Polycystic ovary syndrome (PCOS), a common reproductive endocrine disorder in women, is one of the leading causes of ovulatory infertility in women of reproductive age. Due to its heterogeneous etiology, complex symptoms, and challenging treatment, PCOS has become a focal point of research in gynecological and reproductive medicine globally. The pathogenesis of PCOS is complex and may involve regulatory mechanisms such as inflammatory responses, oxidative stress, and cellular autophagy. Crown-like structures (CLSs) refer to pro-inflammatory microenvironments formed by macrophages engulfing adipocytes. The inflammatory disorders induced by CLSs are one of the key factors contributing to the development of PCOS and its complications. Current studies have indicated that the obese status in PCOS accelerates the formation of CLSs, and the density of CLSs can predict the progression of metabolic disorders and influence the outcomes of various metabolic diseases. Traditional Chinese Medicine (TCM) offers the unique advantages of a holistic view, four diagnostic methods, and syndrome differentiation and treatment to ameliorate the symptoms and signs of PCOS through multiple levels, pathways, and targets. Although studies on the mechanisms of metabolic diseases and CLS formation have been reported in China and abroad, there is still a lack of literature on the correlation between CLSs and PCOS, as well as reviews on TCM interventions targeting CLSs for treating this disease. Therefore, this paper summarized the correlation between obese PCOS and CLSs and reviewed recent studies on TCM interventions based on CLS formation (adipose tissue-macrophage inflammatory crosstalk) in the treatment of obese PCOS, aiming to provide new research perspectives for the prevention and treatment of PCOS using TCM.

3.Clinical observation of nicorandil combined with different doses of tirofiban in the treatment of elderly patients with acute ST-segment elevation myocardial infarction

OBJECTIVE To explore the clinical effect and safety of nicorandil combined with different doses of tirofiban in the treatment of elderly patients with acute ST-segment elevation myocardial infarction (STEMI). METHODS A total of 162 elderly patients with STEMI admitted to our hospital from June 1, 2022 to June 1, 2024 were retrospectively enrolled. All patients received percutaneous coronary intervention (PCI) and conventional treatment of STEMI, and used nicorandil (5 mg each time, tid) and tirofiban. According to the use of tirofiban, the patients were divided into conventional-dose group (n=104) and half-dose group (n=58). Patients in the conventional-dose group received an intracoronary injection of 10 μg/kg tirofiban, followed by intravenous infusion of 0.1 μg/(kg·min) for 48 h; patients in the half-dose group received an intracoronary injection of 5 μg/kg tirofiban, followed by intravenous infusion of 0.05 μg/(kg·min) for 48 h. Related indexes of PCI (the proportion of patients with grade 3 of thrombolysis in myocardial infarction, no reflow in infarct related artery, percentage decrease in total ST-segment elevation >50% on electrocardiogram 2 hours after PCI), cardiac function parameters before and after treatment (troponin I, N-terminal pro-brain natriuretic peptide contents and left ventricular ejection fraction), bleeding events during treatment (gingival bleeding, epistaxis, mucosal bleeding, gastrointestinal bleeding) and other adverse events (all-cause death, non-fatal reinfarction, hypotension, ventricular fibrillation, acute heart failure) were compared between 2 groups. RESULTS There were no significant differences in related parameters of PCI, cardiac function parameters after treatment, the incidences of gingival bleeding, epistaxis and gastrointestinal bleeding, the total incidence of bleeding events, or the incidence of other adverse events during treatment between 2 groups (P>0.05), but the incidence of mucosal bleeding in the conventional-dose group was significantly higher than the half-dose group (P<0.05). CONCLUSIONS The clinical effect of nicorandil combined with half-dose tirofiban in the treatment of elderly patients with acute STEMI is comparable to that of nicorandil combined with conventional dose of tirofiban, but the mucosal bleeding risk of the former is lower than that of the latter. Therefore, patients at risk of mucosal bleeding are more suitable to use the previous regimen.
4.Value of artificial intelligence in assisting ultrasound residents training for the identification,measurement and diagnosis of fetal nuchal translucency thickness

Objective:To explore the clinical application value of artificial intelligence(AI)-assisted training in enhancing the accuracy of nuchal translucency(NT)identification,standardization of measurement,and diagnostic efficacy for abnormalities among ultrasound residents.Methods:A retrospective collection of 300 standard fetal NT ultrasound images was conducted at the Center for Medical Ultrasound,Suzhou Hospital Affiliated of Nanjing Medical University from January 2018 to June 2024. The AI model performed NT measurements and diagnoses once. Four sonographers of different seniority levels(including two resident physicians)independently conducted NT measurements and diagnoses twice. Prior to the experiment,the middle-age and resident sonographers had uniformly completed traditional theory training. Following the first independent measurements,the two resident sonographers received additional AI-assisted training,after which all 4 sonographers performed the second independent measurements. A fetal medicine expert evaluated blindly all the results and compared the differences in NT recognition accuracy,measurement standard rate and diagnosis accuracy between the middle-age sonographer(traditional training only)and two resident sonographers(traditional + AI-assisted training).Results:For the middle-aged sonographer who only received traditional lecture-based training,the accuracy of NT recognition,standardization rate of measurement,or diagnostic accuracy were not significantly improved befroe and after the training,and the diffrence was not statistically significant( χ2=0.189,1.887,0.326;all P>0.05). In contrast,the second-year resident(Resident 2)and first-year resident(Resident 1),who received both traditional lecture-based training and AI training,demonstrated some improvements in the accuracy of NT measurement site recognition,though the differences were not statistically significant( χ2=1.301,2.418;all P>0.05). However,both residents did significant improvements in the standardization rate of NT measurement( χ2=25.768,17.035;all P<0.05). In terms of diagnostic accuracy,Resident 1 did significant improvement( χ2=10.180, P<0.05),while Resident 2 also did some improvement,though the difference was not statistically significant( χ2=2.573, P>0.05). Conclusions:The AI-assisted training system enhances the ability of ultrasound resident sonographers to recognize,measure,and diagnose NT,providing a novel and efficient training model for standardized residency training in ultrasound specialties.
5.Diagnostic value of fetal cardiac ultrasound screening views in the first trimester for congenital heart disease

