1.Targeted therapies and immunotherapies for unresectable cholangiocarcinoma.

Cholangiocarcinoma (CCA) is a fatal malignancy with steadily increasing incidence and poor prognosis. Since most CCA cases are diagnosed at an advanced stage, systemic therapies, including chemotherapy, radiotherapy, targeted therapy, and immunotherapy, play a crucial role in the management of unresectable CCA. The recent advances in targeted therapies and immunotherapies brought more options in the clinical management of unresectable CCA. This review depicts the advances of targeted therapies and immunotherapies for unresectable CCA, summarizes crucial clinical trials, and describes the efficacy and safety of different drugs, which may help further develop precision and individualization in the clinical treatment of unresectable CCA.

2.Functional Connectivity Encodes Sound Locations by Lateralization Angles.

The ability to localize sound sources rapidly allows human beings to efficiently understand the surrounding environment. Previous studies have suggested that there is an auditory "where" pathway in the cortex for processing sound locations. The neural activation in regions along this pathway encodes sound locations by opponent hemifield coding, in which each unilateral region is activated by sounds coming from the contralateral hemifield. However, it is still unclear how these regions interact with each other to form a unified representation of the auditory space. In the present study, we investigated whether functional connectivity in the auditory "where" pathway encoded sound locations during passive listening. Participants underwent functional magnetic resonance imaging while passively listening to sounds from five distinct horizontal locations (-90°, -45°, 0°, 45°, 90°). We were able to decode sound locations from the functional connectivity patterns of the "where" pathway. Furthermore, we found that such neural representation of sound locations was primarily based on the coding of sound lateralization angles to the frontal midline. In addition, whole-brain analysis indicated that functional connectivity between occipital regions and the primary auditory cortex also encoded sound locations by lateralization angles. Overall, our results reveal a lateralization-angle-based representation of sound locations encoded by functional connectivity patterns, which could add on the activation-based opponent hemifield coding to provide a more precise representation of the auditory space.

With the growing emphasis on maternal and child oral health, the significance of managing oral health across preconception, pregnancy, and infancy stages has become increasingly apparent. Oral health challenges extend beyond affecting maternal well-being, exerting profound influences on fetal and neonatal oral development as well as immune system maturation. This expert consensus paper, developed using a modified Delphi method, reviews current research and provides recommendations on maternal and child oral health management. It underscores the critical role of comprehensive oral assessments prior to conception, diligent oral health management throughout pregnancy, and meticulous oral hygiene practices during infancy. Effective strategies should be seamlessly integrated across the life course, encompassing preconception oral assessments, systematic dental care during pregnancy, and routine infant oral hygiene. Collaborative efforts among pediatric dentists, maternal and child health workers, and obstetricians are crucial to improving outcomes and fostering clinical research, contributing to evidence-based health management strategies.

4.Effect of picroside Ⅱ on the malignant progression of non-small cell lung cancer

OBJECTIVE To investigate the effect and mechanism of picroside Ⅱ on the malignant progression of non-small cell lung cancer (NSCLC). METHODS A549 cells were divided into the control group, picroside Ⅱ low-, medium- and high- concentration groups, K6PC-5 [sphingosine kinase 1 (SPHK1) activator] group, and picroside Ⅱ high-dose+K6PC-5 group. Cell proliferation, migration and invasion were detected. Besides, the expression of proliferating cell nuclear antigen (PCNA), matrix metalloproteinase-2 (MMP-2), MMP-9, SPHK1, sphingosine-1-phosphate receptor 3 (S1PR3) and extracellular signal-regulated kinase 1/2 (ERK1/2) protein in the cells were also observed. BALB/c nude mice were subcutaneously inoculated with A549 cell suspension to establish NSCLC xenograft models. Then they were assigned to the nude mouse-control group, nude mouse-picroside Ⅱ low-, medium- and high-dose groups, nude mouse-K6PC-5 group, and nude mouse-picroside Ⅱ high-dose+K6PC-5 group (with 5 mice in each group) to investigate the effect of picroside Ⅱ on their tumor mass and volume. RESULTS Compared with the control group, the OD450 values, EdU-positive cell rates, scratch healing rates, cell invasion number, and the relative expression levels of PCNA, MMP-2, MMP-9, SPHK1, S1PR3 and ERK1/2 protein in the low-, medium- and high-concentration groups of picroside Ⅱ were significantly decreased. Compared with the nude mouse-control group, the tumor mass and volume in the nude mouse-low-, medium- and high-dose groups of picroside Ⅱ were significantly decreased or shrunk. The changes of above indicators were concentration/dose-dependent (P<0.05). The changing trend of the corresponding indicators in the K6PC-5 ZYTS181) group and the nude mouse-K6PC-5 group was opposite (P<0.05). Compared with the picroside Ⅱ high-concentration group or the nude mice-picroside Ⅱ high-dose group, the above quantitative indicators in the picroside Ⅱ high- concentration+K6PC-5 group cells and the nude mouse-picroside Ⅱ high-dose+K6PC-5 group nude mice were significantly increased or enlarged (P<0.05). CONCLUSIONS Picroside Ⅱ may inhibit the malignant progression of NSCLC by inhibiting SPHK1/sphingosine-1-phosphate/S1PR3 signaling pathway.
5. Effects of low concentration atropine on macular microcirculation in children with different degrees of myopia

