1.Effect of symptoms of attention deficit hyperactivity disorder in children on parental emotions

Objective:To investigate children's attention deficit hyperactivity disorder symptoms and their im-pact on parental anxiety and depression,as well as the mediating role of parent-child relationship.Methods:A total of 472 children with ADHD who met the diagnostic criteria of the Diagnostic and Statistical Manual of Mental Dis-orders,Fourth Edition(DSM-Ⅳ)were selected.The severity of ADHD symptoms was assessed using the Attention Deficit Hyperactivity Disorder Rating Scale(ADHD-RS),parent-child relationships were assessed using the Parent-Child Relationship Test(PCRT),and parental anxiety and depression were assessed using the Generalized Anxiety Disorder 7-item Scale(GAD-7)and the Patient Health Questionnaire-9(PHQ-9),respectively.Mediation analysis was conducted following structural equation modeling construction.Results:There was a positive correlation be-tween ADHD-RS scores and PCRT scores,parental GAD-7 scores,and PHQ-9 scores(r=0.19-0.29,Ps<0.001).PCRT scores significantly mediated the relationship between ADHD-RS total scores and parental GAD-7 and PHQ-9 scores(β=0.02-0.03,Ps<0.05).PCRT scores did not significantly mediate the relationship between ADHD-RS inattention scores and parental GAD-7 and PHQ-9 scores(β=0.01-0.02,Ps>0.05).PCRT scores significantly mediated the relationship between ADHD-RS hyperactivity/impulsivity scores and parental GAD-7 and PHQ-9 scores(β=0.02,Ps<0.05).Conclusion:The symptoms of ADHD in children are associated with parental anxiety and depression symptoms,with parent-child relationships mediating this relationship.
2.Research on the construction and application of an intelligent internet of things-enabled dental chair platform based on dental chair domain interconnection

To address the problem of data silos in dental specialties caused by equipment heterogeneity, this study developed an Intelligent Internet of Things (IoT)-enabled dental chair platform (hereinafter referred to as the intelligent platform) based on the concept of medical-engineering integration. The platform adopts a three-tier chair-domain interconnection architecture: the bottom tier integrates multi-source sensors and standardized interfaces for automated data acquisition and linkage with hospital information systems; the middle tier provides clinic-level management and remote teaching collaboration; and the top tier employs a blockchain-based secure cloud database for resource allocation and data management. Clinical validation at The Affiliated Stomatological Hospital of Nanjing Medical University demonstrated that, compared with a control group from the same period in 2023, the trial group achieved a 38.0% increase in average daily patient visits (80.6±6.8 vs. 58.4±5.2, t=15.16, P<0.001), a 24.6% reduction in average treatment time [(36.1±6.3) min vs. (47.9±8.5) min, t=7.72, P<0.001], a 39.2% reduction in waiting time [23.3 (16.5, 30.1) min vs. 38.3 (28.3, 48.3) min, U=32.00, P<0.001], a 30.4% reduction in equipment idle rate [8.7% (5.1%, 12.3%) vs. 12.5% (7.4%, 17.6%), U=251.00, P=0.003], and an increase in patient satisfaction from 88.2% (1 519/1 723) to 94.3% (2 186/2 318) ( t=7.26, P<0.001). User research confirmed that the functions most favored by clinicians and patients were "dental chair parameter updating and clinical data integration" [74.7% (80/107)] and "chairside display of diagnostic images" [76.8% (119/155)], respectively. Looking forward, the intelligent platform has the potential to integrate artificial intelligence-assisted diagnosis and 5G-enabled multicenter collaboration to further expand its clinical applications and accelerate the digital transformation of dental healthcare.
3.The effects of subchronic aluminum exposure on nerve growth factor in hippocampus of offspring rats

