1.Current status of talent cultivation in sports rehabilitation in China: based on World Health Organization rehabilitation competency framework

ObjectiveTo analyze the current status and existing challenges in talent cultivation for sports rehabilitation in China based on World Health Organization rehabilitation competency framework (RCF). MethodsData were collected from 104 higher education institutions nationwide that offer undergraduate programs in sports rehabilitation, including enrollment scale, regional distribution, degree conferral and training curriculum. Descriptive statistics and content analysis were used to examine the status and characteristics from three dimensions: institutional distribution, competency development and resource allocation, in terms of institutional type, regional distribution density, curriculum structure differences, competency-oriented training approaches and resource allocation patterns. ResultsTalent cultivation in sports rehabilitation in China currently faced a three-dimensional dilemma involving competency, resources, and public cognition. In terms of competency structure, a disconnect existed between medical fundamentals and exercise practice: physical education institutions provided insufficient medical training, whereas medical institutions lacked systematic instruction in exercise techniques. Regarding resource allocation, significant regional disparities were observed, with institutions heavily concentrated in Southwest (24.0%), East China (19.2%), and North China (15.4%), while Northwest China accounted for only 3.8%, forming a pronounced east-west gap. At the cognitive level, the public generally perceived sports rehabilitation as a service exclusive to athletes, and within the discipline, divergences persisted among the medical-oriented, sports-oriented, and integrated schools of thought. These challenges collectively hindered the quality of talent supply and regional balance. ConclusionTalent cultivation in sports rehabilitation in China is at a pivotal stage of transition from rapid expansion to quality enhancement. It is necessary to realign the knowledge structure and competency system of training programs with the five core competency domains of RCF.
2.Five-year survival analysis and influencing factors of elderly lung cancer patients with chronic obstructive pulmonary disease in Mianyang City

Objective To study the five-year survival status and influencing factors of elderly patients with lung cancer complicated with chronic obstructive pulmonary disease (COPD). Methods A cohort study was conducted to follow up 450 patients with lung cancer and chronic obstructive pulmonary disease who were hospitalized in our hospital from January 2018 to December 2023. The endpoint of the follow-up was the end of a five-year period or death. The Life Tables method was used to calculate survival rates and plot survival curves. The Cox proportional hazards model was used to analyze the influencing factors of five-year survival. Results The results indicated that the overall five-year survival rate of patients was 4.89%, and it decreased year by year. Cox regression analysis showed that age, gender, family functioning, and psychological status significantly influenced patient survival rate (all P<0.05). Stratified analysis found that the smoking status, family functioning, and psychological status of male patients all had an impact on survival rate (all P<0.05), while the psychological status of female patients had a more significant impact on survival (P=0.008). Conclusion This study provides a scientific basis for comprehensive intervention of elderly lung cancer patients with COPD. It is recommended that clinical attention should be paid to psychological and family factors to improve patient prognosis.
3.Modern Pharmacological Mechanisms and Clinical Applications of Xuan-dredging Wind Medicinals: A Review

Since Li Dongyuan formally proposed the concept of "wind medicinals" (Feng Yao),their clinical application has been highly valued by physicians throughout history. However,influenced by the evolution of the term and connotation of "wind medicinals" in modern times,its conceptual understanding,leading to a decline in clinical utilization. Since the new century,Professor Wang Mingjie has integrated LIU Wanxu's sweat pore (Xuanfu) theory into the reinterpretation of wind medicinals,proposing the "Xuanfu-dredging wind medicinal theory", which has gained widespread recognition in academic circles,revitalizing their clinical application. This study traces the origin of the Xuan-dredging wind medicinals theory and reviews their current functions and clinical applications,finding that the theoretical framework is preliminarily established. Characterized by their pungent and dispersing properties,wind medicines act by opening the Xuanfu throughout the body,exerting therapeutic effects such as dispelling pathogens,resolving stagnation,and enhancing treatments like blood-activation,spleen-fortification,and heat-clearing. They are widely used,showing advantages in treating systemic diseases including ophthalmic and cardiovascular/cerebrovascular disorders. Modern pharmacological research indicates preliminary consensus on hypotheses of cerebral,intestinal,hepatic,and renal Xuanfu. studies on formulas (e.g.,Qufeng Tongqiao Fang),single herbs (e.g.,Mahuang and Gegen),and active constituents (e.g.,tetramethylpyrazine) provide evidence that wind medicines improve key mechanisms like blood-brain barrier function and cerebral microcirculation (material basis of cerebral Xuanfu),supporting their use in brain disorders (e.g.,cerebral ischemia,depression). Despite clinical and pharmacological support,the clinical application system for wind medicines remains incomplete. Future efforts should focus on high-quality clinical research and mechanistic studies to establish personalized application systems,enhance Xuanfu opening practices,and ensure the effectiveness and safety of wind medicines.
4.Ethical issues and reflections on clinical research of radiopharmaceuticals

