1.Effects of continuous nursing based on Siebens domain management model in patients with Parkinson's disease after deep brain stimulation

Objective:To explore the effect of continuous nursing based on Siebens domain management model in patients with Parkinson's disease after deep brain stimulation.Methods:From January 2021 to June 2022, convenience sampling was used to select 169 Parkinson's disease patients who underwent deep brain electrical stimulation in the Functional Neurosurgery of Beijing Tiantan Hospital as the study subject. The patients were divided into a control group of 84 cases and an observation group of 85 cases using the random number table method. The control group received routine continuous nursing, while the observation group received the continuous nursing based on Siebens domain management model on the basis of the control group. This study compared the Activity of Daily Living (ADL) Scale, Morisky Medication Adherence Scale (MMAS), 39-item Parkinson Disease Questionnaire (PDQ-39), Parkinson's Disease Sleep Scale (PDSS) scores, and unexpected program control rate between two groups of patients after six months of discharge.Results:After six months of discharge, the ability of daily living score, quality of life score, and sleep quality score of both groups of patients improved compared to before intervention ( P<0.05), and the medication adherence of the observation group patients improved compared to before intervention ( P<0.05), and the differences were statistically significant. The ability of daily living score, medication adherence, quality of life score, and sleep quality score of the observation group patients after six months were higher than those of the control group, and the differences were statistically significant ( P<0.05). Within six months after discharge, the unexpected program control rate of the observation group was statistically lower than that of the control group ( P<0.05) . Conclusions:In the treatment and discharge rehabilitation process of Parkinson's patients treated with deep brain electrical stimulation, adopting continuous nursing based on the Siebens domain management model can help ensure patient medication compliance, improve postoperative sleep quality and quality of life, reduce unexpected program control frequency, and have certain clinical practice value.
2.Role of a continuously developed information traceability system in the management of foreign medical instrument packages

Objective:To investigate the role of a continuously developed information traceability system in the management of foreign medical instrument packages in central sterile supply department (CSSD).Methods:A total of 350 foreign medical instrument packages processed by CSSD during May to June 2021 in Shenzhen Hospital, University of Chinese Academy of Sciences were included in the control group. These packages were managed with the original information traceability system. A total of 375 foreign medical instrument packages processed by CSSD during July to August 2021 in Shenzhen Hospital, University of Chinese Academy of Sciences were included in the observation group. These packages were managed with a continuously developed information traceability system. Step-by-step reminding and early warning control were performed according to the problems encountered in actual operation. Quality management indexes of foreign medical instrument packages and the incidence of adverse events were compared between the two groups.Results:The percentages of good cleaning and functional integrity of the foreign medical instrument packages, functional integrity of sterile barrier after sterilization, timely monitoring of biological culture results, standardized handover, postoperative cleaning and detoxification in the observation group were 96.0% (360/375), 98.7% (370/375), 99.5% (373/375), 98.9% (371/375), and 99.7% (374/375), which were significantly higher than those in the control group [84.2% (295/350), 92.6% (325/350), 91.4% (320/350), 89.1% (312/350), 84.9% (297/350), χ2 = 28.48, 15.40, 27.72, 31.80, 58.12, all P < 0.05]. The percentages of instrument breaking, instrument mixed or damaged, wet packages, information label error, oversized and overweighed packages in the observation group were 0.8% (3/375), 0.3% (1/375), 0.8% (3/375), 0.3% (1/375), and 0.5% (2/375), respectively, which were significantly lower than those in the control group [4.3% (15/350), 4.6% (16/350), 3.4% (12/350), 10.0% (35/350), 7.7% (27/350), χ2 = 9.08, 14.65, 6.17, 36.34, 24.31, all P < 0.05]. Conclusion:A continuously developed information traceability system is very important for management of medical instrument processed in CSSD. A developmental traceability system not only continuously improves quality control of foreign medical instrument and decreases the incidence of adverse events, but also increases the responsibility and enthusiasm of staff, thereby ensuring the safety for patients.
3.Epidemiology of classic human astrovirus among infants and young children with acute diarrhea in a hospital outpatient in Shanghai, 2012-2016

