1.Configurational pathways to career growth of public hospital managers based on qualitative comparative analysis

Objective:To explore the causal configurations influencing the career growth of public hospital managers and provide references for promoting their professional development.Methods:From July to December 2023, managers from 9 public hospitals in 3 prefecture-level cities or autonomous prefectures of a central China province were surveyed through a combination of stratified sampling and purposive sampling. The questionnaire included three validated scales: the career growth scale, organizational career management scale, and person-organization fit scale. Using career growth as the outcome variable, and hospital level, organizational career management, person-organization fit, educational background, and employment type as antecedent conditions, fuzzy-set qualitative comparative analysis was applied to identify high-growth configuration pathways.Results:A total of 276 valid questionnaires were collected. Scores for career growth, organizational career management, and person-organization fit were (3.29±0.79), (3.54±0.85), and (3.51±0.85), respectively. The consistency of necessary condition tests for all factors was below 0.9, indicating none were indispensable prerequisites, supporting a configurational analysis of sufficient conditions. Five distinct pathways for career growth of public hospital managers were identified: resource-oriented growth (consistency=0.88), person-organization fit-driven growth (consistency=0.89), organization-dependent growth (consistency=0.91), expertise-driven growth (consistency=0.88), and stable adaptation growth (consistency=0.87). Organizational career management and person-organization fit emerge as core conditions.Conclusions:The study reveals five heterogeneous pathways for managers′ career growth, emphasizing multidimensional complexity. Hospitals should design personalized development plans aligned with pathway characteristics, particularly by strengthening organizational career management and enhancing person-organization fit, to foster managers′ holistic growth and organizational advancement.

3.Analysis on the changes of measurement indicators of Chinese Journal of Trauma from 2014 to 2023

Objective:To analyze the changes of measurement indicators of Chinese Journal of Trauma (hereinafter termed as the "Journal") from 2014 to 2023. Methods:The data from the Annual Report for Chinese Academic Journals Impact Factors ( natural science and engineering technology) were used to aggregate, analyze, and evaluate the clout index, impact factor, number of articles published, number of citations that can be cited, and total citation, etc. of the Journal from 2014 to 2023. Results:The changes in measurement indicators of the Journal from 2014 to 2023 were as follows: (1) The clout index averaged 555.652, with slight decrease in the first 6 years and rapid increase in the following 4 years. (2) The composite impact factor averaged 1.270, and the comprehensive impact factor averaged 1.174. (3) The average number of articles published was 303, and the average number of citations that can be cited was 213, both of which had decreased over the years. (4) The average composite total citation was 2 556 times, keeping over 2 100 times per year. The average comprehensive citation count was 1 987 times. (5) The average ratio of fund papers was 0.54, with the average citing half-life of 5.3 years and average cited half-life of 4.4 years. The average number of citing journals was 290, with average number of cited journals of 534. The ratio of other-citations to total-citations had been consistently at a high level with an average of 0.92. The total download volume averaged 18 200 times. The mass index averaged 3.146, with the level of the index in 2023 five times as much as that in 2015.Conclusion:The clout index, impact factor and mass index of the Journal have steadily increased over the past 10 years, while the number of citations that can be cited has slightly decreased.
4.Economic burden of cerebral infarction patients in a tertiary general hospital in Guangzhou

Objective This paper aims to analyze average hospitalization expenses,focusing on the structure of per pa-tient's personal costs,and the influencing factors,to provide reference for reducing the disease economic burden on patients.Methods Descriptive statistical analysis was conducted to compare hospitalization expenses and their structures among cerebral infarction patients in a tertiary general hospital in Guangzhou from 2015 to 2022.Multilinear analysis was used to study the influ-encing factors of patient's personal costs.Results The growth of hospitalization expenses of cerebral infarction patients slowed down,with an average annual growth rate of 2.86%.The structure of hospitalization expenses was gradually optimized,with the average technical and labor costs accounting for 47.41%.The personal costs of cerebral infarction patients showed an increasing trend,with an average annual growth rate of 5.96%.This trend was influenced by factors such as hospitalization days,case clas-sification,payment patterns,and patient sources.Conclusion The economic burden of cerebral infarction remains heavy.Hos-pitals should continue to optimize clinical diagnosis and treatment practices to enhance clinical pathway management.Govern-ments should improve the social security system to reduce the economic burden of disease on patients.
5.Expression and biological processes of CHI3L2 in brain glioma and its impact on patient prognosis

