1.Bibliometric analysis of research process and current situation of brain aging and exosomes

BACKGROUND:In recent years,with the rapid development of biomedicine,the study of brain aging and exosomes has attracted more and more attention,but there is no bibliometrics analysis in this field. OBJECTIVE:To objectively analyze domestic and foreign literature on brain aging and exosomes in the past 15 years,to summarize the research status,hot spots,and development trends in this field. METHODS:Using the core database of Web of Science as a search platform,we downloaded the literature on brain aging and exosomes published from the establishment of the database to December 28,2022,and analyzed the data from the aspects of country or region,institution,author,keywords,and co-cited literature using CiteSpace 6.1.R6 visualization software. RESULTS AND CONCLUSION:A total of 1 045 research articles were included,and the number of publications on brain aging and exosomes research both domestically and internationally was showing an increasing trend year by year.The United States ranked first with 429 papers,and China ranked second with 277 papers.Louisiana State University ranked first with 16 articles.Professor Lukiw Walter J from Louisiana State University in the United States was the author with the highest number of publications,and Professor Bartel DP from the Massachusetts Institute of Technology was the author with the most citations.The most prolific Journal was the International Journal of Molecular Sciences.Alzheimer's disease,microRNA,gene expression,extracellular vesicles,exosomes,oxidative stress,and biomarkers are the most relevant terms.According to the research on hot topics,biomarkers have become a new research hotspot.The above results indicate that the research on brain aging and exosomes has gradually increased in the past 15 years.The research direction has gradually shifted from the initial exploration of the expression of miRNAs in central nervous system diseases related to brain aging to the search for biomarkers that can identify and diagnose neurodegenerative diseases.The study of exocrine miRNAs to protect central nervous system from damage has emerged as promising therapeutic strategy.
2.Coronary artery stenosis associated with right ventricular dysfunction in acute pulmonary embolism: A case-control study.

BACKGROUND:
The potential impact of pre-existing coronary artery stenosis (CAS) on right ventricular (RV) function during acute pulmonary embolism (PE) episodes remains underexplored. This study aimed to investigate the association between pre-existing CAS and RV dysfunction in patients with acute PE.
METHODS:
In this multicenter, case-control study, 89 cases and 176 controls matched for age were enrolled at three study centers (Peking Union Medical College Hospital, Fuwai Hospital, and the Second Affiliated Hospital of Harbin Medical University) from January 2016 to December 2020. The cases were patients with acute PE with CAS, and the controls were patients with acute PE without CAS. Coronary artery assessment was performed using coronary computed tomographic angiography. CAS was defined as ≥50% stenosis of the lumen diameter in any coronary vessel >2.0 mm in diameter. Conditional logistic regression analysis was used to evaluate the association between CAS and RV dysfunction.
RESULTS:
The percentages of RV dysfunction (19.1% [17/89] vs. 44.6% [78/176], P <0.001) and elevated systolic pulmonary artery pressure (sPAP) (19.3% [17/89] vs. 39.5% [68/176], P = 0.001) were significantly lower in the case group than those in the control group. In the multivariable logistic regression model, CAS was independently and negatively associated with RV dysfunction (adjusted odds ratio [OR]: 0.367; 95% confidence interval [CI]: 0.185-0.728; P = 0.004), and elevated sPAP (OR: 0.490; 95% CI: 0.252-0.980; P = 0.035), respectively.
CONCLUSIONS
Pre-existing CAS was significantly and negatively associated with RV dysfunction and elevated sPAP in patients with acute PE. This finding provides new insights into RV dysfunction in patients with acute PE with pre-existing CAS.

3.Quality Control and Analysis of Treatment for Hospitalized Cancer Patients:Interview and Medical Records Study from Nine Hospitals in Beijing

