1.Research progress in perioperative immunotherapy for non-small cell lung cancer

It is very limited that the benefit of perioperative chemotherapy in early non-small cell lung cancer (NSCLC), and the 5-year survival rate is only 5% higher than surgery. Antibodies that block programmed cell death protein 1/programmed death-ligand 1 significantly improve the survival of advanced NSCLC. The value of immunotherapy in early NSCLC is also being explored. This paper firstly summarized and analyzed the progress of immunotherapy in the perioperative period of NSCLC. Secondly, the safety and feasibility of surgical resection after neoadjuvant immunotherapy were discussed. Finally, the clinical value of different therapeutic efficacy prediction indicators was summarized, in order to clarify the current status of immunotherapy in the perioperative period, so as to improve the clinical benefits of early NSCLC patients.
2.Comparison of Perioperative and Long-Term Outcomes Between Simple and Complex Segmentectomies for Treatment of ≤2 cm Solid Pulmonary Nodules

Objective To compare the prognostic differences between simple and complex segmentectomies. Methods We conducted a retrospective cohort analysis of patients with solid pulmonary nodules (≤2 cm) who underwent segmentectomy. Recurrence-free survival (RFS) and local recurrence rates were evaluated. Results We included57 patients undergoing complex segmentectomy and 53 patients undergoing simple segmentectomy. Among patients who did not receive adjuvant therapy, those in the complex group had a significantly lower five-year RFS than those in the simple group (69.86% vs. 85.97%, P=0.04). Furthermore, the local recurrence rate was significantly higher in the complex group (18.75% vs. 4.65%, P=0.003) than in the simple group. Conclusion For solid pulmonary nodules (≤2 cm), complex segmentectomy is associated with inferior local control and worse RFS than simple segmentectomy.
3.Bidirectional regulation of distal colon motility in rats with electroacupuncture of different intensities at "Tianshu" (ST25).

OBJECTIVE:
To observe the distribution characteristics of sensitization areas on the body surface in the rat models with functional constipation and diarrhea, explore the regulatory patterns of electroacupuncture (EA) of different intensities at "Tianshu" (ST25) on distal colon motility, and clarify the roles of the neurons of different subtypes in the enteric nervous system (ENS) displayed in the regulatory effect.
METHODS:
Of 90 SD male rats of SPF grade, 15 rats were randomized into a normal group, a constipation group and a diarrhea group, 5 rats in each one. The stool form and fecal water content, as well as the distribution of the Evans blue (EB) extravasation on the body surface after the intravenous injection with EB on the tails were observed. Eighteen rats were randomized into a normal +2 mA group, a normal +4 mA group and a normal + 6 mA group, 6 rats in each one. Using physiological signal acquisition system, the area under the curve and the average amplitude of colon peristalsis were recorded and analyzed, and the immediate effect on distal colon peristalsis observed after EA with different intensities at "Tianshu" (ST25). Thirty rats were randomized into a normal group, a constipation group, a diarrhea group, a constipation +2 mA group, and a diarrhea +6 mA group, 6 rats in each one, so as to observe the cumulative effect on colon motility disorder in the rat models of constipation and diarrhea after EA at "Tianshu" (ST25). Twelve rats were randomized into a constipation +2 mA group and a diarrhea +6 mA group, 6 rats in each one, to observe the immediate effect on colon motility disorder in the rat models of constipation and diarrhea after EA at "Tianshu" (ST25). Fifteen rats were randomly divided into a normal group, a constipation group, a diarrhea group, a constipation +2 mA group, and a diarrhea + 6 mA group, 3 rats in each one. Using the whole-mount staining technique, the expression of vesicular acetylcholine transporter (VAChT)-positive neurons and nitric oxide synthase (nNOS)-positive neurons in ENS was detected. According to the group divisions, the functional constipation models were established by intragastric administration of loperamide hydrochloride (10 mg/kg, once daily, for consecutive 7 days), and the functional diarrhea models were prepared by intragastric administration of folium sennae decoction (10 mL/kg, once daily, for consecutive 2 days). The interventions were delivered with EA of different intensities (the electric current of 2, 4 or 6 mA) at bilateral "Tianshu" (ST25), separately, with the continuous wave and the frequency of 10 Hz used.
RESULTS:
Compared with the normal group, the fecal amount was decreased, and the fecal water content was reduced in the rats of the constipation group (P<0.001); and loose stool was presented and the fecal water content increased in rats of the diarrhea group (P<0.001). EB extravasation on the body surface happened in the region from T6 to S2 of the rats in the constipation and diarrhea groups, and it was more concentrated in the lower abdominal and the lower back regions from T10 to L3. Compared with the indexes before EA, in the normal +2 mA group and the normal +4 mA group, the areas under the curve and the average amplitude of the distal colon peristalsis were higher during EA delivery (P<0.01, P<0.05), showing a stimulatory immediate effect; and the post-effect was obtained after EA at 2 mA. Whereas, these two indexes were declined during EA in the rats of the normal +6 mA group (P<0.001), showing an inhibitory immediate effect. After many interventions with EA, when compared with those before EA, the above two indexes rose in the constipation +2 mA group (P<0.05, P<0.01), and they were dropped in the diarrhea +6 mA group (P<0.01, P<0.05). The area under the curve of the colon peristalsis in the constipation +2 mA group was higher than that of the constipation group (P<0.001), and that in the diarrhea +6 mA group was lower compared with that in the diarrhea group (P<0.001). The stimulatory effect of EA on colon motility in the constipation +2 mA group was stronger than that of the normal + 2 mA group (P<0.05), and its inhibitory effect was not different statistically in comparison between the normal +6 mA group and the diarrhea +6 mA group (P>0.05). In ENS of the distal colon, after EA at 2 mA, the proportion of VAChT-positive neurons was higher than that of the activated nNOS-positive neurons (P<0.001); and after EA at 6 mA, the activated nNOS-positive neurons were dominant (P<0.001).
CONCLUSION
In the functional constipation and diarrhea rat models, the sensitization areas on the body surface are centralized in the lower abdominal and the lower back regions of T10 to L3. Electroacupuncture at "Tianshu" (ST25) has a bidirectional regulatory effect on distal colon motility, and this effect is coordinated with the intensity of electroacupuncture, and may be mediated by ENS neurons of different subtypes.

