1.Differential expression and molecular genetic characteristics of CD36 antigen in platelet donor database in Shaanxi province, China

Objective: To screen the frequency of CD36 antigen expression in platelet donor database in Shaanxi province and analyze the molecular genetic characteristics of samples with CD36 antigen deficiency and low expression. Methods: A total of 525 platelet donors samples were randomly collected during May 2023. CD36-FITC monoclonal antibody was used for immunofluorescence labeling, and flow cytometry was applied to detect the expression of CD36 antigen on platelets. For samples with CD36 antigen deficiency on platelets, the expression of CD36 on monocytes was further detected. Samples with CD36 antigen deficiency and low expression were sequenced and analyzed. Results: Among the 525 blood samples, 99.24% (521/525) showed positive expression of CD36 antigen. There were differences in the expression intensity of CD36 antigen, with low expression accounting for 3.43% (18/525) and CD36 antigen deficiency accounting for 0.76% (4/525), all of which were type Ⅱ deficiency. The exon mutation frequency of CD36 type Ⅱ deficiency and low expression samples was 31.82% (7/22), and the exon mutation types were 121-1_126delGCAAGTT, 329-330delAC, 1142T>G, 1204-1246dupl 43bp, 1221G>A, and 1228-1239delATTGTGCCTATT. All four cases of CD36 type Ⅱ deficiency had a 121-6 T>C mutation in intron 3. All CD36 low expression samples had a mutation of 282-10A>G, and 121-6T>C mutation rate was 61.1%(11/18). Conclusion: There were differences in the expression of CD36 antigen in the platelet donor database in Shaanxi province, which may be caused by multiple molecular genetic variations. The frequency of CD36 antigen deficiency in Shaanxi was lower than that of Han, Zhuang and Yao populations in southern China. This study provides references for solving the CD36 antibody mediated transfusion reaction and auxiliary treatment of diseases caused by CD36 antigen deficiency in the future. It also provides a basis for investigating the molecular mechanisms of CD36 deficiency and low expression.

The rapid development of artificial intelligence (AI), especially generative AI (GenAI), has already brought, and will continue to bring, revolutionary changes to our daily production and life, as well as create new opportunities and challenges for diagnostic and therapeutic practices in the medical field. Haihe Hospital of Tianjin University collaborates with the National Supercomputer Center in Tianjin, Tianjin University, and other institutions to carry out research in areas such as smart healthcare, smart services, and smart management. We have conducted research and development of a full-process disease diagnosis and treatment assistance system based on GenAI in the field of smart healthcare. The development of this project is of great significance. The first goal is to upgrade and transform the hospital's information center, organically integrate it with existing information systems, and provide the necessary computing power storage support for intelligent services within the hospital. We have implemented the localized deployment of three models: Tianhe "Tianyuan", WiNGPT, and DeepSeek. The second is to create a digital avatar of the chief physician/chief physician's voice and image by integrating multimodal intelligent interaction technology. With generative intelligence as the core, this solution provides patients with a visual medical interaction solution. The third is to achieve deep adaptation between generative intelligence and the entire process of patient medical treatment. In this project, we have developed assistant tools such as intelligent inquiry, intelligent diagnosis and recognition, intelligent treatment plan generation, and intelligent assisted medical record generation to improve the safety, quality, and efficiency of the diagnosis and treatment process. This study introduces the content of a full-process disease diagnosis and treatment assistance system, aiming to provide references and insights for the digital transformation of the healthcare industry.

3.USP51/GRP78/ABCB1 axis confers chemoresistance through decreasing doxorubicin accumulation in triple-negative breast cancer cells.

Recent studies have indicated that the expression of ubiquitin-specific protease 51 (USP51), a novel deubiquitinating enzyme (DUB) that mediates protein degradation as part of the ubiquitin‒proteasome system (UPS), is associated with tumor progression and therapeutic resistance in multiple malignancies. However, the underlying mechanisms and signaling networks involved in USP51-mediated regulation of malignant phenotypes remain largely unknown. The present study provides evidence of USP51's functions as the prominent DUB in chemoresistant triple-negative breast cancer (TNBC) cells. At the molecular level, ectopic expression of USP51 stabilized the 78 kDa Glucose-Regulated Protein (GRP78) protein through deubiquitination, thereby increasing its expression and localization on the cell surface. Furthermore, the upregulation of cell surface GRP78 increased the activity of ATP binding cassette subfamily B member 1 (ABCB1), the main efflux pump of doxorubicin (DOX), ultimately decreasing its accumulation in TNBC cells and promoting the development of drug resistance both in vitro and in vivo. Clinically, we found significant correlations among USP51, GRP78, and ABCB1 expression in TNBC patients with chemoresistance. Elevated USP51, GRP78, and ABCB1 levels were also strongly associated with a poor patient prognosis. Importantly, we revealed an alternative intervention for specific pharmacological targeting of USP51 for TNBC cell chemosensitization. In conclusion, these findings collectively indicate that the USP51/GRP78/ABCB1 network is a key contributor to the malignant progression and chemotherapeutic resistance of TNBC cells, underscoring the pivotal role of USP51 as a novel therapeutic target for cancer management.
4.MRI-based habitat radiomics for evaluating lymph node metastasis in renal cell carcinoma