Objective To analyze the current quality of treatment for hospitalized cancer patients in Bei-jing,identify major issues in treatment practices,and propose improvements.Methods Nine hospitals in Beijing were selected for examination.Expert on-site interviews and medical record sampling were conducted.The"Bei-jing Cancer Diagnosis and Treatment Quality Control Checklist"was used to assess the hardware,management,anti-cancer drug therapy,radiation therapy,and surgical treatment during cancer treatment at these hospitals from January to October 2023.The relevant problems were analyzed.Results Among the nine hospitals,two(22.2%)were equipped with laminar flow rooms,and three(33.3%)had intravenous drug preparation centers.In terms of institutional management,seven hospitals(77.8%)had standardized anti-cancer drug prescription authority management,eight(88.9%)had complete emergency plans,and five(55.6%)had oncology specialist pharmacists.Regarding anti-cancer drug therapy,the areas with higher completion rates included pathology diag-nosis support(97.6%),routine pre-treatment examinations(96.3%),adverse reaction evaluation(92.7%),discharge summaries(95.1%),and admission records(91.5%).However,the accuracy of tumor staging before treatment(70.7%)and the evaluation of therapeutic efficacy after drug treatment(76.9%)needed improvement.The oncology specialty significantly outperformed the non-oncology specialty in terms of the accuracy rate of TNM staging(86.0%vs.46.9%,P<0.001),the completeness of informed consent forms(100%vs.68.8%,P<0.001),the completeness of drug indication evaluation(96.0%vs.78.1%,P=0.025),the completeness of admission medical history records(98.0%vs.81.3%,P=0.008),the rationality of drug dosage(96.0%vs.75.0%,P=0.005),the rationality of drug infusion time(100%vs.62.5%,P<0.001),and the rationality of the order of drug infusion(100%vs.87.5%,P=0.010).Although the quality of radiation therapy was high,the subsequent evaluation of therapeutic efficacy(39.3%)requires enhancement.In surgical treatment,the preoper-ative pathology diagnosis support rate(78.1%)and the accuracy of tumor staging(37.5%)were relatively low,indicating issues with incomplete preoperative evaluation and the absence of multidisciplinary discussions.Conclusions There remains significant room for improvement in the quality of cancer treatment in China.It is recommended to standardize tumor staging assessment processes,strengthen entry assessments for non-oncology departments,promote the implementation of multidisciplinary treatment models,and establish a multi-department collaborative management model.Continuous monitoring of cancer diagnosis and treatment quality indicators is es-sential to promote ongoing improvements in cancer treatment quality.
4.Effect and mechanism of the Hypericum japonicum-Rehmannia glutinosa-Salvia plebeian compound in alleviating inflammation,promoting autophagy,and mitiga-ting liver injury

Based on network pharmacology,through molecular docking and experimental validation,the study explored the mechanism of the Hypericum japonicum-Rehmannia glutinosa-Salvia ple-beian compound(HRS)in the treatment of liver injury.Mice were randomly divided into a control group(CON group),a model group(CCL4 group),a high-dose drug group(HRS-H group),and a low-dose group(HRS-L group).A mouse liver injury model was established using CCL4 induction,liver tissue pathological morphology was observed,and the relative expression levels of liver in-flammatory cytokine genes was measured.Active ingredients of traditional Chinese medicine and targets related to Chinese medicine and diseases were obtained from databases such as Herb,TCM-SP,PubChem,Swiss Target Prediction,Gene Cards and DisGeNET.The intersection of targets was used to obtain potential drug targets.The potential targets were analyzed for protein-protein inter-action(PPI)using the string database and a network diagram of"drug-active component-intersec-tion target"was constructed using Cytoscape.DAVID database was used for GO and KEGG path-way analysis,and Auto Dock Tools software was used for molecular docking.Finally,the results of molecular docking by examining the expression of key target genes and downstream genes such as those related to the PI3K-AKT pathway and the autophagy pathway were experimentally valida-ted.Results:Animal experiment results showed that compared to the CON group,the CCL4 group of mice exhibited disrupted liver architecture,hepatocyte steatosis,vacuolization,and extensive in-flammatory cell infiltration.These characteristics were ameliorated by drug treatment groups with the HRS-H group demonstrating superior effects compared to the HRS-L group.RT-qPCR results from mouse livers showed significantly increased relative expression of TNF-α and INOS mRNA compared to the CON group in the CCL4 group(P<0.01),and significantly increased IL-1β mR-NA relative expression(P<0.05).Compared to the CCL4 group,the HRS-H group showed signifi-cantly decreased TNF-α,INOS,and IL-1β mRNA relative expressions(P<0.01).155 potential tar-gets for HRS in alleviating liver damage were identified through network pharmacology,with top-ranked key target points including STAT3,SRC,PIK3R1,PIK3CA,AKT1,HSP90A11,EGFR,and ESR.Key active ingredients included Tetramethoxyluteolin,Hispidulin,Eupafolin,Kaempferol,and Eupaformonin.GO enrichment analysis yielded 940 entries,and KEGG enrichment analysis yielded 177 biological pathways.Molecular docking results showed a strong binding ability between the main components of HRS and key target points.RT-qPCR results showed increasing trends for EGFR,PI3KCA,HSP90A11,and NF-κB mRNA compared to the CON group in the CCL4 group,significantly increased AKT1 mRNA relative expression(P<0.05),significant decreases in ULK1,ATG5,LC3B,and ATG7 mRNA relative expressions(P<0.05),and extremely significant decreases in PTEN,ATG13,BECLIN-1,ATG16L1,ATG12,ATG4B,and ATG3 mRNA relative expressions(P<0.01).Compared to the CCL4 group,the HRS-H group showed significantly de-creased PI3KCA,HSP90A11,and NF-κB mRNA relative expressions(P<0.05),extremely signif-icantly decreased EGFR,AKT1,and mTOR mRNA relative expressions(P<0.01),increased ULK1 relative expression trends,significantly increased PTEN,ATG16L1,ATG5,LC3B,and ATG7 mRNA relative expressions(P<0.05),extremely significantly increased ATG13,BECLIN-1,ATG12,ATG4B,and ATG3 mRNA relative expressions(P<0.01).Conclusion:The HRS ex-erts hepatoprotective effects through multi-component,multi-pathway approaches,with alleviating inflammation and promoting hepatocyte autophagy through PI3K-AKT pathway likely being im-portant mechanisms for its protective effects.