[摘 要] 目的:基于生物信息学方法探究几丁质酶-3样蛋白2(CHI3L2)在脑胶质瘤中的表达和生物学过程及其对患者临床预后的影响。方法:以中国脑胶质瘤基因组图谱(CGGA)为训练集(n = 325)、癌症基因组图谱(TCGA)为验证集(n = 702),对CHI3L2与脑胶质瘤患者临床病理特征的关系、预后价值和生物学过程进行交叉验证分析。用Kaplan-Meier法进行生存分析,采用Cox回归模型分析CHI3L2表达及相关临床病理特征与脑胶质瘤患者预后的关系,利用受试者工作特征(ROC)曲线分析CHI3L2在脑胶质瘤诊断中的价值,用GO、KEGG及GSVA途径分析CHI3L2在脑胶质瘤中的潜在生物学过程,构建CHI3L2的列线图以校准曲线及C-Index值来评估预测的准确性。WB法和qPCR 法检测CHI3L2在正常星形胶质细胞HA1800、胶质瘤U215和U87细胞中蛋白质与mRNA水平表达的影响。选取长沙市中心医院病理科保存的7例正常脑组织、5例低级别胶质瘤(LGG, WHOⅠ~Ⅱ级)和6例胶质母细胞瘤(GBM,WHO Ⅳ级)标本进行免疫组化染色分析,验证CHI3L2在正常脑组织和不同级别脑胶质瘤组织中的表达情况。结果:CHI3L2在GBM(P < 0.000 1)、非1p/19q编码(P < 0.000 1)、IDH-野生型(P < 0.000 1)、非MGMT甲基化(P<0.01)患者中显著表达,对GBM具有一定的预测价值,并且是脑胶质瘤患者总生存期(OS)的独立预后因素(P < 0.001)。构建的列线图对脑胶质瘤患者的生存预后预测性良好。CHI3L2与LGG和GBM的免疫细胞浸润水平、肿瘤免疫微环境和免疫细胞均有显著关系。脑胶质瘤中CHI3L2蛋白(P < 0.05)和mRNA(P < 0.000 1)的表达水平与更高的恶性程度相关,免疫组化的结果进一步验证了这个发现。结论:CHI3L2的表达与脑胶质瘤的恶性程度、临床病理特征及预后关系密切,并且参与脑胶质瘤的肿瘤微环境和免疫浸润,有望成为脑胶质瘤治疗策略中的一个新靶点。

7.Effects of Rehabilitation Training Combined with Acupuncture on Upper Limb Function of Stroke Patients Based on Kinect

Objective To investigate the effect of rehabilitation training combined with acupuncture ( RTA) on upper limb function of stroke patients by Kinect. Methods Stroke patients with upper limb dysfunction werrandomly divided into control group (rehabilitation training) and treatment group ( RTA), with 15 cases in each group. The modified Barthel Index ( MBI), Fugl Meyer assessment ( FMA), and Wolf motor function test (WMFT) were compared between two groups before and after treatment. The changes in motor time ( MT), motor unit number (MUN), index of curvature (IC), elbow flexion angle (EFA), shoulder flexion angle (SFA),and shoulder adduction angle ( SAA) during three actions, namely, placing forearm on the table, extending elbow and drinking water, were evaluated by Kinect and then compared between two groups before and after treatment. Results After 6 weeks of intervention, the scores of MBI, FMA, WMFT and elbow extension in treatment group were higher than those in control group (P<0. 05). The scores of MBI, FMA, WMFT and three actions after treatment were higher than those before treatment (P<0. 05). For three actions, the improvement of MT, MUN, IC, EFA, SFA, and SAA in treatment group were better than those in control group ( P< 0. 05). Compared with pre-treatment, for the action of forearm placement on the table and elbow extension, both treatment group and control group showed an increase in EFA (P<0. 05), and a decrease in MT, MUN, IC, SFA and SA (P<0. 05). For the action of drinking water, both treatment group and control group showed an increase in EFA and SAA (P< 0. 05), and a decrease in MT, MUN, IC and SAA ( P< 0. 05). Conclusions RTA can improve the upper limb function of stroke patients. Kinect can accurately reflect the changes in upper limb function of stroke patients, and it is suitable for clinical work.
