1.Methods and application effects of simultaneous correction of sunken upper eyelid during double eyelid plasty

Objective:To investigate the method and clinical efficacy of simultaneous correction of sunken upper eyelid during double eyelid plasty.Methods:A retrospective analysis was conducted on patients with single eyelids and sunken upper eyelid treated at the Department of Plastic Surgery, the First People’s Hospital of Zhengzhou, from October 2022 to February 2024. All patients underwent incisional double eyelid plasty with individualized correction based on depression severity. For mild depression, a combination of pretarsal orbicularis oculi muscle flap folding and orbital septum fat flap transposition was performed for correction. For moderate depression, additional autologous stromal vascular fraction (SVF)-gel grafting would be performed if residual depression persisted after the aforementioned approach. For severe depression, if the orbital fat was sufficient, the orbicularis oculi muscle flap folding, orbital fat flap repositioning and autologous SVF-gel transplantation were used. If there was insufficient orbital fat, the correction involved the use of a orbicularis oculi muscle flap and SVF-gel transplantation. Postoperative follow-up was conducted regularly, and related complications were statistically analyzed. Six months after the surgery, two blinded plastic surgeons evaluated outcomes via visual analog scale (VAS), including five aspects: symmetry of palpebral folds(including width and curvature of the double eyelid fold), fold contour, depression improvement, scar quality, overall eyelid aesthetics. Each aspect was rated on a scale from 1 to 10, with higher scores indicating better outcomes. Patient satisfaction was scored separately (1-5 per item), with higher scores indicating greater satisfaction.Results:A total of 64 patients were enrolled, including 4 males and 60 females; the age ranged from 19 to 66 years[ (32.0±9.7) years]. The degree of sunken upper eyelid was mild in 33 cases, moderate in 19 cases and severe in 12 cases. All patients underwent double eyelid plasty and orbicularis oculi flap folding. Based on this, 45 cases (33 mild and 12 moderate cases) underwent orbital septum fat flap transposition, 11 severe cases underwent SVF-gel grafting, and 8 cases (7 moderate and 1 severe cases) required combined approaches to correct depression. During the procedure, 19 cases (38 sides) received (1.8±0.8) ml of SVF-gel injection. Among these cases, one case with severe sunken upper eyelid who did not receive orbital fat transfer was injected with high density fat (left side: 0.6 ml, right side: 0.8 ml) due to insufficient SVF-gel volume. Two cases with severe sunken upper eyelid who only had autologous SVF-gel transplantation did not fully correct the depression within 3 months post-surgery received a second SVF-gel injection[(1.0±0.1) ml]. Follow-up was conducted for 6 to 12 months after surgery. All patients experienced varying degrees of eyelid edema, which typically lasted 2 to 3 months. Ecchymosis (34 sides), conjunctival congestion (3 sides), and temporary ptosis (5 sides) all resolved within 2 weeks. Pigmentation (14 sides) and scar hyperplasia (3 sides) disappeared or stabilized within 6 months. No patients developed complications such as incision infection, hematoma, fat liquefaction, local skin unevenness, or induration. The surgeons’ VAS scores of the above five indicators were all > 8 points, and the satisfaction scores of patients for the five indicators were all > 4 points. In both scoring, the improvement of sunken upper eyelid scored the highest, which were (9.2 ± 0.9) points and (4.8 ± 0.6) points respectively.Conclusion:For patients with single eyelids and varying degrees of sunken upper eyelid, performing double eyelid plasty and orbicularis oculi muscle flap folding, individualized correction is achieved through autologous fat redistribution techniques, based on the severity of the sunken upper eyelid and the amount of orbital fat. This single procedure can restore upper eyelid volume and rejuvenate the appearance, resulting in a natural and aesthetically pleasing double eyelid with minimal complications and high patient satisfaction.