Objective:To investigate the epidemiological characteristics of classic human astrovirus (HAstV) among children under five years old with acute diarrhea, and to understand the role of HAstV in children acute diarrhea.Methods:A total of 1 010 fecal specimens were collected in 1 010 outpatients under five years old with acute diarrhea admitted to Children′s Hospital of Fudan University, Shanghai from January 2012 to December 2016. Reverse transcription polymerase chain reaction (PCR) or PCR was used for screening classic HAstV, group A rotavirus, norovirus and adenovirus. Genotypes of classic HAstV were determined by nucleotide sequencing and phylogenetic tree analysis.Results:The overall positive rate of classic HAstV was 2.7%(27/1 010). The detection rates of classic HAstV from 2012 to 2016 were 6.9%(10/144), 3.5%(5/144), 2.1%(3/144), 1.5%(4/265) and 1.6%(5/313), respectively. Almost 96.3%(26/27) of children infected with HAstV were 0 to 36 months of age. The prevalence of classic HAstV infections displayed a typical autumn/winter seasonality except in 2016. All the positive classic HAstV strains were genotyped as HAstV-1 with two lineages of HAstV-1a and HAstV-1b. Among them, the lineage of HAstV-1a was the predominant subtype (63.0%, 17/27). There were 77.8%(21/27) of the children with acute diarrhea only infected with classic HAstV, whereas for the remaining cases a variety of other enteric viruses were detected (three cases co-infected with HAstV and group A rotavirus, two cases co-infected with HAstV and adenovirus, and one case co-infected with HAstV, group A rotavirus and adenovirus).Conclusions:Children infected with HAstV are mainly less than 36 months of age. Although the genotype of classic HAstV detected in this study is single, but the lineages are in a state of dynamic change. Long-time and continuous monitor for the epidemiology of classic HAstV is needed to avoid outbreak of diarrhea in children.

5.Interventional effects of comprehensive nursing program in patients with low-grade glioma in sports areas and epilepsy

Objective:To explore the effects of perioperative comprehensive nursing program on the postoperative management of patients with low-grade glioma in sports area and epilepsy.Methods:Totally 130 patients with low-grade glioma in sports areas and epilepsy who chose to undergo craniotomy at the No.12 Ward Department of Neurosurgery in Beijing Tiantan Hospital, Capital Medical University from May 2018 to April 2019 were selected and divided into the observation group ( n=74) and the control group ( n=56) according to the admission time. Patients in the observation group adopted the comprehensive nursing program, while patients in the control group received routine care. Skin redness events, falls/falling out of bed, average length of hospital stay and patient satisfaction of the two groups of patients were observed and statistically analyzed. Results:The incidence of skin redness in the observation group was lower than that in the control group (4.05% vs. 16.07%, P=0.030) ; the incidence of falls/falling out of bed in the observation group was lower than that in the control group (2.70% vs. 12.50%, P=0.015) ; the average length of hospital stay in the observation group was lower than that in the control group [ (7.6±0.3) vs. (11.3±0.5) d, P< 0.01]; and the differences were all statistically significant. Conclusions:Comprehensive nursing can reduce the incidence of skin redness and falls/falling out of bed events in patients with low-grade glioma in sports areas and epilepsy, shorten their average postoperative hospital stay, and improve their satisfaction.
6.Application of fiber Raman endoscopic probe in the diagnosis of gastric cancer.

