1.Heterogeneity of sub-dimensions of satisfaction with the quality of assistive devices from the perspective of self-care ability stratification

ObjectiveTo explore the differences in attention to the sub-dimensions of satisfaction with the quality of assistive devices among people with different self-care abilities in China, identify the key driving factors, and provide a basis for the precision design and service provision of assistive devices. MethodsBased on the 2023 national survey data, involving 14 030 people with functional impairments, self-care ability was taken as the core independent variable, eight sub-dimensions of satisfaction as dependent variables, and variables such as gender, age, educational level and residential type were controlled. Univariate analysis was performed using Chi-square test with Bonferroni correction, and a binary Logistic regression model was constructed to identify influencing factors. Meanwhile, reliability and validity tests, endogeneity tests (instrumental variable method and propensity score matching) and heterogeneity tests were conducted. ResultsAmong the eight satisfaction sub-dimensions, six presented significant inter-group differences, with Bonferroni correction (threshold = 0.00625). Following binary Logistic regression and endogeneity correction, significant inter-group heterogeneity was confirmed in dimensions such as size and shape. For the affordability dimension, the main effect of self-care ability was not statistically significant, yet prominent urban-rural heterogeneity was observed. Specifically, taking the fully independent (self-care) group as the reference, the fully dependent group attached significantly greater importance to safety (B = 0.253, P < 0.001), comfort (B = 0.153, P = 0.001) and ease of use (B = 0.316, P < 0.001); the partially dependent group showed the highest level of attention to lightweight (B = 0.094, P = 0.027) and durability (B = 0.254, P < 0.001); and the fully independent group demonstrated a relatively stronger preference for aesthetics. ConclusionStratified functional demands, driven by self-care ability, exist in the satisfaction of individuals with functional impairments with assistive devices in China. The policy formulation and product design of assistive devices should shift to a precision-oriented paradigm: prioritize the guarantee of safety, comfort and ease of use for fully dependent groups, optimize lightweight performance and durability for partially dependent groups, enhance aesthetics and social acceptance for fully independent groups, roll out price subsidy policies for urban price-sensitive groups, and strengthen the supply of core functional services for rural groups. This approach will comprehensively improve the adaptation effectiveness of assistive devices and the well-being of users.
2.Evaluation of assistive device adaptation and analysis of influencing factors among people with dysfunction

Objective To investigate the current status of assistive device adaptation among people with dysfunction in China,identi-fy unmet assistive needs for assistive products,and determine key factors influencing attitudes toward assistive device use.Methods Based on data from the Chinese segment of the Global Rapid Assistive Technology Assessment(rATA)survey,5 935 individuals with dysfunction from Beijing,Anhui,Fujian,Hubei,Sichuan and Shaanxi were included.The adaptation status of assistive devices was analyzed,and a theoretical framework was constructed using the tech-nology acceptance model.Multiple linear regression analysis was applied to identify factors influencing attitudes toward assistive device use.Results The utilization rate of rehabilitation assistive devices among people with dysfunction was 65.9%,with a notable prevalence of multiple functional impairments.Fit rate was the highest(68.44%)in individuals with visual im-pairments,while those with communication impairments had the lowest(1.52%).Fit rates were significantly low-er among rural residents and older adults.Overall satisfaction with assistive devices was relatively high(4.11±0.71).Accessibility of the home environment(B=0.396,P<0.001),accessibility of the public environment(B=0.204,P<0.001),functional utility of assistive devices(B=0.204,P<0.001),and distance to obtain the devices(B<-0.112,P<0.01)were key factors affecting satisfication toward assistive device use,while the source of payment showed no significant impact(P>0.05).Conclusion Significant structural imbalances and group disparities exist in the adaptation of assistive devices among people with dysfunction in China.The phenomenon of multiple functional impairments has not received suffi-cient attention,and there remains a substantial unmet need for assistive products among non-fitted groups.Al-though overall satisfaction with assistive devices is high,influencing factors are complex.Continuous improve-ment of accessibility in both private and public environments,promotion of personalized adaptation services,and optimization of assistive device service networks are essential to enhance adaptation accuracy,sustained use,and subjective well-being among people with dysfunction.

4.Unmet needs for assistive technology and its related factors for persons with physical disabilities in Chengdu,Chi-na

Objective To investigate the unmet needs for assistive technology for people with physical disabilities in Chengdu,and analyze the related factors. Methods From November,2023 to March,2024,the persons with physical disabilities in Chengdu were selected from Sichuan Individuation service platform,and investigated using World Health Organization rapid Assistive Tech-nology Assessment. Results A total of 558 questionnaires were set up,and 527 effective questionnaires retured.26.8%of them reported un-met needs for aids,with the highest need for mobility aids(66.0%).Lack of support(54.9%),high price(26.3%)and lack of knowledge about aids(20.3%)were the main reasons for not obtaining the aids they needed.Loss of spouse(OR=3.615),serious mobility impairment(OR>2.926)and serious self-care impairment(OR>2.781)were the risks of unmet needs for aids. Conclusion It is important to popularize policies and products of aids,pay attention to personal adaptation for people with different barriers,and strengthen the service system,to meet the needs of people with disabilities.

