1.Correlation Between Traditional Chinese Medicine Syndrome Type of Unstable Angina Pectoris and Iron Metabolism Indices

ObjectiveTo investigate the correlation between iron metabolism parameters and various syndrome types of unstable angina pectoris (UAP). MethodsA cross-sectional study was conducted from October 2021 to October 2023, encompassing 213 patients diagnosed with UAP at Xiyuan Hospital of Chinese Academy of Chinese Medical Sciences. Additionally, 30 healthy individuals were selected as control cases. Single-factor analysis was used to investigate the differences in clinical data among different Traditional Chinese Medicine (TCM) syndrome types of UAP and their correlation with iron metabolism indices. The study conducted a comparative analysis of the aforementioned clinical data among patients with and without heat-toxic and blood-stasis syndrome. Logistic regression was used to analyze the correlation between TCM syndrome types and related factors. The receiver operator characteristic (ROC) curve was employed to assess the predictive value of iron metabolism indices, along with their sensitivity and specificity. ResultsCompared to those in the control group, serum iron (SI) and serum ferritin (SF) levels were significantly increased in the UAP group (P<0.01), while transferrin (TRF) and total iron binding capacity (TIBC) levels were decreased (P<0.01). However, there was no significant difference in unsaturated iron binding capacity (UIBC). Multivariate binary Logistic regression analysis identified apolipoprotein A1 (ApoA1), homocysteine (HCY), high-sensitivity C-reactive protein (hs-CRP), and SF as independent influencing factors for the UAP patients (P<0.05, P<0.01). Additionally, statistically significant differences were observed in SI, SF, TRF, and TIBC among 213 patients with different TCM types (P<0.01). Patients with heat-toxic and blood-stasis syndrome had higher SI and SF values than those without the syndrome (P<0.01), while their TIBC and TRF values were lower (P<0.01). Multivariate binary logistic regression analysis showed that SI and LDL-C levels were closely associated with the differentiation of heat-toxic and blood-stasis syndrome. ConclusionUAP patients often experience iron metabolism disorders, and the heat-toxic and blood-stasis syndrome are significantly correlated with iron metabolism parameters. The SI and LDL-C levels have high specificity and sensitivity in diagnosing heat-toxic and blood-stasis syndrome.
2.Application of CT myocardial perfusion imaging combined with plaque quantification technique in assessing reperfusion injury in patients with myocardial ischemia

Objective To discuss the application of CT myocardial perfusion imaging(CT-MPI)combined with plaque quantification technique in assessing reperfusion injury in patients with myocardial ischemia.Methods The clinical data of 100 patients with myocardial ischemia,who were admitted to the People's Hospital of Xinjiang Uygur Autonomous Region of China from June 2019 to June 2024,were retrospectively analyzed.CT-MPI and plaque quantification technique examination were performed in all patients before percutaneous coronary intervention(PCI).According to the presence or absence of reperfusion injury after PCI,the patients were divided into non-injury group(n=38)and injury group(n=62).The CT-MPI parameters,including time-to-peak(TTP),myocardial blood flow(MBF)and myocardial blood volume(MBV),and the plaque quantification technique parameters,including eccentricity index(EI),remodeling index(RI),calcified plaque volume(CPV),minimum lumen area(MLA),narrowness degree(ND),non-calcified plaque volume(NCPV),total plaque volume(TPV)and plaque length(PL),were compared between the two groups.Receiver operating characteristic(ROC)curve was used to analyze the value of CT-MPI and plaque quantification technique in assessing reperfusion injury in patients with myocardial ischemia,and logistic regression analysis was used to determine the factors influencing reperfusion injury.Results TTP in the injury group was longer than that in the non-injury group,while MBF and MBV in the injury group were lower than those in the non-injury group(all P<0.05).No statistically significant differences in El,RI,CPV and MLA existed between the two groups(all P>0.05).ND,NCPV,TPV and PL in the injury group were higher than those in the non-injury group(P<0.001).ROC curve analysis showed that the area under ROC curve(AUC)of combination use of TTP MBF,MBV,ND,NCPV,TPV and PL for evaluating reperfusion injury in patients with myocardial ischemia was 0.914,which was higher than the values obtained by the separate evaluation of the above seven indicators,and the results were 0.781,0.784,0.760,0.791,0.775,0.792 and 0.798 respectively(all P<0.05).Logistic regression analysis revealed that TTP,MBF,MBV,ND,NCPV,TPV and PL were the risk factors for reperfusion injury in patients with myocardial ischemia(all P<0.05).Conclusion CT-MPI combined with plaque quantification technique can effectively evaluate the reperfusion injury in patients with myocardial ischemia,providing important reference for the diagnosis and treatment of reperfusion injury,therefore,it has high clinical application value.

