1.Unilateral biportal endoscopic transforaminal lumbar interbody fusion reduces paravertebral muscle atrophy and enhances recovery compared with Wiltse-transforaminal lumbar interbody fusion in lumbar degenerative disease: a retrospective study in a Chinese cohort

Methods:
Fifty patients who underwent UBE-TLIF and 50 patients who underwent W-TLIF, each with >2 years of follow-up, were retrospectively analyzed. Outcomes included operative parameters, time to postoperative mobilization, paravertebral muscle atrophy and fat infiltration rates, clinical scores (Visual Analog Scale [VAS], Oswestry Disability Index [ODI], Japanese Orthopaedic Association [JOA]), modified Macnab criteria, fusion rates, and complications.
Results:
Compared with W-TLIF, the UBE-TLIF group had significantly less intraoperative blood loss, shorter operative times, and lower postoperative drainage volumes (p <0.05). The UBE-TLIF group showed faster postoperative recovery and shorter hospital stays. At 6 months, 1 year, and 2 years, W-TLIF patients had higher multifidus and erector spinae atrophy, and greater paravertebral muscle fat infiltration (p <0.05). The UBE-TLIF group also had lower VAS and ODI scores at 1 year and 2 years (p <0.05) and fewer surgical complications (6% vs. 10%). Fusion rates (94% vs. 92%) and modified Macnab outcomes (88% vs. 86%) were comparable (p >0.05).
Conclusions
UBE-TLIF is associated with reduced intraoperative trauma, quicker recovery, and fewer complications. In the long-term, it better preserves paravertebral muscle integrity and provides superior pain and functional outcomes.
2.High expression of E2F2 in clear cell renal cell carcinoma and its association with prognosis and tumor immune microenvironment

Objective To investigate the expression characteristics of the transcription factor E2F2 in clear cell renal cell carcinoma (ccRCC), its impact on patient prognosis, and its potential role in the tumor immune microenvironment, with validation using clinical samples and functional assays. Methods RNA sequencing data and clinical information of ccRCC patients were obtained from the TCGA database; GSE53757 and GSE66272 datasets were downloaded from the GEO database as external validation cohorts. The expression difference of E2F2 between ccRCC tissue and normal tissue was compared, and the relationship between E2F2 expression and clinical pathological characteristics was analyzed. Kaplan-Meier (KM) survival analysis with log-rank test was performed to evaluate the effects of E2F2 on overall survival (OS), progression-free survival (PFS), and disease-specific survival (DSS). The prognostic efficacy of E2F2 in predicting 1-, 3- and 5-year survival was evaluated using receiver operating characteristic (ROC) curve. GSEA was performed to identify E2F2-related signaling pathways, and the ESTIMATE and CIBERSORT algorithms were used to assess the relationship between E2F2 expression level and tumor immune cell infiltration and immune microenvironment. Twenty pairs of surgically resected and pathologically confirmed ccRCC tissues and matched adjacent non-tumor tissues were collected from our hospital, and immunohistochemistry (IHC) was performed to detect the protein expression of E2F2. Human ccRCC cell line 786-O was used for functional assays;shRNA was used to knock down E2F2 expression (constructing stable shE2F2#1 and shE2F2#2 cell lines), and qRT-PCR and Western blot were performed to verify knockdown efficiency, followed by MTT, colony formation, and Transwell migration assays to evaluate the effects of E2F2 on the biological behaviors of ccRCC cells. Results E2F2 was significantly upregulated in ccRCC tissues (the same results in the verification set). Higher E2F2 expression was associated with advanced histological grade, clinical stage and higher TNM classification (P<0.05). KM analysis showed that patients in the E2F2 high-expression group had worse OS, PFS, and DSS (P=0.019, 0.036, <0.001); the ROC curves showed area under the curve (AUC) of E2F2 of predicting 1-, 3- and 5- year survival of ccRCC patients were 0.844, 0.851 and 0.815, respectively. GSEA revealed that the E2F2 low-expression group was enriched in multiple metabolism-related pathways, whereas the E2F2 high-expression group was associated with immune-related pathways. Immune analysis demonstrated that high E2F2 expression was associated with increased immune scores, increased proportion of immune cells, higher tumor mutation burden, and potentially stronger immunotherapy response. IHC results showed that the E2F2 immunoreactivity score in ccRCC tissues was significantly higher than that in adjacent non-tumor tissues (P<0.05). Functional assays indicated that E2F2 knockdown significantly inhibited the proliferation, colony formation, and migration of ccRCC cells. Conclusion E2F2 is highly expressed in ccRCC and may be involved in tumorigenesis and progression through modulation of the immune microenvironment. Its expression level is closely associated with patient prognosis, and E2F2 has the potential to serve as a prognostic biomarker and immunotherapeutic target in ccRCC.
3.Current status and influencing factors of work readiness in patients returning to work after lung cancer surgery

