1.Research on the mechanism of Chaiqi yigan granules against liver cancer via the ferroptosis pathway

OBJECTIVE To explore the mechanism of Chaiqi yigan granules (CQYG) against liver cancer through the ferroptosis pathway. METHODS Network pharmacology combined with ferroptosis-related database was used to screen key targets and main effective components of CQYG against liver cancer via regulating ferroptosis; molecular docking technology was employed to analyze the binding ability of main active components to key targets. Human liver Huh-7 cells were divided into blank serum control (CON) group, CQYG drug-containing serum (CQYGKL) group, ferroptosis inducer (RSL3) group, mammalian target of rapamycin complex 1 (mTORC1) inhibitor (RMC-5552) group, mTORC1 agonist (CCT007093) group, and CCT007093+CQYGKL group. The levels of Fe 2+ , malondialdehyde (MDA), and glutathione (GSH) in the cells were detected in the former three groups; mRNA expressions of mammalian target of rapamycin (mTOR), sterol regulatory element-binding protein 1 (SREBP1), and stearoyl-CoA desaturase 1 (SCD1), protein expressions of SREBP1 and SCD1 as well as phosphorylation levels of mTOR and ribosomal S6 kinase (S6K) proteins were detected in all groups. RESULTS Key targets of CQYG for anti-liver cancer through the ferroptosis pathway were mTOR, SREBP1, SCD1,etc. The main active components included quercetin, tanshinone Ⅱ A , baicalein, etc. The binding energies of main active components to key targets were all less than -5 kJ/mol. Compared with CON group, the levels of Fe 2+ and MDA in the cells in CQYGKL group and RSL3 group were significantly increased, while the levels of GSH were significantly decreased ( P <0.05). mRNA expressions of mTOR, SREBP1 and SCD1, protein expressions of SREBP1 and SCD1, as well as the phosphorylation levels of mTOR and S6K proteins were significantly decreased in the CQYGKL group, RSL3 group, and RMC-5552 group, whereas all the above indicators were significantly increased in the CCT007093 group ( P <0.05). Compared with CCT007093 group, the changes in all the above indicators were significantly suppressed in the CCT007093+CQYGKL group ( P <0.05). CONCLUSIONS CQYG may induce ferroptosis by inhibiting mTORC1/SREBP1/SCD1 axis, thereby exerting anti-liver cancer effects.

3.Association of urinary serine protease Corin with clinical staging in early diabetic kidney disease

Objective:To investigate the level of urinary serine protease(Corin) in early diabetic kidney disease(DKD) and its correlation with clinical stage.Methods:One hundred and seventy-three patients with type 2 diabetes mellitus(DM) from two tertiary A hospitals in Henan, diagnosed between April 2023 and December 2023 were selected as the research group, and 120 healthy subjects were selected as the control group. Basic clinical information and laboratory data were collected, and urinary Corin level was detected. DM patients were classified into G1-G5 stages based on estimated glomerular filtration rate(eGFR), and those in the early DKD stages(G1-G3) were further divided into A1-A3 subgroups based on urinary albumin/creatinine ratio(ACR). Spearman correlation analysis was performed to assess relationships between urinary Corin and other indicators, linear regression analysis identified factors influencing urinary Corin in early DKD patients, logistic regression analysis evaluated the risk factors for early DKD, and receiver operating characteristic(ROC) curve analysis determined the diagnostic value of urinary Corin in early DKD. Results:Urinary Corin levels were significantly higher in early DKD patients compared to healthy controls, with levels increasing as ACR rose( P<0.05). Urinary Corin was positively associated with serum creatinine( r=0.570), urea( r=0.458), cystatin C( r=0.693), ACR( r=0.616), urinary transferrin( r=0.448), urinary α1 microglobulin( r=0.507), urinary n-acetyl-β-D-glucosaminase( r=0.388) and A subgroup( r=0.692) while was negatively correlated with eGFR( r=-0.647), albumin( r=-0.312)(all P<0.05). eGFR was the only independent factor affecting urinary Corin. After adjusting for confounding factors in logistic regression analysis, urinary Corin was still an independent influencing factor for early DKD. ROC curve analysis indicated that urinary Corin had a diagnostic AUC of 0.842(95% CI 0.791-0.892, P<0.001), with a cut-off value of 2 226.04 pg/mL, sensitivity of 0.712, and specificity of 0.858 for early DKD diagnosis. Conclusions:Urinary Corin was elevated in early DKD patients and correlated with clinical stage. Urinary Corin is an independent factor of early DKD, and a reliable predictor of early DKD diagnosis.

