1.Banxia Xiexin Decoction reshapes tryptophan metabolism to inhibit progression of colon cancer.

This study explores the effect and mechanism of Banxia Xiexin Decoction(BXD) in inhibiting colon cancer progression by reshaping tryptophan metabolism. Balb/c mice were assigned into control, model, low-dose BXD(BXD-L), and high-dose BXD(BXD-H) groups. Except the control group, the other groups were subcutaneously injected with CT26-Luc cells for the modeling of colon cancer, which was followed by the intervention with BXD. Small animal live imaging was employed to monitor tumor growth, and the tumor volume and weight were measured. Hematoxylin-eosin(HE) staining was used to observe the pathological changes in mouse tumors. Immunohistochemistry was used to detect Ki67 expression in tumors. Immunofluorescence and flow cytometry were used to detect the infiltration and number changes of CD3~+/CD8~+ T cells in the tumor tissue. Enzyme-linked immunosorbent assay(ELISA) was employed to measure the levels of interferon-gamma(IFN-γ) and interleukin-2(IL-2) in tumors. Targeted metabolomics was employed to measure the level of tryptophan(Trp) in the serum, and the Trp content in the tumor tissue was measured. Western blot and RT-qPCR were employed to determine the protein and mRNA levels, respectively, of indoleamine 2,3-dioxygenase 1(IDO1), MYC proto-oncogene, and solute carrier family 7 member 5(SLC7A5) in the tumor tissue. Additionally, a co-culture model with CT26 cells and CD8~+ T cells was established in vitro and treated with the BXD-containing serum. The cell counting kit-8(CCK-8) assay was used to examine the viability of CT26 cells. The content of Trp in CT26 cells and CD8~+ T cells, as well as the secretion of IFN-γ and IL-2 by CD8~+ T cells, was measured. RT-qPCR was used to determine the mRNA levels of MYC and SLC7A5 in CT26 cells. The results showed that BXD significantly inhibited the tumor growth, reduced the tumor weight, and decreased the tumor volume in the model mice. In addition, the model mice showed sparse arrangement of tumor cells, varying degrees of patchy necrosis, and downregulated expression of Ki67 in the tumor tissue. BXD elevated the levels of IFN-γ and IL-2 in the tumor tissue, while upregulating the ratio of CD3~+/CD8~+ T cells and lowering the levels of Trp, IDO1, MYC, and SLC7A5. The co-culture experiment showed that BXD-containing serum reduced Trp uptake by CT26 cells, increased Trp content in CD8~+T cells, enhanced IL-2 and IFN-γ secretion of CD8~+T cells, and down-regulated the mRNA levels of MYC and SLC7A5 in CT26 cells. In summary, BXD can inhibit the MYC/SLC7A5 pathway to reshape Trp metabolism and adjust Trp uptake by CD8~+ T cells to enhance the cytotoxicity, thereby inhibiting the development of colon cancer.

