1.Construction and validation of a prognostic risk assessment model for lung adenocarcinoma based on miR-34 family target genes

ObjectiveTo establish a tumor prognostic risk assessment model related to target genes of the miR-34 family. MethodsTarget genes of the miR-34 family were screened, and the scores of miR-34 target genes were assessed in 16 tumor types. Univariate Cox regression analysis was used to identify the tumor type with the strongest correlation between miR-34 target gene scores and overall survival (OS). Gene Ontology (GO) and Kyoto Encyclopedia of Genes and Genomes (KEGG) analyses were performed to elucidate the functional roles and signaling pathways of miR-34 target genes. A prognostic risk model based on the miR-34 target genes was constructed using univariate Cox and LASSO regression analyses. Quantitative real-time PCR (qPCR) and dual-luciferase reporter assays were conducted to validate whether the target genes bind to miR-34 and measure their RNA expression levels in the relevant tumors. Additionally, the risk score was integrated with other clinical indicators to develop a nomogram prediction model for patient survival. ResultsA total of 65 target genes of the miR-34 family were screened. The cancer type exhibiting stronger correlation between the target gene scores and OS was lung adenocarcinoma (P = 0.003, HR= 5.150). Furthermore, miR-34 target genes were predominantly enriched in oxidative stress pathways and various tumor-related processes. Three genes, LDHA, GALNT7, and SATB2, were identified as core components of the prognostic analysis model for lung adenocarcinoma. Additionally, the constructed nomogram model demonstrated robust predictive performance. ConclusionThe risk model and prognosis model of lung adenocarcinoma constructed based on the key target genes of miR-34 have good predictive performance.

Background With rising obesity rates and earlier hypertension onset among occupational populations, there is an urgent need to elucidate the long-term cardiovascular impacts of dynamic body weight patterns. Current evidence lacks trajectory modeling studies examining occupation-specific prevention strategies. Objective To investigate the association between long-term body mass index (BMI) trajectories and incident hypertension risk in Chinese working adults, and to examine occupation-specific heterogeneity in this relationship. Objective To systematically analyze the characteristics of the disease burden of hypertensive heart disease (HHD) in the elderly (≥60 years) globally and in China from 1990 to 2021, and to predict its future trends from 2022 to 2040, with the aim of providing data support for optimizing comprehensive prevention and control strategies for HHD. Methods Based on the Global Burden of Disease (GBD) 2021 database, the number of prevalent cases and disability-adjusted life years (DALYs) of HHD in the elderly were extracted for the world, China, and five regions categorized by sociodemographic index (SDI). Joinpoint regression was used to analyze the temporal trends of age-standardized prevalence rate and age-standardized DALYs rate of HHD in the elderly. A three-factor decomposition method was applied to evaluate the relative contributions of aging, population growth, and epidemiological changes to the variations in the elderly HHD burden. Additionally, a Bayesian age-period-cohort model was used to predict the elderly HHD burden from 2022 to 2040. Results In 2021, the number of prevalent elderly HHD cases reached 10 283 000 globally and 3 412 400 in China, representing increases of 179.20% and 159.20% respectively, compared with 1990. The DALYs of elderly HHD were 18 812 700 person-years globally and 4 731 400 person-years in China, rising by 76.08% and 29.45% respectively from 1990. Meanwhile, the growth rates of the number of prevalent cases and DALYs of elderly HHD varied across different SDI regions. From 1990 to 2021, the age-standardized prevalence rate of elderly HHD in China, as well as the age-standardized DALYs rate of elderly HHD both globally and in China, showed significant downward trends (all average annual percentage changes<0, all P<0.001). In 2021, the 70-74 years age group accounted for the highest proportion of prevalent cases and DALYs of elderly HHD, both globally and in China. Decomposition analysis revealed that population growth was the dominant factor driving the increase in the elderly HHD burden across all regions. The prediction model results indicated that the number of prevalent cases and DALYs of elderly HHD would continue to rise globally and in China from 2022 to 2040, with the growth rate of the elderly HHD burden in China between 2021 and 2040 expected to exceed the global average. Conclusion Over the past 32 years, although the age-standardized disease rates of elderly HHD have mainly shown a downward trend globally and in China, the absolute number of the disease burden has increased substantially. The projection model indicates a continued upward trajectory, with the growth rate in China higher than the global average. Therefore, there is an urgent need to implement precise prevention and control strategies to effectively mitigate the disease burden of elderly HHD.
7.Evaluation Value of Blood Biomarker Tests for Efficacy of EGFR-TKI in Advanced NSCLC Treatment