AIM: To investigate the effects of 0.01% atropine eye drops on macular blood flow density and retinal thickness in children with different degrees of myopia. METHODS: This was a prospective case-control study. Sixty-four patients (112 eyes) diagnosed with myopia for the first time with 0.01% atropine eye drops before and 6 months after medication were investigated with the uncorrected distance visual acuity (UCVA), axial length (AL), spherical equivalent (SE), macular ganglion cell-inner plexiform layer thicknes (mGCIPL) using slit lamp examination and optical coherence tomography (OCT), vascular density in the macular area and the area of the avascular in the fovea using optical coherence tomography angiography (OCTA) . Changes in various indicators before and after medication were compared. RESULTS: Compared with before medication, the AL of the three groups of myopia patients increased significantly (P<0.01), the difference in low to moderate myopia group was significantly smaller than that in high myopia group. Compared with before medication, SE increased in all three groups of myopia patients, yet there was no statistically significant difference in the low - grade myopia group (P>0.05). The difference was statistically significant between the moderate myopia group and the high myopia group (P< 0.01). Compared with before medication, there was no change in intraocular pressure (IOP) among the three groups of myopic patients (P>0.05). After 6 months of medication, the central circle macular vessel density (cCVD) increased in the low myopia group and moderate myopia group (P<0.01), there was no statistically significant difference in the high myopia group (P>0.05). Before and after medication, there was no significant difference in outer circle macular vessel density (oCVD), inner circle macular vessel density (iCVD), and whole circle macular vessel density (wCVD) among the three myopia groups (P>0.05). The increase in mGCIPL was statistically significant in the low myopia group (P<0.01), but there was no statistically significant difference in the moderate myopia and high myopia groups (P>0.05). There was no significant difference in foveal avascular zone (FAZ) among the three myopia groups before and after medication (P>0.05). There was no correlation between CVD, AL, and SE in the three myopia groups (P>0.01). There was a low correlation between CVD and mGCIPL in the low myopia group (r=0.442, P<0.05), there was no correlation between CVD and mGCIPL in the moderate myopia and high myopia groups (P >0.01). CONCLUSION: 0.01% atropine can significantly reduce the rate of axial and refractive growth in children with low to moderate myopia, increase the density of central macular vessels, and increase the thickness of mGCIPL in children with low to moderate myopia.
6.ZHANG Deying's Cases of Treating Acne from Phlegm