"When warm evil is received,it first attacks the lungs and then spread to the pericardium reversely"is the general rule of warm diseases.Doctors of different dynasties have different views on the phrase"reverse spread to the pericardium",especially the word"reverse".Professor Tong Xiaolin proposed that the heart governs the mind,the pericardium and the heart are connected in qi,and when the heart is affected by evil,the pericardium instead suffers from the evil.The"reverse spread to the pericardium"proposed by Ye Tianshi is actually the spread of warm evil to the brain.Taking meningococcal meningitis as the basic disease,it can be matched one by one with the typical stages of the transmission of Wei-Qi-Ying-Xue.Combined with the theory of Dingjiao,it is believed that the function of"the heart governing the mind"focuses more on the brain in the modern anatomical sense.Combining traditional Chinese medicine's ideas on diagnosis and treatment of warm diseases with modern medicine,revealing the essence of the disease,grasping the core of the pathogenesis,analyzing the word"reverse"from a new perspective,and exploring its true meaning,is of great significance for clarifying its connotation,exploring the development laws of warm diseases,and guiding the diagnosis and treatment of warm disea-ses.
5.Prediction of biochemical recurrence after radical prostatectomy based on MRI intratumoral and peritumoral radiomics combined with pathological parameters

Objective To investigate the efficacy of intratumoral and peritumoral radiomics features besed on biparametric MRI combined with pathological parameters in predicting biochemical recurrence(BCR)after radical prostatectomy(RP).Methods A total of 295 prostate cancer(PCa)patients who underwent RP were retrospectively selected and divided into training set(n=206)and test set(n=89)in a 7∶3 ratio.Subgroups were stratified based on prostate specific antigen(PSA)results during 24-month postopera-tive follow-up.Radiomics features were extracted and integrated from both intratumoral and peritumoral 3 mm dual-regions on T2WI and apparent diffusion coefficient(ADC)sequences.Feature selection was performed using independent sample t-tests,maximum relevance and minimum redundancy(mRMR)algorithm,and least absolute shrinkage and selection operator(LASSO)regression.Binary logis-tic regression analysis was used to screen the independent pathological risk factors.Radiomics models(intratumoral,peritumoral,and combined)and a pathological model were constructed based on the Light gradient boosting machine(LightGBM)algorithm.A nomo-gram model was developed by integrating the pathological model with the combined model.The model efficacy was evaluated via the receiver operating characteristic(ROC)curves,and the DeLong's test and net reclassification improvement(NRI)were used to com-pare the efficacy differences among the models.Results Finally,the optimal feature numbers for the intratumoral,peritumoral,and combined models were 5,6,and 6,respectively.In the test set,the combined model demonstrated significantly higher area under the curve(AUC)than the intratumoral model(0.905 vs 0.815,P<0.05).Ki-67,extracapsular extension,and positive surgical margin were identified as independent pathological predictors of BCR(P<0.05).The nomogram model demonstrated significantly higher AUC than the pathological model(0.931 vs 0.786,P<0.05),with no significant difference versus the combined model(0.931 vs 0.905,P>0.05).Conclusion The nomogram model integrating intratumoral,peritumoral radiomics features with pathological parameters can effectively predict the occurrence of BCR within 2 years after PCa surgery.

