ObjectiveTo examine the prevalence of hypertension and abnormal electrocardiogram (ECG) among the noise-exposed population in Songjiang District, Shanghai, and to analyze the influencing factors for hypertension and abnormal ECG. MethodsOccupational health examination data, encompassing basic demographic information, physical examinations, audiometric testing, blood pressure measurements, and ECG assessments, were collected from 20 165 noise-exposed workers across all manufacturing enterprise in Songjiang District in 2024. Hypertension and abnormal ECG findings were identified as indicators of health impairment. Categorical data were presented as frequencies and percentages (%). Univariate chi-square tests and multivariate logistic regression analyses were employed to identify factors associated with hypertension and abnormal ECG outcomes. ResultsThe detection rates of hypertension and abnormal ECG among noise-exposed workers in the manufacturing industry in Songjiang District were 22.78% and 13.06%, respectively. Across different enterprise scales, industry categories and economic types, micro-sized enterprises (26.90%, 25.29%), furniture manufacturing enterprises (31.01%, 31.28%) and domestic-funded enterprises (23.43%, 15.57%) had the highest detection rates of hypertension and abnormal ECG. The detection rates of hypertension and abnormal ECG were relatively high among male workers, those aged ≥60 years old and those with a Body Mass Index(BMI) ≥24.0 kg·m^-2. The detection rates of hypertension and abnormal ECG in workers exposed to noise combined with other hazards such as heat stress (27.47%, 16.87%) and benzene (22.22%, 23.46%) were higher than those exposed to noise alone (21.40%, 11.60%) (P<0.01). Compared to workers in the general equipment manufacturing industry, those in the automotive manufacturing industry had a higher risk of hypertension (OR=1.284, 95%CI: 1.098‒1.501), and those in the furniture manufacturing industry had a higher risk of abnormal ECG (OR=2.615, 95%CI: 2.014‒3.385). Micro and small enterprises, BMI ≥24.0 kg·m^-2 and advanced age had potential associations with hypertension and abnormal ECG. In addition, compared to the group exposed to noise alone, the group with noise and other physical hazards had a higher risk of hypertension (OR=1.146, 95%CI: 1.071‒1.288) and abnormal ECG (OR=1.179, 95%CI: 1.072‒1.297). Compared to the normal group, the risks of hypertension and abnormal ECG in workers with abnormal high-frequency hearing threshold for both ears increased by 21.5% (OR=1.215, 95%CI: 1.082‒1.364) and 21.0% (OR=1.210, 95%CI: 1.006‒1.410), respectively. ConclusionDifferent types of employers (economic type, industry category, enterprise size) and individual characteristics of workers (gender, age, length of service, BMI, exposure to other physical hazards, and abnormal high-frequency hearing threshold in both ears) impacted the association between noise exposure and hypertension/ECG abnormalities. Employers should enhance the management of occupational noise. In addition to hearing impairment, attention should be paid to abnormal changes in blood pressure and ECG of workers, and corresponding intervention measures should be implemented to reduce the occurrence and development of cardiovascular diseases among workers exposed to noise.

OBJECTIVE:Dry needling therapy is widely used in various myofascial pain syndromes.The purpose of this study was to systematically evaluate the clinical effect of dry needling in the treatment of knee joint diseases.METHODS:PubMed,EBSCO,ScienceDirect,Web of Science,CINAHL,Cochrane Library,CNKI and other databases were searched for relevant literature.Randomized controlled trials with dry needling as the main treatment method and patients diagnosed with knee joint disease were selected.Two evaluators independently screened the articles,scored the methodological quality,and extracted the data.The main indicators were visual analog scale score,and the secondary indicators were the Western Ontario McMaster Universities(WOMAC)score,pressure pain threshold,knee mobility and Kujala score(knee function score scale).RESULTS:A total of 15 randomized controlled trials involving 698 patients were included in the Meta-analysis.The results showed that compared with non-dry needling,dry needling at myofascial trigger points had a significant advantage in visual analog scale score,the WOMAC pain score and WOMAC stiffness score[MD=-0.63,95%confidence interval(CI):-1.06 to-0.19,P=0.005;MD=-0.74,95%CI:-1.32 to-0.17,P=0.01;MD=-0.43,95%CI:-0.77 to-0.09,P=0.01).However,there was no significant advantage in WOMAC total score,WOMAC functional score,pressure pain threshold,knee mobility,and Kujala score.CONCLUSION:Dry needling can effectively treat knee pain and stiffness;however,the clinical advantages of dry needling in improving other knee joint dysfunction and its follow-up effect are not well documented.Therefore,in some older patients with chronic knee pain or joint stiffness,the use of dry needling can be carefully considered.

