BackgroundIndividuals with chronic multimorbidity exhibit a high prevalence of sleep disturbances, which impair their physical and psychological well-being and impede chronic disease management. Although sleep quality is closely influencing with psychological functioning, empirical evidence regarding the longitudinal trajectories of sleep quality and their influencing factors in this population remains insufficient. ObjectiveTo investigate the longitudinal trajectories of sleep quality and associated factors among patients with chronic multimorbidity from hospitalization through 6 months post-discharge, aiming to facilitate the early identification of high-risk individuals and the development of personalized sleep interventions. MethodsA total of 228 inpatients with chronic multimorbidity, primarily with cardiovascular and metabolic chronic conditions, were enrolled via random sampling at The Third People's Hospital of Fuyang from January 2023 to September 2024. Baseline assessments were conducted during hospitalization using the general socio-demographic questionnaire, the Sleep Dysfunction Rating Scale (SDRS), the Ruminative Response Scale (RRS), and the Hospital Anxiety and Depression Scale (HADS). Sleep quality was further evaluated using the SDRS at 1-, 3-, and 6-month post-discharge. Latent class growth model was employed to delineate trajectories of sleep quality. Multivariate logistic regression analysis was subsequently performed to identify influencing factors of sleep quality. ResultsA total of 201 patients with chronic diseases were included.Based on four predefined assessment time points (baseline, 1 month, 3 months, 6 months post-discharge), the latent class growth model identified three distinct trajectories of sleep quality: persistent high sleep disturbance (n=72, 35.82%), moderate sleep disturbance-progression (n=73, 36.32%), and low sleep disturbance (n=56, 27.86%). Multivariate logistic regression analysis revealed that, compared with the low sleep disturbance group, multiple factors increased the odds of membership in the persistent high sleep disturbance group: per capita monthly household income of <3 000 yuan (β=13.131, P<0.01) or 3 000–5 000 yuan (β=5.913, P<0.05), aged 45–65 years (β=9.536, P<0.05), presence of 3 (β=7.792, P<0.05) or >3 comorbid chronic conditions (β=6.626, P<0.05), as well as higher RRS (β=0.334, P<0.01) and HADS score (β=1.628, P<0.01). Additionally, patients aged 45–65 years (β=2.777, P<0.05), with 3 (β=3.802, P<0.01) or >3 comorbid chronic conditions (β=2.463, P<0.01), and with higher RRS (β=0.111, P<0.01) and HDAS scores (β=0.350, P<0.05) also demonstrated significantly increased odds of belonging to the moderate sleep disturbance–progression group. ConclusionThere is significant population heterogeneity in sleep quality trajectories among patients with chronic multimorbidity, with age, per capita household monthly income, number of comorbidities, rumination, and anxiety/depression symptoms being identify as the primary influencing factors.

ObjectiveTo explore the preparation method and performance characteristics of a new double-network hydrogel based on carboxymethyl chitosan (CMCS)/silk fibroin (SF)/copper-doped bioactive glass (CuBG). MethodsPolyethylene glycol diphenyldehyde (PEGDE) was used as a cross-linking agent to cross-link CMCS, which was then blended with enzymatically cross-linked SF and CuBG nanoparticles to prepare a double-network hydrogel. The optimal ratio of CMCS/SF/CuBG double-network hydrogels was determined by gelation time and rheological analysis. The injectability of each group of gel was evaluated through injectability testing and rheological viscosity analysis. The swelling properties of the hydrogels were measured using the gravimetric method. The cytocompatibility of the hydrogels was evaluated using cell counting kit-8 (CCK-8) assay and live/dead cell staining. ResultsThe gelation time of the CMCS single-network hydrogel exceeded 4 minutes, and its mechanical strength was poor. With the addition of SF and CuBG, the gelation time of the hydrogel was shortened, the mechanical strength was enhanced, and the swelling index decreased. Hydrogel injection could form clear and continuous patterns. The CCK-8 and cell live/dead staining results showed that the survival rate of HUVECs cultured in leaching solution of each group of hydrogels was greater than 90%, with complete cell morphology and good proliferation status. ConclusionThe CMCS/SF/CuBG double-network hydrogel exhibits a suitable gelation time, stable mechanical properties, excellent swelling rate, and exhibits no cytotoxicity. These findings indicate that the hydrogel has considerable potential for biomedical applications.

