1.Study on the compatibility stability of 16 commonly used intravenous antibiotics in ICU

OBJECTIVE To systematically analyze the compatibility stability of commonly used intravenous antibiotics in the intensive care unit (ICU), and to provide evidence-based support for rational clinical drug use. METHODS Medication data from the ICU of Hebei General Hospital between January and December 2024 were extracted from the Prescription Automatic Screening System. Commonly used intravenous antibiotics and other intravenous drugs in the ICU were selected through consultations with critical care and pharmacy experts in Hebei province, drug package inserts and compatibility information retrieved from Micromedex, Trissel’s Injectable Drug Handbook and PubMed. The physicochemical stability of drug combinations was analyzed. In addition, Cytoscape 3.10.2 software was used to construct a drug compatibility network for identifying high-risk drugs. RESULTS &CONCLUSIONS A total of 904 pairwise drug combinations involving 16 antibacterial agents and 65 intravenous drugs were collected. Among them, 549 combinations (60.7%) were compatible, 88 combinations (9.7%) were incompatible, 82 combinations (9.1%) had conflicting evidence, and 185 combinations (20.5%) lacked valid data support. High-risk combination drugs primarily involved Amphotericin B for injection, Ceftazidime for injection, Imipenem-cilastatin for injection, Ceftriaxone sodium for injection, Vancomycin hydrochloride for injection, etc. The main risk factors for drug-drug incompatibility included drug concentration, temperature, mixing rate, pH, and chemical structure. In clinical practice, drugs and diluents should be selected rationally based on specific compatibility data, and research and monitoring of drug compatibility should be further strengthened.

Stereotactic radiotherapy is widely favored because of its high treatment precision and less fractionations.ZND-A is a new domestic gamma-ray cone-beam focused stereotactic radiotherapy system.Herein the technical characteristics of ZND-A system are described in detail from the aspects of the treatment frame,gamma-ray module,collimator module,six-dimensional treatment couch module and image-guided system module,and the main parameters are compared with the mainstream gamma knife equipments at home and abroad.With reference to Response Evaluation Criteria in Solid Tumors(RECIST 1.1),the initial efficacy of the patients treated by the ZND-A system is analyzed to evaluate the advantages and disadvantages of the ZND-A system for providing a reference for the hospital clinical use of this type of gamma knife.
3.The Analysis of Discrepancies in Reimbursement Benefits for Medications under China's Outpatient Security Policy in the Case of Patients with Inflammatory Bowel Disease

Objective:Taking Inflammatory Bowel Disease(IBD)patients as an example,to explore the impact of disparities in regional outpatient security policies on medication reimbursement benefits for patients,and to provide insights for promoting the reform and development of equal outpatient security policy.Methods:39 cities from 9 provinces in China were selected as research samples to analyze the types,coverage,and benefits of their outpatient security policies.Indicators such as the individual out-of-pocket ratio,deductible,and specified reimbursement rate were used to simulate the actual reimbursement ratio for IBD patients using negotiated drugs.Results:Under the general outpatient coordination policy,the average actual reimbursement rate for medications in IBD patients was 26.36%for residents and 36.47%for employees.Under the outpatient chronic and special disease policy,the average actual reimbursement rate was 42.49%for residents and 50.94%for employees,while patients receiving drug treatment under the outpatient special drug policy have an average actual reimbursement rate of 51.62%(for residents)and 64.92%(for employees).Conclusion:Under China's outpatient security policies,there are significant disparities in reimbursement benefits for IBD patients across different regions.Therefore,it is recommended to strengthen the coordination of outpatient security policies across regions,optimize policy design,and provide patients with more equitable and accessible medical coverage.

7.Study on the anti-pulmonary fibrosis effect of linarin in vivo and in vitro and its mechanism