Objective:To explore the value of fetal heart quantification technology in assessing the morphology and function of the fetal heart during normal pregnancy.Methods:This prospective cohort study selected normal fetuses from healthy pregnant women who underwent prenatal ultrasound examinations at Hunan Provincial People's Hospital from January 2023 to October 2024. Using the GE Voluson E10 color Doppler diasonography, routine obstetric ultrasound and fetal echocardiography were performed to assess fetal growth and development and to exclude intracardiac and extracardiac malformations. Clear four-chamber view (4CV) dynamic images of the heart showing the endocardium (duration ≥3 s) were collected. Speckle-tracking analysis was performed using fetal heart quantification software. The measured indicators included the global spherical index (GSI), end-diastolic length of the heart (L-ED), end-diastolic width of the heart (W-ED), and the global longitudinal strain (GLS), fractional area change (FAC), and 24-segment spherical index (SI) of the left ventricle (LV) and right ventricle (RV). The cases were divided into five groups based on gestational age at the time of prenatal ultrasound: 20 +0 to 23 +6, 24 +0 to 27 +6, 28 +0 to 31 +6, 32 +0 to 35 +6, and 36 +0 to 40 +6 weeks. One-way analysis of variance, two independent samples t-test, univariate linear regression analysis, and Pearson correlation analysis were used to explore the differences in the above indicators among different gestational age groups and their correlation with gestational age. Results:A total of 200 pregnant women were included in the cohort, four cases were excluded due to poor image quality that prevented accurate tracking and measurement of relevant indicators. Ultimately, 196 cases (20 +0 to 23 +6 weeks 40 cases, 24 +0 to 27 +6 weeks 34 cases, 28 +0 to 31 +6 weeks 41 cases, 32 +0 to 35 +6 weeks 48 cases, and 36 +0 to 40 +6 weeks 33 cases) were included in the study, with a successful image analysis rate of 98.0%. (1) There were statistically significant differences in 4CV L-ED, 4CV W-ED, LV-FAC, and RV-FAC among the groups at 20 +0 to 23 +6, 24 +0 to 27 +6, 28 +0 to 31 +6, 32 +0 to 35 +6, and 36 +0 to 40 +6 weeks [4CV L-ED: 28.0±3.0, 32.6±4.3, 40.9±4.3, 46.7±4.8, 53.1±5.8, F=3.72; 4CV W-ED: 21.9±1.8, 25.1±4.2, 31.7±3.0, 37.4±4.0, 42.0±4.9, F=2.61; LV-FAC: (51.4±8.0)%, (49.0±10.4)%, (47.3±7.3)%, (43.1±7.5)%, (40.7±8.2)%, F=2.94; RV-FAC: (49.9±10.8)%, (46.2±12.0)%, (46.3±8.3)%, (43.2±8.0)%, (41.9±5.6)%, F=3.09; all P<0.05].(2) The size of the normal fetal heart gradually increased with gestational age, while the heart morphology remained relatively stable (4CV L-ED and 4CV W-ED were positively correlated with gestational age, with regression coefficients of 1.313 and 1.325, respectively, both P<0.001;LV-FAC and RV-FAC were negatively correlated with gestational age with regression coefficients of -0.783 and -0.552, respectively, both P<0.001; GSI, LV-GLS and RV-GLS had no correlations with gestational age, all P>0.05). (3) The SI of LV segments 1 to 17 were higher than the SI of the corresponding RV segments, and the SI of RV segments 20-24 were higher than that of the corresponding LV segments (all P<0.001). Conclusion:Fetal heart quantification technology has a certain value in the assessment of fetal cardiac morphology and function.
4.Forensic Research Progress on Bongkrekic Acid Poisoning

Bongkrekic acid(BA)is a toxin with stable properties and no distinctive smell.It exists in common foods such as fermented edible grain products,potato products,spoiled tremella fuciformis and auricularia polytricha,as well as auricularia polytricha that has been soaked too long.It can easily cause food poisoning.At present,there is still a lack of complete method to detect BA,and no spe-cific antidote of BA has been found.Therefore,BA poisoning is easy to be misdiagnosed or missed diagnosed,and its mortality rate remains high.In recent years,studies have revealed the toxic mecha-nism of BA and found that BA can inactivate some enzymes containing thiol groups(-SH)and in-hibit the synthesis and transport of adenosine triphosphate(ATP),causing damage to liver,kidney,brain and other parenchymal organs.This article reviews the autopsy cases and literature of deaths caused by BA poisoning at home and abroad,systematically summarizes the epidemiology,clinical manifestations,pathological changes,toxicological mechanisms,detection methods,forensic diagnostic key points and challenges of BA in forensic medicine,with the aim of providing a reference for foren-sic identification of related cases.
5.Analysis of Coordination Patterns and Energy Flow in Patellofemoral Pain Syndrome During Sit-to-Stand Transitions