[Objective]To summarize the characteristics and experience of Professor ZHANG Deying in treating acne from the perspective of phlegm syndrome.[Methods]By following the clinical study of Professor ZHANG's clinic,the medical records of acne treatment were sorted out,three typical cases were selected,and combined with the classical theories of traditional Chinese medicine,the unique insights and clinical experience of Professor ZHANG on phlegm syndrome theory were analyzed and summarized.[Results]According to the physical condition of people and the characteristics of acne,Professor ZHANG points out that the etiology and pathogenesis of acne are phlegm heat or phlegm fire invading the head,chest and causing local flesh rot.According to the spleen and stomach of middle-Jiao,the phlegm is a pathogenic factor of soil.In the treatment,the purpose of reducing phlegm can be achieved by reducing soil,multiplying wood to reduce soil and producing metal to eliminate soil,and then using heat-clearing drugs to clear the evil of upper-Jiao phlegm heat and cure acne.At the same time,it should be noted the relationship among the five elements,the phlegm is too excessive,easy to block the kidney water and cause kidney deficiency,the first treatment is reducing phlegm,after the removal of phlegm,tonifying the kidney can be effective,even some patients do not need to be tonified the kidney,and the kidney will slowly recover.[Conclusion]Professor ZHANG has unique understanding of the theory of phlegm syndrome,pointing out that the pathogenesis of acne is mostly phlegm fire or phlegm heat stagnation in the muscle surface,and the flesh rot.The main treatment should be to clear phlegm heat.Phlegm is the pathogenic factor of soil.Besides the method of reducing soil,according to the theory of five elements,there are also methods of multiplying wood to reduce soil and producing metal to eliminate soil.
7.Mechanism of Treatment of Hepatocellular Carcinoma with Traditional Chinese Medicine Based on Epigenetic Regulation: A Review

Hepatocellular carcinoma (HCC) is the sixth most common cancer in the world. In recent years, the clinical early diagnosis and treatment protocols of HCC have been improved, whereas the prognosis of patients is still not satisfactory, which is due to the fact that the mechanism of HCC development has not been fully elucidated. Therefore, it is of great significance to explore the molecular mechanisms and key regulatory links of hepatocellular carcinoma development to further improve the diagnosis and treatment of HCC in China. Epigenetics has become a research hotspot because of its reversibility and easy regulation. According to relevant studies, HCC involves the accumulation of multiple genetic and epigenetic changes during the initiation, promotion, and progression stages. HCC is categorized as infantile malnutrition with accumulation, hypochondriac pain, tympan ites, and abdominal mass in traditional Chinese medicine (TCM). In the treatment of HCC, TCM with low toxicity, multi-targets, and multi-mechanisms can inhibit tumor growth, alleviate the clinical symptoms, and enhance the quality of life of the patients. Chinese medicines and their active ingredients exert anti-HCC effects through epigenetic regulation of DNA methylation, histone modification, and non-coding RNA. Abnormal gene expression due to epigenetic regulation disorders is involved in all stages of HCC development. There are few studies on epigenetic regulation in TCM treatment of HCC, and there is still much room for development in basic and clinical trials. This paper reviews the mechanism of epigenetic regulation in HCC and summarizes the experimental results of TCM research on the related mechanism, with a view to providing a theoretical basis for future research on the mechanism of HCC development and clinical diagnosis and treatment of hepatocellular carcinoma with TCM.
8.Effects of Daizong Prescription on Glycogen Metabolism in White Adipose Tissue of Obese Mice