Objective:To investigate the diagnostic value of fetal cardiac ultrasound view visualization in the first trimester for congenital heart disease(CHD).Methods:A retrospective analysis was performed on 13 323 singleton fetuses who underwent first-trimester(11-13 +6 weeks)ultrasound screening at the Ultrasound Medicine Center,the Affiliated Suzhou Hospital of Nanjing Medical University from January 2018 to June 2024. Cardiac views including the four-chamber view(4CV),left ventricular outflow tract view(LVOT),and Results:The study group showed significantly higher rates of "poorly visualized" 4CV,LVOT,and 3VT than the control group(2.70% vs. 0.14%, P=0.005;36.49% vs. 4.76%, P<0.001;36.49% vs.2.46%, P<0.001). The efficacies of combination 1(any view abnormal)and combination 2(any view "poorly visualized" or "abnormal")were comparable,with AUCs of 0.86 and 0.85( P=0.424). The AUCs of combination 3(3VT "poorly visualized" or any view "abnormal")and combination 4(4CV "poorly visualized" or any view "abnormal")were 0.88 and 0.86( P=0.424),all significantly higher than combination 5(LVOT "poorly visualized" or any view "abnormal",AUC=0.84,all P<0.05). Conclusions:"Poorly visualized" cardiac views in the first trimester demonstrate good diagnostic efficacy for CHD,particularly when 3VT or 4CV are affected,warranting heightened clinical vigilance for fetal cardiac anomalies.
6.Risk of CHF complicated with deep venous thrombosis:a logistic regression analysis based on neutrophil count and coagulation parameters

Objective To analyze the risk of chronic heart failure(CHF)complicated with deep ve-nous thrombosis by using logistic regression model based on neutrophil and coagulation parame-ters.Methods A retrospective analysis was conducted on 132 CHF patients admitted to our hos-pital from August 2021 to August 2023.According to deep venous thrombosis occurred or not,they were divided into thrombosis group(44 cases)and non-thrombosis group(88 cases).The neutrophil count and coagulation parameters were measured in all subjects after admission.Binary logistic regression analysis was used to construct a prediction model based on neutrophil count and coagulation parameters.ROC curve was plotted to analyze the predictive value of the count and parameters in predicting deep venous thrombosis in CHF patients.Results The thrombosis group had significantly higher serum neutrophil count and D-Dimer(D-D)and fibrinogen(FIB)levels than the non-thrombosis group(P<0.01),but no statistical differences were observed be-tween them in terms of activated partial thromboplastin time,prothrombin time and thrombin time(P>0.05).Binary logistic regression analysis suggested that serum neutrophil count and D-D and FIB levels were independent risk factors for deep venous thrombosis in CHF patients(OR=2.778,95%CI:1.672-4.617,P=0.000;OR=12.089,95%CI:4.310-33.903,P=0.000;OR=2.357,95%CI:1.170-4.746,P=0.016),and the prediction model was manifested as logit(P)=-13.908+1.022 × neutrophil+2.492 × D-D+0.857 × FIB.ROC curve analysis showed that the AUC value of serum neutrophil count,D-D and FIB levels alone and combined in predicting deep venous thrombosis were 0.772,0.82 9,0.712 and 0.913,respectively,with a sensitivity of 70.45%,72.73%,47.73%and 81.82%,and a specificity of 80.68%,78.41%,96.59%and 96.59%,separate-ly.The combined prediction model showed better efficiency than the single indicator(P<0.01).Conclusion Neutrophil count and coagulation parameters are closely associated with the occur-rence of deep venous thrombosis in CHF patients.Early combined determination of the above three indicators can improve the prediction.

10.Research progress about traditional Chinese medicine in the intervention of gastric cancer by regulating Th17/Treg balance

Gastric cancer (GC) is a common malignant tumor of the digestive tract. T helper cells 17 (Th17) and T regulatory cells (Treg) are differentiated subsets of CD4+T cells. Th17/Treg imbalance has been shown to be closely related to the progression of GC. Traditional Chinese medicine (TCM) can not only improve the survival prognosis of GC patients, but also play a role in enhancing the efficacy and reducing the toxicity of postoperative chemotherapy for GC. This paper systematically sorted out the action rules of TCM in the intervention of GC by regulating Th17/Treg balance. The results showed that the TCM compound could regulate the balance of GC Th17/Treg by invigorating the spleen and invigorating Qi, warming Yang, removing blood stasis and detoxifying. The mechanism of regulating Th17/Treg balance in the intervention of GC is mainly to inhibit the excessive differentiation of Th17 and Treg and the overexpression of transcription factors and cytokines, reverse the excessive drift of GC Th17/Treg balance to Th17 or Treg, and thus restore the immune balance of GC Th17/Treg.