Thermal ablation (TA) is a widely applied minimally invasive treatment for benign thyroid nodules and low-risk papillary thyroid microcarcinoma. Compared to conventional surgery, TA offers advantages such as minimal trauma, rapid recovery, and no scarring. However, this procedure may lead to various complications, including intraoperative pain, nerve injury, hemorrhage, tracheal injury, skin burns, vasovagal reactions, nodule rupture, and thyroid dysfunction. Although TA demonstrates excellent safety and efficacy, further standardization of procedural protocols is necessary to minimize the incidence of complications.
3.Application of ultrasound, genetic testing, and clinical features in malignancy prediction of Bethesda III thyroid nodules: potential to avoid unnecessary surgery

Objective:To evaluate the predictive role of ultrasound, genetic testing, and clinical features in the malignancy risk of Bethesda Ⅲ thyroid nodules, and to explore strategies for optimizing treatment decisions.Methods:This retrospective study included 227 Bethesda Ⅲ thyroid nodules from patients who underwent surgical treatment at the Thyroid Surgery Department of Nanjing Drum Tower Hospital between Jan. 2020 and Dec. 2023. All patients underwent ultrasound evaluation and fine-needle aspiration. For nodules diagnosed as ultrasound, genetic testing, and clinical features were analyzed using univariate and multivariate regression to assess their association with malignancy.Results:Among the 227 nodules, 214 were malignant, resulting in a malignancy rate of 94.2%. The malignancy rate of thyroid nodules was 94.2%. In univariate analysis, age ( P=0.016), BRAF V600E gene mutation ( P<0.001), nodule size ( P=0.002), and TIRADS ( P<0.001) were significantly associated with malignancy in Bethesda Ⅲ thyroid nodules. Multivariate analysis confirmed that age ( OR=0.939, P=0.049) and BRAF V600E gene mutation ( OR=24.641, P<0.001) were significantly associated with thyroid nodule nature and served as independent predictive factors for malignancy. Conclusions:Genetic testing is an important method for predicting the malignancy of Bethesda Ⅲ thyroid nodules, and ultrasound also has high clinical value in assessing the malignancy risk of nodules. While some clinical features are highly correlated with nodule characteristics, they may not be practical in clinical application. For nodules classified as TIRADS 3 through ultrasound evaluation and negative for BRAF mutations, continued observation may be considered, whereas TIRADS 5 nodules or nodules with BRAF mutations should be prioritized for surgical treatment.

6.Association between depression and death in the maintenance hemodialysis population: a cohort study

Objective:To identify factors associated with depressive symptoms in maintenance hemodialysis patients and to examine the relationship between these symptoms and mortality.Methods:Between January and December 2019, patients who received maintenance hemodialysis in the Blood Purification Center of the Second Affiliated Hospital of Nanjing Medical University were enrolled in a prospective cohort study. Depressive symptoms were assessed using the internationally validated patient health questionnaire-8 (PHQ-8). Sleep quality and anxiety were measured with the Pittsburgh sleep quality index (PSQI) and the generalized anxiety disorder-7 (GAD-7) scale, respectively. Follow-up continued until December 31, 2022, with all-cause mortality as the primary outcome. Ordinal logistic regression was used to identify independent predictors of depression severity. Cox proportional hazards models evaluated the association between depressive symptoms and mortality.Results:A total of 532 maintenance hemodialysis (MHD) patients completed the study. Among them, 177 (33.3%) exhibited depressive symptoms. Compared with patients without depression, those with mild or moderate-to-severe depression were older [median age: 58 (50, 66) vs. 60 (55, 65) vs. 55 (46, 64)], more likely to smoke [35.9% (51/142) vs. 40.0% (14/35) vs.26.2% (93/355)], had poorer sleep quality [PSQI: 9 (6, 13) vs. 12 (9, 17) vs. 5 (3, 9)], and higher anxiety levels [GAD-7: 1 (0, 3) vs. 3 (1, 6) vs. 0 (0, 1)], the differences among the three groups were statistically significant (all P<0.05). Ordinal logistic regression identified smoking status, history of diabetes or cardiovascular disease, hemoglobin level, PSQI score, and GAD-7 score as independent predictors of depression severity ( OR=1.60, 1.80, 1.81, 0.98, 3.67, 8.67; all P<0.05). After a median follow-up of 40 (35, 44) months, 109 patients died, including 66 (60.6%) from cardio-cerebrovascular causes and 24 (22.0%) from infections. Kaplan-Meier analysis revealed significantly lower cumulative survival in the depression group compared to the non-depression group ( P<0.001). Cox regression analysis demonstrated that depressive symptoms remained independently associated with all-cause mortality after adjusting for confounders ( HR=1.06, 95% CI 1.00-1.13, P=0.048), with an even stronger association observed for patients with PHQ-8 scores≥2.9 ( HR=1.10, 95% CI 1.03-1.16, P=0.005). However, the associations between depression and cardio-cerebrovascular mortality ( P=0.111) or infection-related mortality ( P=0.509) were not statistically significant. Conclusions:Depressive symptoms are prevalent among maintenance hemodialysis patients and are independently associated with increased all-cause mortality. Smoking, comorbid diabetes or cardiovascular disease, low hemoglobin level, poor sleep quality, and anxiety are risk factors contributing to depression. Maintenance hemodialysis patients with PHQ-8 scores≥3 should be considered at heightened risk for mortality.
7.Anesthesia management for posterior correction of scoliosis in a child with absence of the right pulmonary artery:a case report

