Objective To investigate the association between the variability of remnant cholesterol inflammatory index(RCII),a novel composite biomarker,and the risk of stroke,in order to provide a theoretical basis for stroke prevention.Methods A prospective cohort study was conducted on 38 659 Kailuan individuals who took annual physical examinations in 2006,2008,and 2010.These subjects were grouped based on the quartiles of RCII variability,which was represented by standard deviation(SD)and average real variability(ARV),and were followed up every 2 years,with the occurrence of stroke(including ischemic and hemorrhagic strokes),death,or the end of follow-up on December 31,2022 as the endpoints.Kaplan-Meier method was used to calculate the cumulative incidence rate of endpoint events across different groups,and log-rank test was used to compare the difference of cumulative incidence of endpoint events in each group.Multivariate Cox proportional hazards regression model was adopted to analyze the association between RCII variability and risk of stroke.Results Among the 38 659 participants,a total of 2 539 strokes occurred during a mean follow-up period of 11.22±2.26 years.After adjusting confounding factors,when the participants were grouped by the quartiles of RCII-SD,the hazard ratio(HR)for stroke was 1.034(95%CI:0.917～1.167,P=0.584),1.146(95%CI:1.018～1.290,P=0.025),and 1.209(95%CI:1.066～1.370,P=0.003),respectively in the Q2,Q3,and Q4 groups,when compared with the Q1 group(Ptrend<0.05).When they were grouped by the quartiles of RCII-ARV,the HR for stroke was 1.008(95%CI:0.894～1.136,P=0.901),1.109(95%CI:0.986～1.248,P=0.085),and 1.152(95%CI:1.018～1.303,P=0.025),respectively,in the Q2,Q3,and Q4 groups,when compared with the Q1 group.Furthermore,both sensitivity and stratified analyses yielded similar results.Conclusion RCII variability is significantly associated with stroke,and the risk of stroke is gradually increasing with increment of the variability.Countermeasures Relevant authorities can focus on reducing RCII variability as a central objective by establishing regular monitoring mechanism,strengthening lifestyle interventions,and standardizing dietary,exercise,and weight management in order to suppress the index fluctuations.The principle of stable lipid-lowering in medication and optimization of therapeutic regimens with stable efficacy should be emphasized to prevent the risk of additional vascular damage.

Objective To explore imaging manifestations of crossed ectopic fused kidney.Methods Data of 12 patients with crossed ectopic fused kidney were retrospectively analyzed,and the imaging manifestations of crossed ectopic fused kidney were observed.Results L type crossed ectopic fused kidney was detected in 5 cases,while inferior ectopic type(Ii type),S-shaped type and lump type(O type)ectopic fused kidney was found each in 2 cases,respectively,and doughnut or disc type(C-C type)ectopic fused kidney was noticed in 1 case.The ectopic fused kidney presented with empty one side renal cavity,while the morphology and position of the contralateral kidney were generally normal in all 12 cases,which crossed the midline and fused with the contralateral in situ kidney,including right to left crossed ectopic fusion in 8 cases and left to right crossed ectopic fusion in 4 cases.The ureters of the crossing kidney went across the midline,opened into the bladder trigone on the contralateral side in 11 cases or prostate in 1 case.Combined reproductive system abnormalities were found in 3 cases,including unicornous uterus,bicornuate uterus and cryptorchidism each in 1 case.Two cases were found complicated with abnormal inferior vena cava,including 1 case of both side inferior vena cava and 1 case of left inferior vena cava.Conclusion The imaging manifestations of crossed ectopic fused kidney had characteristics.Complete displaying of reveal fused kidney,ureteral course and opening was helpful to accurate diagnosis and classification of crossed ectopic fused kidney.

