1.Mechanism of Qili Qiangxin Capsules in Treating Chronic Heart Failure Based on Theory of Collateral Diseases

Chronic heart failure (CHF) is the terminal stage of various heart diseases, with high morbidity, mortality, and hospitalization rate. According to the theory of collateral diseases, the core pathogenesis of CHF is blood stasis of collaterals, water retention and stagnation, and pathogen accumulation in collaterals. Accordingly, the treatment should focus on reinforcing healthy Qi to warm Yang, activating blood to dredge collaterals, and excreting water to alleviate edema. On this basis, Qili Qiangxin capsules (QLQX) are created. This prescription can effectively treat chronic heart failure. Modern studies have shown that QLQX contains a variety of pharmacological components such as flavonoids, alkaloids, terpenoids, phenylpropanoids, phenolic acids, and cardiac glycosides. QLQX can improve the cardiac function, inhibit myocardial fibrosis, improve hemodynamics, mitigate inflammation, reduce cardiomyocyte apoptosis, and regulate the nervous system, with mild adverse reactions. This study analyzed the etiology and pathogenesis of CHF based on the theory of collateral diseases, explored the relationship between the prescription and syndrome, and delved into the material basis and mechanism of QLQX in the treatment of CHF, aiming to provide reference for the clinical application and scientific research of QLQX.

ln the context of the high-quality development of public hospitals,research management is transitioning from a results-oriented approach to a full-process management model,necessitating the exploration of a governance system that integrates topic guidance,process support,and outcome transformation.Based on the Knowledge-to-Action and SECl model,it analyzes the construction logic and application effectiveness of the nursing research rounds mechanism at a teritary grade A hospital in Hainan Province.Through a"three-tier stratification+dual-track collaboration+full-process closed-loop"design,the mechanism bridges the chain from"clinical problem identification to research path optimization to outcome transformation",significantly enhancing research participation,research plan standardization,and research output.The findings validate the practical value of the research rounds mechanism in promoting knowledge transformation,capability enhancement,and organizational knowledge retention.
3.Hematopoietic stem cell and kidney transplantation from the same donor in a patient with acute myeloid leukemia and literature review

The paper reports a 32-year-old female acute myeloid leukemia patient who developed graft-versus-host disease after paternal hematopoietic stem cell transplantation, which subsequently led to renal thrombotic microangiopathy. She subsequently required a kidney transplant from the same donor 5 years later due to renal failure. Considering that both the bone marrow and kidney were from the same donor and the recovery of renal function was favorable, immunosuppressive therapy was discontinued after a short course of anti-rejection treatment, with maintained stable kidney function. This case suggests that under the condition of high chimerism, allogeneic hematopoietic stem cell transplantation and kidney transplantation from the same donor can achieve immune tolerance, potentially improving solid organ transplantation success rate. The findings provide a novel therapeutic approach for solid organ transplantation following allogeneic hematopoietic stem cell transplantation.
4.Deep Learning of Contrast-Enhanced Lung Ultrasonography for Predicting EGFR Mutation Status in Peripheral Non-Small Cell Lung Cancer

Purpose To develop an integrate model combining deep learning features from contrast-enhanced lung ultrasonography with clinical characteristics for predicting epidermal growth factor receptor mutation status in peripheral non-small cell lung cancer.Materials and Methods This retrospective study included 117 patients with pathologically confirmed non-small cell lung cancer from the First Affiliated Hospital of Guangxi Medical University(July 2021 to February 2024).Patients were randomly divided into training(n=93)and test(n=24)sets at an 8∶2 ratio.Regions of interest were delineated at the peak enhancement phase of contrast-enhanced lung ultrasonography.Various deep learning convolutional neural networks were pretrained,with ResNet18 selected as optimal for feature extraction.Deep learning,clinical,and integrated models were constructed using naive Bayesian algorithm.Performance was evaluated via receiver operating characteristic and calibration curves,while class activation mapping and Shapley additive explanation values provided model interpretability.Results In the training set,the deep learning,clinical and integrated models achieved area under the curve of 0.93(95%CI 0.88-0.98),0.86(95%CI 0.68-1.00),and 0.91(95%CI 0.85-0.97),respectively.Corresponding test set area under the curve were 0.81(95%CI 0.72-0.90),0.56(95%CI 0.33-0.80),and 0.87(95%CI 0.72-1.00).Both deep learning and integrated models significantly outperformed the clinical model in training(Z=2.380,P=0.017;Z=2.597,P=0.009)and test sets(Z=2.034,P=0.042;Z=2.577,P=0.010).The integrated model demonstrated excellent calibration and predictive performance.Conclusion The integrated model combining deep learning features from contrast-enhanced lung ultrasonography with clinical characteristics effectively predicts epidermal growth factor receptor mutation status in peripheral non-small cell lung cancer.

8.Diagnostic value for the fine needle aspiration biopsy and contrast-enhanced ultrasound in thyroid imaging reported and data system Grade 4 nodules.