Talents are the main force for the development of traditional Chinese medicine(TCM), and the construction of TCM talents and the reformation of talent evaluation system are essential to promote the inheritance and innovation of TCM. At present, we are still exploring and developing in the fields of the formulation, implementation and evaluation indicators of TCM talent evaluation system. However, there are shortcomings and difficulties. For instance, insufficient stratification in the evaluation, excessive emphasis on the quantity of achievements, neglecting the quality of the achievements and the actual contribution, imperfect assessment indicators, and the weak characteristics of TCM. Therefore, national ministries and commissions have jointly issued a document requesting to break the four only and set a new standard, in order to promote the construction of a scientific and technological talent evaluation system oriented by innovation value, ability and contribution. For the evaluation of TCM clinical talents, China Association for Science and Technology commissioned China Association of Chinese Medicine to build the China Clinical Cases Library of TCM(CCCL-TCM), which aims at collecting the most authoritative and representative TCM clinical cases and exploring the advantages of applying clinical cases as masterpiece of achievement in TCM clinical talents evaluation. CCCL-TCM can promote the construction of a talent evaluation system that is more in line with the development characteristics of TCM industry, and to carry out relevant pilot in TCM colleges and institutions across the country in order to promote the reformation of TCM talent evaluation system.
4.Construction, reliability and validity of a professional identity questionnaire for psychiatrists

BackgroundDomestic researches on the professional identity of psychiatrists are still relatively rare up to now, and effective evaluation tools remain inadequate. ObjectiveTo construct a professional identity questionnaire for psychiatrists with high reliability and validity, so as to provide an effective tool for the evaluation of professional identity of psychiatrists. MethodsA self-designed questionnaire titled "Professional Identity Questionnaire" was compiled. After preliminary screening of items and expert consultation, an initial questionnaire was formed and administered to a sample of 400 psychiatrists, and the final questionnaire was assessed by item analysis, reliability analysis and validity analysis. ResultsThe Cronbach's α coefficient was 0.930 for the final questionnaire, and ranged from 0.817 to 0.920 for each dimension. The value of KMO was 0.904 (χ2=2 931.652, P<0.01). The cumulative variance contribution rate of factors extracted from exploratory factor analysis was 68.306%. Confirmatory factor analysis showed that the integrity of fit indexes were χ2/df=1.234, GFI=0.905, NFI=0.916, RMSEA=0.036, CFI=0.983, IFI=0.983. The final version of the professional identity questionnaire for psychiatrists consisted of 23 items relating to 4 dimensions, namely, professional cognition, professional behavior, professional emotion and professional values. ConclusionThe professional identity questionnaire for psychiatrists has good reliability and validity, and can be used as an effective tool to evaluate the professional identity of psychiatrists. 5.Effect of acupuncture on children with attention deficit hyperactivity disorder

ObjectiveTo observe the effect of acupuncture on children with attention deficit hyperactivity disorder (ADHD). MethodsFrom August to December, 2022, 54 children with ADHD from the outpatient clinic of the Fourth People's Affiliated Hospital of Jiangsu University were randomly divided into control group (n = 27) and observation group (n = 27). All the patients accepted electroencephalogram (EEG) neurofeedback training, while the observation group accepted acupuncture in addition, for twelve weeks. They were assessed with Integrated Visual and Auditory Continuous Performance Test (IVA-CPT), Conners Parent Symptom Questionnaire (PSQ), and EEG before and after treatment. ResultsThe comprehensive control quotient, comprehensive attention quotient and hyperactivity quotient of IVA-CPT increased in the observation group after treatment (t > 3.889, P < 0.001), and they were more in the observation group than in the control group (t > 2.040, P < 0.05); while all the factors of PSQ reduced in the observation group (t > 6.630, P < 0.001), and they were less in the observation group than in the control group (t > 3.871, P < 0.001); the mean frequency of β and sensorimotor rhythms increased in the observation group (t > 12.432, P < 0.001), and they were more in the observation group than in the control group (t > 7.561, P < 0.001); the mean frequency of θ reduced in the observation group (t = 9.966, P < 0.001), and it was less in the observation group than in the control group (t = 7.257, P < 0.001). ConclusionCombination of acupuncture is more effective on EEG in children with ADHD to improve attention, control and core symptoms.
6.Mechanism of Zhizi Prescription in Protection of CCl4-induced Acute and Subacute Liver Injury in Mice