Objective To explore the application effect of enteral nutrition-related diarrhea in postoperative esophageal cancer patients.Methods Based on literature search and expert meeting,a management process for enteral nutrition-associated diarrhea in postoperative esophageal cancer patients was constructed.A convenience sampling method was used to select a total of 68 patients with enteral nutrition-related diarrhea after esophageal cancer surgery admitted to the thoracic surgery department of a tertiary A cancer hospital in Jiangsu Province.Among them,patients admitted from January 2022 to December 2022 were set as an experimental group.The experimental group was implemented the management process for enteral nutrition-associated diarrhea in postoperative esophageal cancer patients.Those admitted from January 2021 to December 2021 were set as a control group with routine nursing.Then,the time of stopping diarrhea,the King's of Stool Chart(KSC-Tr)diarrhea score,and abnormal incidence of nutrition-related indexes,electrolytes abnormalities(low sodium,low potassium,and low calcium)were compared between 2 groups.Results The time of stopping diarrhea,KSC diarrhea score after 3 days of intervention and the time to achieve target feeding volume of the experimental group were lower than those in the control group,and the difference was statistically significant(P＜0.05).There were no significant differences in hemoglobin,albumin,prealbumin after 3 days of intervention,the incidence of electrolyte abnormalities(low sodium,low potassium,and low calcium)after 3 days of intervention,and the BMI index after 7 days of intervention between 2 groups(P＞0.05).Conclusion The management process for enteral nutrition-associated diarrhea in postoperative esophageal cancer patients can reduce the time of diarrhea,improve the severity of diarrhea,and shorten the time to achieve the target feeding,but has no significant change in the incidence of electrolyte abnormalities.

Objective:To analyze the status and influencing factors of empathy fatigue in hospice nurses based on ABC-X model (A: stressor event; B:resources available to a family; C: family sperceptions of the stressor; X: likelihood of crisis), so as to provide a reliable basis for developing comprehensive intervention strategies.Methods:A total of 325 nurses engaged in hospice care in China from April 2022 to June 2022 were selected by convenient sampling method. The influencing factors of empathy fatigue of hospice care nurses were analyzed by ABC-X model (working environment, resilience and coping style). The hospice care nurses were investigated by self-made general questionnaire, Chinese version of Empathy Fatigue Scale, Simplified Coping Style Questionnaire, Coping Style Resilience Scale and Nursing Work Environment Scale. The statistical analysis was performed by SPSS.26.0 statistical software.Results:There were 316 females and 9 males with age of (33.0 ± 7.9) years old. The total score of empathy fatigue in 325 hospice nurses was (91.16 ± 9.60) points, the scores of empathy satisfaction, ocupational burnout and secondary traumatic stress were (31.35 ± 6.01), (28.43 ± 5.86), (31.38 ± 5.76) points respectively. The scores of positive coping style, negative coping style, psychological resilience and nursing working environment were (37.46 ± 5.69), (21.28 ± 6.90), (89.84 ± 16.46), (117.13 ± 19.95) points respectively. The negative predictive factor for empathy satisfaction among nurses with the professional title of palliative care ( t=-4.22, P<0.05), and the positive predictive factors for simple coping strategies, psychological resilience, and nursing work environment ( t=4.52, 3.05, 9.03, all P<0.05), could explain 56.7% of the total variation. Psychological resilience, simplified coping strategies, nursing work environment were negative predictive factors for occupational burnout among hospice nurses ( t=-6.93, -3.54, -2.51, all P<0.05), while work nature was a positive predictive factor ( t=2.36, P<0.05), which could explain 49.4% of the total variation. Simplified coping strategies, psychological resilience, and nursing work environment were all negative predictors of secondary traumatic stress in hospice nurses ( t=-5.40, -3.25, -5.95, all P<0.05), which could explain 48.8% of the total variation. Conclusions:Based on the ABC-X model, it is found that the empathic fatigue of hospice nurses is mainly affected by the nursing work environment, mental resilience and coping styles. It is necessary for nursing managers to actively take measures to improve the working environment and coping styles of nurses, enhance their mental resilience and reduce their empathic fatigue.

Oromucosal drug delivery preparations offer advantages such as convenient administration,suitability for patients with dysphagia,rapid onset of action,and avoidance of first-pass metabolism in the liver.The 2020 edition of the Chinese Pharmacopoeia,EP11.0,BP2022,USP44-NF39,and JP18 all include relevant standards for the quality control of different oromucosal drug delivery systems.This article compares the differences in general re-quirements for oromucosal formulations among different countries and provides an overview of inspection items for marketed oral mucosal formulations and those documented in pharmacopoeias both domestically and internationally.Foreign pharmacopoeias include a wide range of oromucosal drug delivery formulations,with more refined quality control measures for systemic action.These findings can serve as a reference for the improvement and enhancement of standards for oromucosal drug delivery systems in China.

