1.Mechanism by which esketamine improves postoperative cognitive impairment in rats with hip fracture through AMPK/SIRT1/PGC-1α signaling pathway

OBJECTIVE To investigate the mechanism by which esketamine improves postoperative cognitive impairment in rats with hip fracture based on the AMP-activated protein kinase (AMPK)/silencing information regulatory factor 1 (SIRT1)/ peroxisome proliferator activated-receptor-γ coactivator-1α (PGC-1α) signaling pathway. METHODS Rats with hip fracture surgery were assigned into model group, esketamine group (10 mg/kg), inhibitor group (250 μg/mL AMPK inhibitor Compound C), and esketamine+inhibitor group (10 mg/kg esketamine + 250 μg/mL Compound C), and rats undergoing sham surgery were used as the control group, with 12 rats in each group. New object recognition and Barnes maze experiments were used to E-mail:448231@163.com evaluate cognitive function in rats. The levels of serum tumor necrosis factor-α (TNF-α), interleukin-1β (IL-1β),superoxide dismutase (SOD), malondialdehyde (MDA), gamma-aminobutyric acid (GABA), dopamine (DA) and glutamate (Glu), and the apoptosis of hippocampal neurons were detected. The pathological morphology of the hippocampal tissue and the ultrastructure of mitochondria were observed. The mRNA expression of B-cell lymphoma-2 (Bcl-2) and Bcl-2-associated X protein (Bax),the mRNA and protein expression of AMPK, SIRT1 and PGC-1α, as well as the expression of phosphorylated (p)-AMPK in hippocampal tissue, were detected. RESULTS Compared with the control group, the hippocampal neurons in the model group of rats were disordered, with more neurons necrotic and swollen mitochondria;the new object recognition index, the SOD, GABA, DA levels, Bcl-2, AMPK, SIRT1 and PGC-1α mRNA expression levels, and p-AMPK, SIRT1, PGC-1α protein expression levels were significantly reduced, while the latency and number of errors for locating unknown holes, the TNF-α, IL-1β, MDA and Glu levels, neuronal cell apoptosis rate, and Bax mRNA expression levels were significantly increased/prolonged (P<0.05). Compared with the model group, the esketamine group showed reduced pathological damage to the hippocampal tissue of rats, and the new object recognition index, the SOD, GABA and DA levels, the Bcl-2, AMPK, SIRT1 and PGC-1α mRNA expression levels, and p-AMPK, SIRT1, PGC-1α protein expression levels were significantly increased,while the latency and error frequency for locating unknown holes, TNF-α, IL-1β, MDA and Glu levels, neuronal cell apoptosis rate, and Bax mRNA expression levels were significantly decreased (P<0.05);the inhibitor group showed the opposite trend of changes in these indicators compared to the esketamine group (P<0.05).AMPK inhibitor could reverse the improvement effect of esketamine on the above indicators after hip fracture surgery in rats (P<0.05). CONCLUSIONS Esketamine may improve postoperative inflammatory response and oxidative stress levels in rats with hip fracture by activating the AMPK/SIRT1/PGC-1α signaling pathway, inhibiting neuronal cell apoptosis, improving mitochondrial structure, and promoting postoperative cognitive function recovery.

The paper introduces Professor ZHUANG Lixing's academic thought on Daoqi Tongjing (directing qi to preserve essence) needling technique and the clinical experience. Based on Huangdi Neijing (the Yellow Emperor's Inner Classic), Dongyuan needling technique and Professor JIN Rui 's Daoqi Tongjing needling method, and by inheriting the valuable experience from the ancient masters and associating with his own clinical practices, Professor ZHUANG Lixing fully displayes the characteristics of principles, methods, formulas, acupoints and techniques of acupuncture in diagnosis and treatment of diseases. He integrates the thought of regulating the mind with Daoqi Tongjing needling. This needling method focuses on directing qi through mind regulation and needle manipulation, in which, the operation steps are refined. Besides, this needling method involves both the reinforcing and reducing techniques. The satisfactory effect of this needling has been obtained in clinical treatment for many disorders such as qi reversion and disharmony of yin and yang.

