1.Qualitative research on the practice status of community nurses under the background of medical union - based on the perspective of community nurses

Objective:To understand the current situation of the specialized nursing alliance team after training, and to provide countermeasures for the further construction of the specialized nurse linkage team in medical institutions.Methods:From April to July in 2024, the descriptive qualitative research method was used, 25 community nursing professionals were interviewed by the objective sampling, and the contents were analyzed and refined by the Colaizzi 7-step analysis method.Results:A total of 25 community specialist nurses were all females, aged 31 - 47 years old. Four themes and 15 sub-themes were extracted: internal benefits after the linkage of the specialized nursing alliance team, external benefits after the linkage of the specialized nursing alliance team, existing difficulties in the construction of the specialized nursing alliance team, and future needs of the specialized nursing alliance team construction. Based on this analysis, the present situation of the team construction of specialized nursing alliance is made.Conclusions:Under the background of medical union, the team construction of specialized nursing alliance meets the needs of patients and policy trends, and has achieved phased results. In the future, it is still necessary to further improve the professional ability of community specialized nurses and strengthen multi-channel sustainable cooperation, including reshaping the structure of medical resources, strengthening capital investment, improving the utilization rate of information technology and strengthening the assessment mechanism to promote the improvement of the specialized nursing alliance team.

3.Construction of competency index system for clinical research nurse based on the iceberg model

Objective:To construct a scientific and objective competency index system for clinical research nurse,providing a reference for establishing a systematic and specialized training system.Methods:A preliminary framework and corresponding indicators for the competency of clinical research nurse were developed through literature analysis with the guidance of the iceberg model.The expert meeting method was utilized to determine the framework,and the Delphi method and Analytic Hierarchy Process were employed to screen indicators and calculate weights.Finally,the competency index system for clinical research nurses was established.Results:In this research,9 experts participated in the expert meeting,confirming that this competency framework consists of two dimensions:knowledge skills and self-concept.The knowledge and skills dimension encompasses five roles of clinical research nurses,and based on this,four levels were further divided.In addition,the self-concept dimension is divided into three aspects.A panel of 17 experts was selected to evaluate these indicators through two Delphi rounds.The effective response rates were 100%and 88.2%,respectively.The expert authority coefficients were 0.968 and 0.963,respectively,while the Kendall's coefficient of concordance were 0.244 and 0.403,respectively(P<0.001).In the end,87 indicators for clinical research nurse competency were developed.Conclusion:Under the guidance of the iceberg model,the competency index system for clinical research nurse constructed in this study is scientific and reliable,closely aligned with clinical practice,and possesses specialized characteristics.It can be used to evaluate the competencies of clinical research nurses and provides a basis for conducting relevant training and improving position management.

5.Application of animation video in epidural anesthesia in elderly patients with preoperative visit

Objective To explore the application effect of postural animation video instruction mode in elderly patients undergoing epidural anesthesia. Methods A total of 300 patients with epidural anesthesia were registered from January 2015 to February 2016. The patients were divided into trial group and control group with 150 cases each. The trial group was preoperatively interviewed by the postural anatomy video instruction mode, and the control group was only preoperatively interviewed by oral interpretation. The epidural anesthesia effect and anxiety between the two groups were compared. Results The difference of anesthesia posture and anatomical position, the average time of epidural puncture, the success rate of one puncture and the degree of anxiety before and after mission were 90.67% (136/150), (14.50 ± 10.20) s, (425.60 ± 78.30) s, 97.33% (146/150) and (21.65 ± 6.62) points respectively. The control group was 22.00%(33/150), (112.60 ± 60.80) s, (589.40 ± 90.70)s, 82.67%(124/150) and (6.65 ± 5.40) points respectively. There were significant differences between the two groups (χ2=8.79, t=13.54-19.88, all P < 0.01). Conclusions The postural anatomical video teaching mode can relieve anxiety, improve the degree of fit of epidural anesthesia and the success rate of puncture, shorten the time of posture, thus shorten the average time of epidural puncture, improve the quality of nursing in the operating room, work efficiency.