Objective:To construct a core competency indicator system for oncology advanced practice nurses.Methods:This study is a cross-sectional study. A preliminary draft of the core competency indicator system for oncology advanced practice nurses was developed through literature review and expert group coordination from June to November 2022. The core competency indicator system for oncology advanced practice nurses was established using the Delphi method for expert consultation and the analytic hierarchy process.Results:A total of 54 experts from 11 hospitals and four medical schools in 10 provinces and municipalities directly under the central government across the country were included in two rounds of expert consultation. The effective response rates of the questionnaire were all 100%, with an expert authority coefficient of 0.90, Kendall coordination coefficients of 0.089 to 0.179 and 0.101 to 0.176 ( P<0.01). The final established core competency indicator system for oncology advanced practice nurses included seven primary indicators and 69 secondary indicators. Conclusions:The core competency indicator system for oncology advanced practice nurses is comprehensive and has the characteristics of specialized oncology nursing, and the construction process is scientific and reliable, laying the foundation for future training of oncology advanced practice nurses.

Objective To employ a mouse model of ABI3BP gene deletion for the detection of postnatal changes in body weight and glucose metabolism and establish a different method of creating a mouse model of low birth weight.Methods Heterozygote mice were mated to produce ABI3BP gene knockout homozygote(ABI3BP-/-)mice,heterozygote(ABI3BP+/)mice,and wild-type(WT)mice.Adult mice from all three groups were evaluated for glucose metabolism markers,including the fasting blood glucose level,glucose tolerance,and insulin tolerance.Additionally,body weight was measured at various postnatal time periods,and the weight ratio of critical organs in adulthood was calculated.Results The gene sequencing result of the polymerase chain reaction product of ABI3BP-/-mice showed that frameshift mutations occurred in the knockout region,with quantitative reverse-transcription polymerase chain reaction analysis demonstrating significantly reduced ABI3BP expression in ABI3BP-/-mice compared with that in WT mice.Notably,the birth weight of ABI3BP-/-mice(1.25±0.08 g)was markedly lower than that of WT mice(1.34±0.12 g)(P＜0.05).Conversely,the weight of adult(120 d)ABI3BP-/mice(27.70±1.93 g)was significantly higher than that of WT mice(23.64±1.34 g)(P＜0.01).The ratios of key organ weights to body weight were not significantly different between the groups(P＞0.05).Fasting blood glucose and insulin tolerance tests showed no significant variations between the groups.However,glucose tolerance tests indicated that ABI3BP-/-mice had lower blood glucose levels(15.68±7.04 mmol/L)than WT mice(23.01±5.75 mmol/L).Conclusions Deletion of the ABI3BP gene result in mice with low birth weight,poor growth recuperation,and inadequate glucose tolerance in adulthood,similar to the clinical growth traits of low-birth-weight human neonates.Therefore,this mouse model is a promising choice for the study of low birth weight.

Objective To explore the effect of acupoint application and emotional nursing in patients with advanced lung cancer undergoing chemotherapy. Methods A total of 124 patients with advanced lung cancer who underwent chemotherapy in our hospital were randomly divided into two groups, with 62 patients per group. The control group received routine care, while the observation group received acupoint application combined with emotional care. The changes in psychological state and quality of life, occurrence of nausea and vomiting before and after nursing intervention were observed in both groups. The changes of blood interleukin-4 (IL-4), vascular endothelial growth factor (VEGF) and interferon-γ (INF-γ) were detected by radioenzyme-linked immunoassay. Results Before the intervention, there was no statistically significant difference in the Hamilton Anxiety Scale (HAMA) scores and Hamilton Depression Scale (HAMD) scores between the two groups (P>0.05). After the intervention, the HAMA and HAMD scores of the observation group were lower than those of the control group (P < 0.05). Before the intervention, there was no statistically significant difference in the scores of symptom dimension and function dimension between the two groups (P>0.05). After the intervention, the score of symptom dimension in the observation group was lower than those in the control group, while the score of function dimension was higher than those in the control group (P < 0.05). After the intervention, the incidence of nausea and vomiting decreased, and the score of the European Organization for Research and Treatment of Cancer Quality of Life Questionnaire Core 30 (EORTC QLQ-C30) improved (P < 0.05). After treatment, the INF-γ levels in both groups were higher than before treatment, while VEGF and IL-4 levels were lower than before treatment (P < 0.05). After treatment, the INF-γ level in the observation group was higher than that in the control group, while the VEGF and IL-4 levels were lower than those in the control group (P < 0.05). Conclusion Acupoint application combined with emotional nursing can improve bad mood, reduce incidence of nausea and vomiting, and improve quality of life and nursing effectiveness in patients with advanced lung cancer chemotherapy.