OBJECTIVE:
To develop a fiber Raman endoscopic probe that can be integrated in a gastroscope and evaluate its value in the diagnosis of gastric cancer.
METHODS:
The Raman spectra of gastric cancer tissues and normal tissues were obtained using the fiber Raman endoscopic probe and confocal microRaman spectroscopy. After preprocessing with smoothing, baseline elimination and normalization, the spectroscopic data were analyzed by the principle component analyses combined with stechiometry. Based on the pathological results, the diagnostic accuracy, sensitiveness and specificity of Raman spectroscopy combined with stechiometry were evaluated.
RESULTS:
The fiber Raman endoscopic probe and microRaman spectroscopy revealed significantly different Raman spectra between gastric cancer tissues and normal tissues. The diagnostic accuracy, sensitiveness and specificity of the fiber Raman endoscopic probe was 80.56%, 88.89%, and 84.72% for gastric cancer, respectively.
CONCLUSIONS
The fiber Raman endoscopic probe combined with stechiometry provides an effective modality for the diagnosis of gastric cancer and can well distinguish gastric cancer tissue from normal gastric tissues.

7.Application of scenario simulation in the training of junior neurosurgery nurses

ObjectiveTo explore and evaluate the application effect of scenario simulation teaching method in the standardized training of neurosurgery junior nurses. MethodsTotally 38 junior nurses in Neurosurgery Department of Beijing Tiantan Hospital Affiliated to Capital Medical University from January 2016 to December 2018 were selected as the subjects in the study and randomly divided into control group (n=19) and observation group (n=19)by simple random sampling method. The control group was given the standardized training process of our hospital for stage training, while the observation group was added scenario simulation teaching method on that basis. The differences of theoretical and operational assessment scores, operational assessment time, emergency response ability, team cooperation ability and patient satisfaction between the two groups were compared. ResultsThe results of operation assessment, emergency response ability, patient satisfaction and team cooperation ability of junior nurses in the observation group were better than those in the control group (P< 0.05). There was no significantly statistical difference between the two groups in theoretical and operational assessment results (P>0.05). ConclusionsOn the basis of standardized training and neurosurgery scenario simulation training, the operation ability, emergency response ability,medical cooperation ability and patient satisfaction of junior neurosurgery nurses can be improved.
8.Research on the relationship between urinary microalbumin/creatinine and serum uric acid in patients with diabetic nephropathy

Objective To analyze the urine trace albumin(mALb)/creatinine(Cr) ratio and blood uric acid(UA),and other various metabolic index level in patients with diabetic nephropathy(DN),combined with clinical data such as patients' age,body mass index(BMI),course of diseases,to explore the related mechanism of occurrence and development of DN.Methods 76 DN patients were selected.The microalbuminuria group(urinary mALb/Cr<300μg/mg) had 46 cases,the clinical albuminuria group(urinary mALb/Cr≥300μg/mg) included 30 cases,another 49 diabetic patients without kidney damage were seleted as control group.The urinary mALb/Cr,blood UA,fasting blood glucose(FBG),triacylglycerol(TG),total cholesterol(TC),high-density lipoprotein(HDL),low density lipoprotein(LDL),glycosylated hemoglobin(HbA1c) levels were determined.The BMI and the length of the course of the disease calculate.Results The patients' age,course of the disease,urinary mALb/Cr,blood UA,FBG,TC,TG,LDL,HbA1c and BMI level in the clinical albuminuria group and microalbuminuria group were significantly higher than those in the control group,the differences were statistically significant (F=6.18,12.48,141.43,12.48,8.49,4.98,6.18,3.89,3.17,3.89,all P<0.05).The high uric acid hematic disease rates of the clinical albuminuria group and microalbuminuria group were 26.09% and 26.09%,which were significantly higher than 10.20% of the control group,the differences were statistically significant(x2=4.074,24.833,all P<0.05).Urinary mALb/Cr was positively correlated with age,duration,BMI,UA,TG,TC,LDL,FBG,HbA1c(r=0.120,0.299,0.148,0.340,0.157,0.149,0.103,0.487,0.103).Multiple linear stepwise regression analysis suggested that duration,blood UA,FBG were independent risk factors of urinary mALb/Cr;TG,BMI,urinary mALb/Cr were independent risk factors for blood UA.Conclusion Urinary mALb/Cr and blood UA are the independent risk factors,high uric acid hematic disease may participate in the development process of DN,and diabetes duration,UA,BMI,TG,TC,LDL,FBG,HbA1C associated with increased urinary mALb/Cr excretory DN patients,the effective monitoring can improve the symptoms of DN and quality of life.
9.Characteristics of molecular epidemiology of human astrovirus in hospitalized and outpatient children with acute diarrhea