Objective:To study the effects of aluminum exposure on learning and memory and the expression of nerve growth factor(NGF)in the hippocampus of offspring rats,and to investigate the mechanism by which aluminum impairs learning and memory function.Methods:Forty pregnant Wistar rats were randomly assigned to the Control group(Control),low-dose Al group(Al-L),medium-dose Al group(Al-M)and high-dose Al group(Al-H).The off-spring rats were fed with Al through breast milk from birth to weaning,while the rats in the control group were fed with distilled water.The maternal rats Al-L,Al-M and Al-H groups drank distilled water solution containing 2.0,4.0 and 8.0 g/L AlCl3,respectively.After weaning,the offspring rats in the aluminum exposure group drank distilled water so-lution containing 2.0,4.0 and 8.0 g/L AlCl3 by themselves until the 90th day after birth to establish the offspring rat model of subchronic aluminum exposure.After aluminum exposure,the shuttle box test was used to detect the learning and memory ability of offspring rats,and the body weight of offspring rats and hippocampus were weighed to evaluate the effect of aluminum exposure.The expression of nerve growth factor(NGF)protein in hippocampus of offspring rats was detected by Western blot,and the expression of NGF mRNA in hippocampus of offspring rats was detected by real time RT-PCR.Results:The body weight of offspring rats in Al-H group was significantly lower than that in the other three dose groups.In the shuttle box test,compared with the control group,the active avoidance response and passive avoid-ance response of the offspring rats in the aluminum exposure group showed a downward trend with the increase of alumi-num exposure dose,indicating that the learning and memory ability of the offspring rats in the aluminum exposure group was impaired.Compared with the control group,the NGF protein content and NGF mRNA expression in the hippocam-pus of offspring rats in the aluminum exposure group were significantly decreased.Conclusion:Subchronic aluminum exposure down-regulates the expression of NGF in the hippocampus,which may cause learning and memory impairment in offspring rats.

7.Effect of Mahuang Xixin Fuzitang on Migration of Dendritic Cells in Mice by Regulating Rho/ROCK Signaling Pathway

ObjectiveTo investigate the inhibitory effect of Mahuang Xixin Fuzitang on the migration of dendritic cells (DCs) in mice and its underlying mechanism. MethodMouse bone marrow-derived DCs were isolated and cultured. The morphological changes of the cells at different stages were observed under a microscope, and the CD11c+ proportion was detected by flow cytometry to identify DC purity. Cells were treated with Mahuang Xixin Fuzitang (1, 2, 5, 10, 20, 40, 50, 100 g·L-1) for 24 hours, and the effect of Mahuang Xixin Fuzitang on cell proliferation was assessed using the cell counting kit-8 (CCK-8) assay to determine the appropriate concentrations for treatment. After modeling by lipopolysaccharide (LPS) induction, DCs were divided into a blank group, a model group, and Mahuang Xixin Fuzitang groups (2, 4, 8 g·L-1). The expression of surface molecules CD80, CD86, and major histocompatibility complex-Ⅱ (MHC-Ⅱ) were detected by flow cytometry. Transwell chamber assay was used to observe cell migration. The levels of chemokine C-C-primitive receptor 7 (CCR7) and chemokine C-X-C-primitive receptor 4 (CXCR4) on the cell surface were detected by enzyme-linked immunosorbent assay (ELISA). The expression of filamentous actin (F-actin) in the cell microfilament cytoskeleton was detected by immunofluorescence (IF) staining. Real-time quantitative polymerase chain reaction (Real-time PCR) was employed to determine the mRNA expression levels of Ras homolog family member A (RhoA) and Rho-associated coiled-coil containing protein kinase 1 (ROCK1). Western blot analysis was performed to detect the protein expression of RhoA and ROCK1. ResultCompared with the blank group, the model group exhibited significantly higher expression levels of CD80, CD86, and MHC-Ⅱ (P<0.01), a significantly increased number of cells migrating to the lower chamber (P<0.01), and significantly elevated levels of CCR7 and CXCR4 (P<0.05, P<0.01). Additionally, F-actin expression was significantly increased (P<0.01), and both RhoA and ROCK1 mRNA and protein expression levels were significantly higher (P<0.05, P<0.01). Compared with the model group, treatment with Mahuang Xixin Fuzitang (2, 4, 8 g·L-1) for 24 hours resulted in significantly lower expression levels of CD80, CD86, and MHC-Ⅱ (P<0.01), a significantly reduced number of cells migrating to the lower chamber (P<0.05), and significantly decreased levels of CCR7 and CXCR4 (P<0.05, P<0.01). Furthermore, F-actin expression was significantly reduced (P<0.01), and both RhoA and ROCK1 mRNA and protein expression levels were significantly decreased (P<0.05, P<0.01). ConclusionMahuang Xixin Fuzitang can inhibit the migration of DCs in mice, and its mechanism of action may be related to reducing the activity of the Rho/ROCK signaling pathway, thereby affecting changes in the cell cytoskeleton.
8.Analysis of Pathogenesis and Treatment Principles of Adolescent Idiopathic Scoliosis Based on Theory of"Five Viscera Devia-tion"

