1.Comparison of postoperative efficacy between leuprorelin and mifepristone in the treatment of endometriosis

OBJECTIVE To compare the effects of leuprorelin and mifepristone on sex hormone levels, ovarian function, adverse reactions, and recurrence in patients with endometriosis (EMs) after surgery. METHODS A total of 178 patients who underwent surgical treatment for EMs in Ji’an Central People’s Hospital from September 2021 to September 2023 were randomly divided into the leuprorelin group and the mifepristone group, with 92 cases in each group. Medication was initiated on days 1 to 5 of the first menstrual cycle following surgery. Patients in the leuprorelin group received subcutaneous injections of Leuprorelin acetate microspheres, 3.75 mg per time, once every four weeks, for a total of 6 injections. Patients in the mifepristone group took 12.5 mg of Mifepristone capsules orally once daily for six consecutive months. Visual analog scale (VAS), serum sex hormone levels [follicle-stimulating hormone (FSH), luteinizing hormone (LH), and estradiol (E2)], ovarian function indicators [anti- Müllerian hormone (AMH), antral follicle count (AFC)], and T helper 1 cell (Th1)/Th2 shift markers [interferon-γ (IFN-γ), interleukin 2 (IL-2), IL-4, and IL-10] were compared between the two groups before surgery and after treatment. Incidence and outcome of adverse reactions and recurrence within one year were also compared between the two groups. RESULTS There were no statistically significant differences in baseline indicators between the two groups before surgery (P>0.05). After treatment, both groups showed significantly lower VAS scores for chronic pelvic pain, dysmenorrhea and dyspareunia, and significantly reduced serum FSH, LH, E2, IL-4 and IL-10 levels compared to before surgery (P<0.05), while serum IFN-γ and IL-2 levels were significantly increased (P<0.05); the leuprorelin group showed significantly greater improvements than the mifepristone group in all these indicators (P<0.05). After treatment, serum AMH levels in both groups were significantly lower than before surgery levels, while AFC was significantly increased (P<0.05); the leuprorelin group had significantly higher serum AMH levels and more AFC compared to the mifepristone group (P<0.05). There were no significant differences in the incidence of adverse reactions and outcome rates between the two groups (P>0.05). During one year of follow-up after discontinuation, the recurrence rate in the leuprorelin group was significantly lower than in the mifepristone group (1.15% vs. 10.99%, P<0.05). CONCLUSIONS Both leuprorelin and mifepristone are effective therapeutic drugs for EMs, but the former has advantages in alleviating pain, regulating serum sex hormone levels, protecting ovarian function, regulating immune function and reducing recurrence rates.
2.Prognostic Factors of Liposarcoma in Head and Neck

Objective To explore the pathogenesis and prognostic factors of liposarcoma in the head and neck region, and simultaneously analyze the efficacy of different treatment regimens. Methods A retrospective analysis was performed on all patients with primary untreated head and neck liposarcoma who were diagnosed and underwent surgical treatment at our hospital from January 2008 to January 2024. All patients were monitored during follow-up, and their prognoses were analyzed using SPSS software. Results A total of 30 patients were included in the study. Liposarcoma accounted for up to 60% of the cases in the orbit, while the remaining liposarcomas were primarily located in various interspaces of the neck. Dedifferentiated liposarcoma was the most common type, comprising 33%, while myxoid pleomorphic liposarcoma was the rarest at 4%. The tumor pathological type (P<0.001) and Ki67 (P=0.014) significantly affected the tumor control rate. However, an analysis of disease-specific survival rates revealed no significant differences across various factors (all P>0.05). Conclusion The prognosis of head and neck liposarcoma is better compared to that of liposarcomas in other parts of the body. However, myxoid pleomorphic liposarcoma, pleomorphic fat sarcoma, and high Ki67 levels are indicators of poor prognosis. Additionally, postoperative adjuvant radiotherapy does not significantly enhance disease-specific survival rates.
3.Prediction of pathological classification of ground glass nodules based on artificial intelligence CT quantitative parameters and histogram parameters

Objective:To analyze the prediction of pathological classification of ground glass nodules based on artificial intelligence computed tomography (CT) quantitative parameters combined with histogram parameters.Methods:The clinical data of 268 suspected patients with ground glass nodules admitted to Foshan Fosun Chancheng Hospital from June 2021 to June 2023 were retrospectively selected as the research subjects. They were divided into pre invasive lesions group (100 cases) and invasive lesions group(168 cases) according to pathological classification. Basic data of patients with different pathological classifications and the CT characteristics were compared, the prediction of pathological classification of ground glass nodules based on CT quantitative parameters combined and histogram parameters were analyzed by receiver operating characteristic (ROC) curve.Results:The edge and boundary of the tumor, shape of the lesion, the peripheral signs of the lesion and the boundary between the two groups had statistical differences ( P<0.05). The CT quantitative parameters of maximum diameter, lesion volume, average CT value in the invasive lesions group and pre invasive lesions group had statistical differences: (15.29 ± 3.20) cm vs. (9.75 ± 2.14) cm, (1.54 ± 0.31) cm 3 vs. (0.51 ± 0.10) cm 3, (- 328.16 ± 46.35) HU vs. (-541.25 ± 100.30) HU, P<0.05. The CT histogram parameters of inproportion of solid components, entropy and maximum CT value in the invasive lesions group and pre invasive lesions group had statistical differences: (66.39 ± 13.25)% vs. (42.65 ± 11.20)%, 4.31 ± 0.52 vs. 3.32 ± 0.39, (-75.34 ± 21.27) HU vs. (-141.72 ± 32.43)HU, P<0.05. Compared with the single prediction of CT quantitative parameters and CT histogram parameters, the combined prediction of the two parameters had higher value in predicting different pathological subtypes of ground glass nodules (the area under the curve was 0.877, P = 0.001). Conclusions:The combined detection of CT quantitative parameters and histogram parameters based on artificial intelligence can effectively evaluate the invasion status of ground glass nodules, which is beneficial for improving the detection of different pathological types of ground glass nodules.
4.Mechanism of effect of Paeoniflorin on oral submucosal fibrosis based on molecular dynamics simulation and network pharmacology