Body weight abnormalities, including overweight, obesity, and underweight, have become a dual public health challenge in Chinese adults: overweight and obesity lead to a variety of chronic complications, while underweight increases the risks of malnutrition, sarcopenia, and organ dysfunction. To systematically address these issues, multidisciplinary experts in endocrinology, sports science, nutrition, and psychiatry from various regions have held multiple weight management seminars. Based on the latest epidemiological data and clinical evidence, they expanded the guideline to include assessment and intervention strategies for underweight, in addition to the core content of obesity management. This guideline outlines the etiological mechanisms, evaluation methods, and multidimensional management strategies for overweight and obesity, covering key areas such as diagnosis and assessment, medical nutrition therapy, exercise prescription, pharmacological intervention, and psychological support. It is intended to provide a scientific and standardized approach to weight management across the adult population, aiming to curb the rising prevalence of obesity, mitigate complications associated with abnormal body weight, and improve nutritional status and overall quality of life.
5.Influence of perioperative blood glucose level on postoperative acute kidney injury and rehabilitation in diabetic patients undergoing coronary artery bypass grafting

Objective To investigate the effects of perioperative blood glucose(BG)levels on postoperative acute kidney injury(AKI)and rehabilitation in diabetic patients with coronary heart disease after coronary artery bypass grafting(CABG).Methods Randomly select data from 142 patients who underwent CABG treatment at The Fuwai Cardiovascular Hospital in Qingdao from January to December 2022,and compare the BG indicators,AKI incidence,cardiac function improvement and quality of life between good control group(n=57)and poor control group(n=85).Results The operation time,ventilator use time,postoperative ICU time,hospital stay time and various BG indexes in the good control group were significantly lower than those in the poor control group(P<0.05 or P<0.01).27 patients(19.01%)developed AKI after surgery,and the incidence of AKI in the good control group was significantly lower than that in the poor control group(10.53%vs 24.71%,P<0.05).After treatment,the cardiac function indexes in both groups were significantly improved compared with those before surgery,and the good control had significantly better cardiac function and quality of life than the poor control(P<0.05).Conclusions The incidence of AKI after CABG in patients with diabetes and coronary heart disease was 19.01%.Good perioperative BG level control can reduce the risk of postoperative AKI and help early postoperative recovery.
6.Efficacy and safety of tegoprazan in the treatment of gastroesophageal reflux disease:a systematic review and Meta-analysis

