1.Value of metabolic markers combined with anthropometric indicators in predicting and risk stratification of metabolic dysfunction-associated fatty liver disease and establishment of a nomogram model

ObjectiveTo develop a novel clinical predictive model for metabolic dysfunction-associated fatty liver disease (MAFLD) based on metabolic markers and anthropometric indicators, and to provide a more effective tool for the early screening and intervention of MAFLD. MethodsA retrospective analysis was performed for 2 824 individuals who underwent abdominal color Doppler ultrasound at Health Examination Center of The First Hospital of Lanzhou University from January 1, 2024 to January 1, 2025, and at a ratio of 7∶3, they were randomly divided into training set with 1 976 patients and validation set with 848 patients. Clinical data, serological markers, and abdominal ultrasound results were collected from all patients, and triglyceride-glucose (TyG) index, triglyceride-to-high-density lipoprotein cholesterol (TG/HDL-C) ratio, and anthropometric indicators were calculated. The independent samples t-test was used for comparison of normally distributed or approximately normally distributed continuous data between two groups, and the Mann-Whitney U test was used for comparison of continuous data with skewed distribution between two groups; the chi-square test or the Fisher’s exact test was used for comparison of categorical data between groups. The multivariate logistic regression analysis was used to identify independent predictive factors for MAFLD and intermediate- to high-risk MAFLD. Five risk prediction models were established for MAFLD based on the independent influencing factors, and a nomogram was plotted. The receiver operating characteristic (ROC) curve was plotted to assess model performance, and the area under the ROC curve (AUC) was calculated. The calibration curve was used to evaluate the predictive accuracy of the models, and decision curve analysis was used to assess the clinical practicability of the models. These models were then compared with traditional models. ResultsAmong the 1 976 individuals in the training set, 937 (47.42%) were diagnosed with MAFLD, and 423 (21.41%) were diagnosed with intermediate- to high-risk MAFLD; among the 848 individuals in the validation set, 406 (47.88%) were diagnosed with MAFLD. The multivariate logistic regression analysis showed that male sex (odds ratio [OR]=0.23, 95% confidence interval [CI]: 0.13 — 0.39, P<0.05), waist circumference (OR=1.11, 95%CI: 1.06 — 1.17, P<0.05), alanine aminotransferase (ALT) >40 U/L (OR=2.24, 95%CI: 1.44 — 3.51, P<0.05), high-density lipoprotein cholesterol (HDL-C) (OR=0.07, 95%CI: 0.04 — 0.15, P<0.05), TyG index (OR=8.27, 95%CI: 5.09 — 13.44, P<0.05), TG/HDL-C ratio (OR=0.84, 95%CI: 0.71 — 0.99, P<0.05), A Body Shape Index (ABSI) (OR=0.45, 95%CI: 0.39 — 0.52, P<0.05), and body roundness index (BRI) (OR=2.31, 95%CI: 1.50 — 3.55, P<0.05) were independent influencing factors for MAFLD, and male sex (OR=0.17, 95%CI: 0.10 — 0.31, P<0.05), age (OR=1.09, 95%CI: 1.07 — 1.11, P<0.05), hemoglobin (OR=0.98, 95%CI: 0.97 — 0.98, P<0.05), platelet count (OR=0.81, 95%CI: 0.70 — 0.93, P<0.05), fasting blood glucose (OR=0.80, 95%CI: 0.71 — 0.89, P<0.05), triglycerides (OR=0.14, 95%CI: 0.07 — 0.29, P<0.05), TG/HDL-C ratio (OR=0.78, 95%CI: 0.67 — 0.91, P<0.05), TyG index (OR=5.26, 95%CI: 3.32 — 8.33), waist circumference (OR=2.50, 95%CI: 1.72 — 3.61, P<0.05), ABSI (OR=0.58, 95%CI: 0.51 — 0.66, P<0.05), and BRI (OR=0.01, 95%CI: 0.00 — 0.21, P<0.05) were independent influencing factors for intermediate- to high-risk MAFLD. Among the five models established, model 5 (incorporating sex, ALT elevation, HDL-C, TyG index, TG/HDL-C ratio, waist circumference, and ABSI) had the best performance, with an AUC of 0.917 (95%CI: 0.905 — 0.929) in the training set and 0.911 (95%CI: 0.892 — 0.930) in the validation set. The calibration curve showed that model 5 had good predictive accuracy, and the decision curve analysis confirmed its clinical practicability. ConclusionThe predictive model for MAFLD constructed based on metabolic markers and anthropometric indicators has good discriminatory ability and can be used to assess the risk of MAFLD. In addition, this study shows that waist circumference, TyG index, TG/HDL-C ratio, ABSI, and BRI are independently associated with intermediate- to high-risk MAFLD, but further studies are needed to confirm their value in predicting liver fibrosis progression.