Objective To analyze the levels of serum CTCs and ctDNA in NSCLC patients receiving first-line EGFR-TKI treatment, and to explore the clinical value of CTCs and ctDNA detection in assessing the efficacy of treatment for advanced lung cancer. Methods A total of 109 NSCLC patients receiving first-line EGFR-TKI treatment were enrolled. Serum tumor markers CEA, CTCs, and ctDNA were detected at baseline and after one month of treatment. Chest CT scans were performed, and treatment efficacy was evaluated based on RECIST1.1 criteria. CTCs were counted by enrichment-staining-computational algorithm to analyze malignant features, while ctDNA was assessed using digital PCR. Results Survival rate was low in patients with abnormal CEA and ctDNA tests at baseline and in patients with reduced serum CTCs after treatment. In the SD subgroup of patients with brain metastases and advanced stage, the PFS benefit was low. Conclusion Patients in the SD subgroup have significantly higher recurrence risks than those in the PR or CR subgroups. Therefore, CTC and ctDNA testing should be applied to patients in the SD subgroup to identify high-risk patients with poor response to EGFR-TKI treatment, intervene with additional treatment promptly, and obtain long progression-free survival.
8.The mechanism of miR-224-5p on proliferation, apoptosis, invasion, and migration of human hepatoma cells HepG2

Objective:
To investigate the mechanism of miR-224-5p on proliferation, apoptosis, invasion and migration of human hepatocellular carcinoma HepG2 cells.
Methods :
The RNA expression levels of miR-224-5p and early growth responsive gene 2(EGR2) in patients with hepatocellular carcinoma were obtained from the TCGA dataset. Normal human hepatocytes LO2 and hepatoma cells HepG2 were cultured in vitro, and the HepG2 cells were transfected with lentiviral vectors(knockdown of miR-224-5p), small interfering RNA fragments or overexpression vectors(interference and overexpression of EGR2). The expression levels of miR-224-5p and EGR2 in hepatocellular carcinoma cDNA chips and cells were detected by quantitative real-time PCR(qPCR). The expression level of EGR2 protein was detected by Western blot. Dual luciferase reporter gene assay was used to detect the binding of miR-224-5p to EGR2. HepG2 cells positive rate were detected by EdU assay, apoptosis rate was detected by flow cytometry, cell invasion number was detected by Transwell assay, and cell mobility was detected by scratch assay.
Results :
Compared with paracancerous tissues, the expression of miR-224-5p was increased and the expression of EGR2 mRNA decreased in HCC tissues. Compared with LO2 group, the expression of miR-224-5p in HepG2 cells increased, and the expression of EGR2 mRNA and protein decreased. Compared with the Lv-sh-NC group, the 24 h EdU positive cell rate, cell invasion number, and 48 h cell mobility of HepG2 cells in the Lv-sh-miR-224-5p group decreased, while the apoptosis rate increased. Compared with Oe-NC group, 24 h EdU positive cell rate, cell invasion number, and 48 h cell mobility of HepG2 cells in Oe-EGR2 group decreased, while apoptosis rate increased. Compared with Lv-sh-NC group, the expression of EGR2 protein in Lv-sh-miR-224-5p group increased. Compared with Lv-sh-miR-224-5p+si-NC group, 24 h EdU positive cell rate, cell invasion rate, and 48 h cell mobility of HepG2 cells in Lv-sh-miR-224-5p+si-EGR2 group increased, while apoptosis number decreased.
Conclusion
miR-224-5p can promote proliferation, invasion, and migration of HepG2 cells and inhibit apoptosis via binding with EGR2.
9.Laparoscopic modified transcystic biliary drainage for the treatment of biliary stones and diagnosis of biliary disease