Objective:To evaluate the efficacy of preoperative prediction of regional lymph node (RLN) metastasis in renal cell carcinoma (RCC) using a machine learning model based on habitat imaging radiomics from renal MRI.Methods:This cross-sectional study retrospectively analyzed 220 patients with RCC who underwent nephrectomy and RLN dissection at four medical centers of Chinese PLA General Hospital from January 2010 to August 2023. The cohort included 65 patients with RLN metastasis and 155 without. A stratified random sampling method was used to divide 175 patients from the first medical center into a training set ( n=140) and an internal test set ( n=35) in an 8∶2 ratio, while 45 patients from the third, fourth, and fifth medical centers constituted the external test set. The primary RCC lesions were categorized into 15 habitat subregions based on corticomedullary-phase enhancement and T 2WI signal intensity on MRI, and the volume fractions of different subregions were analyzed. In the training cohort, radiomics features derived from the habitat subregions were used to construct a radiomics model employing various machine learning algorithms, including extremely random trees (ET), gradient boosting decision trees (GBDT), random forest (RF), and support vector machine (SVM). The optimal model was selected and combined with RLN short-axis diameter to develop a combined model. The efficacy of each model in predicting RLN metastasis was evaluated using the receiver operating characteristic (ROC) curve. Results:The volume fraction of hyper-enhanced hyper-intense regions in the non-metastatic group was significantly higher than that in the metastatic group (0.05±0.09 vs. 0.02±0.03; t=3.00, P=0.003). Among the machine learning models constructed using 15 optimal habitat radiomics features, the SVM model demonstrated the best performance, with area under the ROC curve (AUC) values of 0.85 (95% CI 0.72-0.98) in the internal test set and 0.82 (95% CI 0.67-0.98) in the external test set, surpassing those of the ET, GBDT, and RF models. The combined model, integrating the SVM model with RLN short-axis diameter, achieved AUC values of 0.94 (95% CI 0.85-1.00) in the internal test set and 0.89 (95% CI 0.78-1.00) in the external test set, with RLN short-axis diameter contributing AUC values of 0.81 (95% CI 0.66-0.96) and 0.81 (95% CI 0.68-0.94), respectively. The diagnostic sensitivity of the combined model was 91.7% in the internal test set and 85.7% in the external test set, with specificities of 78.3% and 67.7%, respectively. Conclusion:The combined model based on MRI habitat imaging radiomics and RLN short-axis diameter demonstrates excellent preoperative assessment capability for RLN metastasis in RCC.
5.Analysis of inhalant and food allergen-specific IgE in a children′s hosipital in Suzhou City from 2017 to 2024

To investigate the distribution of common inhalant and food allergens test results in children in Suzhou by gender, age and disease group, and to analyse the changes in allergen distribution in the different years, to provide theoretical guidance for the diagnosis, treatment and monitoring of children′s allergic diseases in Suzhou City. A retrospective cross-sectional study was conducted to analyze the 2 736 children (1 654 males and 1 082 females) who were diagnosed and tested serum content of specific immunoglobulin E (sIgE) antibodies to inhalant and food allergens in the Children′s Hospital of Soochow University between October 2017 and June 2024. The allergen sIgE positive rates epidemiological characteristics were statistically analyzed by Chi square test by grouping based on different year, gender, age and disease. The results showed that a total of 2 736 children screened for allergens, with an overall positive allergen sIgE rate of 73.06%(1 999/2 736), and the top five allergen sIgE positive rates were Dermatophagoides farinae (40.75%, 1 115/2 736), Dermatophagoides pteronyssinus (38.78%, 1 061/2 736), milk (34.65%, 948/2 736), egg whites (32.68%, 894/2 736) and molds and yeasts mixes (mx2) (24.82%, 679/2 736). The positive rates of food allergen sIgE were higher in the 2017-2019 (60.46%, 370/612) than the 2020-2022 (53.79%, 149/227) and after the 2023-2024 (48.46%, 895/1 847) ( χ2=27.059, P<0.001); The positive rates of inhaled allergen sIgE were lower in the 2017-2019 (40.03%, 245/612) than the 2020-2022 (52.71%, 146/227) and the 2023-2024 (56.04%, 1 035/1 847)( χ2=47.223, P<0.001). The positive rates of inhaled allergen sIgE in males (54.96%, 909/1 654) were higher than those in females (47.78%, 517/1 082) ( χ2=13.497, P<0.001). The total positive rate for the food allergen sIgE was highest at 1-3 years of age (67.55%, 589/872) and then gradually decreased with age ( χ2=194.095, P<0.001); The total positive rate of inhaled allergen sIgE was lowest at the age of less than 1 year (8.33%, 22/206) and then gradually increased with age ( χ2=300.329, P<0.001). The positive rates of food allergen sIgE, in descending order from high to low were asthma (59.40%), atopic dermatitis and eczema (48.10%), other groups (48.02%), allergic rhinitis (45.73%) and urticaria (40.00%); The positive rates of inhaled allergen sIgE were asthma (71.05%), allergic rhinitis (63.57%), atopic dermatitis and eczema (62.17%), urticaria (40.00%) and other groups (49.76%), and the difference between disease subgroups were statistically significant( χ2 were 64.841 and 19.055, P<0.05). In conclusion, the top four allergen sIgE positive rates for children in the Suzhou area were Dermatophagoides farinae, Dermatophagoides pteronyssinus, milk and egg white. Total sIgE positive rates for food allergens decreased progressively in the 2017-2019, 2020-2022 and 2023-2024, and total positive rates for inhalant allergens increased progressively in the 2017-2019, 2020-2022 and 2023-2024. The distribution of allergen positive varies with gender, age and disease.
6.Tocilizumab desensitization therapy of refractory systemic juvenile idiopathic arthritis: a case report and literature review