Objective To analyze the multi-target mechanism of Paeoniflorin in the intervention of oral submucosal fibrosis(OSF)systematically,based on molecular dynamics simulation and network pharmacology.Methods Identify potential targets of Paeoniflorin were predicted by using database.OSF-related disease targets and identified drug-disease intersecting targets were screened.Gene Ontology(GO)and Kyoto Encyclopedia of Genes and Genomes(KEGG)enrichment analysis were conducted to validate the molecular binding capabilities between Paeoniflorin and core targets.Finally,molecular dynamics simulations were performed to verify binding stability.Results A total of 20 overlapping targets were identified,including key genes such as transforming growth factor(TGF)-β1,interleukin(IL)-6,and tumor necrosis factor(TNF)-α.TGF-β1,IL-6,and TNF formed the core hub.The enrichment analysis revealed that the target molecules were significantly enriched in the TGF-β1,phosphatidylinositol 3-kinease-actin(PI3K-Akt),and nuclear factor κB(NF-κB)signaling pathways.Molecular docking confirmed high affinity binding of Paeoniflorin to targets including TGF-β1,while molecular dynamics simulations verified stable interactions between Paeoniflorin and both TGF-β1 and B-cell lymphoma-2 targets.Conclusion This study revealed that Paeoniflorin inhibits the inflammatory-fibrotic cascade of OSF through synergistic regulation of TGF-β1/Smad,PI3K-Akt and NF-κB pathways.

6.Analysis of prognostic factors for chondrosarcoma of the larynx

OBJECTIVE To investigate the prognostic factors of chondrosarcoma of the larynx,deeply analyze its clinical data,and provide a theoretical basis for better treatment of chondrosarcoma of the larynx.METHODS A retrospective analysis was conducted on the complete clinical data of patients with primary chondrosarcoma of the larynx admitted to the Department of Otolaryngology Head and Neck Surgery,Beijing Tongren Hospital,Capital Medical University from January 2010 to December 2024.RESULTS A total of 15 patients were included,including 11 males and 4 females,with a gender ratio of 11∶4.The average age of onset was 57.3 years,and the average clinical symptom duration was 12.2 months.The tumors were mainly located in the cricoid cartilage in 11 patients,in the arytenoid cartilage in 2 patients,and in the thyroid cartilage in 2 patients.Tumor grading showed that 7 patients were grade I and 8 were grade II.Four patients underwent transoral laser minimally invasive surgery,2 patients underwent partial laryngectomy+tracheotomy,and 9 patients underwent total laryngectomy/cervical lymph node dissection+tracheostomy.The 5-year overall survival rate was 85.7%,the 5-year disease-specific survival rate was 100%,and the 5-year local-regional control rate was 90.9%.Gender,tumor location,tumor grade,Ki-67,tumor size,and whether larynx preservation surgery was performed did not affect the local-regional control rate or disease-specific survival rate.CONCLUSION Laryngeal chondrosarcoma generally has a longer disease history and is difficult to detect.The pathological type is mostly well-differentiated.Regional or distant metastasis is rare,and the long-term survival rate is good.Surgical resection is the preferred treatment option.On the basis of not reducing the tumor control rate,surgery that prioritizes preserving laryngeal function should be given priority,while comprehensive treatment is generally not recommended.
7.Effect of positive ruminative thinking training on negative emotions in depression patients during modified electroconvulsive therapy

Objective To explore the effect of positive ruminative thinking training(PRT)on negative emotions during modified electroconvulsive therapy(MECT)in depression patients.Method A total of 80 patients with depression who received MECT at Ganzhou Third People's Hospital from February 2024 to February 2025 were selected as subjects.Using a random number table method,they were divided into control group(n=40)and observation group(n=40).The control group received standard interventions,while the observation group received PRT in addition to control group's interventions.Both groups continued treatment until the completion of MECT,with all patients receiving 8-12 sessions of MECT.Negative emotions,self-efficacy,and the positive and negative ruminative thinking(PNR)scale were compared between two groups.Before and after intervention,adverse reactions were evaluated in both groups post-treatment.Results After intervention,the scores of anxiety,depression scale,and negative factor rumination thinking in observation group were lower than those in control group.The scores of general self-efficacy scale and positive factor rumination thinking were higher than those in control group,and the differences were statistically significant(P<0.05).The total incidence of adverse reactions in observation group was lower than that in control group(P<0.05).Conclusion PRT can reduce negative emotions and negative rumination thinking in depression patients during MECT,improve positive rumination thinking and self-efficacy.