Background:
s/Aims: Non-invasive models stratifying clinically significant portal hypertension (CSPH) are limited. Herein, we developed a new non-invasive model for predicting CSPH in patients with compensated cirrhosis and investigated whether carvedilol can prevent hepatic decompensation in patients with high-risk CSPH stratified using the new model.
Methods:
Non-invasive risk factors of CSPH were identified via systematic review and meta-analysis of studies involving patients with hepatic venous pressure gradient (HVPG). A new non-invasive model was validated for various performance aspects in three cohorts, i.e., a multicenter HVPG cohort, a follow-up cohort, and a carvediloltreating cohort.
Results:
In the meta-analysis with six studies (n=819), liver stiffness measurement and platelet count were identified as independent risk factors for CSPH and were used to develop the new “CSPH risk” model. In the HVPG cohort (n=151), the new model accurately predicted CSPH with cutoff values of 0 and –0.68 for ruling in and out CSPH, respectively. In the follow-up cohort (n=1,102), the cumulative incidences of decompensation events significantly differed using the cutoff values of <–0.68 (low-risk), –0.68 to 0 (medium-risk), and >0 (high-risk). In the carvediloltreated cohort, patients with high-risk CSPH treated with carvedilol (n=81) had lower rates of decompensation events than non-selective beta-blockers untreated patients with high-risk CSPH (n=613 before propensity score matching [PSM], n=162 after PSM).
Conclusions
Treatment with carvedilol significantly reduces the risk of hepatic decompensation in patients with high-risk CSPH stratified by the new model.
3.Risk factors for suicidal ideation in bereavement callers to the Beijing psychological support hotline

Objective:To explore the risk factors of suicidal ideation in bereavement callers of Beijing psy-chological support hotline.Methods:Bereavement callers from 2022-2023 hotline callers who were assessed for grief reactions were included in this study.Callers were assessed for current suicidal ideation by asking them if they had suicidal ideation within two weeks.The general demographic data of bereavement callers were collected,and the distress,hope and suicide risk factors of bereavement callers were assessed by hotline interview.The depression de-gree of bereavement callers was assessed by diagnostic screening scale for depression.Logistic regression analysis was used to explore the risk factors of suicidal ideation.Results:Seventy-seven point two percent(1 028/1 332)of callers reported suicidal ideation.Those who were over 25 years old(OR=0.50),with 10-16 years of education(OR=0.59),with over 16 years of education(OR=0.28),and had a high sense of hope(OR=0.41)were less likely to have suicidal ideation.Those with previous suicide history(OR=2.70),high levels of distress(OR=1.72),and high levels of depression(OR=2.86)were more likely to have suicidal ideation.Conclusion:A history of previous suicide,high levels of distress,and high levels of depression among bereavement callers were risk fac-tors for suicidal ideation.
4.Medication regularity and mechanism of prescriptions containingtraditional Chi-nese medicine compound formulas in treatment of sepsis based on data mining and network pharmacology

This study aims to investigate the medication rules of patented traditional Chinese medi-cine(TCM)compound formulas and molecular mechanisms of core drugs for treating sepsis using data mining and network pharmacology approaches.In the present study,we first searched the PubMed database,Web of Science database,and the China National Knowledge Infrastructure(CNKI)since the establishment of the library to April 30,2024 for the relevant literature on the treatment of sepsis by traditional Chinese medicine.The prescriptions were then statistically ana-lyzed for drug frequency and association analysis to obtain the core drugs.Then we screened the ef-fective active ingredients of the core drugs by TCMSP and other database platforms,obtained sep-sis-related genes in GeneCards and other databases,and statistically intersected targets,and predic-ted the mechanism of action of the core TCMs by subjecting the intersected targets to PPI analy-sis,GO function and KEGG pathway enrichment analysis.Finally,the relationship between key tar-gets and herbal components was examined in reverse by molecular docking method.The results showed that 64 compound formulas were obtained,with a total of 150 Chinese medicines,which were mostly sweet in taste,cold in nature,and belonged to the spleen,stomach and intestinal me-ridians.According to the association rules,the core drugs were identified as"mirabilite-peach ker-nel-rheum officinale".There were 79 intersecting targets between the core drugs and sepsis,with core targets such as IL-1β,EGFR and SRC.MAPK,TNF,IL-17 and other signaling pathways are involved to mediate inflammatory responses,apoptosis and other biological processes to exert ther-apeutic effects on sepsis.The molecular docking results indicated that the docking activity of the key targets with the main components of the drug,and sennoside E_qt has the lowest binding ener-gy and the best docking activity with SRC.In conclusion,this study showed that the prescription of Chinese medicine for sepsis is mostly based on tonifying the spleen and clearing heat.The mecha-nism of action of the core drug"mirabilite-peach kernel-rheum officinale"in the treatment of sep-sis is multilevel and multifaceted,which provides a certain theoretical basis for the treatment of sepsis by traditional Chinese medicine.
5.Distinct Distribution Patterns of Pain-Responsive and Itch-Sensitive Nerve Terminal in Mouse Skin