Objective To observe the effects of Daizong Prescription on glycogen metabolism in adipose tissue of obese mice;To explore its regulatory mechanism in activating browning in the white adipose tissue.Methods A obesity model was established by feeding high-fat diet to C57BL/6J mice.The obese mice were divided into model group,metformin group(0.15 g/kg),and Daizong Prescription low-(0.20 g/kg)and high-dosage(0.40 g/kg)groups.Mice fed a standard diet were set as the normal group,with 12 mice in each group.Each medication group was given corresponding drugs by gavage for 6 consecutive weeks.Body mass and fasting blood glucose were monitored,serum triglycerides(TG),total cholesterol(TC),high-density lipoprotein cholesterol(HDL-C),and low-density lipoprotein cholesterol(LDL-C)contents were measured.Brown adipose tissue from the interscapular region and white adipose tissue from the inguinal,perirenal and epididymal region were collected,the adipose tissue mass was measured,and the body fat coefficient was calculated.HE staining was performed to observe morphological changes in adipose tissue,PAS staining was used to observe glycogen distribution in adipose tissue,immunohistochemistry staining was performed to detect the expressions of Gys2,Ppp1r3c,and GSK-3β in inguinal white adipose tissue.Results Compared with the normal group,the body mass and fasting blood glucose in different time points of the model group significant increase(P<0.05,P<0.01),and serum TC and HDL-C contents significantly increased(P<0.01);the mass and body fat coefficient of white adipose tissue in inguinal,perirenal,and epididymis significantly increased(P<0.01),the cells in white adipose tissue in inguinal were hypertrophic and appeared as large vacuoles,with less glycogen accumulation,the expressions of Gys2 and Ppp1r3c significantly decreased(P<0.01).Compared with the model group,the mice in Daizong Prescription high-dosage group showed a significant decrease in body mass and fasting blood glucose at 4 and 6 weeks of administration(P<0.05,P<0.01),and the contents of serum TG,TC,HDL-C,and LDL-C were significantly decreased(P<0.01);the mass and body fat coefficient in white adipose tissue of perirenal and epididymal significantly decreased(P<0.05,P<0.01),and the mass of inguinal white adipose tissue significantly decreased(P<0.05),multiple irregularly shaped small vacuoles could be seen in inguinal white adipose tissue,accompanied by nuclear aggregation and increased glycogen accumulation,the expressions of Gys2 and Ppp1r3c significantly increased(P<0.01).There was no significant difference in the expression of GSK-3β inguinal white adipose tissue of mice among the groups.Conclusion Daizong Prescription can increase the activity of Gys2 by upregulating the expression of Ppp1r3c,promote glycogen synthesis,induce browning of adipose tissue,increase fat heat production,and improve obesity and related disorders of glycolipid metabolism.
9.Levels and clinical significance of serum miR-651 and miR-630 in patients with cervical cancer

Objective To investigate the levels and clinical significance of serum microRNA(miR)-651 and miR-630 in patients with cervical cancer.Methods From June 2017 to May 2020,108 cervical cancer patients accepted by our hospital were collected as the cervical cancer group.Meantime,100 cervical intraepithelial neoplasia(CIN)patients treated in our hospital were regarded as the CIN group,and 110 healthy individuals who underwent physical examination were regarded as the control group.Real-time fluorescence quantitative PCR(qRT-PCR)was applied to detect serum levels of miR-651 and miR-630,while analyzing the relationship between serum miR-651 and miR-630 as well as clinical features and prognosis of patients.Results The serum levels of miR-651 and miR-630 in the cervical cancer group and CIN group were obviously lower than those in the control group(P<0.05),while the serum levels of miR-651 and miR-630 in the cervical cancer group were obviously lower than those in the CIN group(P<0.05).The expression levels of serum miR-651 and miR-630 were positively corre-lated(r = 0.542,P<0.05).The serum levels of miR-651 and miR-630 were related to HPV infection,differentia-tion,lymph node metastasis,and FIGO staging(P<0.05).The overall survival rate of patients with low levels of miR-651 and miR-630 was lower than that of patients with high levels.Cox regression analysis showed that,HPV infection,degree of differentiation,lymph node metastasis,FIGO staging,miR-651,and miR-630 were all influ-encing factors for the prognosis of cervical cancer patients(P<0.05).Conclusion The serum levels of miR-651 and miR-630 in cervical cancer patients decrease,which are related to HPV infection,differentiation,lymph node metastasis,FIGO staging,and prognosis.
10.Nursing care of a child with monocytopenia and mycobacterial infection syndrome and pulmonary alveolar proteinosis

To summarize the nursing experience of hematopoietic stem cell transplantation for a child with monocytopenia and mycobacterium infection syndrome complicated with pulmonary alveolar proteinosis.The key aspects of nursing encompass the establishment of a multidisciplinary diagnostic and treatment team,as well as the enhancement of warehousing preparation.Fine nursing plays a pivotal role in infection prevention and control,while also promoting hematopoietic reconstruction.Vigilant monitoring of condition changes is crucial to proactively prevent cardiopulmonary failure.Moreover,an intensified management approach towards underlying diseases should be implemented alongside predictive nursing interventions.The utilization of precision medication plans allows for the observation of drug efficacy and adverse reactions.Narrative nursing serves as a foundation to alleviate the child's inner concems,while personalized follow-up plans ensure the continuity of high-quality care.With careful treatment and care,the child successfully underwent hematopoietic stem cell transplantation.After 41 days post-transplantation,the child was discharged from the hospital after a successful recovery.The follow-up after a month showed good progress.
