Objective To investigate the correlation between serum vascular endothelial growth factor(VEGF)level and the development and severity of diabetes mellitus cystoid macular edema(CME).Methods A total of 57 patients(57 eyes)with diabetic CME(the case group)and diabetes without eye complications(the control group)admitted to the Vitreoretinal Surgery Department of Tangshan Eye Hospital from June 2023 to June 2024 were prospectively selected, and all of them underwent systematic ophthalmic specialty examination and serum VEGF detection.Multivariate Logistic regression analysis was used to identify the risk factors of diabetes mellitus CME.The diagnostic value of VEGF in patients with diabetes CME was analyzed by receiver operating characteristic(ROC)curve.Spearman correlation was used to analyze the relationship between VEGF levels and the severity of CME in patients with diabetes, such as corrected visual, number of cystoid edema and non-round index of macular foveal avascular zone(FAZ).Results Compared with the control group, the course of diabetes in the case group was longer, the incidence of hypertension was higher, levels of glycosylated hemoglobin(HbA1c),serum creatinine and serum VEGF were higher, and the estimated glomerular filtration rate(eGFR)was lower(P<0.05).Multivariate Logistic regression results showed that long duration of diabetes, increased levels of HbA1c and VEGF were risk factors for CME in diabetes mellitus patients, while elevated eGFR was protective factor(P<0.05).ROC curve analysis showed that the area under the curve(AUC)of VEGF in diagnosing diabetes mellitus CME was 0.758(95%CI:0.669-0.834).The AUC of the combined application of diabetes duration,HbA1c,eGFR and VEGF in the diagnosis of diabetes mellitus CME was 0.916(95%CI:0.877-0.941),which was significantly higher than that of the application of VEGF alone(P<0.05).The VEGF level was negatively correlated with the corrected visual acuity[0.34(0.24,0.44)]in patients with diabetic CME,and positively correlated with the detection of cystoid macular edema[3(2,5)]and FAZ non-roundness index[1.16(1.08,1.20)](rs were 0.771,0.700,respectively,P<0.05).Conclusion Serum VEGF levels are closely related to the onset and severity of CME,and which can be used as a reliable reference index for the diagnosis of CME.

ObjectiveTo analyze the evolutionary characteristics and genetic variations of the HA (hemagglutinin) and NA (neuraminidase) genes of influenza A(H1N1) viruses in Shanghai during 2024, to investigate their transmission patterns, and to evaluate their potential impact on vaccine effectiveness. MethodsFrom January to October 2024, throat swab specimens were collected from influenza like illness (ILI) patients at 4 hospitals in Shanghai. Real-time fluorescence ploymerase chain reaction (RT-PCR) was used for virus detection and isolation of H1N1 influenza viruses. Forty influenza A(H1N1) virus strains were sequenced using Illumina NovaSeq 6000 platform, followed by phylogenetic analyses, genetic distance analysis, and amino acid variation analyses of HA and NA genes. ResultsPhylogenetic tree of the HA and NA genes revealed that the 40 influenza A(H1N1) virus strains circulating in Shanghai in 2024 exhibited no significant geographic clustering, with a broad origin of strains and complex transmission chains. Genetic distance analyses demonstrated that the average intra-group genetic distances of HA and NA genes among the Shanghai strains were 0.005 1±0.000 6 and 0.004 6±0.000 6, respectively, which were comparable to or higher than those observed in global surveillance strains. Both HA and NA genes displayed frequent mutations. Compared to the 2023‒2024 and 2024‒2025 Northern Hemisphere A(H1N1) vaccine strains (WHO-recommended), the HA proteins of 40 Shanghai strains exhibited amino acid substitutions at positions 120, 137, 142, 169, 216, 223, 260, 277, 356 and 451, with critical mutations at positions 137 and 142 located within the Ca2 antigenic determinant. Furthermore, mutations in the NA protein were observed at positions 13, 50, 200, 257, 264, 339 and 382. ConclusionThe genetic background of the 2024 Shanghai influenza A(H1N1) virus strains is complex and diverse, and antigenic variation may affect vaccine effectiveness. Therefore, it is recommended to enhance genomic surveillance of influenza viruses, evaluate vaccine suitability, and implement more targeted prevention and control strategies against imported influenza viruses.

Objective To investigate the application value of multi slice spiral computed tomography(MSCT)in the diagnosis of bilateral Wilms tumor(BWT)in children. Methods The clinical and CT data of 7 children with BWT confirmed by clinical, imaging and pathology were analyzed retrospectively, and all cases underwent CT plain scanning and enhancement CT.Results Two cases were complicated with hypospadias,1 case with cryptorchidism, and 1 case with WAGR syndrome. There were 19 lesions in 14 kid-neys in 7 cases. The lesions were single in 10 kidneys and multiple in 4 kidneys on CT.CT flat scan showed that there were 3 solid lesions,16 cystic lesions, and there were different degrees of necrotic cystic changes inside,2 combined with bleeding,10 calcification,15 clear boundaries, and 4 exudation around.CT enhancement showed that the tumor body was unevenly strengthened, the necrotic cystic area was not strengthened, and the residual renal parenchyma was significantly strengthened. The typical signs were crescent signs and cuddle-ball signs, with 1 case of left renal venous thrombosis and 1 case of the invasion of renal pelvis. Conclusion MSCT can be used in the preoperative diagnosis of BWT in children,to evaluate the effect of chemotherapy and the postoperative evaluation, and to follow-up with and without recurrence and metastasis,so as to provide an important value for clinical diagnosis and treatment.

Aerosol microorganisms are important constituents of aerosols. They participate in physical and chemical reactions in the air and are also closely related to disease transmission and human health. With the SARS-CoV-2 pandemic, aerosol microorganisms have become a public health topic of great concern. Studying the composition and influencing factors of the air microbiome therefore has significant public health implications. Due to the limitations of traditional technologies for sampling and determination, the aerosol microbiome has not been fully understood. However, with the development and maturity of high-throughput sequencing technology, the aerosol microbiome has shown promising research prospects. This article reviews the composition, characteristics, detection methods and influencing factors of aerosol microorganisms, providing basic knowledge for further research on the air microbiome. In-depth research on microbial aerosols has significant implications on urban air quality control, national security and public health.