Radiopharmaceuticals play an important role in the diagnosis and treatment of cardiovascular and cerebrovascular diseases, malignant tumors, central nervous system diseases, and other diseases. Under the urgent need for clinical diagnosis and treatment as well as medical development, the clinical research of radiopharmaceuticals has become a hotspot in international research. By analyzing the current situation of clinical research on radiopharmaceuticals in Europe, America, and China, the ethical issues of clinical research on radiopharmaceuticals were elaborated from four aspects, including lack of relevant laws and regulations, a higher risk of radiopharmaceuticals, dilemmas in ethical review, and insufficient radiation protection. Response principles and measures were proposed from four aspects, including improving regulations and policies, enhancing radiological protection for all parties involved in the research, strengthening ethical review, and reinforcing the training of relevant personnel, to enhance the quality and level of clinical research on radiopharmaceuticals.
5.Decision-making behavior in patients with depressive disorder and its relationship with depressive and anxiety symptoms

BackgroundPatients with depressive disorder often exhibit impaired decision-making functions. However, the relationship between decision-making abilities and depressive and anxiety symptoms in these patients remains unclear. ObjectiveTo explore the characteristics of decision-making behavior in patients with depressive disorder, and to analyze its relationship with clinical symptoms. MethodsA total of 48 patients diagnosed with depressive disorder according to the Diagnostic and Statistical Manual of Mental Disorders, fourth edition (DSM-IV) were recruited from the Department of Psychosomatic Medicine of the Affiliated Hospital of Southwest Medical University from October 2020 to May 2023. Concurrently, 52 healthy individuals matched for age and gender were recruited from Luzhou as the control group. Beck Depression Inventory (BDI) and Beck Anxiety Inventory (BAI) were used for assessment, and decision-making behavior was evaluated using Probabilistic Reversal Learning (PRL) task. Indicators assessed included the number of trials to criterion, perseverative errors, win-stay rate and lose-shift rate. Spearman correlation analysis was used to assess the correlation between BDI and BAI scores and PRL task indicators. ResultsThe depression group showed a significantly higher lose-shift rate compared with the control group (t=3.684, P<0.01). There were no statistically significant differences between two groups in trials to criterion, perseverative errors and win-stay rate (t=0.329, 0.132, 0.609, P>0.05). In depression group, BDI and BAI scores were positively correlated with the win-stay rate(r=0.450, 0.398, P<0.01). ConclusionPatients with depressive disorder are more likely to change their decision-making strategies following negative outcomes. Furthermore, the severity of depressive and anxiety symptoms is associated with a greater propensity to maintain existing decisions after receiving positive feedback. 6.Clinical course, causes of worsening, and outcomes of severe ischemic stroke: A prospective multicenter cohort study.