ObjectiveTo explore the mechanism of Huashi Baidu granules in treating influenza A (H1N1) virus infection based on the gut-lung axis. MethodsBALB/c mice were randomly assigned into a blank control group, a model control group, a positive control (oseltamivir phosphate capsules, 19.5 mg·kg-1) group, and a Huashi Baidu granules (17.81 g·kg-1) group. Mice in other groups except the blank control group were instilled nasally by H1N1 viral fluid for the modeling of influenza viral pneumonia and treated with corresponding drugs once daily by gavage for 5 consecutive days. The mouse lung index was calculated, and pathological changes in the lung and colon tissue were observed. Western blot was employed to quantify the expression levels of nucleotide-binding oligomeric domain-like receptor protein 3 (NLRP3), cysteinyl aspartate-specific proteinase -1 (Caspase-1), apoptosis-associated Speck-like protein containing a CARD (ASC), gasdermin D (GSDMD), interleukin-18 (IL-18), IL-1β, and colonic tight junction protein 1 (CLDN1), and Occludin in the lung and colon tissue. Non-targeted metabolomics was employed to analyze the metabolic profiles of the lung tissue and intestinal contents. The 16S rRNA sequencing was performed to analyze the gut microbiota structure in intestinal contents. Enzyme-linked immunosorbent assay (ELISA) was employed to measure the 5-hydroxytryptamine (5-HT) levels in the serum, lung tissue, and colon tissue. ResultsCompared with the blank control group, the model control group showed elevated lung index, marked inflammatory cell infiltration and alveolar structural destruction in the lung tissue, obvious inflammatory cell infiltration and mucosal epithelial damage in the colon tissue, upregulated protein levels of NLRP3 inflammasome pathway proteins NLRP3, Caspase-1, GSDMD, IL-1β, and IL-18 in the lung and colon tissue (P<0.05, P<0.01), downregulated protein levels of CLDN1 and Occludin in the colon tissue (P<0.05, P<0.01). Compared with the model control group, Huashi Baidu Granules reduced the lung index (P<0.01), alleviated pathological damage in the lungs and colon, inhibited the expression of NLRP3, Caspase-1, GSDMD, IL-1β, and IL-18 in lung and colon tissue (P<0.05, P<0.01), and upregulated the expression of CLDN1 and Occludin in the colon tissue (P<0.05, P<0.01). Metabolomics analysis showed that Huashi Baidu Granules downregulated the 5-HT pathway and arachidonic acid metabolism pathway in the lung and colon tissue, while upregulating nucleotide metabolism and TCA cycle. Compared with the blank control group, the model control group exhibited elevated levels of 5-HT in the serum, lung tissue, and colon tissue (P<0.01). The intervention with Huashi Baidu Granules lowered the levels of 5-HT in the serum (P<0.05) and the lung and colon tissue (P<0.01). Huashi Baidu Granules increased the diversity of gut microbiota, enriched beneficial bacteria such as Blautia, and inhibited pathogenic bacteria such as Desulfovibrio. ConclusionHuashi Baidu Granules significantly alleviate lung inflammation and pathological damage in mice infected with influenza A (H1N1) virus, reshape the intestinal microecology, and repair the intestinal barrier. It may exert cross-organ regulation by inhibiting the cascade of "gutogenic 5-HT release-circulatory transport-pulmonary 5-HT receptor activation-NLRP3 inflammasome assembly." It exerts therapeutic effects on influenza viral pneumonia through the gut-lung axis. This study provides a modern biological interpretation for the TCM theories of interior-exterior relationship between the lung and large intestine and simultaneous treatment of the lung and intestine.

Thrombocytopenia is a common complication during the treatment of malignant tumors. It can lead to insufficient doses of chemotherapy drugs or delayed chemotherapy, shorten patients’ survival time, and affect prognosis. Thrombocytopenia has two types: cancer treatment-induced thrombocytopenia and tumor-associated immune thrombocytopenia. The latter is relatively rare, and its pathogenesis may be related to immune dysregulation. Current studies have shown that gene polymorphism and methylation are involved in tumor-associated immune thrombocytopenia. The pathogenesis and treatment of tumor-associated immune thrombocytopenia are discussed in this article.