Objective To systematically review the efficacy and safety of tegoprazan in the treatment of gastroesophageal reflux disease(GERD).Methods PubMed,Embase,Web of Science,Cochrane Library,SinoMed,CNKI,VIP,and WanFang Data databases were electronically searched to collect randomized controlled trials(RCTs)on tegoprazan for GERD treatment from inception to January 1,2025.Two researchers independently screened the literature,extracted data and assessed the risk of bias of the included studies.Meta-analysis was performed using RevMan 5.4 software and Stata 16 software.Results A total of 7 RCTs were included,involving 1,329 patients.The results of Meta-analysis showed that there was no statistically significant difference in the overall effective rate between the tegoprazan group and the control group(placebo or proton pump inhibitors)[RR=1.08,95%CI(0.99,1.17),P=0.07].There was also no statistically significant difference in the cure rate between the tegoprazan group and the control group(proton pump inhibitors)[RR=0.99,95%CI(0.96,1.02),P=0.53].Comparing the incidence of treatment-emergent adverse events(TEAE)and serious adverse events(SAE)during treatment between the tegoprazan group and the control group,no statistically significant differences were found[TEAE:RR=0.90,95%CI(0.62,1.32),P=0.60;SAE:RR=0.61,95%CI(0.26,1.48),P=0.28].In terms of specific adverse event,the incidence of abnormal liver function was significantly higher in the tegoprazan group compared to the control group[RR=7.60,95%CI(1.40,42.27),P=0.02],while the incidence of other adverse reactions showed no significant differences(P>0.05).Conclusion Tegoprazan has relatively good overall efficacy and safety in the treatment of GERD,and its efficacy is similar to that of proton pump inhibitors,which can be used as an alternative treatment for proton pump inhibitors.Due to the limited quality and quantity of the included studies,more high-quality RCTs are needed to verify the above conclusion.
7.Epidemiological characteristics and trend of hospitalization of patients with herpes zoster in Beijing, 2017-2022

Objective:To analyze the epidemiological characteristics and trend of hospitalization of the patients with herpes zoster in Beijing from 2017 to 2022.Methods:In this retrospective study, the information of hospitalization of herpes zoster patients were collected from all medical institutions at the first level and above in Xicheng, Changping, and Miyun districts of Beijing. The age and gender specific hospitalization rates and age-standardized hospitalization rates were calculated. Joinpoint regression model was used to explore the trend of the hospitalization rates, and the influencing factors of the hospital stay length and complications were analyzed.Results:The age-standardized hospitalization rate of the patients with herpes zoster was 10.82/100 000-18.43/100 000 in Beijing from 2017 to 2022 [annual percent change (APC) =5.86%, 95% CI: -2.80%-15.98%]. The age-standardized hospitalization rate of the cases with herpes zoster as the main diagnosis showed an upward trend (APC=11.35%, 95% CI: 7.21%-16.23%). The age-standardized hospitalization rate showed an upward trend in women (APC=14.34%, 95% CI: 7.95%-22.37%). The hospitalization rate showed a downward trend in age group 30-39 years (APC=-24.92%, 95% CI: -48.56% - -1.85%) and showed upward trends in age group 70-79 years and 80-109 years (APC=23.18%, 95% CI: 13.53%-35.58%; APC=4.90%, 95% CI: 1.18%-9.19%). Complications occurred in 66.28% (680/1 026) of the patients. The median hospital stay length was 9 (5,15) days, and the patients with high age (≥80 years) and two or more complications had longer hospital stay, which were 12 (6, 23) and 14 (7, 27) days respectively ( P<0.001). Conclusions:The hospitalization rate in women and the elderly aged ≥70 years with herpes zoster as the main diagnosis showed upward trends in Beijing in recent years. The elderly aged ≥80 years usually had longer hospital stay, showing a relatively disease burden level. More attention should be paid to development of intervention strategies, such as vaccine, for this population.
8.Construction of a health education checklist for safety management of oral antitumor drugs in tumor patients

Objective:To construct a health education checklist for safety management of oral antitumor drugs in tumor patients, so as to provide a guidance for clinical nurses in practice, thereby improving the drug safety of patients taking oral antitumor drugs.Methods:Based on the cognition-behavior theoretical framework, a preliminary draft of a health education checklist for safety management of oral antitumor drugs in tumor patients was developed through literature review. The Delphi method was used to conduct consultations with 17 experts from July to September 2024 to revise and add or delete the contents of the checklist based on the experts' assignments of importance to the indicators at each level and the textual comments made.Results:A total of two rounds of expert consultation were conducted. The final established health education checklist for safety management of oral antitumor drugs in tumor patients included 4 primary and 28 secondary indicators. In the two rounds of consultation, the positive coefficient of experts was 100.0%, the coefficient of expert authority was 0.93, the coefficients of variation for the two rounds of expert ratings were 0 to 0.173 and 0 to 0.151, and the coefficients of Kendall's concordance were 0.141 (χ 2=74.461, P<0.001) and 0.113 (χ 2=59.549, P=0.002) , respectively. Conclusions:The health education checklist for safety management of oral antitumor drugs in tumor patients has good scientific, reliable and clinical practical value, which can provide scientific, standardized and convenient practical guidance for clinical nurses to implement health education on drug safety management for patients taking oral antitumor drugs.
9.Impact of Qiangxin Tang on Myocardial Fibrosis in Rats with Chronic Heart Failure Mediated by BNIP3/NIX/FUNDC1 Pathway and Myo-cardial Mitochondrial Autophagy