[Objective]To analyze the pathogenesis and treatment of adolescent idiopathic scoliosis(AIS)in traditional Chinese medicine based on the theory of"five viscera deviation",and develop the idea of its diagnosis and treatment in traditional Chinese medicine.[Methods]By reviewing the ancient Chinese medicine literature related to AIS,and based on the theory of"five viscera deviation"in Huangdi Neijing,this research summarized the etiology,pathogenesis and therapeutic rules of Chinese medicine in AIS through the literature study,and explored the guiding role of Chinese medicine theory of"five viscera deviation"for the treatment and regimen of AIS.[Results]Literature research has found that the performance of the"five viscera deviation"in Huangdi Neijing is highly consistent with the lesion site and symptom characteristics of AIS.Based on the academic connotation of the theory of"five viscera deviation"in Chinese medicine,the etiology of AIS is based on endowment insufficiency,with dietary disorders and injuries caused by fatigue as the triggers,and the mechanism of the disease is based on the disorders of the spleen and kidney and the five viscera deviation,with the tendons out of the grooves and the bones out of joints as the symptoms,and the treatment should be based on the principle of adjusting the spleen and kidney,rectifying the five viscera,and emphasizing on both the tendons and the bones and the combination of internal and external treatments.The treatment should be a combination of internal medicine,external treatment,guiding and dietary therapy.[Conclusion]AIS is highly related to"five viscera deviation".Based on the theory of"five viscera deviation",it discusses the disease mechanism and treatment rules of AIS,which can provide Chinese medicine ideas for the clinical diagnosis and treatment of AIS,and worthy of attention,further exploration and clinical application.
9.Endoscopic dilation guided by two guidewires: a novel method in establishing channels in percutaneous nephrolithotomy

【Objective】 To explore the safety and efficacy of a novel endoscopic two-wire guided dilation in the creation of channels in percutaneous nephrolithotomy (PCNL). 【Methods】 Clinical records of 180 patients undergoing PCNL during Oct.2020 and Oct.2022 were retrospectively analyzed. The patients were divided into three groups, 60 in AMD group (fascial amplatz dilation), 60 in OSD group (one shot dilation) and 60 in END group (endoscopic dilation). Time to establish channels, operating time, failure of access, stone clearance rate, drop in hemoglobin, embolization rate, fever rate, blood transfusion rate and postoperative hospitalization were compared among the three groups. 【Results】 There were no significant differences in the general data among the three groups (P>0.05). Compared with AMD and OSD groups, END group needed significantly reduced time to establish the first channel [(5.6±0.8) min vs. (4.9±1.4) min vs. (4.2±0.5) min, (P<0.05)] . Compared with OSD group, END and AMD groups had significantly more hemoglobin drop [(14.0±17.6) g/L vs. (19.4±12.6) g/L vs. (10.2±6.8) g/L, (P<0.05)] . There were no significant differences in terms of failure of establishing channels, operating time, stone clearance rate, embolization rate, fever rate, blood transfusion rate and postoperative hospitality. Four patients needed selective renal artery embolization (1 case in AMD group and 3 in OSD group). No serious complications such as organ injuries, septic shock or death occurred. 【Conclusion】 Endoscopic two-wire guided dilation is simple, with few complications and good application value.
10.Application and Considerations on"Internet+"Healthcare in Outpatient Special Disease Management

Purpose/Significance According to the experience of Sichuan Provincial People's Hospital in implementing the internet outpatient special disease services,the paper puts forward some thoughts and suggestions,so as to provide references for further promoting the application of"internet+"healthcare in outpatient special disease management.Method/Process Based on the operational data of the internet hospital,the paper analyzes the operation status and existing problems of internet outpatient special disease services from the aspects of patient visits,departments,and complaints feedback.Result/Conclusion At present,there are problems such as low utiliza-tion rate of internet outpatient special disease services,poor prescription circulation,and untimely drug delivery.It is necessary to opti-mize online service for key groups,establish the regional electronic prescription sharing platform,improve the drug distribution system,strengthen cooperation between medical institutions at different levels,and build a regional outpatient special disease management system.