Kidney deficiency syndrome is a common clinical syndrome in traditional Chinese medicine (TCM). With the progress of science and technology, clinical and animal experiments on kidney deficiency syndrome have made remarkable progress. Research on kidney deficiency and the nature of "kidney" involves a large number of physiological and pathological bases, which are closely related to physiological and pathological links in the human body, among which the neuroendocrine-immune network shares the closest relationship. However, there are still many challenges in modern research on kidney deficiency syndrome, such as expert consensus on clinical diagnostic criteria and evaluation indexes and optimization of animal experimental models. In the past decade, a large number of clinical and animal experiments have been reported in the literature on kidney deficiency syndrome, among which the literature focusing on the combination of disease and syndrome is predominant, and most of them focus on kidney Yang deficiency and kidney Yin deficiency, involving the exploration of many pathological mechanisms. Research on the mechanisms related to kidney deficiency syndrome encompasses multiple signaling pathways and various biochemical indicators, including the phosphatidylinositol 3-kinase/protein kinase B/nuclear factor-erythroid 2-relatedfactor-2(PI3K/Akt/Nrf2) signaling pathway, the Toll-like receptor 4/myeloid differentiation factor88/nuclear factor-κB(TLR4/MyD88/NF-κB) signaling pathway, the Janus kinase 2/signal transducer and activator of transcription 3(JAK2/STAT3) signaling pathway, Osteoprotectin/nuclear factor-κB receptor activator ligand/receptor activator of nuclear factor-κB (OPG/RANKL/RANK) signaling pathway. The biochemical indicators cover the cyclic adenosine monophosphate/cyclic guanosine monophosphate (cAMP/cGMP) ratio, Na+-K+-ATPase activity, Ca2+-Mg2+-ATPase activity, adrenocorticotropic hormone (ACTH), polycorticosterone (CORT), 17-OHCS, and other sex hormone indicators, providing crucial reference values for diagnosing kidney Yang deficiency or kidney Yin deficiency. The literature related to kidney deficiency syndrome over the past decade was collated and excavated, with a view to providing a reference for research on kidney deficiency syndrome.

Objective:To investigate the relationship between different obesity metabolic phenotypes and the incidence of new carotid artery plaque. Methods:The present study is a retrospective cohort study, collecting individuals from the Health Management Center of the Second Affiliated Hospital of Dalian Medical University who had two or more cervical vascular color ultrasound examinations and met the inclusion criteria from 2014 to 2022, and collected their baseline clinical data. According to whether the subjects were obese and had metabolic syndrome, they were divided into metabolically healthy non-obese group, metabolically unhealthy non-obese group, metabolically healthy obese group, and metabolically unhealthy obese group. The first physical examination time of the subjects was taken as the starting point of follow-up, and cervical vascular color ultrasound was performed during the follow-up physical examination, with the outcome event being carotid artery plaque. Kaplan-Meier survival curve analysis was used to analyze the cumulative incidence of carotid artery plaques in the four groups and log-rank test was performed, and a multifactorial Cox proportional hazards model was used to analyze the relationship between different obesity metabolic phenotypes and the risk of carotid artery plaque incidence. Results:A total of 4 890 subjects were enrolled, aged (45.4±9.6) years, and 2 754 (56.3%) males. The follow-up time was 1.14(0.93, 2.20) years. Compared with the other 3 obesity metabolic phenotypes, the incidence of carotid plaques in the metabolically unhealthy obesity group was the highest (15.4% (286/1 861)). Kaplan-Meier survival curve analysis showed that the cumulative incidence of carotid plaques in metabolically unhealthy obese subjects was about 2.962 times that of metabolically healthy non-obese subjects (log-rank P<0.001). Multivariate Cox regression results showed that the risk of carotid plaque in metabolically unhealthy obese subjects was 1.650 times that of metabolically healthy non-obese subjects (95% CI: 1.203-2.264, P=0.002). Conclusion:Metabolically unhealthy obesity phenotype is an independent risk factor for carotid plaque.

Chinese Journal of Endocrinology and Metabolism 2024;40(9):752-757
Objective:To investigate the relationship between obesity and incident chronic kidney disease(CKD) in a population undergoing health check-ups.Methods:This is a retrospective cohort study. A total of 31 251 participants who had at least 2 health physical examinations in the Health Management Center of the Second Affiliated Hospital of Dalian Medical University from January 2017 to December 2022 and met the inclusion criteria were selected. The participants were divided into normal body weight group, overweight group, and obese group according to baseline body mass index. Cox proportional hazard regression model was used to analyze the relationship between obesity and new-onset CKD, and the dose-response relationship between body mass index and CKD was analyzed with restricted cubic splines. Results:Multivariate Cox regression analysis showed that the risk of developing CKD increased by 13%( HR=1.13, 95% CI 1.01-1.25) and 55%( HR=1.55, 95% CI 1.36-1.76) in the overweight and obese group compared to the normal weight group. Subgroup analysis indicated that obese women had a higher risk of developing CKD compared to men. There was a " U-shaped" correlation between body mass index and CKD in male population, with the lowest risk of CKD occurring at body mass index of 19.6-24.2 kg/m 2. In women, the relationship between body mass index and CKD was approximately linear, with the risk of CKD gradually increasing when body mass index exceeded 22.5 kg/m 2. Conclusions:Obesity is an independent risk factor for new-onset CKD, and obese women have a higher risk of developing CKD than men. Regarding CKD prevention, men are advised to maintain a higher level of body weight within the normal range of body mass index, while women are encouraged to control their weight to a lower level within the normal body mass index range.
