Cardiac remodeling in microgravity exerts effects on astronauts'health and space mission efficiency. This paper analyzes the etiology, pathogenesis, prevention, and treatment of cardiac remodeling in microgravity based on the Trinity View of"Physique-Qi-Spirit"of traditional Chinese medicine. The microgravity environment disrupts the heaven-earth qi interaction law described in"Suwen Yin Yang Ying Xiang Da Lun":"Earth qi ascends to form clouds through heavenly evaporation, while heaven qi descends as rain".This disruption causes upward reversal of human qi and cephalad fluid shift, consequently influencing the dynamic balance of the physique-qi-spirit system and inducing cardiac remodeling. The core pathogenesis initiates with qi upward reversal at the initial stage, advances through spiritual disturbance during progression, and culminates in physical impairment at the terminal stage. We therefore propose a holistic"physique-qi-spirit"co-regulation strategy implemented via three therapeutic principles:regulating qi,stabilizing spirit, and restoring physique. This approach aims to restore the dynamic balance of the"physique-qi-spirit"system in microgravity, apply the concept of"preventive treatment of diseases",holistically prevent or delay cardiac remodeling, and provide Traditional Chinese Medicine solutions for safeguarding astronauts'cardiovascular health.

The"ying-wei imbalance"theory originates from Inner Canon of Yellow Emperor, which refers to the dynamic pathological process of yingfen and weifen circulation, distribution, posture, strength, and weakness due to internal and external disturbance, emphasizing that"ying-wei imbalance"is the key to disease occurrence. The sanjiao,as the"envoy of primordial qi,"is central to yingfen and weifen, and the coordinated operation of yingfen and weifen depends on the promotion of the qi of sanjiao. Therefore, based on the yingfen and weifen theory and sanjiao differentiation and treatment, this paper discusses the etiology and pathogenesis of chronic pulmonary heart disease,as well as the syndrome differentiation approach."Ying-wei imbalance"is the core of this mechanism:in the early stage, the loss of yingfen and weifen nutrition, combined with the deficiency of the qi of sanjiao, allows exogenous pathogens to invade the lungs.During progression, dysfunction in the transportation function of yingfen and weifen, along with the stagnation of the qi of sanjiao, results in pulmonary and cardiac involvement, accompanied by phlegm accumulation and stagnation. The end stage is characterized by the failure of both ying and wei, along with dysfunction of the sanjiao and zang-fu organs, which interact to form a pathological chain of"qi disease involving body."Accordingly, the treatment principle of"harmonizing yingfen and weifen, giving sanjiao treatment"was proposed. This is implemented through three therapeutic strategies:"harmonizing""returning",and"astringing"aimed at restoring balance. In the early stage, treatment focuses on tonifying the lungs and consolidating the exterior to eliminate pathogenic factors and activate stagnation. The selected formula is Buyuan Decoction with modifications to harmonize sanjiao.During the developmental stage, treatment focuses on promoting collaterals and dispersing phlegm, utilizing the modified Xuefu Zhuyu Decocotion to facilitate the restoration of function to the sanjiao. In the final stage,Fuyang Decoction, with modifications,is used to resolve fluid retention and nourish both qi and blood.Discussions are closely focused on the syndrome differentiation system of"sanjiao-yingfen and weifen,"in order to offer a novel perspective on the management of chronic pulmonary heart disease.

Objective To explore the potential pathogenesis of SLE and SS based on GEO database with screening differential expression genes common in systemic lupus erythematosus (SLE) and Sjogren's syndrome (SS),analyzing their functions and identifing their expression levels. Methods The gene expression datasets of SLE and SS whole blood samples were retrieved from GEO database. Differential expression genes in peripheral blood cells of SLE and SS were screened using gene expression datasets GSE50772,GSE81622,GSE84844 and GSE48378,respectively,and the shared differential expression genes of SLE and SS were screened. Functional analysis of differentially expressed genes was performed using Gene Ontology (GO) analysis and the Kyoto Encyclopedia of Genes and Genomes (KEGG). Peripheral blood from SLE and SS patients and healthy controls were collected from March 2024 to April 2024,recruited from the Second Affiliated Hospital of Air Force Medical University. Quantitative fluorescence real-time PCR (qRT-PCR) was used to identify the expression levels of 11 genes with the most significant differences in expression. Results 232 and 110 differentially expressed genes were screened for SLE and SS,respectively, among which 32 genes shared by SLE and SS were up-regulated in expression levels. Functional analysis showed that the 32 differentially expressed genes were mainly enriched in biological processes related to interferon (IFN) signaling pathways, defense response to viruses, response to viruses, negative regulation of viral genome replication, and immune response. KEGG pathway analysis showed that 32 differentialy expressed genes were associated with the process of viral infection. The clinical sample identification results showed that the expression levels of OAS3,IFI44,IFI44L and EPSTI1 were significantly elevated in PBMC of SLE and SS patients. Conclusion This study suggested that changes in biological processes related to IFN signal and viral infection response play important roles in both SLE and SS development, and may be a predisposing factor and potential biomarker for SLE and SS.

Hip surgery is often associated with moderate to severe postoperative pain due to exten-sive invasion.A safe and effective postoperative pain treatment can promote patients'recovering activities and facilitate the early functional recovery of the hip after the surgery.Although the routinely used intrave-nous self-controlled analgesia has an exquisite analgesic effect,it carries the risks of postoperative nausea and emesis, excessive sedation, intestinal paralysis, and even respiratory depression. As an alternative to transversus abdominis plane block(TAPB),lumbar quadratus block(QLB)has been gradually applied to the hip joint surgery. In this paper,we review the anatomical basis and characteristics of QLB and its clini-cal application in hip surgery,in order to provide the postoperative analgesia application of QLB.

Chimeric antigen receptor (CAR)-T cell therapy has achieved remarkable success in the treatment of hematological malignancies. Based on the immunomodulatory capability of CAR-T cells, efforts have turned toward exploring their potential in treating autoimmune diseases. Bibliometric analysis of 210 records from 128 academic journals published by 372 institutions in 40 countries/regions indicates a growing number of publications on CAR-T therapy for autoimmune diseases, covering a range of subtypes such as systemic lupus erythematosus, multiple sclerosis, among others. CAR-T therapy holds promise in mitigating several shortcomings, including the indiscriminate suppression of the immune system by traditional immunosuppressants, and non-sustaining therapeutic levels of monoclonal antibodies due to inherent pharmacokinetic constraints. By persisting and proliferating in vivo, CAR-T cells can offer a tailored and precise therapeutics. This paper reviewed preclinical experiments and clinical trials involving CAR-T and CAR-related therapies in various autoimmune diseases, incorporating innovations well-studied in the field of hematological tumors, aiming to explore a safe and effective therapeutic option for relapsed/refractory autoimmune diseases.