Peritoneal mesothelioma(PeM),which originates in cells in the visceral and mural layers of the peritoneum,is a rare and highly aggressive malignant tumor that accounts for approximately 15%of all mesotheliomas.The disease is characterized by insidious onset and poor prognosis,and its mechanisms of tumorigenesis and progression have not been fully elucidated.Asbestos exposure is a major risk factor for mesothelioma.Effective treatment options alternative to cytoreductive surgery(CRS),hyperthermic intraperitoneal chemotherapy(HIPEC),and platinum/pemetrexed(conventional treatments)are currently lacking.Immunotherapy,which mobilizes the immune system,has shown initial efficacy in patients with advanced mesotheliomas.However,additional real-world clinical trials and studies on immuno-therapeutic targets remain needed.This article reviews progress in PeM-related immunotherapy clinical studies and potential targets in PeM treatment.

8.Moderate-intensity interval aerobic training slows brain aging in rats by inhibiting the apoptosis pathway mediated by miR-21-3p

Objective To examine the impact of moderate-intensity intermittent aerobic training on the expression of microRNA-21-3p(miR-21-3p),so as to establish a theoretical foundation for under-standing the potential mechanisms of exercise interventions mitigating the decline in learning and memo-ry functions associated with brain aging. Methods Forty-five three-month specific pathogen-free(SPF) male Sprague-Dawley(SD) rats were randomly divided into a control group(group C),an aging group (group A),and an aging+exercise group(group AE),following a one-week acclimatization period. Group A and AE were administered intraperitoneal injections of D-galactose(D-gal) for six weeks,and the latter group underwent an eight-week regimen of moderate-intensity interval aerobic training. Behavioral assessments were conducted in all groups 24 hours after modeling. Nissl staining was used to detect the distribution of neurons,while in situ terminal transferase labeling(TUNEL) was employed to identify apoptosis in hippocampal cells. Moreover,small RNA sequencing was conducted to detect miRNA expression,while real-time quantitative polymerase chain reaction(qRT-PCR) was employed to measure the relative content of miR-21-3p. Meanwhile,the targeting relationship between miR-21-3p and tumor protein p53(p53) was confirmed using the dual luciferase reporter gene assay,while the ex-pression levels of p53,the B lymphoblastoma-2 protein family member X(Bax),and the B lympho-blastoma-2(Bcl-2) were measured using Western Blotting. Results ①The results of the Morris water maze positioning navigation experiment before and after the intervention showed that the overall swim-ming distance of rats in groups A and AE was significantly longer than group C(P<0.05,P<0.01),with that of group AE significantly shorter than group A(P<0.01,P<0.05). ② Nissl's staining re-vealed that,compared to group C,hippocampal neurons in group A exhibited disorganization,reduced staining intensity,indistinct nuclei from surrounding tissues,and a scarcity of Nissl bodies,while those of group AE were sparse,with fewer levels and significantly reduced numbers. However,com-pared with group A,the hippocampal neurons in group AE were arranged more neatly,with more lay-ers and more Nissl bodies. ③TUNEL results showed that the apoptosis rate of hippocampal cells in group A and AE was significantly higher than group C(P<0.01,P<0.05),with that of group AE signif-icantly lower than group A(P<0.01). ④ Small RNA sequencing results showed that the expression of plasma exosomes miR-21-3p in group A and AE was significantly higher than group C(P<0.01,P<0.05),with that of group AE significantly lower than group A(P<0.01). ⑤According to the results of qRT-PCR,the expression of miR-21-3p in group A and AE was significantly higher than group C(P<0.01),with that of group AE significantly lower than group A(P<0.05). ⑥Luciferase experiment showed that miR-21-3p had a targeted relationship with p53. Compared with Vector mimics+pcD-NA3.1-p53,miR-21-3p mimics and pcDNA3.1-p53 co-transfected HEK293T cells could significantly improve p53 luciferase activity. ⑦According to Western Blotting results,the protein expressions of p53 and Bax and the ratio of Bax/Bcl2 in group A and AE were significantly higher than group C(P<0.01,P<0.05),but that of Bcl-2 was significantly lower than the latter group(P<0.01). Moreover,the expression of p53 and Bax protein and the ratio of Bax/Bcl-2 in group AE were significantly low-er than group A(P<0.05,P<0.01),but that of Bcl-2 protein was significantly higher than group A(P<0.01). Conclusions It is suggested that 8-week moderate intensity intermittent aerobic training may down-regulate miR-21-3p mediated by peripheral blood exosomes,reduce the expression of miR-21-3p in brain tissue,target miR-21-3p to bind p53,and reduce the occurrence of apoptosis,thus im-proving the learning and memory ability of rats.
9.Risk Factors for Returning of Pediatric Liver Transplant Recipients to the Intensive Care Unit