BACKGROUND:Chronic myofascial trigger points can identify differential metabolite changes through non targeted metabolomics techniques,helping to understand and further explore the pathophysiological processes and pathogenesis of chronic myofascial trigger points from the perspective of endogenous small molecule metabolites. OBJECTIVE:To investigate potential biomarkers and related metabolic pathways based on urine metabolomics in the rat model of chronic myofascial trigger points. METHODS:Sixteen Sprague-Dawley rats were randomly divided into a model group and a normal group.The model group was used to establish a chronic myofascial trigger point animal model by combining blunt hitting with centrifugal exercise(treadmill slope:-16°,running speed:16 m/min,training time:90 minutes each),once a week for 8 continuous weeks,with 4 weeks off.After 12 weeks of modeling,the metabolic cage method was used to collect urine from rats at 24 hours after modeling.Liquid chromatography-mass spectrometry non-targeted metabolomics technology was used to detect metabolic profiles in the urine samples,screen common differential metabolites,and conduct bioinformatics analysis. RESULTS AND CONCLUSION:Compared with the normal group,there were 32 differential metabolic markers in the model group,of which 21 were upregulated and 11 were downregulated.A total of 14 differential metabolites were identified as potential biomarkers based on the value of variable important in projection greater than 3.The enrichment analysis of the Kyoto Encyclopedia of Genes and Genomes indicated that the formation of chronic myofascial trigger points is closely related to metabolic pathways such as primary bile acid biosynthesis and arachidonic acid metabolism.

ObjectivesTo summarize the clinical and pathological features of patients with congenital liver fibrosis （CHF）， and to investigate the differences in clinical and pathological features between patients in different age groups. MethodsA retrospective analysis was performed for the clinicopathological data of 26 patients with pathologically confirmed CHF in China-Japan Friendship Hospital from August 2005 to June 2023， and the patients were stratified by age to investigate the clinical and pathological features of patients in different age groups. ResultsAmong the 26 patients， there were 12 male patients and 14 female patients， with an age of onset of 4 — 61 years. There were 19 patients with portal hypertension type （73.08%）， 2 patients with cholangitis type （7.69%）， 4 patients with mixed type （15.38%）， and 1 patient with occult type （3.85%）. Of all 26 patients， 4 had unknown clinical symptoms， and among the 22 patients with clear clinical symptoms， 10 （45.45%） attended the hospital due to upper gastrointestinal bleeding caused by portal hypertension. Pathological manifestations included roughly normal liver parenchyma separated by fibrous septa in the portal area， with the presence of abnormal reactive bile duct hyperplasia. Denser fibrous septa and a lack of portal veins with the corresponding caliber were observed in 4 pediatric patients with disease onset before the age of 10 years， with a significant reduction or even disappearance of compensatory thin-walled blood vessels. ConclusionPortal hypertension-type CHF is the most common type in clinical practice. Patients with an early age of onset have certain histopathological features of the liver， with the presence of serious complications associated with portal hypertension. Therefore， liver biopsy should be performed for patients suspected of CHF in clinical practice， and genetic testing should be performed when necessary. Early identification and diagnosis are of great importance for improving the prognosis of patients.

BACKGROUND:The regulatory mechanisms of acupotomy intervention for knee osteoarthritis at a transcriptome level are not well understood despite its proven clinical efficacy.OBJECTIVE:Using acupotomy therapy in a rat model of knee osteoarthritis,to conduct transcriptome sequencing and bioinformatics analysis on cartilage samples,along with validation,and to reveal the molecular regulatory mechanisms involved in this therapy for knee osteoarthritis in rats.METHODS:Forty-eight Sprague-Dawley rats were selected and divided into three groups by random number table method,acupotomy group,model group,and sham operation group,with 16 rats in each group.Osteoarthritis models were induced by medial meniscus instability in the acupotomy group and model group.After successful modeling,acupotomy group rats were treated with acupotomy once a week,for 4 weeks in total.After the intervention,cartilage samples from the rat's knee joint were stained with hematoxylin-eosin staining and safranin O-fast green staining,evaluated by Mankin scores,and underwent MicroCT scanning.Serum inflammatory factor levels were detected by Elisa.Transcriptome sequencing was performed on the remaining cartilage samples,and the data were analyzed using R software to identify differential gene expression levels among the groups.Core targets were screened through protein-protein interaction network and Cytoscape and validated using RT-qPCR.RESULTS AND CONCLUSION:Compared with the sham operation group,rats in the model group had rough and uneven articular cartilage surfaces,narrowed joint spaces,destroyed articular surface structures,elevated Mankin scores,and significant increases in serum levels of interleukin-6,tumor necrosis factor-α,and matrix metalloproteinase 13(P<0.05).Compared with the model group,rats in the acupotomy group had smoother articular cartilage surfaces,wider joint spaces,slightly irregular articular surfaces,lower Mankin scores,and significantly lower serum levels of interleukin-6,tumor necrosis factor-α,and matrix metalloproteinase-13(P<0.05).Gene ontology and Kyoto genome and genome encyclopedia analyses involved proteolytic metabolism,autophagy,mitogen-activated protein kinase,nuclear factor kB,and Wnt signaling pathways.Protein-protein interaction network and Cytoscape screened for four key genes,including ataxia-telangiectasia mutations,Myb SWIRM and MPN domain protein 1,heat shock protein 90α1,and NIPBL cohesion-loading factor.The mRNA expression of ataxia-telangiectasia mutations,Myb SWIRM and MPN domain protein 1,heat shock protein 90α1,and NIPBL cohesion-loading factor in the articular cartilage of rats in the model group was lower than that of the sham operation group(P<0.05),while the mRNA expression of ataxia-telangiectasia mutations,Myb SWIRM and MPN domain protein 1,heat shock protein 90α1,and NIPBL cohesion-loading factor in the articular cartilage of rats in the acupotomy group was higher than that in the model group(P<0.05).To conclude,acupotomy treatment of knee osteoarthritis in rats may act on signaling pathways such as MAPK,nuclear factor kB,and Wnt to promote cartilage repair,and is closely related to the expression of genes associated with ataxia-telangiectasia mutations,Myb SWIRM and MPN domain protein 1,heat shock protein 90α1,and NIPBL cohesion-loading factor.

