BACKGROUND:The asymmetrical biomechanical environment of adolescent idiopathic scoliosis can lead to further wedge deformation of the vertebral body, which may affect cardiopulmonary function and compress nerves in severe cases.Adolescent idiopathic scoliosis with different degrees of scoliosis should be treated with exercise, bracing, and surgery. However, the mechanical mechanism of selecting an orthopedic approach remains unclear due to the individual variability of patients.OBJECTIVE:To investigate the biomechanical mechanism of different orthopedic modalities for the treatment of adolescent idiopathic scoliosis to provide a basis for clinical selection of treatment modalities based on the spine model of adolescent idiopathic scoliosis patients.METHODS:Based on the CT images of an adolescent idiopathic scoliosis patient,a scoliosis model(C7-L5)was reconstructed in Mimics software in three dimensions, and lateral thrust force was applied at the T8/T9 thorax and vertical distraction force was applied over the C7 vertebra with the magnitude of 20,40,60,80,100,and 120 N.The intervertebral disc stress and vertebral displacement in concave and convex sides, and Cobb angle of the spine were analyzed under two orthopedic modalities.RESULTS AND CONCLUSION:(1)With lateral thrust, there was no significant change in the C7T1-T7T8 intervertebral disc. The concave and convex stress of T7T8-L4L5 segment decreased first and then increased with the increase of lateral thrust force. The correction effect of lateral thrust on the segment near T8T9 was obvious and weakened with the extension of the segment to the cephalic and caudal ends.At 120 N of lateral thrust, the thoracic Cobb angle changed from 53.2° to 32.5° and the lumbar Cobb angle changed from 50.2° to 43.9°.(2)With the vertical distraction, the thoracic intervertebral disc stresses first decreased and then increased, and all the lumbar disc stresses decreased. The C7 displacement was the most obvious, and the correction effect gradually diminished with the segment extended to the caudal end.At a vertical distraction force of 120 N,the thoracic Cobb angle changed from 53.2° to 39.4° and the lumbar Cobb angle changed from 50.2° to 47.6°.(3)It is concluded that both orthopedic modalities provide improvement in the degree of scoliosis, with the thoracic correction being greater than the lumbar correction. Also, the asymmetric stress distribution on the concave and convex sides is improved, which contributes to normal bone growth.A vertical distraction approach is appropriate for larger Cobb angles, and a lateral thrust approach is appropriate for smaller Cobb angles. The results of this study help to understand the mechanism of spinal orthosis and provide a theoretical basis for the choice of orthopedic approach.

Zika virus(ZIKV)is a mosquito-borne flavivirus associated with neurological complications such as Limbari syndrome in adults and microcephaly in newborns. As there are no drugs or preventive vaccines available to specifically treat ZIKV infection, there is an urgent need to take comprehensive measures to prevent and control ZIKV infection and stop its spread. In this paper,we reviewed the structure and function of ZIKV gene and protein, and summarized the progress in the development of virus-targeting anti-ZIKV drug candidates through high-throughput screening and drug relocation, and summarized the mechanism of action of virus and host cell targeting drug candidates. The research and development trend of small molecule antiviral drugs in the future is prospected, which provides reference for promoting the research and development of new anti-ZIKV drugs.

Objective:To detect the expression levels of various immune cells in peripheral blood, and to ex-plore their relationship with unexplained recurrent spontaneous abortion(URSA)and its clinical significance in di-agnosis and treatment. Methods:A total of 351 patients with URSA who visited Hebei Reproductive Health Hospi-tal between January 2020 and September 2022 were enrolled as the URSA group, while a control group compri-sing 30 healthy women who had experienced uncomplicated term delivery once and had no history of adverse pregnancy outcomes during the same period was selected. The levels of various immune cell subsets in peripheral blood during the luteal phase were measured and compared between the two groups.Receiver operating charac-teristic(ROC)curve analysis was performed to determine the optimal cutoff values of immune cell levels that could assist in URSA diagnosis and treatment, while logistic regression analysis was conducted to identify factors influ-encing URSA occurrence. Results:The number and percentage of CD3+CD8+cells in the peripheral blood of the URSA group were significantly lower compared to those in the control group(P<0.05).Conversely,the per-centage of NK cells was significantly higher in the URSA group than in the control group(P<0.05).Logistic re-gression analysis revealed that the increase in CD3+CD8+cell count in peripheral blood during the luteal phase was a protective factor of URSA(OR<1,P<0.05).ROC curve analysis determined optimal cutoff values for im-mune cell levels to assist in the diagnosing and treating URSA:702.82 cells/μl for CD3+CD8+cell count,28.39%for CD3+CD8+cell percentage, and 12.33%for NK cell percentage. After using the optimal cutoff value to convert continuous variables into binary variables,Logistic regression analysis showed that a CD3+CD8+cell count<702.82 cells/μl and an NK cell percentage>12.33%were independent risk factors for URSA(OR>1,P<0.05).Conclusions:The decrease of CD3+CD8+cells in peripheral blood during midluteal phase is closely related to the occurrence of URSA.The number of CD3+CD8+cells(702.82 cells/μl)and the percentage of NK cells(12.33%)can be used as reference values for the diagnosis and treatment of URSA.

Objective:To investigate the efficacy and safety of high semi-decubitus position in the second stage of labor in epidural analgesic delivery of primipara. Methods:A total of 120 women who gave birth in Hang-zhou women's Hospital from May 1,2021 to July 30,2021 were randomly divided into experimental group(n=60)and control group(n=60)by random number table method. The control group adopted supine position in the sec-ond stage of labor, and the experimental group transferred to high semi-supine position after the supine position was used applied for 30 minutes when the fetal head was not exposed. To compare the effects of different postur-al positions on the duration of the second stage of labor, mode of delivery, postpartum injury, perineal tearing,la-bor force experience and labor control, and neonatal asphyxia. Results:The duration of uterine opening to fetal head exposure, the duration of uterine opening to fetal head crown and the duration of second stage of labor in experimental group were shorter than those in control group. The rate of natural delivery of experimental group was higher than that of control group. The episiotomy rate and the second degree perineal laceration rate were lower than that of control group. The amount of blood loss during delivery and 2h postpartum of experimental group was less than that of control group. The scores of birth experience and birth control of experimental group were higher than those of control group, and the differences were statistically significant(P<0.05).There was no significant difference in 1-minute Agpar score and 5-minute Agpar score between the two groups(P>0.05).Conclusions:The application of high semi-decubitus position under epidural labor analgesia promoted vaginal natural labor, shortened the time of second stage of labor, reduced episiotomy rate and perineal laceration de-gree,reduced postpartum hemorrhage, had positive labor force experience, and increased the sense of labor con-trol.