Objective To explore the impact of Qiangxin Tang on myocardial fibrosis in rats with chronic heart failure by mediating myocardial mitochondrial autophagy through the Bcl-2/adenovirus E1B 19kDa interacting protein 3/NIP3 like protein X and FUN14domain-containing protein 1.Methods Forty Sprague Dawley rats were divided into five groups randomly by the random number table:sham operation group,model group,Qiangxin Tang group,Captopril group,and Qiangxin Tang+Captopril group,with a total of eight rats in each group.Only the sham operation group threaded without ligating the left coronary artery of the heart.In all the other groups,CHF rat models were established after myocardial infarction by ligating the left coronary artery of the heart.From the day after successful modeling,each group was given corresponding drugs by gavage,and the left ventricular myocardial tissue of each group was de-tected.Western blot was used to detect the expression of NIX,BNIP3,and FUNDC1 proteins in each group.ELISA was used to detect brain natriuretic peptide and cardiac troponin T level.Mitochondrial morphology was observed under transmission electron microscopy.Masson staining was used to observe left ventricular myocardial fibrosis.Results Compared with the sham surgery group,the expression of NIX,BNIP3 and FUNDC1 proteins in the model group was significantly reduced,while the levels of BNP and cTnT were significantly increased(P<0.05).The left ventricular myocardial tissue showed significant fibrosisand mitochondrial swelling.Compared with the model group,the expression of NIX,BNIP3,and FUNDC1 proteins was significantly increased in Qiangxin Tang group and Captopril group,while the levels of BNP and cTnT were significantly decreased(P<0.05).The left ventricular myocardial tissue fibrosis was sig-nificantly reduced,and the mitochondrial morphology was mildly altered.Compared with Qiangxin Tang group and Captopril group,the Qiangxin Tang+Captopril group showed significant improvement in the above indicators,and there was no significant difference in the in-dicators between Qiangxin Tang group and Captopril group.Conclusion Qiangxin Tang may promote mitochondrial autophagy and ensure the morphology and function of mitochondrial by upregulating the expression of NIX,BNIP3 and FUNDC1,thereby reducing myocardial injury and fibrosis.In addition,the combination of Qiangxin Tang and Captopril has a better therapeutic effect on CHF.

Objective:To discuss the relationship between the systolic blood pressure (SBP) circadian rhythm and blood pressure variability (BPV) and the left ventricular ejection fraction (LVEF) among 178 patients with hypertension.Methods:This article was a retrospective study. Totally 178 patients with hypertension from January 2020 to January 2022 were selected based on incorporated basis. 24-hour dynamic blood pressure monitoring and echocardiography examination were performed. Data such as patients' SBP circadian rhythm, BPV, heart ultrasound heart dynamic maps were collected, and the relationship between SBP circadian rhythm and BPV and their LVEF was explored.Results:Among the 178 patients, the decreased proportion of SBP circadian rhythm>the proportion of existed SBP circadian rhythm>the proportion of inverted SBP circadian rhythm>the proportion of disappearance of SBP circadian rhythm; patients with disappearance of SBP circadian rhythm were the youngest (63.8 ± 14.5) years old, and patients with inverted SBP circadian rhythm were the oldest (71.5 ± 9.4) years old ( P<0.05); the 24-hour systolic blood pressure standard deviation of SBP circadian rhythm was observed in patients with (13.1 ± 2.8) mmHg