Objective To explore limb coordination patterns and energy flow strategies during the sit-to-stand(STS)transition in individuals with patellofemoral pain(PFP),so as to provide a theoretical evidence for the pathogenesis of PFP and subsequent formulation of treatment and rehabilitation strategies for PFP patients.Methods A totoal of 36 participants was recruited for the STS test.They were divided into the unilateral PFP group(unilateral group),bilateral PFP group(bilateral group),and control group,based on the number of limbs affected by PFP.An infrared motion capture system and a three-dimensioanl force plate were used for motion capture.Visual 3D and Matlab software were used to calculate the trunk and pelvis angles,angular velocities,linear velocities,and proximal and distal joint forces.Additionally,the angles,torques,and joint forces of the hip,knee,and ankle joints,along with the angular and linear velocities of the thigh and shank,were computed.Coupling angles was used to represent coordination patterns via vector coding;the segmental net energy integration method was used to calculate energy flow within segments at each stage.Results For the coordination pattern at frontal plane,the proximal coordination mode frequency of the pelvis-hip coordination in the flexion momentum phase(FMP)was higher in unilateral group than that in bilateral group(P=0.024).In the momentum transfer phase(MTP),the frequency of in-phase coordination in the trunk-pelvis coordination was higher in unilateral group than that in bilateral group(P=0.023),while the frequency of distal coordination was higher in control group than in that in unilateral group(P=0.032).For the knee-ankle coordination pattern,the frequency of distal coordination in control group was lower than that in unilateral and bilateral groups(P=0.025,P=0.005).In segmental energy flow,during the FMP,the energy output from the pelvis during extension phase(MP)was higher in bilateral group than that in control group(P=0.021).Conclusions PFP affects energy flow patterns and coordination patterns at frontal plane during the STS transition.Individuals in unilateral group may engage in lateral pelvic and ankle movements as a dynamic compensation for patellofemoral joint pressure,whereas individuals in bilateral group appear to increase pelvic region energy output and employ a more complex whole-body coordination pattern to compensate for functional deficits in the knee caused by PFP.
6.Feasibility study of using clinical trial individual-level data sample bank as external control to support drug and device development:taking transcatheter aortic valve replacement device as an example

Objective To explore the feasibility and corresponding implementation methods of constructing a sample resource bank based on individual-level data of completed clinical trials and using it to construct external controls for drug/device clinical trials.Methods Taking the pre-marketing clinical trial of transcatheter active valve replacement(TAVR)for the treatment of aortic valve stenosis as an example,the individual-level databases of multiple trials were standardized to form a sample bank.The original data of any trial in the sample bank were selected as the experimental group,and the remaining samples were selected as the control group.The potential confounding was handled by using the propensity score matching and stratification methods to clarify the process of constructing external controls based on the sample bank of individual-level data of clinical trials.Results This study included individual-level data of single-group trials of 4 TAVR devices,with a total of 569 subjects(59.2%male).The number of subjects in Trials 1 to 4 was 120,120,163,and 166,respectively.Propensity score matching enabled the matching of 113,117,125,and 147 subjects with comparable or similar characteristics from individual-level data from other trials,respectively,demonstrating a high matching success rate.The PS score distribution plot after stratification showed that the proportions of subjects in the experimental and control groups in strata 1 to 5 in scheme 1 were 4/103,11/103,22/92,32/87,and 51/64,respectively.For all constructed external controlled trials,a certain number of control samples with similar baseline characteristics to the experimental groups were distributed within each propensity score stratum.The results of the simulation test also reflected the potential differences between different devices in the 12-month all-cause mortality rate.Conclusions The sample bank constructed with individual-level data from clinical trials,as a high-quality data source,can serve as a source of external control for single-arm trials in the same field,and as a useful supplement to the external control scenario of real-world evidence to support drug and device development.At the same time,targeted research on research methods and bias control measures in related fields is also needed.
7.Inhibitory effect of phillyrin on myocardial fibrosis in diabetes mellitus rats by activating PI3K/AKT/eNOS signaling pathway