OBJECTIVE To investigate the anti-pulmonary fibrosis effect of linarin in vivo and in vitro, and investigate its mechanism preliminarily. METHODS C57BL/6J mice were randomly divided into normal group (carboxymethylcellulose sodium), model group (carboxymethylcellulose sodium), positive control group (pirfenidone, 200 mg/kg), linarin low-dose and high-dose groups (12.5, 25 mg/kg), with 8 mice in each group. Except for normal group, pulmonary fibrosis model was induced in other groups. After modeling, they were given relevant medicine intragastrically, once a day, for consecutive 14 d. The general situation of mice was observed, and their lung indexes were measured; the levels of tumor necrosis factor-α (TNF-α) and transforming growth factor-β1( TGF-β1) in serum and interleukin-6 (IL-6) in lung tissue were determined. Hematoxylin-eosin (HE) staining and Masson staining were used to observe the histopathological morphology of lung. The pulmonary fibrosis was scored according to Ashcroft score standard. The expressions of α-smooth muscle actin (α-SMA) and (type Ⅰ collagen, Collagen Ⅰ), phosphorylated extracellular signal-regulated kinase (p-ERK1/2) and TGF-β1 in lung tissues were detected. HFL1 cells were stimulated by TGF- β1 to form pulmonary fibrosis model in vitro, which were divided into normal group, model group and linarin low-, medium- and high-concentration groups (3.7, 7.4, 14.8 mg/L). After being cultured for 48 h, the protein expressions of α-SMA, Collagen Ⅰ and p-ERK1/2 in HFL1 cells were detected. RESULTS In vivo, compared with normal group, the lung index of model group and the levels of TNF- α, TGF- β1 and IL-6 were significantly increased (P<0.01). There were a large number of inflammatory infiltration and cellular fibrosis lesions in the alveoli, and a large number of collagen depositions. The scores of HE staining and Masson staining were significantly increased (P<0.01). The protein expressions of α-SMA, Collagen Ⅰ, p-ERK1/2 and TGF-β1 in lung tissue were up-regulated significantly (P<0.01). Compared with model group, above indexes of mice were improved significantly in linarin high-dose group (P<0.05 or P<0.01), and most of indexes (except for lung index) were improved significantly in linarin low-dose group (P<0.05 or P<0.01). In vitro, compared with blank group, the density of cells in the model group increased, and obvious proliferation and other changes occurred; protein expressions of α-SMA, Collagen Ⅰ and p-ERK1/2 were significantly up-regulated (P<0.05 or P<0.01). Compared with model group, the cell density of each concentration group was decreased and the morphology gradually returned to normal; the expressions of above proteins in linarin high-concentration group and the protein expression of p-ERK1/2 in linarin medium-concentration group were down-regulated significantly(P<0.05 or P<0.01). CONCLUSIONS Linarin may regulate ERK and inflammatory pathways to reduce the inflammatory response, thereby exerting anti-pulmonary fibrosis effect.

Objective:To explore the current status of surgery for portal hypertension to grasp current status and future development of surgery in China.Methods:This study is jointly sponsored by China Hepatobiliary & Pancreatic Specialist Alliance & Portal Hypertension Alliance in China (CHESS).Comprehensive surveying is conducted for basic domestic situations of surgery for portal hypertension, including case load, surgical approaches, management of postoperative complications, primary effects, existing confusion and obstacles, liver transplantation(LT), laparoscopic procedures and transjugular intrahepatic portosystemic shunt(TIPS), etc.Results:A total of 8 512 cases of portal hypertension surgery are performed at 378 hospitals nationwide in 2021.Splenectomy plus devascularization predominated(53.0%)and laparoscopy accounted for 76.1%.Primary goal is preventing rebleeding(67.0%) and 72.8% of hospitals used preventive anticoagulants after conventional surgery.And 80.7% of teams believe that the formation of postoperative portal vein thrombosis is a surgical dilemma and 65.3% of hospitals practiced both laparoscopy and TIPS.The major reasons for patients with portal hypertension not receiving LT are due to a lack of qualifications for LT(69.3%)and economic factors(69.0%).Conclusions:Surgery is an integral part of management of portal hypertension in China.However, it is imperative to further standardize the grasp of surgical indications, the handling of surgical operation and the management of postoperative complications.Moreover, prospective, multi-center randomized controlled clinical studies should be performed.
9.Study on the drying methods of wine-processed Cornus officinalis based on the contents of multi-index components