BACKGROUND:
Severe stroke has high rates of mortality and morbidity. This study aimed to investigate the clinical course, causes of worsening, and outcomes of severe ischemic stroke.
METHODS:
This prospective, multicenter cohort study enrolled adult patients admitted ≤30 days after ischemic stroke from nine hospitals in China between September 2017 and December 2019. Severe stroke was defined as a score of ≥15 on the National Institutes of Health Stroke Scale (NIHSS). Clinical worsening was defined as an increase of 4 in the NIHSS score from baseline. Unfavorable functional outcome was defined as a modified Rankin scale score ≥3 at 3 months and 1 year after stroke onset, respectively. We performed Logistic regression to explore baseline features and reperfusion therapies associated with clinical worsening and functional outcomes.
RESULTS:
Among 4201 patients enrolled, 854 patients (20.33%) had severe stroke on admission. Of 3347 patients without severe stroke on admission, 142 (4.24%) patients developed severe stroke in hospital. Of 854 patients with severe stroke on admission, 33.95% (290/854) experienced clinical worsening (median time from stroke onset: 43 h, Q1-Q3: 20-88 h), with brain edema (54.83% [159/290]) as the leading cause; 24.59% (210/854) of these patients died by 30 days, and 81.47% (677/831) and 78.44% (633/807) had unfavorable functional outcomes at 3 months and 1 year respectively. Reperfusion reduced the risk of worsening (adjusted odds ratio [OR]: 0.24, 95% confidence interval [CI]: 0.12-0.49, P <0.01), 30-day death (adjusted OR: 0.22, 95% CI: 0.11-0.41, P <0.01), and unfavorable functional outcomes at 3 months (adjusted OR: 0.24, 95% CI: 0.08-0.68, P <0.01) and 1 year (adjusted OR: 0.17, 95% CI: 0.06-0.50, P <0.01).
CONCLUSIONS:
Approximately one-fifth of patients with ischemic stroke had severe neurological deficits on admission. Clinical worsening mainly occurred in the first 3 to 4 days after stroke onset, with brain edema as the leading cause of worsening. Reperfusion reduced the risk of clinical worsening and improved functional outcomes.

7.Molecular mechanism of Shenling Baizhu powder in treatment of cancer cachexia based on network pharmacology

Objective To analyze the pharmacological mechanism of Shenling Baizhu powder in the treatment of cancer cachexia based on the network pharmacological method and provide a reference for the clinical application of classical traditional Chinese medicine(TCM) prescriptions. Methods Through TCMSP and BATMAN-TCM databases, the main chemical components and their targets of the TCM prescription of Shenling Baizhu powder were obtained, and the active components of the TCM were screened according to ADME. The main targets of cancer cachexia were obtained through OMIM, Genecards, Disgenet and DRUGBANK databases, and protein interaction analysis was conducted using String platform to build a PPI network. The “drug-active ingredient-target” network of Shenling Baizhu powder was constructed by Cytoscape 3.7.2 software, and then the biological processes and pathways involved were analyzed by using Metascape platform. Finally, molecular docking verification was conducted by Discovery Studio. Results The core active ingredients of Shenling Baizhu powder in the treatment of cancer cachexia were quercetin, kaempferol, pyrolignous acid, stigmasterol, luteolin, β-sitosterol, etc. The core targets were AKT1, TP53, TNF, IL-6, MAPK3, CASP3, JUN, CTNNB1, HIF1A, EGFR, etc. The molecular docking test also showed that the top 10 active ingredients, such as pyrolignous acid, stigmasterol and β-sitosterol, had good binding activities with most of the target sites. The biological pathway of Shenling Baizhu powder in treating cancer cachexia wss mainly to regulate tumor related pathway, metabolism related pathway, inflammatory factors and appetite related pathway. Conclusion This study preliminarily revealed the mechanism of action of Shenling Baizhu powder in treating cancer cachexia with multi components, multi targets and multi pathways, which provided a basis for the clinical development and utilization of Shenling Baizhu powder.
8.Research on the value of narrative medicine in bridging differences in shared decision-making

Shared Decision-Making (SDM) is a crucial concept in modern medicine, emphasizing the joint participation of doctors and patients in the medical decision-making process. However, the authoritative position of doctors and the passive role of patients in traditional medical models often overlook the personal wishes and needs of patients, leading to tense doctor-patient relationships and medical disputes. By listening to and understanding their stories, narrative medicine helps doctors gain a more comprehensive understanding of patients’ situations and balances medical advice with patient needs in the decision-making process. Through systematic literature analysis and theoretical exploration, this paper investigated the application effects and mechanisms of narrative medicine in different medical contexts, as well as analyzed its specific role in the process of SDM. The aim is to explore the value of narrative medicine in bridging differences in SDM, revealing its role in promoting doctor-patient communication, enhancing decision-making participation, and improving medical outcomes. Researches had found that narrative medicine enhanced doctors’ “narrative ability”, promoted emotional communication and trust between doctors and patients, reduced conflicts and misunderstandings in decision-making, and improved patients’ sense of participation and trust, thus playing an important role in SDM. Therefore, by enhancing doctor-patient communication and understanding, promoting SDM and treatment selection between doctors and patients, personalized care and treatment optimization, advocating for doctor-patient co-construction, improving consultation efficiency, restoring the patient’s subject position, and other methods, it can bridge doctor-patient differences, promote communication and enhance decision-making participation, and improve medical outcomes and patient satisfaction.
9.Research on the reconstruction of doctor-patient relationships in patients with sudden deafness from the perspective of narrative medicine