Objective:To study the inhibitory effect of phillyrin(PN)on myocardial fibrosis in diabetes mellitus(DM)rats and its mechanism.Methods:Sixty SD rats were randomly assigned:control group,DM group,low-dose PN group(10 mg/kg),high-dose PN group(30 mg/kg)and LY294002 group(PN 30 mg/kg+PI3K inhibitor 100 mg/kg),with 12 rats in each group,DM model was constructed by streptozotocin.After 8 weeks of drug intervention,FBG was measured with a blood glucose meter,the LVFS,LVEF,LVPWT and E/A were measured by echocardiography;HE staining and Masson staining were used to changes of myocardium,and the CVF was measured;ELISA was used to detect the Vaspin,TNF-α,IL-1β and IL-6 in myocardium;Western blot was used to detect the expressions of α-SMA,FSP-1,CD31,COL1,PI3K,p-PI3K,AKT,p-AKT,eNOS and p-eNOS in myocardium.Results:Compared with the control group,the myocardial cells in DM group were swollen,gaps were enlarged,and collagen fibers were in-creased,the levels of FBG,CVF,Vaspin,TNF-α,IL-1β,IL-6,α-SMA,FSP-1 and COL1 were increased obviously,the levels of LVFS,LVEF,LVPWT,E/A,CD31,p-PI3K/PI3K,p-AKT/AKT,p-eNOS/eNOS were reduced obviously(P<0.05).Compared with the DM group,the swelling of myocardial cells,the normal gap,and the decrease of collagen fibers in the low-dose and high-dose PN groups were reduced,the levels of FBG,CVF,Vaspin,TNF-α,IL-1β,IL-6,α-SMA,FSP-1 and COL1 were decreased obviously,the LVFS,LVEF,LVPWT,E/A,CD31,levels of p-PI3K/PI3K,p-AKT/AKT,p-eNOS/eNOS were increased obviously(P<0.05).LY294002,AKT/eNOS pathway inhibitor,obviously weakened the relieving effect of PN on myocardial fibrosis in DM rats.Conclu-sion:PN may improve cardiac function,reduce myocardial fibrosis and inhibit endothelial mesenchymal transdifferentiation(EndMT)in DM rats by activating PI3K/AKT/eNOS pathway.
8.The Application Effect of Diltiazem Hydrochloride Combined with Sacubitril Valsartan on Chronic Heart Failure after Coronary Intervention

Objective:To investigate the application effect of diltiazem hydrochloride combined with sacubitril valsartan on chronic heart failure after coronary intervention(PCI).Methods:A prospective study was conducted.The cases were included from May 2020 to May 2023.After the inclusion and exclusion criteria were deleted,a total of 88 patients with chronic heart failure after PCI met the research requirements.They were divided into two groups with the same number of cases,namely 44 cases in each group.The control group received oral treatment with sacubitril and valsartan,while the observation group received additional treatment with diltiazem hydrochloride on top of the control group.Compare the clinical efficacy,changes in cardiac function related indicators and exercise endurance before and after treatment between two groups,and conduct a 1-year follow-up of all patients to record the incidence of cardiovascular adverse events during the follow-up period.Finally,compare the changes in quality of life before and after treatment between the two groups of patients.Results:The total effective rate of the observation group was 93.18%,which was higher than the control group's 77.27%(P<0.05);After treatment,the changes of cardiac function related indexes and exercise tolerance were compared.It was found that the N-terminal B-type natriuretic peptide(NT proBNP)(45.14±6.34)pg/mL,LVEDD(51.66±3.04)mm and LVESD(32.63±4.45)mm in the observation group were lower than those in the control group,while the left ventricular ejection fraction(LVEF)(55.57±4.25)%and 6MWT(514.62±34.42)m in the observation group were higher than those in the control group(P<0.05);During the 1-year follow-up,no death occurred in the two groups.The incidence of cardiovascular adverse events in the observation group was 4.55%,much lower than 20.45%in the control group(P<0.05);After treatment,the scores of emotion,body,others and MLHFQ in the observation group(13.53±2.21,14.25±2.63,20.35±4.52,48.13±5.25)were lower than those in the control group(P<0.05).Conclusion:The combination of Diltiazem Hydrochloride and Sacubitril Valsartan has significant therapeutic effects on patients with chronic heart failure after PCI.It can improve patients' cardiac function and exercise endurance,reduce the incidence of cardiovascular adverse events,and improve their quality of life.
