Journal of Traditional Chinese Medicine 2024;65(13):1340-1345
According to the characteristics of pain and disease process, postherpetic neuralgia (PHN) can be classified into the category of "collaterals disease" in traditional Chinese medicine. The key pathogenesis of PHN includes toxin, stasis and deficiency. The main syndromes of collaterals disease are pathogenic toxin burning collaterals, static blood blocking collaterals, and collaterals depletion. Ashi points are mainly selected for treatment of PHN. For the syndrome of pathogenic toxin burning collaterals, dense fire needling is suitable for removing toxin and relieving pain. For the syndrome of static blood blocking collaterals, collateral bloodletting is suitable to remove stasis and relieve pain. For the syndrome of collaterals depletion, the combination of encircling fire needling, hair needling and transverse insertion with filiform needles, and Jiaji acupoint (EX-B2) needling is suitable for nourishing collaterals and relieving pain. In clinical practice, the most appropriate acupuncture regimen can be selected according to the differentiated syndrome of "collaterals disease", and with adoption of the holistic pattern differentiation and treatment, the clinical efficacy can be strengthened.

ObjectiveTo explore the role of efficacy evaluation methods in providing evidence for traditional Chinese medicine (TCM) clinical practice guidelines based on a demonstration study of clinical efficacy evaluation of TCM for allergic rhinitis (AR),aiming to enrich the sources of evidence for guideline development. MethodReal-world data of TCM medication for AR were collected and efficacy evaluation was carried out. SPSS 16.0 software was used to calculate the conformity of clinical syndromes, main prescriptions, and medications to the guidelines. Correlation analysis of efficacy and medications was performed according to guideline conformity to compare the differences in medications between real-world clinical applications and current guidelines. ResultA total of 198 cases were collected and the clinical medications were compared with the relevant guidelines for AR. It was found that the clinical syndrome conformity was above 70%,and in addition to the guideline syndrome, there were also syndromes in six meridians and other mixed syndromes in clinical practice. The guideline conformity of the main prescriptions used in clinical practice showed a positive correlation trend with efficacy. There were some differences between the commonly used drugs in clinical practice and those recommended by the guidelines. For example, for lung Qi deficiency and cold syndrome, drugs such as Angelicae Dahuricae Radix, Magnoliae Flos, and Schisandrae Chinensis Fructus were often used. For kidney Yang deficiency syndrome, drugs such as Atractylodis Macrocephalae Rhizoma and Ostreae Concha were often used. For spleen Qi deficiency and weakness syndrome, drugs such as Poria, Cinnamomi Ramulus, and Magnoliae Flos were used. For lung Qi deficiency and cold syndrome in children, drugs such as Chebulae Fructus, Cicadae Periostracum, Zingiberis Rhizoma, and Ginseng Radix et Rhizoma were used. For lung and spleen Qi deficiency syndrome, drugs such as Dioscoreae Rhizoma, Menthae Haplocalycis Herba, Amomi Fructus, and Zingiberis Rhizoma were used. There were also some differences in the commonly used drugs for adults and children. For example, for lung Qi deficiency and cold syndrome, adults often used Angelicae Dahuricae Radix, Magnoliae Flos, and Tribuli Fructus, while children often used Ginseng Radix et Rhizoma, Paeoniae Radix Alba, and Poria. For lung meridian hidden heat syndrome, adults often used Bupleuri Radix, Asari Radix et Rhizoma, Pinelliae Rhizoma, and Xanthii Fructus, while children often used Houttuyniae Herba, Menthae Haplocalycis Herba, Citri Reticulatae Pericarpium, and Massa Medicata Fermentata. ConclusionEffective medication regimens can be screened out based on efficacy evaluation methods, which can help supplement immediate, objective, and clinically relevant evidence of medication for the development of clinical practice guidelines in TCM from the perspective of clinical effectiveness.

Objective:To use the resting state functional network connectivity (FNC) method based on independent component analysis (ICA) to analyze the characteristics of FNC changes in patients with basal ganglia aphasia (BGA) after stroke, and to explore its occurrence and recovery mechanism under the intervention of acupuncture combined with language rehabilitation training. Methods:Using a prospective observational research method, 16 right-handed BGA patients who were treated at Beijing Hospital of Traditional Chinese Medicine, Capital Medical University from July 2021 to December 2022, as well as 14 healthy subjects matched in age, gender, education level, and handedness, were included. The resting state functional magnetic resonance imaging, demographic information, and Western Aphasia Examination data of healthy subjects and BGA patients before and after intervention were collected. The GIFT toolbox based on MATLAB platform was applied for ICA and resting state brain network FNC analysis. The FNC differences between BGA patients and healthy subjects were compared horizontally, and the FNC changes in BGA patients before and after intervention were compared vertically. Results:Compared with healthy subjects, post-stroke BGA patients showed decreased connectivity between the basal ganglia network, default network, and visual network before intervention, while increased connectivity between the auditory network, right frontoparietal network, and anterior cuneiform network; After the intervention of acupuncture combined with language rehabilitation training, the connectivity between the basal ganglia network, visual network, and anterior cuneiform network decreased, while the connectivity between the anterior convex network and bilateral frontoparietal network decreased, while the connectivity between the default network, auditory network, right frontoparietal network, and visual network increased. The BGA patient group showed enhanced connectivity between the basal ganglia network and the left frontoparietal network before and after intervention. Conclusions:The FNC changes between the basal ganglia network and other brain networks are key to reflecting the mechanism of BGA occurrence and language function recovery. Acupuncture combined with language rehabilitation training may improve language function by enhancing the connectivity between the basal ganglia network and the left frontoparietal network, and the redistribution of attention resources may also be one of the reasons for promoting language function recovery in BGA patients.

Objective:To evaluate the safety and short-term clinical postoperative functional outcomes of a novel 3D-printed porous prosthesis of the distal tibia for the bone defect after tumorectomy. Methods:From December 2017 to December 2019, a total of eight patients diagnosed with malignant bone tumor of the distal tibia were enrolled in this study. All cases received standard preoperative chemotherapy, after which osteosarcoma resection was performed and ankle arthrodesis was reconstructed using a 3D-printed prosthesis developed by our medical center. The contact surface between the distal part of the prosthesis and the talus is a 3D-printed porous surface, which is conducive to ankle fusion. The length of the prosthesis is adjusted by the conical mounting part of the modular prosthesis. The proximal part of the prosthesis can be fixed either biologically or with bone cement. At postoperative follow-up, the function of the fused ankle was assessed by radiographs and the monthly Musculoskeletal Tumor Society (MSTS) score. Results:Of the 8 patients, 5 were male and 3 were female, aged 8-29 years (mean 16.1±7.4 years), including 7 osteoblastic osteosarcomas and 1 telangiectatic osteosarcoma. Among the procedures, the mean length of osteotomy was 16 cm (11-20 cm). The method of fixation of the proximal part of the prosthesis included one case with 3D-printing of trabecular metal bone, one case with autogenous fibular graft, and six cases with bone cement. All patients were followed up for 7-39 months (mean 15.6±10.5 months). The distal prosthesis and talus were completely fused in all cases. The mean fusion duration was 4.3±0.7 months. The mean MSTS score was 84.2%±3.0% (mean 80%-90%). No tumor recurrence, wound complications, or prosthesis loosening were observed during the follow-up period. Conclusion:The novel 3D-printed distal tibial prosthesis is a safe and effective technique for reconstruction of a massive bone defect after tumorectomy of a malignant bone tumor, with high fusion rate, few complications, and satisfactory postoperative function.