ObjectiveTo investigate the protective effect of Zhizi prescription (ZZP) on carbon tetrachloride (CCl4)-induced acute and subacute liver injury and its mechanism. MethodAcute and subacute liver injury animal models were induced. C57 mice were randomly divided into a normal group, model group, obeccholic acid group, ZZP high-dose (0.5 g·kg-1) group, and ZZP low-dose (0.25 g·kg-1) group. According to the experiment design, the serum and liver tissue of mice were collected after the last administration. Hematoxylin-eosin (HE) and Sirius staining was used to observe the liver pathological changes. Serum alanine aminotransferase (ALT), aspartate aminotransferase (AST), total bilirubin (TBIL), liver homogenate hydroxyproline (Hyp), malondialdehyde (MDA), and superoxide dismutase (SOD) levels were determined by kit. The levels of tumor necrosis factor-α (TNF-α) and interleukin-6 (IL-6) in the liver tissue were determined by enzyme-linked immunosorbent assay (ELISA). Real-time fluorescence quantitative polymerase chain reaction (Real-time PCR) was used to detect the mRNA expressions of collagen 1A1 (Col1a1), collagen 3A1 (Col3a1), fibronectin (FN), transforming growth factor β receptor Ⅱ (Tgfbr2) and α-smooth muscle actin (α-SMA) in the liver tissue. ResultIn terms of the acute liver injury, as compared with the normal group, the levels of ALT, AST, TBIL and MDA in the model group were significantly increased (P<0.01), while the activity of liver SOD was significantly decreased (P<0.01). Compared with model group, the ZZP high-dose and low-dose groups both significantly reduced the degree of liver cell injury, and protected the acute liver injury induced by CCl4. The ZZP high-dose group had a better effect than the ZZP low-dose group. In terms of the subacute liver injury, the levels of ALT, AST, MDA,TNF-α and IL-6 in the model group were significantly increased (P<0.01), while the activity of liver SOD was significantly decreased (P<0.01). As compared with the model group, liver Hyp content in the ZZP high-dose and low-dose groups was significantly decreased (P<0.01), and the collagen deposition in liver of both groups was significantly reduced. The ZZP high-dose group also significantly down-regulated the mRNA expressions of α-SMA, Col1a1, Col3a1, FN, and Tgfbr2 in the liver of mice (P<0.05, P<0.01). ConclusionZZP effectively protects the acute and subacute liver injury induced by CCl4, and the protective effect is proportional to its concentration. The mechanism may be related to the increase of the activity of antioxidant enzymes in the liver tissue, the decrease of the level of lipid peroxidation, and the inhibition of inflammatory response, thus reducing collagen deposition and improving early liver fibrosis.
7.Identification of triterpenoid saponins from Anemone rivularis var. flore-minore and study on their antitumor activities

OBJECTIVE To study the triterpenoid saponins from Anemone rivularis var. flore-minore and their antitumor activities. METHODS The n-butanol extract of 70% ethanol extract from rhizome of the plant was separated. The triterpenoid saponins were separated and purified by normal silica gel column chromatography ,reversed phase ODS column chromatography , Sephadex LH- 20 gel column chromatography and semi-preparation high performance liquid chromatography. The structures of these saponins were identified by spectral analysis (NMR and MS )and physical and chemical properties. MTT assay was used to test the proliferation inhibitory activity of the compounds against five kinds of human tumor cells (HL-60 cells,A549 cells,HepG2 cells,HeLa cells and U 87MG cells ). The apoptosis inducing effect of compound 7 on U 87MG cells was evaluated by flow cytometric Annexin V-FITC/PI staining test. RESULTS:Sixteen triterpenoid saponins were obtained and identified as 3 β-O-β-D-xylopyranosyl-(1→2)-α-L-arabinopyranosyl-oleanolic acid-28-O-α-L-rhamnopyranosyl- (1→4) -β-D-glucopyranosyl-(1→6)-β-D-glucopyranoside(1),3β-O-L-arabinopyranosyl oleanolic acid- 28-O-β-D-glucopyranoside(2),saponin B (3), 163.com oleanolic acid- 3β-O-β-D-glucopyranosyl-(1→2)-α-L-arabino- pyranoside(4),HN-saponin F (5),clematoside S (6),prosapogenin CP 4(7),cussonside B (8),pulsatilla saponin C (9), clemastanoside D (10),3 β-O-β-D-glucopyranosyl-(1→2)-β-L-arabinopyranosyl-hederagenin-28-O-β-D-glucopyranoside(11), ciwujianoside C 3(12),ciwujianoside A 1(13),huzhangoside D (14),kalopanaxsaponin B (15)and hederacolchiside E (16). Compounds 3,4,6-9 displayed inhibitory activities on the proliferation of tumor cells to different extent ,and compound 7 had the strongest activity ;compound 7 induced the apoptosis of U 87MG cell so as to inhibit the proliferation of cancer cells in a time-dependent manner. CONCLUSIONS The obtained 16 saponins are all identified as oleanolane-type ,among which compound 1 is a new compound. The monodesmosidic saponins ,the sugar chain of which attached at C- 3 and a free carboxyl at C- 28, possess stronger antitumor activity than others.
8.Simultaneous determination of the contents of eight chemical components in Huaiqin Salve by QAMS