Objective To explore the distribution characteristics and innervation patterns of pain-and itch-sensing nerves in dif-ferent regions of skin tissue.Methods Using Trpv1Cre;Ai9 and SstCre;Ai9 transgenic mouse systems,to analyze the distribution and in-nervation patterns of transient receptor potential vanilloid 1(TRPV1+)pain-sensing nerves and somatostatin(SST+)itch-sensing nerves in the skin of the forelimbs,hindlimbs and dorsal abdomen.Immunofluorescence staining was employed to quantitatively detect the distribution ratios of these two types of sensory neurons in the dorsal root ganglia(DRG).Results Free nerve terminal of both pain-and itch-sensing nerves were observed in the epidermal and dermal layers of skin tissue across all regions,but their morphological and distribution characteristics differed significantly.In the dermis,the two types of nerve fibers exhibited a parallel innervation pattern,while in the epidermis,their distribution tended to be more dispersed.Pain-sensing nerve terminal displayed a dense,filamentous distribu-tion,whereas itch-sensing nerve terminal were arranged in a sparse,bead-like pattern.In the DRG,the proportion of pain-sensing neuronal cell bodies was 76.13%±2.33%,while itch-sensing neuronal cell bodies accounted for only 7.58%±2.59%.Additionally,TRPV 1 was not only expressed in pain-sensing nerve fibers but also showed high expression in the stratum corneum,dermis,and inter-muscular layers of the plantar skin.In contrast,SST was primarily expressed in itch-sensing nerve fibers,with minor expression in peri-follicular cells.Conclusion Pain-and itch-sensing nerve fibers exhibit distinct innervation patterns in the skin.The widespread and dense distribution of pain-sensing nerve terminal may provide a structural basis for the rapid detection of potential noxious stimuli.This study offers morphological evidence to clarify the regulatory mechanisms of peripheral sensory nervous system function.
6.Effects of miRNA-383-5p targeting CIP2A on the proliferation,invasion,migration and apoptosis of bladder cancer cells