OBJECTIVE To investigate the effects of different drying methods on the index components in wine-processed Cornus officinalis so as to optimize drying method.METHODS After processed with wine, C. officinalis decoction pieces were dried with different drying methods (blast drying, far infrared drying, microwave drying, freeze drying, sun drying, shade drying and combined drying). The contents of 5 components such as gallic acid in wine-processed C. officinalis were determined by high- performance liquid chromatography. The contents of total flavonoids in wine-processed C. officinalis were determined by chromogenic method. Analytic hierarchy process was used to evaluate the effects of different drying methods on the contents of components in C. officinalis.RESULTS The contents of gallic acid, 5-hydroxymethylfurfural, monoside, loganin, cornuside and total flavonoids in 22 batches of wine-processed C. officinalis were 1.043 8-1.563 8, 0.648 5-2.358 8, 5.031 0-10.305 7, 6.681 2- 7.534 2, 0.986 5-1.148 8 and 33.657 2-50.741 5 mg/g, respectively. The comprehensive scoring results of analytic hierarchy process showed that the comprehensive score of each component in C. officinalis dried by microwave at 75 ℃ was higher , followed by blast drying at 60 ℃ and far infrared drying at 60 ℃ .CONCLUSIONS The wine-processed C. officinalis could be dried by microwave drying at 75 ℃, blast drying at 60 ℃ or far infrared drying at 60 ℃.
10.Alleviation of amyloid beta-protein induced neurotixicity and synaptic damage in PC12 cells by downregulation of serine racemase

Objective:To investigate the role of down-regulating serine racemase (SRR) in alleviating the β-amyloid peptide (Aβ) induced neurotoxicity and synaptic damage and possible mechanism in PC12 cells.Methods:(1) PC12 cells cultured in vitro were divided into 0, 20, 40 and 80 μmol/L Aβ 25-35 treatment groups; they were treated with 0, 20, 40 and 80 μmol/L Aβ 25-35 for 24 h, respectively; cell counting kit (CCK)-8 was used to detect the survival rate of cells in each group, and Western blotting was used to detect the SRR protein expression. PC12 cells were treated with 40 μmol/L Aβ 25-35 for 0, 12, 24 and 48 h, respectively; cell survival and SRR protein expression were detected by CCK-8 and Western blotting, respectively. (2) PC12 cells were divided into control group, nonsense sequence group, SRR small interfering RNA (siRNA) group 1, SRR siRNA group 2, and SRR siRNA group 3; cells in the later three groups were transfected with SRR nonsense sequence or different SRR siRNA sequences, respectively; 48 h after that, Western blotting was used to detect the SRR protein expression of cells in each group, and SRR siRNA with best effect was selected for subsequent experiments. (3) PC12 cells were divided into control group, AD group, AD+nonsense sequence group, and AD+SRR siRNA group; cells in the latter two groups were transfected with nonsense sequence or SRR siRNA for 48 h, respectively; cells in the latter three groups were added 40 μmol/L Aβ 25-35, and cells in the control group were added same amount of solvent; 24 h after treatment, the SRR protein expression was detected by Western blotting, cell survival was detected by CCK-8, cell apoptosis was detected by Hoechst 33258 fluorescent staining, Caspase 3 activity was detected by enzyme linked immunosorbent assay, and the expressions of activated Caspase 3, N-methyl- D aspartate (NMDA) receptor-associated proteins and postsynaptic dense protein 95 (PSD95) were detected by Western blotting. Results:(1) The survival rate of cells in 0, 20, 40 and 80 μmol/L Aβ 25-35 treatment groups was successively decreased and the SRR protein expression was successively increased, with significant differences ( P<0.05); PC12 cells treated with 40 μmol/L Aβ 25-35 for 0, 12, 24 and 48 h had successively decreased survival rate and successively increased SRR protein expression, with significant differences ( P<0.05). (2) The SRR protein expressions in the SRR siRNA group 1, SRR siRNA group 2 and SRR siRNA3 group 3 were significantly decreased as compared with those in the control group and nonsense sequence group ( P<0.05), and the decrease in the SRR siRNA group 2 was the most obvious. (3) As compared with the control group, the cells in the AD group had significantly increased SRR protein expression and apoptosis rate, statistically decreased cell survival rate, significantly increased Caspase 3 activity and activated Caspase 3 protein expression, significantly increased protein expressions of NMDA receptor 2A (NMDAR2A) and NMDA receptor 2B(NMDAR2B), and statistically decreased PSD95 protein expression ( P<0.05); as compared with cells in the AD group, cells in the AD+SRR siRNA group had significantly decreased SRR protein expression and apoptosis rate, statistically increased cell survival rate, significantly decreased Caspase 3 activity and activated Caspase 3 protein expression, significantly decreased NMDAR2A protein expression, and statistically increased PSD95 protein expression ( P<0.05). Conclusion:Down-regulation of SRR expression can reduce the NMDAR2A protein expression, alleviate the over-activation of NMDA receptor, reduce the cell apoptosis, improve cell survival rate, protect nerve cells, increase PSD95 protein expression, and alleviate synaptic damage in PC12 cells.