Objective:To determine the content of albiflorin, paeoniflorin, benzoylpaeoniflorin, sophoricoside, prim-O-glucosylcimifugin, cimifugin, 5-O-methylvisammioside and sec-O-glucosylhamaudol in Huaiqin Salve with high performance liquid chromatography-quantitative analysis of multi-components by single-marker (HPLC-QAMS) method. Methods:The samples were separated with ZORBAX Eclipse XDB-C18 column by a gradient elution using methanol-acetonitrile (1:2) (A)-0.1% phosphoric acid solution (B) as mobile phase and the flow rate was 1.0 ml/min. The detection wavelength was 230 nm for albiflorin, paeoniflorin and benzoylpaeoniflorin, and 254 nm for sophoricoside, prim-O-glucosylcimifugin, cimifugin, 5-O-methylvisammioside and sec-O-glucosylhamaudol. The column temperature was 30 ℃. Using sophoricoside as an internal standard, the relative correction factors (RCFs) of albiflorin, paeoniflorin, benzoylpaeoniflorin, prim-O-glucosylcimifugin, cimifugin, 5-O-methylvisammioside and sec-O-glucosylhamaudol were calculated. The method was validated by comparison of the quantitative results between external standard method (ESM) and HPLC-QAMS method.Results:Albiflorin, paeoniflorin, benzoylpaeoniflorin, sophoricoside, prim-O-glucosylcimifugin, cimifugin, 5-O-methylvisammioside and sec-O-glucosylhamaudol showed good linear relationship within the range of 0.034 7-0.867 5, 0.063 6-1.590 0, 0.006 9-0.172 5, 0.198 6-4.965 0, 0.092 8-2.320 0, 0.026 6-0.665 0, 0.042 7-1.067 5, 0.020 9-0.522 5 μg ( r ≥ 0.999 1); whose average recoveries ( RSDs) were 98.85% (1.02%), 100.04% (0.67%), 96.92% (1.14%), 100.06% (0.85%), 99.31% (1.39%), 99.16% (1.22%), 98.59% (1.33%) and 97.58% (1.41%), respectively. No significant differences were found in the quantitative analysis of the components with ESM and HPLC-QAMS method. Conclusion:The HPLC-QAMS method could provide the reference for themulti-index component evaluation for Huaiqin Salve.

10.Establishment of personalized locking clavicle plate model and finite element analysis

BACKGROUND:The finite element analysis method is more accurate and fast to construct the three-dimensional model of the human skeleton and design the bone surgical medical instrument.
OBJECTIVE:To establish locking plate model according to the clavicle model, analyze and evaluate stress distribution of locking plate of the finite element model under bending and torsion conditions.
METHODS:Chest scan was carried out in a healthy young adult male by adopting 64-row spiral CT and his two-dimensional image data were gotten. The obtained data were analyzed with Mimics 10.0 software to establish the three-dimensional clavicle finite element model. The clavicle locking fixation plate model was established by applying the UG software. The locking fixation plate was evaluated by utilizing the abaqus software when the plate was bent while down to give force of 200 N, and twisted while 200 N?mm, to simulate the force and analyze the stress distribution of the locking plate.
RESULTS AND CONCLUSION:Based on the original image parameters provided by CT, this experiment produced a three-dimensional model of the clavical titanium plate which fitted better to bones. This model can obtain a single individual, personalized plate by three-dimensional printing technology. The finite element analysis basical y can simulate the actual stress of the plate. For straight plate and“S”-shape plate, in lateral bending and axial torsion loads, the maximum stress distribution of the seven-hole titanium plate is located in the center of the center hole. During actual surgical procedures, clavicle fracture fragments and middle locking hole had stress superposition. If the titanium plate can avoid the stress concentration, it can effectively avoid the occurrence of the broken plate after implantation, provide theoretical guidance for clinical practice, and provide reference and technical route for biomechanical analysis of other types of titanium plate.