Objective The aim of this study was to detect the expression of miR-383-5p in bladder cancer tissues and bladder cancer 5637 cells,BIU-87 cells,TCCSUP cells and HT-1376 cells,and to explore the effects of miR-383-5p on the prolif-eration,invasion,migration and apoptosis of bladder cancer cells by targeting CIP2A.Methods The expression of miR-383-5p was detected by qRT-PCR in human bladder cancer tissues and their corresponding adjacent tissues,5637 cells,BIU-87 cells,TCCSUP cells,HT-1376 cells,human bladder transitional epithelial cells.BIU-87 cells with low miR-383-5p expression were selected for subsequent experiments.BIU-87 cells were divided into the blank group(normal culture),miR-383-5p NC group(negative control,transfected with miR-383-5p negative control),miR-383-5p mimic group(transfected with miR-383-5p mimic),and miR-383-5p mimic+pc-CIP2A group(co-transfected with miR-383-5p mimic and CIP2A overexpression plasmid pc-CIP2A).CCK-8 kit was used to detect the viability of BIU-87 cells in each group;Flow cytometry was used to detect apoptosis of BIU-87 cells;Transwell assay was used to measure cell invasion ability of BIU-87 cells;Scratch assay was used to measure cell migration ability of BIU-87 cells;Western blot was used to determine the expression of proteins related to apoptosis,invasion(MMP-2,MMP-9),and CIP2A/PP2A in BIU-87 cells;The dual luciferase assay was used to verify the targeting relationship between miR-383-5p and CIP2A in BIU-87 cells.Results The expression of miR-383-5p was low in bladder cancer tissues and bladder cancer cells.Compared with the blank group,BIU-87 cells in the miR-383-5p mimic group showed a significant increase the level of miR-383-5p(0.91±0.10 vs.1.67±0.24,P<0.01)and a significant decrease in the expression of CIP2A protein(1.32±0.17 vs.0.45±0.03,P<0.001),the cell viability,invasion,migration abilities,the expression of proteins related to invasion(MMP-2,MMP-9),and the expression of Bcl-2 protein[(100.00±4.36)% vs.(32.15±2.65)% ,(150.20±12.95)vs.(82.35±7.01),(77.91±3.63)% vs.(46.12±2.54)% ,1.02±0.11 vs.0.22±0.04,1.03±0.18 vs.0.21±0.04,1.01±0.14 vs.0.27±0.05,P<0.001];The apoptosis rate,the expression of caspase-3 and Bax proteins related to apoptosis,and PP2A expression were significantly increased[(14.02±2.29)% vs.(38.21±3.20)% ],0.81±0.11 vs.1.78±0.24,0.83±0.12 vs.1.72±0.24,0.27±0.02 vs.0.95±0.16,P<0.001].Compared with the miR-383-5p mimic group,BIU-87 cells in the miR-383-5p mimic+pc-CIP2A group significantly increased the cell viability,invasion,migration abilities,the expression of proteins related to invasion,and the expression of Bcl-2 protein[(32.15±2.65)% vs.(50.18±3.77)% ,(82.35±7.01)% vs.(116.30±13.70),(46.12±2.54)% vs.(58.43±3.15)% ,0.22±0.04 vs.0.60±0.08,0.21±0.04 vs.0.5 8±0.06,0.27±0.05 vs.0.64±0.08,P<0.05];The apoptosis rate,the expression of caspase-3,Bax,and PP2A was signifi-cantly reduced in the miR-383-5p mimic+pc-CIP2A group[(38.21±3.20)% (23.15±2.74)% ,1.78±0.24 vs.1.25±0.21,1.72±0.24 vs.1.23±0.18,0.95±0.16 vs.0.60±0.13,P<0.05].The results of dual luciferase experiments showed a corresponding tar-geting relationship between miR-383-5p and CIP2A.Conclusion Increasing the expression of miR-383-5p can inhibit the prolif-eration,invasion and migration of bladder cancer BIU-87 cells,and enhance the ability of apoptosis,which may be achieved by targe-ted regulation of CIP2A.
7.The impact and applications of high-dose fractionated irradiation on the effector function of NK cells

Objective:To investigate the impact of high-dose fractionated irradiation on NK cell function and immune checkpoint expression.Methods:A total of 10 female C57BL/6N mice aged 6-8 weeks were subcutaneously injected with Lewis lung carcinoma (LLC) cells on the right shoulder to establish a subcutaneous tumor model. Mice were randomly divided into two groups: the control (Ctrl) group and the radiation (RT) group, with five mice in each group. The RT group received fractionated irradiation of 8 Gy × 3 fractions, while the Ctrl group was not irradiated. Three days later, flow cytometry was performed to compare the numbers of NK cells and the expressions of immune checkpoints, including programmed death-1 (PD-1) and T-cell immunoreceptor with Ig and ITIM domains (TIGIT), in the spleen, draining lymph nodes, and tumor tissue between the two groups. The activation state and cytotoxicity of NK cells were assessed by the expression of lysosomal-associated membrane protein 1 (CD107a) on the NK cell surface. Additionally, a second experiment was conducted with 20 tumor-bearing mice divided into four groups: Ctrl, RT, anti-TIGIT (αTIGIT) treatment, and RT combined with αTIGIT treatment (RT+ αTIGIT), with five mice per group. The RT and RT+ αTIGIT groups received fractionated RT, while the αTIGIT and RT+ αTIGIT groups were treated with blocking antibody αTIGIT at 0.5 mg/kg every 3 days and the Ctrl group was treated with isotype control IgG. Tumor size was monitored during the experiment. On day 20, mice were euthanized, Ki67 immunohistochemistry and TUNEL immunofluorescence staining were used to evaluate tumor cell proliferation and apoptosis.Results:Compared with the Ctrl group, fractionated high-dose RT resulted in a decrease in NK cell numbers in the spleen, tumor-draining lymph nodes, and tumor microenvironment in the RT group ( t=6.79-7.54, P<0.05), together with a significant upregulation of the mean fluorescence intensity of PD-1 and TIGIT ( t=4.16-5.65, P<0.05; t=6.12-21.13, P<0.05), and a decrease in CD107a+ expression ( t=5.64-8.99, P<0.05). Compared with the RT group, the RT+ αTIGIT group showed reduced tumor weight ( t=2.50, 9.12, P<0.05), decreased Ki67 expression ( t=12.10, P<0.05), and increased TUNEL expression ( t=5.42, P<0.05). Conclusions:The fractioned high-dose RT can impair NK cell function and upregulate immune checkpoint expression, thereby limiting the immune response. The combination of irradiation with anti-TIGIT antibody treatment can partially reverse this trend and improve tumor treatment efficacy.
8.Risk factors for suicidal ideation among unemployed callers to the Beijing psychological assistance hotline