3.Discussion on Thoughts for Differentiation and Treatment of Gynecological Disease Following Pharmacological Theory Based on Natural Phenomena

Pharmacological theory based on natural phenomena(abbreviated as natural-phenomena-based pharmacology)belongs to the traditional theory of Chinese herbal medicine,and is originated from Huang Di Nei Jing(The Yellow Emperor's Inner Classic).By analyzing the correlation of four properties and five flavors of medicines with the seasonal changes of nature,Zhang Yuansu,a famous physician of Yuan Dynasty,put forward the view that medicines'properties accord with the features of climatic factors of nature,showing that wind has the properties of ascending and generating,heat has the properties of going up and growing,dampness has the properties of transforming and developing,dryness has the properties of descending and reaping,and cold has the properties of sinking and storing,and then established the theory of natural-phenomena-based pharmacology.As a promotion of the theory,Li Dongyuan advocated that the medication should adapt to the seasonal changes and to the ascending,descending,floating and sinking of qi movement of human body.By reviewing the viewpoints of Zhang Yuansu and Li Dongyuan on natural-phenomena-based pharmacology,this paper discussed the application of natural-phenomena-based pharmacological theory,such as wind-medicine having the properties of ascending and generating,heat-medicine having the properties of going up and growing,damp-medicine having the properties of transforming and developing,dry-medicine having the properties of descending and reaping,and cold-medicine having the properties of sinking and storing,in the identification and treatment of gynecological diseases.The unique way of classification of Chinese herbal medicines according to the theory of natural-phenomena-based pharmacology reflects the view of correspondence between human being and nature.And medication following the theory of natural-phenomena-based pharmacology accords with the thinking of traditional Chinese medicine(TCM),and is in line with the seasonal changes,which reflects the biological rhythm of the human body.Identification of gynecological diseases from the disorders of qi movement,and medication following the theory of medicinal-feature-based pharmacology for gynecological diseases in TCM will provide a novel approach to homotherapy for heteropathy of gynecological diseases in TCM.
4.Summary of Zhuang Lixing's Experience in Treating Perimenopausal Generalized Anxiety Disorder with Mind-Regulating Acupuncture Therapy

Perimenopausal generalized anxiety disorder belongs to the category of"depression syndrome"in traditional Chinese medicine.It is caused by disharmony of blood vessels,stagnation of qi,and disturbance of the mind and brain,leading to excessive or insufficient qi in the five zang organs.Based on different clinical manifestations,it can be divided into five types of depression.In clinical treatment,Professor Zhuang Lixing applies the mind-regulating acupuncture therapy,with the main acupoints being Sishenzhen,Shenting(GV24),Yintang(EX-HN3),Shenmen(HT7),and Sanyinjiao(SP6).Additional acupoints can be selected according to the patient's symptoms.During the manipulation,the"guiding qi and harmonizing essence"reinforcing and reducing technique is used to regulate emotions and guide qi to relieve depression.This method has shown excellent clinical efficacy and is suitable for clinical promotion.
5.Pulmonary Hypertension in Pulmonary Lymphangiomyomatosis:a Case Report

Pulmonary lymphangiomyomatosis(LAM)is a rare chronic progressive diffuse cystic lung disease that mainly occurs in women of reproductive age.Pulmonary hypertension is a rare complication of LAM.Currently,there is insufficient evidence on the epidemiology,pathogenesis and treatment strategy of LAM related pulmonary hypertension.We reported a case of a woman at reproductive age with shortness of breath and diagnosed with LAM by the combination of specific lung imaging features and serum vascular endothelial growth factor D.Precapillary pulmonary hypertension was confirmed by right cardiac catheterization.Her condition was stable with Sirolimus and home oxygen therapy.
6.Interstitial Lung Disease-associated Pulmonary Hypertension in a Denture Technician

Pulmonary hypertension is a rare complication of pneumoconiosis,which is caused by long term denture dusk contacting and poor protection.Here we reported a case,who was a denture technician and had been engaged in denture grinding for more than 10 years.According to the specific lung imaging findings and dust exposure history,she was diagnosed with interstitial lung disease and pneumoconiosis (probable).Precapillary pulmonary hypertension was confirmed by right cardiac catheterization.