Objective:To investigate the characteristics of gut microbiota in patients with type 2 diabetes (T2DM) complicated by nonalcoholic fatty liver disease (NAFLD).Methods:A total of 74 patients first diagnosed with T2DM at the Endocrinology Department of Peking Union Medical College Hospital from April 2021 to October 2023 were included. Among them, 28 patients had concurrent NAFLD while 46 patients did not. Additionally, 51 healthy controls were matched (HC group). Clinical laboratory parameters were collected, and 16S rRNA sequencing with fecal samples was conducted to compare the differences in gut microbiota across the groups.Results:Compared to the group with T2DM, patients with concurrent T2DM and NAFLD were younger, had higher level of insulin resistance as assessed by the homeostatic model assessment of insulin resistance, higher body mass index (BMI), and higher triglyceride levels. There was no difference in α-diversity across the three groups ( P>0.05), while there was a significant difference in β-diversity ( P=0.03). The Eubacterium coprostanoligenes, Fusicatenibacter, Parasutterella and Tyzzerella 3 were enriched in the group with concurrent T2DM and NAFLD as shown by the relative abundance, while the relative abundance of Flavonifractor was decreased in this group. Tyzzerella 3 abundance was positively correlated with triglyceride and albumin levels and negatively correlated with high-density lipoprotein cholesterol (HDL-C) levels. Conclusion:Patients with T2DM complicated by NAFLD exhibit dysbiosis in gut microbiota composition and specific genera abundance, with Flavonifractor identified as a potential protective factor for T2DM complicated by NAFLD.

Objective:To investigate the clinicopathological features and genetic mutation status of pulmonary intravascular large B cell lymphoma.Methods:The clinicopathological data of eight patients diagnosed with pulmonary intravascular large B cell lymphoma, from April 2018 to May 2023, were retrospectively analyzed. The genetic profile of six patients was detected via next-generation sequencing (NGS) and followed up.Results:All patients included one male and seven females, with a median age of 64 years (ranging from 45 to 66 years). Respiratory symptoms were the most common (7 cases), B symptoms in two cases, hemophagocytic syndrome in two cases. Multiple diffuse ground-glass opacities in both lungs were observed based on the high-resolution chest CT scan. Six cases of mild to moderate ventilation or diffusion dysfunction were observed based on the pulmonary function tests. Moreover, two cases of hypoxemia and two cases with type Ⅰ respiratory failure were recorded. The serum lactate dehydrogenase level increased (7/8), β2-MG level increased (2/8), neuron-specific enolase level increased (7/8), total number of peripheral blood lymphocytes decreased (7/8), and clinical stages were all stage Ⅳ. The neoplastic lymphoid cells were lodged in the lumina of venules and capillaries of the alveolar septum; the tumor cells were large, with prominent nucleoli and frequent mitotic figures. The malignant cells were detected in the extravascular surrounding lung tissue in all cases. The tumor cells expressed mature B cell-associated antigens CD20 and CD79a, and the vascular endothelial markers CD31 and CD34 showed that the tumor cells were filled in the blood vessels, infiltrated blood vessel walls, and perivascular areas. One case was germinal center-type, seven cases were non-germinal center-type, two cases were double-expressing lymphoma, and all cases were EBER-negative. Furthermore, the top five genes with mutation frequencies detected by NGS were MYD88 (5/6), PIM1 (5/6), CD79B (4/6), TCF3 (4/6), and TP53 (3/6). Of the eight cases, seven patients received R-CHOP-based chemotherapy, six cases had complete remission after chemotherapy, one case died, and one case was lost to follow-up.Conclusions:Pulmonary vascular large B cell lymphoma is rare, which shares similar patterns with interstitial lung disease on imaging. Transbronchial lung biopsy is an effective method to confirm the diagnosis. Immunochemotherapy with BTK inhibitors can provide a survival advantage for patients in the future based on molecular typing.

Schizophrenia(SZ)is a serious mental illness,and traditional diagnostic methods are prone to missed and misdiagnosis.Using machine learning(ML)algorithms can extract SZ relative features from functional MRI(fMRI)data,hence being helpful for diagnosing and predicting treatment response of SZ.The research progresses of ML based on fMRI for diagnosis and treatment of SZ were reviewed in this article.