Objective:To explore the risk factors of suicidal ideation among callers with unemployment issues to the Beijing psychological assistance hotline.Methods:Totally 802 callers from the Beijing psychological assis-tance hotline from 2020 to 2023,who identified unemployment as their primary concern.Suicidal ideation was as-sessed by directly asking callers if they had experienced suicidal thoughts within the past two weeks.General demo-graphic information was collected,along with factors related to suicidal ideation,including distress,hope,depres-sion,substance abuse,physical problems,history of previous suicide attempts,chronic life events,acute life events,history of abuse,fear of being attacked,and family or friends' history of suicide.Results:A total of 517 individuals(64.4%)reported experiencing suicidal ideation within the two weeks prior to their call.Regression analysis indi-cated that high levels of hope(OR=0.32,95%CI:0.23-0.44)were a protective factor against suicidal ideation among unemployed callers.In contrast,severe depressive symptoms(OR=2.47,95%CI:1.77-3.45),a history of previous suicide attempts(OR=1.64,95%CI:1.05-2.56),and chronic life events(OR=1.55,95%CI:1.11-2.16)were identified as significant risk factors for suicidal ideation in this group.Conclusion:A history of prior suicide attempts,chronic life events,and high levels of depression are significant risk factors for suicidal idea-tion among unemployed callers,whereas a high level of hope serves as a protective factor.
9.Effects of Hermetia illucens larvae meal on immune function,intestinal morphol-ogy and microflora of Sichuan white geese

This experiment was conducted to study the effects of replacing feed protein by Herme-tia illucens larvae meal on immune function,intestinal morphology and microflora of Sichuan white geese.A total of 64 healthy 1-day-old Sichuan white geese were randomly allocated into 4 groups with 4 replicates in each group and 4 geese in each replicate,namely the control group,the 2%HILM,4%HILM and 8%HILM groups fed diets contained 0%,2%,4%and 8%of HILM,re-spectively.The experimental period was 40 days.The results showed that compared with the con-trol group,8%HILM increased the levels of serum IgG1,IgG2a and complement C3,and the difference was statistically significant(P<0.01).4%HILM significantly increased the expression level of CD4(P<0.05),and 8%HILM significantly increased the expression level of IL-10(P<0.05).The ratio of villus length to crypt depth(VH/CD)in the jejunum and ileum in 4%HILM group was significantly increased(P<0.05).The SIgA level of jejunum was significantly increased in all HILM replacement groups(P>0.05).The abundance of Bacteroides in 4%HILM group were extremely significantly increased(P<0.01),and the abundance of Bilophila and Bilophila wadsworthia were significantly decreased in all HILM replacement groups(P<0.05).In conclu-sion,HILM can enhance the immune function of Sichuan white geese,improve the intestinal mor-phology of jejunum and ileum,enhance the local mucosal immunity of jejunum,increase the abun-dance of beneficial bacteria in cecum and decrease the abundance of harmful bacteria in cecum,and protect intestinal health.

Danggui Buxue decoction(DBD)is a classic prescription with immunomodulatory and hematopoietic effects.Previous studies have shown the DBD has potential to be used as an oral im-mune booster.However,its immunomodulatory effects and mechanism of action have not been thoroughly studied,especially the protective mechanism of immunomodulatory regulation in the state of immunosuppressive is still unclear.The aim of this study was to explore the protective mechanism of DBD in the immunosuppressive state using network pharmacology combined with animal experiments verification.The active components,core targets and signaling pathways of DBD in treating immunosuppression were obtained using network pharmacology tools.On this ba-sis,the active components of DBD were identified using HPLC-MS,and in vivo studies were con-ducted at the same time.The key active components of DBD obtained using network pharmacology included quercetin,kaempferol and formononetin.The core targets included TP53,RELA,TNF,AKT1,and IL-6.KEGG pathway enrichment analysis showed that phosphoinositide 3-kinase(PI3K)-protein kinase B(AKT)may play an important role in the treatment of immunosuppres-sive diseases using DBD.Molecular docking confirmed that each core target had good binding activ-ity with its corresponding compounds.Animal experiments showed that after DBD intervention,the mRNA gene and protein expression of RELA,TNF,and IL-6 in the serum was significantly down-regulated.The mRNA expression of PI3K and AKT in the ileum and PI3K protein expression were also downregulated.In conclusion,DBD exerts its role in treating immunosuppressive diseases by regulating the PI3K-AKT signaling pathway.