Objective To discuss the perioperative anesthesia management strategy for posterior correction of scoliosis in children with unilateral absence of pulmonary artery(UAPA).Methods Clinical data of one case of an adolescent male with absence of right pulmonary artery receiving posterior scoliosis correction surgery was retrospectively collect-ed;the preoperative evaluation,intraoperative anesthesia management and postoperative pain treatment were analyzed and summarized.Results UAPA is a rare congenital cardiovascular anomaly that can further increase the cardiovas-cular and respiratory risks of surgical treatment in scoliosis patients.As the patient was concurrent with hemoptysis,after multidisciplinary consultation and evaluation,an emergency plan for hemoptysis was developed,and invasive ar-terial pressure monitoring,sufficient analgesia,close respiratory monitoring,and lung protective ventilation strategy was performed intraoperatively.Ensuring respiratory function after surgery,multimodal analgesia was administered.Confirming satisfactory treatment results,the patient was discharged from the hospital.Conclusions For symptomatic UAPA patients,clinicians should focus on the circulation and respiratory system,maintain perioperative hemodynamic stability,prevent hypoxia and carbon dioxide retention so to reduce the risk of complications.

9.Association between depression and death in the maintenance hemodialysis population: a cohort study

10.Role of PPARδ agonist seladelpar in mice with diabetic kidney disease

AIM:This study investigates the protective effect of the selective peroxisome proliferator-activated receptor δ(PPARδ)agonist seladelpar on podocytes in a mouse model of diabetic kidney disease(DKD).METHODS:A DKD mouse model was established using streptozotocin and a high-fat diet.Mice were randomly assigned to three groups:control,DKD with vehicle treatment,and DKD with seladelpar treatment at dosages of 1,5 and 10 mg/kg(6 to 7 mice per group).After four weeks of treatment,mice were euthanized,and various parameters were measured,including urinary albumin,creatinine,blood urea nitrogen,serum triglycerides,and cholesterol using respective kits.Renal tissues were processed for histological examination through HE and PAS staining to assess glomerular pathology.The RNA from glomerular tissues was analyzed via RT-qPCR for podocyte marker(Nphs1,Nphs2,Wt1 and Synpo)expression,and the protein expression was evaluated using Western blot for PPARδ.Immunofluorescence staining of frozen renal cortical sec-tions was conducted to quantify WT1-positive cells and desmin protein levels.Transmission electron microscopy was used to observe glomerular ultrastructural changes.RESULTS:(1)PPARδ protein levels in the glomerular tissue of DKD mice were significantly lower than in controls.(2)Mice treated with 10 mg/kg of seladelpar showed reduced blood glu-cose,triglycerides,and cholesterol levels compared to the diabetic nephropathy group.(3)Both 5 mg/kg and 10 mg/kg se-ladelpar treatment groups exhibited decreased urine albumin/creatinine ratios,along with improvements in glomerular hy-pertrophy and mesangial matrix deposition.Increased mRNA expression levels of Nphs1,Nphs2,Wt1,and Synpo were also observed,alongside an increase in WT1-positive cells and a decrease in desmin expression.Additionally,there was a reduction in podocyte foot process fusion and glomerular basement membrane thickness.(4)RNA sequencing indicated that 5 mg/kg seladelpar treatment modulated PPAR signaling pathway and influenced various metabolic pathways,includ-ing bile acid metabolism,carbon metabolism,glutathione metabolism,glycerophospholipid metabolism,and fatty acid metabolism.CONCLUSION:Seladelpar,a PPARδ agonist,mitigates podocyte injury and reduces proteinuria in DKD mice.Notably,the protective effects of seladelpar are partially independent of its hypolipidemic properties.