Objective:To explore the safety of laparoscopic modified transcystic biliary drainage(modified C-tube technique)in the treatment of biliary stones and application of diagnosing biliary disease.Methods:A retrospective analysis was conducted on the clinical data of 68 cases of biliary stones successfully treated with laparoscopic modified C-tube technique from August 2021 to December 2023.The safety,effectiveness,and area of applications were analyzed.The reliability of the principle of the modified fixation method was verified by using an ex vivo gallbladder.Results:Three cases of ex vivo gallbladder demonstrated that the strength of the modified fixation method was reliable,and the sinus tract formed by suture after immediate extraction of the C tube could be dislocated and closed,resisting the bile outflow caused by the weight of the gallbladder.Among the 68 patients,42 were difficult biliary stones,6 were suspected common bile duct stones,and 5 were extrahepatic bile duct stones combined with intrahepatic bile duct stones.Among them,48 cases underwent choledochoscopy assisted trans-choledochal approach for stone removal,and 10 cases underwent transcystic approach stone removal,Six patients underwent simply basket exploration and removal of stones through the cystic duct(5 patients had no residual stones on postoperative C-tube angiography,1 patient had suspected residual stones,and the patient refused further examination and treatment for no symptom),and 4 patients only underwent biliary drainage through the cystic duct;two patients with retained stone passed after the use of topical nitrate drip infusion via C-tube,seven cases underwent endoscopic retrograde cholangiopancreatography(ERCP),stone removal with the assistance of a C-tube after laparoscopic surgery,and the results were uneventful.The mean surgical time was(131±44)min(76-279 min),the maximum daily drainage volume of the C-tube was(401±235)mL/d(10-1 150 mL/d),the hospital stay was(8.6±3.6)d(2-19 d),and the mean time of C-tube removal was(11±6.9)d(5-46 d).There were 14 overall complications,including 2 residual stones,and 12 C-tube related complications,comprising of 1 gradeⅢ a,2 grade Ⅱ,and 9 grade Ⅰ.There were 9 cases of C-tube related adverse events that did not cause complications,including 3 of early detachment,2 of displacement,and 4 of deep insertion.The median follow-up time after surgery was 21(2-30)months,and 5 patients had recurrent stones.Among them,4 patients had slow contrast outflow during cholangiogram,and 1 patient had obvious pancreaticobiliary reflux.55 patients underwent C-tube amylase measurement,and 9 cases showed a significant increase in bile amylase(349-44 936 U/L),suggesting the presence of pancreaticobiliary reflux.Conclusion:Laparoscopic modified C-tube technique can be effectively used in the treatment of biliary stones,which is relatively safe and can be attempted in the diagnosis of biliary diseases.
10.Research progress of autophagy-lysosomal pathway dysfunction in ocular surface diseases associated with diabetes mellitus

Autophagy-lysosomal pathway (ALP) is the degradation system that remove unfolded proteins and damaged organelles in cells, and plays an important role in maintaining intracellular homeostasis.The process of autophagy mainly includes autophagosome formation, autophagosome-lysosome fusion, and degradation of cargoes in mature lysosomes by lysosomal enzymes.Dry eye disease, meibomian gland dysfunction and keratopathy are common ocular surface diseases associated with diabetes mellitus, and clinical manifestations include dry eyes, reduced tear secretion, persistent corneal epithelial defects, neuropathy (decreased corneal sensitivity) and endothelial cell dysfunction.Aberrant expression of gene, oxidative stress and inflammation related advanced glycosylation end products and reactive oxygen species are significant pathogenesis of ocular surface diseases related to diabetes.Moreover, the above pathogenesis involves defects of autophagy regulatory gene, abnormal expression of autophagy related protein and activation of autophagy signaling pathway which lead to the defects of ALP such as autophagosome lysosome fusion disorder, accumulation of cargoes and abnormal lysosomal function, and the deficiency of autophagy further promoting the oxidative stress and release of inflammatory factors.The occurrence and development of ocular surface diseases associated with diabetes are closely related to the defects of ALP.This article reviews the basic research status between the defects of ALP and diabetic ocular surface diseases to provide new ideas for the mechanism and treatment research.