Patients with sudden deafness encounter greater psychological challenges and communication barriers after experiencing sudden hearing loss, and traditional medical models often fail to adequately address their unique needs. This paper analyzed the current situation of emotional and behavioral changes in patients with sudden deafness, and the gap between their expectations and the reality of medical care. From the perspective of narrative medicine, the theory and characteristics of the reconstruction of the doctor-patient relationships in patients with sudden deafness were explored. The results showed that narrative medicine can enhance patients’ emotional resonance and understanding, improve the efficiency and quality of doctor-patient communication, promote the formulation of personalized treatment plans, and enhance treatment adherence and satisfaction. Based on these results, strategies and pathways for the reconstruction of doctor-patient relationships for patients with sudden deafness were proposed, including building empathetic bridges and tapping into mechanisms of emotional resonance within narrative medicine; optimizing communication strategies and promoting the application of narrative techniques in doctor-patient dialogues; connecting narrative pathways and advocating the exploration of stories and strategies in personalized treatments; as well as facilitating treatment adherence and making full use of the psychodynamic effects of narrative medicine. Narrative medicine, as a patient-centered medical practice, can effectively promote the reconstruction of doctor-patient relationships, enhance treatment effectiveness, and offer a more humane treatment experience for patients.
10.Impact of childhood trauma on internet addiction in medical students: the mediating role of perceived stress

BackgroundWith the rapid development of the networking technologies, internet addiction has increasingly become a serious mental health issue. Previous studies have revealed the link between childhood trauma and internet addiction, while the mediating role of perceived stress in this link is not yet clear. ObjectiveTo investigate the role of medical students' perceived stress in the relationship between childhood trauma and internet addiction, so as to provide references for the intervention of internet addiction. MethodsFrom February to March 2023, a random sampling technique was used to select 1 232 undergraduate students from the School of Clinical Medical Sciences of Southwest Medical University as research subjects. The Childhood Trauma Questionnaire-Short Form (CTQ-SF), Perceived Stress Scale (PSS), Internet Gaming Disorder Scale (IGDS), and Bergen Social Media Addiction Scale (BSMAS) were used for assessment. Pearson's correlation coefficients were calculated. The mediation effect of perceived stress in the relationship between childhood trauma and internet addiction was tested using Model 4 in the SPSS Process 4.1, and Bootstrapping procedure involving 5 000 replicates was employed to confirm the statistical significance. ResultsA total of 1 016 (82.47%) valid completed questionnaires were gathered. The CTQ-SF scores of medical students were positively correlated with PSS scores, IGD scores, and BSMAS scores (r=0.583, 0.474, 0.465, P<0.01). PSS scores were positively correlated with IGD scores and BSMAS scores (r=0.369, 0.479, P<0.01). Childhood trauma in medical students was found to positively predict perceived stress (β=0.191, P<0.01), social media addiction (β=0.160, P<0.01), and internet gaming disorder (β=0.106, P<0.01). Perceived stress played a significant mediating role in the relationship between childhood trauma and internet gaming disorder, indirect effect value was 0.018 (95% CI: 0.009~0.027), accounting for 16.98%. Perceived stress also exhibited a significant mediating role in the relationship between childhood trauma and social media addiction, indirect effect value was 0.063 (95% CI: 0.048~0.079), accounting for 39.38%. ConclusionChildhood trauma in medical students may affect internet gaming disorder and social media addiction through perceived stress.