Objective To investigate the current status of work readiness and its influencing factors among patients returning to work after lung cancer surgery. Methods A retrospective study was conducted on young and middle-aged postoperative lung cancer patients who were treated at the Department of Thoracic Surgery, West China Hospital, Sichuan University from March to September 2023 and returned to their jobs. Data were collected through a general information questionnaire, readiness for return-to-work scale (RRTW), general self-efficacy scale (GSES), and simplified coping style questionnaire (SCSQ). Univariate and multivariate logistic regression analyses were used to explore the factors affecting the work readiness of patients returning to work. Results A total of 219 patients were included, with 59 males and 160 females aged 18-60 years. Among the postoperative lung cancer patients returning to work, 73.1% were in the active maintenance stage of return-to-work readiness with a RRTW score of (17.59±1.48) points, and 26.9% were in the uncertain maintenance stage with a RRTW score of (16.22±1.50) points. Multivariate logistic regression analysis showed that patients aged≤30 years (OR=52.381), employees of enterprises and institutions (OR=7.682), agricultural, pastoral, fishery, forestry laborers (OR=15.665), positive coping (OR=5.043), those with higher self-efficacy (OR=1.157) and engaged in physically demanding work (OR=2.449) had higher return-to-work readiness, while patients with≥2 children (OR=0.055), self-payment (OR=0.044), and those working more than 40 hours per week (OR=0.122) had lower return-to-work readiness. Conclusion The return-to-work readiness of young and middle-aged postoperative lung cancer patients needs to be improved, and occupation, job nature, main coping styles, general self-efficacy and other factors are associated with return-to-work readiness.
4.Research on application of a teaching model based on three-dimensional visualization technology in cul-tivating clinical decision-making ability for severe pancreatitis in standardized residency training

Objective To explore the application effect of three-dimensional(3D)visualization technology in teaching rounds for severe pancreatitis during standardized residency training.Methods Forty residents undergoing standardized training in the Department of Pancreatic Surgery,Union Hospital,Tongji Medical College,Huazhong University of Science and Technolo-gy from February 2024 to February 2025 were selected as the study subjects and randomly divided into a traditional teaching rounds group(n=20)and a 3 D visualization teaching rounds group(n=20).The traditional group followed the standard teach-ing rounds procedure,while the 3D visualization group incorporated 3D visualization models into the standard teaching rounds.A questionnaire survey was used to assess the satisfaction of the two groups with the teaching rounds.Results Analysis of the questionnaire results showed that the 3D visualization teaching group had significantly higher scores than the traditional teaching group in overall satisfaction(4.20±0.77 vs.3.10±1.17),helpfulness in understanding the disease(4.55±0.69 vs.3.20±1.20),teaching interactivity(4.40±0.68 vs.3.05±1.19),improvement in clinical thinking ability(4.50±0.51 vs.3.05±1.23),helpfulness in clinical decision-making(4.35±0.67 vs.3.20±1.32),and assessment scores(88.55±6.35 vs.81.10±7.17),with all differences being statistically significant(P<0.05).Conclusion The 3D visualization technology en-hances residents' learning interest in teaching rounds for severe pancreatitis during standardized training,while also strengthening their understanding and mastery of professional knowledge.This teaching method received positive feedback and can be promoted in teaching rounds.

High altitude heart disease(HAHD)is a chronic mountain sickness in which the body is exposed to high altitude(>2 500 m)hypobaric hypoxia environment for a long time.HAHD has high morbidity and poor prognosis,and pulmonary hypertension is the main causative mechanism for its develop-ment.The phosphodiesterase-5 inhibitor sildenafil has become a hot drug for the treatment of pulmonary hypertension.This paper reviews the progress of HAHD and discusses the mechanism of action and effectiveness of sildenafil in the treatment of HAHD,with a view to providing a basis for the treatment of HAHD with sildenafil.
6.Clinical effect of dual mobility total hip arthroplasty for the treatment of femoral neck fracture of the hemiplegic side

Objective To investigate the clinical effects and safety of dual mobility total hip arthroplasty for the treatment of femoral neck fracture of the hemiplegic side.Methods Patients with hemiplegia and femoral neck fracture who underwent hip arthroplasty in our hospital from March 2021 to March 2023 were selected as the research objects,and they were randomly divided into the dual mobility group(dual mobility total hip arthroplasty was performed)and the hemiarthroplasty group(hemiarthroplasty was performed),with 31 cases in each group.The perioperative status,postoperative hip function and postoperative complications of patients in the two groups were analyzed and compared.Results The operative time of patients in the dual mobility group was longer than that in the hemiarthroplasty group,the difference was statistically significant(P<0.05),but there was no statistically significant difference in intraoperative blood loss,total drainage volume 3 days after operation or hospital stay between the two groups(P>0.05).One month after surgery,there was no significant difference in hip Harris score between the two groups(P>0.05).At 6 months and 12 months after surgery,the hip Harris scores of patients in the dual mobility group were better than those in the hemiarthroplasty group,and the differences were statistically significant(P<0.05).In the case of different preoperative muscle strength of the hemiplegic side,the hip Harris scores of patients in the dual mobility group at 6 months and 12 months after surgery were better than those in the hemiarthroplasty group,and the differences were statistically significant(P<0.05).The incidence of postoperative complications was 12.9%(4/31)in the dual mobility group,and it was 22.6%(7/31)in the hemiarthroplasty group,without significant difference between the two groups(P>0.05).Conclusion For patients with femoral neck fracture and decreased muscle strength of affected limb due to hemiplegia,the use of dual mobility total hip arthroplasty can achieve good postoperative function and anti-dislocation effect.
7.Development of a knowledge-attitude-practice assessment scale for home prevention and management of parastostomy hernia in ostomy patients and the test of its reliability and validity