Objective To compare the molecular epidemic characteristics of human astrovirus (HAstV) between outpatient and hospitalized children with acute diarrhea,and to investigate the relationship between HAstY infection and diarrhea in children.Methods A total of 298 cases were randomly collected from hospitalized children from January 2008 to December 2010 in Children's Hospital of Fudan University,and 360 specimens were collected from outpatients with acute diarrhea from August 2010 to July 2011.Reverse transcriptase polymerase chain reaction (RT-PCR) was used to detect rotavirus (RV),human calicivirus (HuCV),HAstV and human adenovirus (HAdV).H AstV genotype was determined by gene sequencing and phylogenetic analysis.Results Epidemiology of HAstV in hospitalized children was as follows:among the included 298 samples,HAstV was detected in 27.2% (81/298) of the patients,compared with 33.9% (42/124),33.8% (25/74) and 14.0% (14/100),respectively from 2008 to 2010.HAstV diarrhea occurred throughout the year and peaked in January,March,and April.95.1% (77/81) of the infected children were 0-35 months old.All the episodes of HAstV were mixed with other diarrhea virus infection.Molecular epidemiology of HAstV in outpatient children with diarrhea was as follows:the overall incidence of HAstV was 1.9 % among the 360 cases (7/360).The seasonal distribution of HAstV's gastroenteritis showed a peak in November.All the outpatient children were 0-35 months old.Three cases were single infection with HAstV and the others were coinfection with RV (3 cases) or HAdV (1 case).All of the detected HAstV,either in inpatients or outpatients,belonged to HAstV-1.Conclusions The detection rate of HAstV in hospitalized children is significantly higher than that in outpatients.Most HAstV infections in hospitalized children are ascribed to nosocomial infections.Most episodes of HAstV infection were accompanied with other diarrhea viruses infection.HAstY single infection is seen in outpatient children while the detection rate is very low,implying that HAstV co-infection with other viruses plays a main role in diarrhea in most instances.
10.Virtual touch tissue imaging quantification in differential diagnosis of benign and malignant TI-RADS 4 thyroid nodules

Objective To observe the value of virtual tough tissue imaging quantification (VTIQ) in differential diagnosis of thyroid imaging reporting and data system(TI-RADS) 4 thyroid nodules.Methods A total of 185 patients with 192 TI-RADS 4 nodules were included in this study.The nodules were divided into three groups according to the maximum size as follows:Group Ⅰ,the maximum size≤0.6 cm;Group Ⅱ,0.6 cm< the maximum size≤ 1.0 cm;Group Ⅲ,the maximum size > 1.0 cm.Shear wave velocities (SWV) of nodules were measured by means of VTIQ technique.With pathological diagnosis as the gold standard,SWV value of benign and malignant nodules were analyzed and ROC curve was drawn to assess the diagnostic efficiency.Results By the ROC curve test,at SWV cut-off values of 2.44 m/s for group Ⅰ and group Ⅱ,2.49 m/s for group Ⅲ,the sensitivity were 79.0 %,76.0 %,88.6%,specificity were 88.6%,89.5 %,93.7 %,accuracy were 83.5 %,81.8 %,90.1%,Youden index were 0.68,0.66,0.82,respectively.Conclusions VTIQ can reflect the hardness of TI-RADS 4 nodules,the value of the differential diagnosis of such nodules is high,convenient,noninvasive and not limited by the size of nodules.
