic mutations of GNE myopathy

Objective To investigate clinical and myopathological features and genetic mutations of GNE myopathy.Methods The clinical manifestations and pathological findings of 4 patients were reviewed who had been treated for GNE myopathy at Department of Neurology,The People's Hospital of Zhengzhou University from January 2016 to July 2017.The exons of GNE gene were sequenced to detect gene mutations in all of them.Results Onset of the disease started at late adolescence or early adulthood in the 4 patients (2 siblings).Gait disturbance appeared as an initial symptom.The disease progressed slowly and the distal muscles were more affected than the proximal ones.Muscle magnetic resonance imaging revealed that the soleus and the anterolateral muscles of the lower leg were the most severely involved and the internal and posterior compartments of the thigh muscles were more involved than the anterior one.The characteristic pathological finding was presence of rimmed vacuoles.Two siblings (Patients 1 and 2) both had the same compound heterozygous mutations of p.G395R and p.D207V,patient 3 had the compound heterozygous mutations of p.D207V and p.G166D,and patient 4 had the homozygous mutation ofp.G237D.Conclusions Sparing of the quadriceps relatively is a very unique feature of GNE myopathy.Missense mutations (G395R and G237D) may be 2 new ones in the GNE gene.
5.Expression of TM4SF1 in breast cancer tissue and its clinical significance

Objective:To investigate the expression of transmembrane 4 super family 1 (TM4SF1)in breast cancer tissue,and to elucidate its clinical significance and explore the related molecular biological mechanisms. Methods:A total of 190 cases of human breast cancer,110 cases of paracancerous tissue and 110 cases of normal breast tissue were collected.Immunohistochemistry was used to detect the expression levels of TM4SF1 mRNA in breast cancer tissue,paracancerous tissue,and normal breast tissue;Western blotting method was used to detect the expression levels of TM4SF1 in breast cancer tissue,paracancerous tissue,and normal breast tissue;RT-PCR method was used to detect the expression levels of TM4SF1 mRNA in breast cancer tissue,paracancerous tissue, and normal breast tissue.The positive expression rates of TM4SF1 in breast cancer tissue of the breast cancer patients with different clinicopathological features were detected.Results:The positive expression rate of TM4SF1 in the breast cancer tissue was significantly higher than those in paracancerous tissue and normal breast tissue (P <0.05);there was no significant difference in the positive expression rates of TM4SF1 between paracancerous tissue and normal breast tissue (P = 0.531);the expression of TM4SF1 was not correlated with age,but was closely correlated with tumor size,differentiation degree,lymph node metastasis and tumor stage (P <0.05);the positive expression rate of TM4SF1 in basal like breast cancer tissue was higher than those in the other three types of tissues (P <0.05).The results of Western blotting showed that the expression level of TM4SF1 in breast cancer tissue was higher than those in paracancerous tissue and normal breast tissue (P < 0.05 ), but there was no significant difference in the expression level of TM4SF1 between paracancerous tissue and normal breast tissue (P >0.05). The results of RT-PCR showed that the expression level of TM4SF1 mRNA in breast cancer tissue was higher than those in the paracancerous tissue and normal breast tissue (P <0.01);there was no significant difference in the expression level of TM4SF1 mRNA between paracancerous tissue and normal breast tissue (P > 0.05 ). Conclusion:The TM4SF1 is highly expressed in breast cancer tissue. TM4SF1 may affect the occurrence, development and distant metastasis of breast cancer through various mechanisms.TM4SF1 may be a potential target for the treatment of breast cancer.

7.A survey on diseases suffering in 5 068 members of rescue teams in Weachuan earthquake

Objective To investigate the incidence of diseases in member of rescue teams in the Wenchuan earthquake in Sichuan Province.Method Twenty days afer the Wenchuan earthquake in 2008,5 068 resucing staff of rescue learns as random and cluster sample were investigated and analyzed to find the factors impacting their fitness associated with their characters,diseases,work,and encampment styles.Results Members of rescue teams suffered from the following diseases:insect dermatitis (18.37%),tinea pedis (10.83%),acute upper respiratory infection (10.56%)and solar dermatitis (6.20%).Contrasted by work tasks,staff resucing on the front line had the highest incidence of acute upper-respiratory infection(11.04%) ,logistics workers had the highest incidence of tinea pedis(15.21% ),and more patients of acute gastroenteritis,insect dermatitis,gingivitis emerged from medical personnel group(P<0.05).The encampment sites were devided into three sorts:plain land,hillside and valley.The staff stayed at valley had higher incidence of acute upper respiratory infection (24.90%),insect dermatitis (36.50%) and tinca pedis (20.02%)than those worked at plain land and hillside (P<0.01).The incidence of acute gastroerrteritis(1.26%) ha staff resides hillside is lower than that in valley or plain land (P<0.05).Conclusions In the period of rescue actions in the guake-hit region,the members of rescue teams suffered from famihar diseases such as dermatitis and acute upper-respiratory infection,etc.The incidence of diseases is associated with their work tasks and the topography of cantonment.