The clinical data of a child with refractory systemic juvenile idiopathic arthritis (sJIA) who was admitted to the Children′s Hospital, Zhejiang University School of Medicine in October 2023 were analyzed, and literature review was conducted.The patient was a 9-year-old boy, who was admitted due to recurrent fever, accompanied by rash and joint pain for more than 4 years.Four years ago, he was diagnosed with macrophage activation syndrome and sJIA.In the past 4 years, the condition recurred 3 times under treatment with glucocorticoids, Cyclosporine, and Methotrexate.He previously experienced allergic shock after 5 minutes of infusion of Tocilizumab.Due to Tocilizumab allergy, he was treated with Janus kinase inhibitors, but the disease still recurred.After informed consent was obtained from the guardian, 240 mg of Tocilizumab was administrated for desensitization once every 2 weeks and the process went smoothly.The patient has been followed up for more than 6 months and no allergic reactions had occurred.The condition was well controlled, and the Prednisone acetate tablet had been reduced to 2.5 mg orally once a day.At present, there are 10 patients undergoing desensitization treatment both domestically and internationally, including 2 males and 8 females, aged between 2.5 and 54.0 years.All of them have achieved a success in desensitization.Therefore, Tocilizumab desensitization therapy is of great significance for patients who are allergic to biologics in clinical practice.
7.Retrospective study on misidentification of bone injuries

Objective This study aims to investigate controversial cases of forensic clinical re-identification of fractures,exploring the characteristics,causes,and countermeasures related to identification errors in primary bone injuries,complications,and subsequent changes.The goal is to provide identification strategies for similar cases regarding the collection of identification materials,timing,and examination method selection,ultimately establishing a paradigm for such identifications.Methods A total of 103 cases of clinical re-identification of fractures accepted by the West China Forensic Identification Center from 2020 to 2024 were collected,and the data from initial identifications and re-identifications were retrospectively analyzed.Results Male cases accounted for 69.90%of the re-identifications,with disability grade(67.96%)and injury degree(30.10%)being the primary concerns.Individual requests represented a high proportion(92.86%)in the initial assessment of disability levels,while unit or joint requests dominated the re-assessment(92.86%).The agreement rates for disability grade and injury degree were 55.26%and 59.38%,respectively.The reassessment of disability grades primarily involved fractures of limb long bones,spine,and ribs,with 75.53%of opinions resulting in downgraded disability levels.Rib,orbital,and nasal bone fractures were the main focus in injury degree reassessments,with 84.62%of opinions indicating aggravated injuries.The consistency rates for fracture identification in disability grade assessments was 92.21%,while rates for injury degree and sequelae were 65.63%and 48.94%,respectively.Inconsistencies in identifying damage facts—including the presence of fractures,distinguishing between fresh and old fractures,and determining the nature of fractures and sequelae—were primarily noted in rib,orbital,and nasal bone fractures.The utilization rate of CT metadata in initial evaluations(25.00%)was significantly lower than in re-evaluations(95.00%).The identification time for joint mobility dysfunction after fracture in re-identifications was significantly longer than in initial identifications(P=0.0002),and the identification time for cases with agreement was significantly shorter than for cases with disagreement(P=0.036).Conclusion Image data type and identification timing are critical factors that may influence the accuracy and consistency of forensic clinical identification of bone injuries.When necessary,dynamic CT metadata in conjunction with image post-processing technology can be routinely employed to identify fractures of the ribs,orbital bones,or nasal bones,thereby reducing the risk of misidentification.