Objective To explore the risk factors for the returning of pediatric liver transplant recipi-ents to the intensive care unit(ICU)and provide reference for the clinical decision-making after surgery.Methods A retrospective analysis was conducted with the information of all the pediatric patients who underwent liver transplantation in Ren Ji Hospital,Shanghai Jiao Tong University School of Medicine and were returned to the ICU from 2019 to 2021.The patients returned to the ICU during hospitalization and the reasons for the return were recorded.Each patient of ICU return was matched with three pediatric patients who did not return to the ICU during hospitalization.The basic information,the vital signs and laboratory indicators on the day of transfer from ICU,immunosuppressants and drug concentrations were compared between the two groups.Multivariate Logistic regression analysis was performed to explore the risk factors for the returning of pediatric liver transplant recipients to the ICU.Results The returning rate of pediatric liver transplant recipients to the ICU was 4.36%,and it was 16.00%within 48 h.The main reasons for the return included respiratory complications,abdominal infections,and hepatic vascular occlusion.Multivariate Logistic regression analysis showed that post-operative red blood cell transfusion(OR=4.554,95%CI=1.743-11.901,P=0.002)and high serum level of uric acid(OR=1.005,95%CI=1.001-1.009,P=0.014)were the risk factors for returning to the ICU.High diastolic blood pressure(OR=0.922,95%CI=0.885-0.960,P<0.001)and high total protein level(OR=0.937,95%CI=0.891-0.986,P=0.012)were the protective factors for returning to the ICU.Conclusion Post-operative red blood cell transfusion and high serum level of uric acid are independent risk factors for the returning of pediatric liver transplant recipients to the ICU.

Purpose To explore the value of iodine concentration(IC)and effective atomic number(Zeff)derived from dual-layer spectral detector CT in diagnosing the regional lymph nodes of gastric cancer.Materials and Methods From May 2021 to September 2022,108 patients pathologically confirmed with gastric cancer(92 non-metastatic lymph nodes and 116 metastatic lymph nodes),who underwent dual-phase enhanced dual-layer spectral detector CT scanning preoperatively in the Second Affiliated Hospital of Guangzhou University of Chinese Medicine,were included in the study.The imaging and clinical data of all cases were collected,retrospectively.According to preoperative CT images,the short diameter,margin and enhancement homogeneity of the lymph nodes were evaluated.The IC and Zeff were obtained from the delineated region of interest of the solid portion of the maximum layer of the target lymph nodes,and the normalization IC(nIC)and normalization Zeff(nZeff)were calculated.The difference of parameters between two groups were compared.The efficiency of differential diagnosis and combined diagnosis of each parameter was analyzed.The area under the receiver characteristic curve(AUC)for each parameter were compared.Results There were statistical differences in short diameter,margin and enhancement homogeneity between the two groups(t/x2=-7.575,30.971,20.913,all P<0.001).In the benign lymph nodes group,all spectral parameters in the arterial phase(AP)were lower than those in malignant lymph nodes,while the parameters in the venous phase(VP)were higher(t=-10.723-12.610,all P<0.001).Among the three conventional features,the AUC of short diameter was the highest(0.749).Compared with the AUC values of ICAP and ZeffVP,the AUC values of short diameter and quantitative parameters had no statistical difference(P>0.05),but were lower than those of other spectral parameters.Among the spectral parameters,the AUC value of nICVP was the highest(0.925).The sensitivity and specificity of the spectral parameters were higher than those of conventional features.The diagnostic efficacy of the multi-parameter combined model was superior to that of the univariate model.The AUC value of the combined model with conventional features was 0.815.Among the combined model based on CT spectral parameters,the diagnostic efficacy of model combined with the parameters of arteriovenous phase was the highest(AUC=0.993).Conclusion The quantitative parameters of dual-phase enhancement derived from dual-layer spectral detector CT can effectively evaluate the characteristics of gastric cancer regional lymph nodes,and the diagnostic efficiency is higher than the conventional characteristics of lymph nodes.The multi-parameter combined model can improve the diagnostic efficiency.