Porcine reproductive and respiratory syndrome virus(PRRSV)mainly causes sow abor-tion,stillbirth,mummified fetus and respiratory symptoms in piglets.Since first reported in China in 1996,the virus complexity has increased significantly in more than 20 years of genetic evolution,bringing huge economic losses to the pig industry.In recent years,with the emergence of various PRRSV recombinant virus strains,preventing and controlling this epidemic became increasingly difficult.The purpose of this article is to comprehensively review the genome structure and func-tion of PRRSV,RNA virus recombination mechanism,main types of recombination,and the epi-demic status and recombination for the dominant epidemic PRRSV strains,in order to provide clues for in-depth research on gene recombination of PRRSV,thus providing the theoretical sup-port for formulating scientific prevention and control strategies.
7.Application of a student-faculty collaborative dynamic question bank based on outcome based education in the teaching of maternal and infant nursing

Objective:To explore the application effect of a student-faculty collaborative dynamic question bank based on outcome based education (OBE) in the teaching of maternal and infant nursing.Methods:A total of 204 junior nursing undergraduates (Grade 2021 and Grade 2022) enrolled at Shantou University Medical College between September 2023 and January 2025 were selected by convenience sampling. Ninety-two students from Grade 2021 were assigned to the control group, and 112 students from Grade 2022 were assigned to the intervention group. The control group received the traditional teaching syllabus before the course, used the People's Medical Publishing House question bank for self-testing, and the original static self-constructed exam question bank in the Kaoyi system for assessment. The intervention group practiced and was assessed using a student-faculty collaborative dynamic question bank based on OBE. The intervention lasted for one semester. Course performance and satisfaction were compared between the two groups.Results:The number of questions in the bank increased from 347 to 2 058. After course completion, the intervention group had higher stage test scores, final exam scores, and overall course scores than the control group ( P<0.05). The intervention group also reported higher levels of overall course satisfaction, perceived personal ability improvement, and satisfaction with course content and instructors than the control group ( P<0.05) . Conclusions:The student-faculty collaborative dynamic question bank based on OBE can effectively improve students' academic performance and satisfaction, and is worthy of wider application.
8.Application of In-fusion Cloning Technology as a Teaching Example in the Molecular Biology Laboratory(A National First-class Undergraduate Course)

In-fusion cloning technology,as a revolutionary and efficient molecular biology tool,has been applied in multiple research fields such as basic biology,biotechnology,and biomedicine.In this article,we introduce a teaching reform project suitable for undergraduate students in the course of"Molecular Bi-ology Laboratory",which utilizes in-Fusion cloning technology to construct a prokaryotic expression vector for alkaline phosphatase mutant genes.Through specific teaching cases,we systematically explored the design and implementation of experimental projects,and focused on analyzing the key and difficult points of the teaching content.Our teaching practice has found that the implementation of this educational re-form project has achieved very good results in enhancing students' core biological literacy,bioinformatics skills,research thinking,and innovation abilities.At the same time,the application of this technology can significantly improve the quality of experimental teaching,providing new ideas and practical refer-ences for promoting the reform and innovation of National First-Class Courses.
9.Development and reliability and validity of the Questionnaire on Pain Nursing Competency evaluation of nursing students

Objective:To develop and validate the Pain nursing Competency Evaluation Questionnaire for Nursing Students to provide an effective tool for measuring the pain management competency of nursing students in China.Methods:The questionnaire was constructed through literature review, semi-structured interviews, focus group discussions, Delphi expert consultation, and a pre-survey. From September 2023 to January 2024, a convenience sampling method was used to select 250 nursing students from Hangzhou Normal University and Lishui University in Zhejiang Province for the survey. Reliability and validity of the developed questionnaire were tested. A random sample of 30 nursing students was selected for retesting after two weeks.Results:A total of 10 female experts were consulted through correspondence. The Pain Care Competency Evaluation Questionnaire for nursing students consists of 36 items. Through exploratory factor analysis, five common factors were extracted: pain health education, comprehensive pain assessment, pain screening and assessment, analgesic interventions, and analgesic side effects nursing, which together explained 61.695% of the variance. The content validity of the questionnaire was 0.96, and the item-level content validity index ranged from 0.900 to 1.000. The overall Cronbach′s α coefficient was 0.924, and the Cronbach′s α coefficients for the five dimensions ranged from 0.856 to 0.915. The test-retest reliability was 0.831. Conclusions:The Pain Care Competency Evaluation Questionnaire for nursing students developed in this study has good reliability and validity. It can be used as a tool to assess nursing students′ competency in pain care and provides a reference for the design and optimization of pain care courses and clinical practice programs for nursing students in undergraduate institutions.
10.Reflections on the ethical implications of genetic testing in assisted reproduction

The application of genetic testing technologies in assisted reproduction, such as preimplantation genetic testing (PGT) and carrier screening for monogenic diseases, has provided infertile couples with more reproductive options and played a crucial role in the prevention of genetic disorders, significantly improving reproductive health. However, the widespread use of these technologies has also raised various ethical challenges, including the uncertainty of mosaic embryo transfer and its implications for reproductive rights, the cost-effectiveness debate surrounding PGT for structural rearrangement for chromosomal inversion carriers, the predictive accuracy and ethical boundaries of polygenic embryo screening, and the ethical concerns related to extensive carrier screening, such as information overload, restricted informed choice, disputes over screening scope, and disparities in healthcare access. The reproductive medicine ethics committee plays a central role in addressing these challenges by overseeing ethical reviews of technological applications, ensuring patients' informed consent, balancing technological innovation with ethical responsibility, and promoting social equity. This article explores the ethical challenges brought by the application of technologies such as PGT and carrier screening in assisted reproduction, and proposes corresponding suggestions based on ethical principles framework, in order to promote the standardized application of genetic testing technology in reproductive medicine.