Objective To investigate the effects of dulaglutide combined with insulin glargine on glucose and lipid metabolism, pancreatic islet function, oxidative stress and bone metabolism indicators in patients with type 2 diabetes mellitus. Methods A total of 98 patients with type 2 diabetes mellitus were randomly divided into insulin glargine group and combined group, with 49 cases in each group. The insulin glargine group was treated with insulin glargine and metformin, while the combined group was treated with dulaglutide, insulin glargine and metformin. The levels of blood glucose, blood lipids and pancreatic islet function were compared between the two groups before and after treatment; enzyme immunoassay analyzer was used to detect the levels of oxidative stress indicators; immunoturbidimetry was used to detect bone metabolism indicators; the urinary microalbumin to creatinine ratio (UACR) was calculated in both groups, and the therapeutic effect was compared between the two groups. Results After treatment, the levels of fasting plasma glucose (FPG), 2-hour postprandial glucose (2 hPG) and glycated hemoglobin (HbA1c) in the combined group were significantly lower than those in the insulin glargine group (P<0.05). After treatment, the levels of total cholesterol (TC), triglycerides (TG) and low-density lipoprotein cholesterol (LDL-C) in the combined group were significantly lower than those in the insulin glargine group, while the level of high-density lipoprotein (HDL) was significantly higher than that in the insulin glargine group (P<0.05). After treatment, the homeostatic model assessment of insulin secretion (HOMA-IS) in the combined group was significantly higher than that in the insulin glargine group, while the homeostatic model assessment of insulin resistance (HOMA-IR) was significantly lower than that in the insulin glargine group (P<0.05). After treatment, the levels of catalase (CAT) and osteocalcin (OC) in the combined group were significantly higher than those in the insulin glargine group, while the levels of lipid peroxide (LPO) and β-collagen degradation products (β-CTX) were significantly lower than those in the insulin glargine group (P<0.05). After treatment, the UACR in the combined group was (33.26±3.37) mg/g, which was significantly higher than (25.49±2.83) mg/g in the insulin glargine group (P<0.05). The total effective rate in the combined group was 93.88%, which was significantly higher than 79.59% in the insulin glargine group (P<0.05). Conclusion Dulaglutide combined with insulin glargine can effectively regulate the blood glucose and lipid levels of patients with type 2 diabetes, improve the function of pancreatic islets, alleviate the oxidative stress reaction, improve bone metabolism, and regulate the UACR level.

Objective:To establish and effect evaluate of management process of Da Vinci robotic surgical instrument based on bar code, and evaluate its application effect.Methods:This study was a randomized controlled trial using convenient sampling method. A total of 172 Da Vinci robotic surgeries from Jiangsu Cancer Hospital from September to November 2020 were selected as the control group, and 118 Da Vinci robotic surgeries from September to November 2021 were selected as the observation group. The control group adopted the traditional Da Vinci surgical instrument management mode, while the observation group adopted the management mode of Da Vinci surgical instrument based on bar code. The incidence of management problems related to Da Vinci surgical instruments, the number of reported adverse events of instruments rate and the impact of staff work effect were compared between two groups.Results:The incidence rates of non-traceable information, counting error, usage charge and bar code irregularity were 9.3% (11/118), 0, 0.8% (1/118) and 2.5% (3/118), respectively, which were lower than 26.7% (46/172), 17.4% (30/172), 12.2% (21/172) and 16.9% (29/172) of the control group. The differences were statistically significant ( χ2 values were 12.89-24.68, all P<0.01). The reporting rate of device adverse events in the control group was 0, and the observation group was 4.2% (5/118), and the difference between the two groups was statistically significant ( χ2=7.42, P<0.05). The number of sterilization package turnover sets in the observation group was 2(2, 3), which was lower than 2(2, 4) of the control group, and the difference was statistically significant ( Z=-2.50, P<0.05). The average amount of pre-purchased surgical instruments per surgical instrument in the observation group was 7 858.0 (6 712.1, 14 119.9), which was lower than that of the control group of 7 858.0 (7 858.0, 2 0385.0), and the difference was statistically significant ( Z=-2.97, P<0.05). Conclusions:The management process of Da Vinci Xi robotic surgical instrument based on bar code can reduce the incidence of related management problems, increase the reporting rate of adverse events, improve the work efficiency of the management of mechanical arm to optimize hospital of reusable medical equipment management level, thus further ensure medical safety and improve medical quality.
