Cancer Research on Prevention and Treatment 2026;53(9):718-726
Objective To investigate the prognostic value of albumin-corrected anion gap (ACAG) in patients with advanced lung cancer receiving immune checkpoint inhibitors (ICIs). Methods A total of 637 patients with advanced lung cancer who received ICI treatment were enrolled. Baseline clinical data and laboratory parameters were collected, and ACAG was calculated. The optimal prognostic Cut-off value for ACAG (11.58 mmol/L) was determined using X-Tile software, and patients were divided into low (≤11.58 mmol/L) and high (>11.58 mmol/L) ACAG groups. Propensity score matching was performed to control for confounding factors. Survival curves were plotted using the Kaplan-Meier method. A Cox proportional hazards regression model was employed to analyze prognostic factors, and subgroup analyses were conducted. Results Among 637 patients, 440 patients remained in the matched cohort. Multivariate Cox analysis showed that after adjusting for clinical and laboratory parameters, ACAG >11.58 mmol/L was an independent risk factor for OS (HR=1.74, 95%CI: 1.02–2.98, P=0.044) but was not an independent predictor for PFS. After adjusting for different sets of covariates, stratified analysis further confirmed that ACAG >11.58 mmol/L was consistently associated with a significantly shortened OS. Survival curves demonstrated that the median OS in the high ACAG group (47.0 months) was significantly lower than that in the low ACAG group. Subgroup analysis indicated that the predictive value of ACAG for OS was more pronounced in males, patients aged <65 years, and patients with squamous cell carcinoma than in other patients. Conclusion In patients with advanced lung cancer treated with ICIs, a high baseline ACAG level (>11.58 mmol/L) is an independent risk factor for shortened OS but is not significantly associated with PFS. ACAG may serve as a simple and effective biological marker for assessing overall survival prognosis in patients with advanced lung cancer undergoing immunotherapy.

OBJECTIVE To investigate the cognition, attitudes, and current practices of Chinese physicians regarding biosimilars and their clinical switching, so as to provide a basis for the rational clinical application of biosimilars in China and the improvement of related policies.METHODS A national cross-sectional online questionnaire survey was conducted in 2025. Quota sampling was adopted, with quotas based on physicians’ sex, city tier, and specialty. Questionnaire invitations were distributed to licensed physicians, and physicians who had prescribed monoclonal antibody biosimilars were included for analysis. The questionnaire covered their familiarity with biosimilars, attitudes, clinical switching practices, and reasons. Data were processed using reliability and validity tests, dispersion analysis, and cross-tabulation analysis.RESULTS The questionnaire scale showed good reliability and validity, with Cronbach’s α coefficient >0.7 and KMO value >0.6. A total of 2 071 survey invitations were distributed, and 300 valid questionnaires were returned, with an effective response rate of 14.5%. Among the respondents, 75.3% had prescribed 3-5 types of monoclonal antibody biosimilars; only 35.3% were aware that China currently has no clear regulatory provisions on biosimilar switching; 46.3% and 47.3% of respondents believed that there was no difference in safety and efficacy, respectively, between biosimilars and originator products; 63.0% of respondents were concerned about the immunogenicity risk of long-term switching between originator drugs and biosimilars. A total of 92.3% of respondents had experience with biosimilar switching, 87.7% of which involved switching from originator products to biosimilars, and 28.2% of respondents had completed three or more switches in the same patient. Among the primary reasons for switching, non-medical factors accounted for 57.4%, higher than medical factors at 42.6%; “lower treatment cost after switching” was the most important reason for switching; 95.3% of respondents believed that informed consent should be obtained from patients before switching.CONCLUSIONS The surveyed physicians had basic knowledge of biosimilars, but their understanding of the relevant regulations of the national drug regulatory authority was insufficient. Although most physicians were concerned about the risks of non-medical switching, clinical switching was mostly driven by non-medical factors. It is recommended to strengthen policy education for medical professionals and standardize clinical switching behavior. The National Medical Products Administration should also promptly improve the interchangeability review standards and post-marketing surveillance system for biosimilars, so as to promote the rational use of biosimilars.

ObjectiveTo investigate the incidence, glucolipid metabolism characteristics, and associated factors of postoperative overweight/obesity in children and adolescent with craniopharyngioma. MethodsA retrospective study was conducted using follow-up data from 44 patients (aged 0-18 years) diagnosed with craniopharyngioma who underwent surgical treatment and received follow-up care at Sun Yat-sen Memorial Hospital, Sun Yat-sen University, between January 2006 and February 2024. The incidence and glucolipid metabolism characteristics of overweight/obesity were summarized, and potential influencing factors were analyzed. ResultsA total of 44 patients were enrolled. The mean age at surgery was (8.57 ± 4.25) years, and the mean age at last follow-up was (16.02 ± 5.36 )years. The mean follow-up duration was (7.45 ± 4.64) years (range, 8 months to 17.5 years). The preoperative incidence of overweight/obesity was 40.91% (predominantly overweight, 27.27%), which increased to 68.18% postoperatively (predominantly obesity, 50.00%). Postoperatively, all patients (44/44, 100%) underwent fasting plasma glucose (FPG) testing, among whom 2 (4.55%) had impaired fasting glucose (IFG). Fasting insulin (FINS) was measured in 20 patients, and insulin resistance was identified in 11 (55.00%). Oral glucose tolerance testing (OGTT) with insulin release assessment was performed in 12 patients, and all (100%) exhibited abnormal insulin secretion, including 5 (41.67%) with hyperinsulinemia and 3 (25.00%) diagnosed with diabetes mellitus (DM). All patients (44/44, 100%) underwent lipid profile assessment. The incidence of dyslipidemia was 81.82% (36/44), including elevated non-high-density lipoprotein cholesterol (29/44, 65.91%), hypertriglyceridemia (29/44, 65.91%), low high-density lipoprotein cholesterol (22/44, 50.00%), and mixed hyperlipidemia (13/44, 29.55%). Logistic regression analysis indicated that preoperative overweight/obesity and postoperative abnormal glucose metabolism were significant factors associated with postoperative overweight/obesity. Compared with normal-weight patients, those with postoperative overweight/obesity had higher FINS levels (14.22 mU/L vs. 3.57 mU/L), higher insulin resistance index(HOMA-IR) (2.88 vs. 0.52), and a higher incidence of abnormal glucose metabolism (77.78% vs. 23.08%). However, no significant difference was observed in the incidence of dyslipidemia (94.44% vs. 73.08%). ConclusionsChildren and adolescent with craniopharyngioma exhibit a higher incidence of overweight/obesity than healthy peers preoperatively, which becomes more pronounced after surgery. This is accompanied by a high incidence of glucolipid metabolic disorders, placing these patients at increased risk of metabolic syndrome, which warrants clinical attention and regular metabolic assessment. In patients who develop postoperative obesity, particular attention should be given to the risk of glucose metabolism abnormalities, and further monitoring of glucose homeostasis is recommended.