To study the clinical application value of ultrasound-guided fine needle aspiration biopsy (US-FNAB) and contrast-enhanced ultrasound (CEU) in the diagnosis of thyroid imaging reported and data system Grade 4 (TI-RADS 4) nodules.
Methods: A retrospective analysis of 134 patients with thyroid nodules surgery were selected, and their results of preoperative color Doppler ultrasonography were TI-RADS 4. The data of US-FNAB and CEU before operation and the results of pathological section after operation were collected. The pathological results were taken as the gold standard, and the specimens obtained by US-FNAB puncture were used for HE staining and cytological diagnosis. The sensitivity, specificity, accuracy and the cost were calculated for CEU and US-FNAB, respectively. The diagnostic efficacy of the 2 methods was compared.
Results: Of 134 thyroid nodules, there were 131 malignant nodules (97.76%) and 3 benign ones (2.24%). The sensitivity of US-FNAB and CEU were 87.02% and 93.89% respectively. The specificity of US-FNAB and CEU were 100.00% and 66.67%. The accuracy of US-FNAB and CEU were 87.31% and 93.28% respectively. Comparisons of the diagnostic accuracy were performed by χ2 test. There was no significant difference in sensitivity between CEU and US-FNAB (P>0.05). However, the sensitivity of US-FNAB and CEU were 87.50% and 100.00%, respectively, when the maximum diameter of nodule was less than 10 mm, and there was statistical significance (P<0.05). The sensitivity of US-FNAB and CEU were 92.73% and 85.45%, respectively, when the maximum diameter of nodule was more than 10 mm, and there was no statistical significance (P>0.05). The cost and risk of US-FNAB was higher than those of CEU.
Conclusion: The sensitivity of US-FNAB is higher than that of CEU for thyroid nodules with the diameter larger than 10 mm. With high detection rate, good safety and low cost, CEU can still be used for thyroid nodules with the diameter less than 10 mm, which is diagnosed as negative nodules by US-FNAB.

9. Clinicopathologic features of myxoid adrenocortical adenomas

Objective:
To study the clinicopathologic characteristics, immunophenotype, pathologic diagnosis and differential diagnosis of myxoid adrenocortical adenomas.
Methods:
The clinical data, histological features and immunohistochemical results of 4 cases of myxoid adrenocortical adenomas were analyzed, which were collected from January 2014 to December 2016 at Guangdong General Hospital, with review of literature.
Results:
Four cases of myxoid adrenocortical adenomas were presented. The patients ages ranged from 26 to 45 years (mean =35 years). Microscopically, it showed a typical morphology, characterized by small-sized tumor cell cords or pseudo-glands embedded in an abundant extracellular myxoid matrix. Immunohistochemical staining showed tumor cells were strongly positive for Melan A, vimentin and focally for α-inhibin, one case showed strong and diffuse positivity for CAM5.2, and two cases showed diffuse positivity for synaptophysin, while negative for CgA, S-100 protein, epithelial antigen, CK7, CK20 and CKpan.
Conclusions
Myxoid adrenocortical adenomas are extremely rare, which may cause confusion with metastatic well-differentiated neuroendocrine tumours, sex cord-stromal tumoursor metanephric adenoma. Recognition of this entity would be beneficial for pathologists to avoid misdiagnosis, and unnecessary treatment.
10.A novel role for tanshinoneⅡA in modulation of dendritic cells maturation and function

Objective:Bone marrow-derived dendritic cells ( DCs) were extracted and gave tanshinone ⅡA to intervene ,and we observed the change of DCs function ,which investigate the effects of tanshinone ⅡA in immune system.Methods:Extract bone mar-row-derived DCs,and cells were cultured in complete medium with 10 ng/ml GM-CSF and IL-4.On day 5,magnetic cell sorting was used to purify DCs,and the purify must be up to 90%.Then,a certain concentration of tanshinone ⅡA or LPS was gave to the cell cul-ture,and cells and supernatant were collected for following experiments .We used flow cytometry to detect phenotype , and enzyme-linked immunosorbent assay(ELISA) was used to detect cytokine production of TNF-αand IL-12.Also,allogeneic mixed lymphocyte reaction was used to detect the ability of DCs to induce T cell proliferation and polarization .Results:When the concentration of tanshi-none ⅡA was 500 ng/ml,the inhibition of secretion of TNF-αwas maximal.So we chose that concentration .Compared with the control group,DCs in experimental group had reduced expression of CD 80,CD86 and MHCⅡ(P<0.05).Compared with the control group ,DCs in experimental group displayed the reduced level of IL-12 and TNF-α(P<0.05).Compared with the control group,DCs in experimen-tal group had reduced ability to induce T cell proliferation (P<0.05).Compared with the control group ,DCs in experimental group in-duced T cell to secret increased level of IL-4,and reduced level of IFN-γ(P<0.05).Conclusion:TanshinoneⅡA can inhibit the den-dritic cells maturation induced by LPS ,which takes part in immune system and autoimmune diseases .
