Objective To explore the therapeutic effect of flexible ureteroscopic lithotripsy(FURL)by using flexible ureteroscope combined with flexible vacuum suction ureteral access sheath in the treatment of renal calculi with diameter 3-4.5 cm and CT value≤1000 HU.Methods A Retrospective analysis was conducted on clinical data of 42 patients with 3-4.5 cm renal calculi and CT value≤1000 HU from March 2023 to March 2025.A disposable flexible ureteroscope combined with flexible vacuum suction ureteral access sheath was used to fragment the calculi with holmium laser, and the fragments were then aspirated. Results Two cases had their stones left untreated when the operation time exceeded 2.5 h,resulting in the termination of the surgery. The remaining40 cases were successfully treated with the operation. The operation time was 85-150 min, with an average of(113.9±17.8)min.The postoperative hospital stay was2-5 d,with an average of(2.2±0.6)d.Postoperative fever occurred in 2 cases, and hematuria appeared in 5 cases, all of which were cured by conservative treatment. On the first postoperative day, residual stones were found in 4 cases. At 1 month after surgery, residual stones were detected in 3 cases. One case was treated with extracorporeal shock wave lithotripsy(ESWL)and the stones were expelled after 1 month. The other 2 cases(those whose surgery was terminated)underwent a second FURL and achieved successful stone clearance. Conclusion FURL for 3-4.5 cm renal calculi with CT value≤1000 HU by using flexible ureteroscope combined with flexible vacuum suction ureteral access sheath is safe and effective.

Neuropathic pain is frequently comorbidity with cognitive deficits. Neuralized1 (Neurl1)-mediated ubiquitination of CPEB3 in the hippocampus is critical in learning and memory. However, the role of Neurl1 in the cognitive impairment in neuropathic pain remains elusive. Herein, we found that lumbar 5 spinal nerve ligation (SNL) in male rat-induced neuropathic pain was followed by learning and memory deficits and LTP impairment in the hippocampus. The Neurl1 expression in the hippocampal CA1 was decreased after SNL. And this decrease paralleled the reduction of ubiquitinated-CPEB3 level and reduced production of GluA1 and GluA2. Overexpression of Neurl1 in the CA1 rescued cognitive deficits and LTP impairment, and reversed the reduction of ubiquitinated-CPEB3 level and the decrease of GluA1 and GluA2 production following SNL. Specific knockdown of Neurl1 or CPEB3 in bilateral hippocampal CA1 in naïve rats resulted in cognitive deficits and impairment of synaptic plasticity. The rescued cognitive function and synaptic plasticity by the treatment of overexpression of Neurl1 before SNL were counteracted by the knockdown of CPEB3 in the CA1. Collectively, the above results suggest that the downregulation of Neurl1 through reducing CPEB3 ubiquitination and, in turn, repressing GluA1 and GluA2 production and mediating synaptic plasticity impairment in hippocampal CA1 leads to the genesis of cognitive deficits in neuropathic pain.

Objective To explore the efficacy of negative pressure ureteral access sheath combined with disposable flexible ureteroscope(UAS+FRUS)in the treatment of renal calculi of 2-3 cm,so as to provide reference for the treatment. Methods A retrospective analysis was conducted on 130 cases of renal calculi of 2-3 cm treated with surgery in Xiamen Third Hospital during Sep.2021 and Sep.2023,including 68 cases with UAS+FRUS and 62 cases with super-mini percutaneous nephrolithotripsy(SMP).The perioperative indexes and stone-clearance rate(SFR)were compared between the two groups. Results All operations were successful. There were no statistically significant differences in the total SFR and incidence of complications(5.88%vs.9.67%)between the two groups 3 days(88.24%vs.90.32%)and 1 month(91.18%vs.93.55%)after surgery(P>0.05).For patients with lower calyceal calculi with infundibulopelvic angle(IPA)<45°,the SFR of the UAS+FRUS group was significantly lower than that of the SMP group(57.14%vs.100%,P<0.05).The UAS+FRUS group had a longer operation time than the SMP group[(104.94±8.79)minutes vs.(77.98±6.60)minutes,P<0.001],higher hospitalization costs[(23 112.82±1152.34)yuan vs.(21 975.84±1512.24)yuan,P<0.001],less postoperative decrease in hemoglobin[(6.71±2.07)g/L vs.(9.81±4.80)g/L,P<0.001],and shorter postoperative hospitalization time[(3.28±0.51)d vs.(5.58±0.71)d,P<0.001].The UAS+FRUS group had lower postoperative VAS score at 6,24,and 48 hours than the SMP group[(6.38±0.69)vs.(7.87±0.88);(3.62±0.73)vs.(5.81±0.83)and(3.12±0.33)vs.(3.81±0.60)],with statistical significance(P<0.05).Conclusion Both surgical methods have a high SFR in the treatment of renal calculi of 2-3 cm.SMP has the advantages of short operation time, low hospitalization costs, and high SFR for lower calyx calculi, while UAS+FURS has the advantages of little bleeding, minimal trauma, and short hospital stay.Surgeons can make reasonable choices based on the patients'condition and willingness, combined with their own surgical experience.

ObjectiveTo investigate the tumor-suppressing effect of Shenqi Yiliu prescription combined with cisplatin in hepatoma H22-bearing mice based on the phosphatase and tensin homolog deleted on chromosome ten (PTEN)/phosphatidylinositol-3-kinase (PI3K)/protein kinase B (Akt) pathway. MethodH22-bearing mice were prepared and randomized into model group, cisplatin group, and cisplatin combined with high-, medium-, and low-dose Shenqi Yiliu prescription groups, with 10 mice in each group. Another 10 healthy mice were randomly selected as normal group. Shenqi Yiliu prescription was given by gavage with the high, medium, low dose of 54.06, 27.03, 13.515 g·kg-1·d-1, respectively, and cisplatin (2.5 mg·kg-1) was administered by intraperitoneal injection, twice a week. Normal group and model group received normal saline. After 13 days of treatment, mice were killed and the tumor inhibition rate was calculated. The pathomorphological changes of tumor were observed based on hematoxylin-eosin (HE) staining, and enzyme-linked immunosorbent assay (ELISA) and immunofluorescence method were used to detect the content of cyclin-dependent kinase inhibitor 1A (p21) and cyclin-dependent kinase inhibitor 1B (p27) in tumor tissue of mice. The levels of PTEN, PI3K and phosphorylated protein kinase B (p-Akt) in tumor tissue were measured by Western blot. ResultCompared with the model group, cisplatin alone and cisplatin in combination with the high-, medium-, and low-dose Shenqi Yiliu prescription decreased tumor mass (P<0.05), particularly the cisplatin in combination with the high-dose Shenqi Yiliu prescription. Necrosis of the tumor tissue was observed in each group, especially the cisplatin combined with high-dose Shenqi Yiliu prescription group. As compared with the model group, cisplatin alone and cisplatin in combination with the high-, medium-, and low-dose Shenqi Yiliu prescription raised the expression of p21, p27, and PTEN (P<0.05) and lowered the expression of PI3K and p-Akt (P<0.05), particularly the cisplatin in combination with high-dose Shenqi Yiliu prescription. ConclusionShenqi Yiliu prescription may regulate the expression of key molecules in PTEN/PI3K/Akt signaling pathway, thereby upregulating the expression of downstream proliferation inhibitors p21 and p27, further suppressing the tumor in H22-bearing mice, and enhancing the effect of chemotherapy.