Objective To develop a knowledge-attitude-practice assessment scale for home prevention and man-agement of parastostomy hernia in ostomy patients,and to test its reliability and validity,to provide a reliable tool for evaluating the knowledge and practice level of home prevention and management of parastostomy hernia in pa-tients with ostomy.Methods Based on the theory of knowledge,attitude and practice,the initial scale was generat-ed through literature review,semi-structured interviews,expert correspondence,research group discussion and pre-sur-vey.From May to December 2023,685 patients with ostomy from the ostomy clinic of a tertiary hospital in Tianjin were investigated by purpose sampling method,and the reliability and validity were tested.Results A total of 673 valid questionnaires were collected,the results showed that the knowledge-attitude-practice assessment scale for pre-venting and managing parastomal hernia of ostomy patients at home included 3 dimensions of knowledge,attitude and practice,with a total of 27 items.The response rates of 2 rounds of the questionnaire were both 100%;the au-thority coefficients were all 0.915;the Kendall's coefficients of concordance were 0.279 and 0.301,respectively.The content validity index of each item was 0.875~1.000;the content validity index of the average scale level was 0.988;after 2 exploratory factor analysis,the cumulative variance contribution rate was 65.920%.The results of con-firmatory factor analysis showed that the model had a good fit,with a chi-square degree of freedom ratio(/df)of 1.945,the fitting goodness index(GFI)of 0.851,the comparative goodness of fit index(CFI)of 0.919,the root means square error of approximation(RMSEA)of 0.060.The Cronbach's alpha coefficient result of the scale is 0.957,with a test-retest reliability of 0.826 and a half reliability of 0.934.Conclusion The knowledge-attitude-practice assessment scale has good reliability and validity,and can be used as an assessment tool for home prevention and management of parastostomy hernia in ostomy patients.
8.Predictive value of CHE and sST2 for short-term death in patients with myocardial infarction and heart failure

Objective:To investigate the predictive value of serum cholinesterase(CHE)and soluble growth stimula-tion gene 2 protein(sST2)for short-term death in patients with myocardial infarction and heart failure.Methods:A total of 100 patients with myocardial infarction and heart failure admitted in Nanjing Benq Hospital between March 2021 and March 2023 were screened.After 6-month follow-up,patients were grouped according to pres-ence of death.Multivariate Cox regression was used to analyze the factors associated with death during 6-month follow-up in patients with myocardial infarction and heart failure.The predictive value of CHE,sST2 and their combined detection for short-term death in patients with myocardial infarction and heart failure was analyzed by receiver operating characteristic(ROC)curve.Kaplan-Meier survival curve was used to compare short-term sur-vival rate between myocardial infarction and heart failure patients with different CHE and sST2 levels.Results:During 6-month follow-up,46 cases died.Compared to those in survival group,patients in death group had sig-nificant higher heart rate(HR)[(82.20±8.09)beats/min vs.(71.54±6.97)beats/min],mean arterial pressure(MAP)[(126.58±5.38)mmHg vs.(104.79±4.94)mmHg]and sST2[(76.48±4.82)ng/ml vs.(40.62±4.96)ng/ml],and significant lower CHE[(3.47±0.26)IU/L vs.(5.07±0.80)IU/L](P<0.001 all).Multivariate Cox regression showed that HR(HR 1.046,95%CI 1.002~1.092,P=0.040),MAP(HR 1.988,95%CI 1.298~2.455,P<0.001),and sST2(HR 1.068,95%CI 1.014~1.125,P=0.013)were independent risk factors for short-term death in patients with myocardial infarction and heart failure,while CHE was its independent protec-tive factor(HR=0.252,95%CI 0.145~0.561,P=0.023).ROC curve analysis indicated that the area under the curve(AUC)of CHE,sST2 and their combination for diagnosing short-term death in patients with myocardial in-farction and heart failure was 0.609(95%0.504~0.707),0.630(95%0.525~0.726)and 0.939(95%0.871~0.977)respectively,and the diagnostic efficacy of combined detection was significantly higher than CHE and sST2 alone(Z=5.814,5.524,P<0.001 all).Kaplan-Meier survival curve showed that the survival rate of patients with low CHE level was significantly lower than that of patients with high CHE level,and the survival rate of pa-tients with high sST2 level was significantly lower than that of patients with low sST2 level(Log-rank x2=2.415,2.354,P<0.001 all).Conclusion:CHE and sST2 were independent influencing factors for death during 6-month follow-up in patients with myocardial infarction and heart failure;their combined detection had good predictive value for short-term death in these patients.