Endometriosis(EMs)is a chronic inflammatory disease.It has been proved that inflammatory microenvironment is the key role in EMs occurance and development.Chinese medicine posits that the theory of"same disease of blood and water",which generally refers to"intermingling dampness and blood stasis",serves as a crucial pathogenesis in EMs.The inflammatory microenvironment is the main manifestation of dampness and blood stasis in EMs,while the insufficiency of vital qi results in endogenous dampness.The stagnation of blood stasis serves as the foundation for the formation of intermingling dampness and blood stasis.Based on contemporary theories in molecular biology,the macrophage-mediated inflammatory microenvironment serves as the foundation for the formation of dampness,while the participation of macrophages in angiogenesis is crucial for the formation of blood stasis.Investigating the intrinsic connection between the inflammatory microenvironment in EMs and the theory of"same disease of blood and water"can contribute to a better understanding of the pathogenesis of EMs from a novel perspective.Remodeling of the inflammatory microenvironment in endometriosis by targeting macrophages may provide a new strategy for EMs treatment.
8.Effects of coronavirus disease 2019 vaccination on seizures in patients with epilepsy

Background::Given that seizures may be triggered by vaccination, this study aimed to evaluate the risk and correlative factors of seizures in patients with epilepsy (PWE) after being vaccinated against coronavirus disease 2019 (COVID-19).Methods::This study retrospectively enrolled PWE who were vaccinated against COVID-19 in the epilepsy centers of 11 hospitals in China. We divided the PWE into two groups as follows: (1) patients who developed seizures within 14 days of vaccination were assigned to the SAV (with seizures after vaccination) group; (2) patients who were seizure-free within 14 days of vaccination were assigned to the SFAV (seizure-free after vaccination) group. To identify potential risk factors for seizure reccurence, the binary logistic regression analysis was performed. Besides, 67 PWE who had not been vaccinated were also included for elucidating the effects of vaccination on seizures recurrence, and binary logistic regression analysis was performed to determine whether vaccination would affect the recurrence rate of PWE who had drug reduction or withdrawal.Results::The study included a total of 407 patients; of which, 48 (11.8%) developed seizures within 14 days after vaccination (SAV group), whereas 359 (88.2%) remained seizure-free (SFAV group). The binary logistic regression analysis revealed that duration of seizure freedom ( P < 0.001) and withdrawal from anti-seizure medications (ASMs) or reduction in their dosage during the peri-vaccination period were significantly associated with the recurrence of seizures (odds ratio= 7.384, 95% confidence interval = 1.732–31.488, P = 0.007). In addition, 32 of 33 patients (97.0%) who were seizure-free for more than three months before vaccination and had a normal electroencephalogram before vaccination did not have any seizures within 14 days of vaccination. A total of 92 (22.6%) patients experienced non-epileptic adverse reactions after vaccination. Binary logistic regression analysis results showed that vaccine did not significantly affect the recurrence rate of PWE who had the behavior of ASMs dose reduction or withdrawal ( P = 0.143). Conclusions::PWE need protection from the COVID-19 vaccine. PWE who are seizure-free for >3 months before vaccination should be vaccinated. Whether the remaining PWE should be vaccinated depends on the local prevalence of COVID-19. Finally, PWE should avoid discontinuing ASMs or reducing their dosage during the peri-vaccination period.

METHODS:
Open the green passage, simplify the procurement process, carry out emergency procurement of related materials, ensure timely delivery of prevention materials, distribute them at different levels, and strengthen the warehouse management of prevention materials.
RESULTS:
The scheme of emergancy supplies was constantly improved, and the supply of prevention materials was completed with good quality.
CONCLUSIONS
Using scientific and efficient management methods, the supply of prevention materials in medical institutions has been guaranteed, which has experience and reference significance for the prevention and control of similar public health emergencies in the future.

10.Progress of the possible mechanism of leucine-rich repeat kinase 2 affecting thyroid cancer via regulating autophagy

Thyroid cancer is the most common malignant tumor of the endocrine system, and the incidence is increasing year by year, which seriously threatens people's health. Autophagy is a programmed mode of death that can be used as a potential target for anti-tumor therapy and plays an important regulatory role. Leucine-rich repeat kinase 2 (LRRK2) is a protein kinase encoded by PARK8 gene. The recent studies have confirmed that autophagy is closely related to thyroid cancer. This paper analyzes the possible regulatory mechanism of LRRK2 affecting thyroid cancer through autophagy, providing new ideas for basic research and clinical diagnosis and treatment of thyroid cancer.