6.Survey on vision-related quality of life for visual impairment students in special school

Objective To investigate visual impairment students' quality of life and its influencing factors in Braille learning class at Quanzhou Special School. Methods November, 2020, 52 students (aged seven to 26) with the best corrected distance visual acuity of the better eye above 0.02 were investigated with near visual acuity, contrast sensitivity and Chinese-version Low Vision Quality of Life Questionnaire (CLVQOL). The subjects were divided into second grade blindness group, first grade low vision group and second grade low vision group according to the best corrected distance visual acuity of the better eye. Results There were significant differences in distance visual acuity (Z = 45.671, P < 0.001), near visual acuity (Z = 24.972, P < 0.001), and contrast sensitivity (CS) ( Z = 13.285, P = 0.001) among three groups. There was a correlation between near visual acuity and distance visual acuity (r = 0.74, P < 0.001), CS to distance visual acuity (r = -0.58, P < 0.001) and near visual acuity (r = -0.57, P < 0.001), score of CLVQOL and CS (r = 0.44, P < 0.001). There were significant differences in the total score (Z = 10.145, P = 0.006), distance visual acuity subscale (Z = 13.586, P = 0.001), psychological adjustment subscale (Z = 7.824, P = 0.020), reading and fine work subscale (Z = 7.923, P = 0.019) of CLVQOL among the three groups. Conclusion Quality of life is different with the visual impairment for students in special school, especially the distance visual acuity, psychological adjustment and fine reading. CS correlates to the quality of life of visually impaired students, which needs to be a part of evaluation of visual function.
7.Analysis of Medication Rules of the Prescription Containing Strychnos nuxvomica in the Dictionary of Traditional Chinese Medicine Prescription

OBJECTIVE: To provide reference for the clinical use and dosage form development of Strychnos nuxvomica. METHODS: The prescriptions containing S. nuxvomica were collected and sorted from the Dictionary of Traditional Chinese Medicine Prescription, and then formula name, prescription source, name of treatment disease, composition, usage and dosage, medication route were input into Access 2010 and database was established. According to the usage, these prescriptions were divided into internal use and external use, and the frequencies of diseases treated by internal/external use prescriptions were counted respectively. Apriori algorithm of SPSS Modeler 18.0 software was used to analyze the drug combination rule of compound prescription containing S. nuxvomica. RESULTS: A total of 78 prescriptions were included, of which 42 were oral prescriptions, 33 were external prescriptions and 3 were both oral and external prescriptions. Internal prescriptions were used to treat diseases such as atrophy-flaccidity disease, arthralgia, hemorrhoids, anal fistula, bones and muscles pain. The external prescriptions were mainly used to treat skin diseases, tumors, lump. Internal and external prescriptions could treat sore, injuries and fractures. In internal prescriptions, S. nuxvomica was often compatible with Boswellia carterii, myrrha, Daemonorops draco, Manis pentadactyla, pyritum, Angelica sinensis, Ephedra sinica, Aconitum kusnezoffii. In external prescriptions, S. nuxvomica was often compatible with Radix Paeoniae, Rehmannia glutinosa, Phellodendron chinense, Coptis chinensis, Scutellaria baicalensis, Angelica sinensis, Angelica dahurica, Buthus martensii. Most of S. nuxvomica were used after processing, and mainly took sesame oil as an auxiliary material (77.97%). CONCLUSIONS: Diseases treated with the prescriptions containing S. nuxvomica can be classified into three types of diseases, i.e. polymyalgia rheumatica and paralysis, bruise and bone fracture, carbuncle and stubbornness tinea. In external prescriptions, S. nuxvomica is often compatible with drug for clearing heat and drying dampness, cooling blood and activating blood, and relieving pain. In internal prescriptions, S. nuxvomica is often compatible with the agents for blood-activating and stasis-resolving medicinal. In addition, the processing of S. nuxvomica with sesame oil is an important measure to ensure its safety.
8.Thompson procedure for chronic mallet finger with tendon defect

Objective To evaluate the Thompson procedure in the treatment of chronic mallet finger with tendon defect.Methods From July 2014 to October 2016,9 cases of chronic mallet finger with tendon defect were treated by Thompson procedure at Department of Hand Surgery,Wuhan Puai Hospital.They were 4 males and 5 females,aged from 25 to 56 years (average,36 years).Their disorder was on the left side in 3 cases and on the right side in 6 ones,involving 2 index fingers,3 middle fingers,3 ring fingers and one little finger.The ranges of motion (ROM) for distal and proximal interphalangeal joints were measured after operation.Operative complications were observed and recorded.Dargan functional assessment was used to evaluate the outcomes at the final follow-up.Results All the 9 patients were followed up for 5 to 36 months (average,16.5months).The wounds healed primarily without such complications as infection,skin breakage,abnormal fingertip sensation or nail deformity.All cases of mallet finger malformation were corrected.About 10 weeks after operation,one case presented with mild mallet finger malformation which was completely corrected after active fixation for 8 weeks.The Dargan assessment at the final follow-up showed 8 excellent and one good cases.Conclusion Thompson procedure can lead to satisfactory outcomes and limited complications in the treatment of chronic mallet finger with tendon defect.
9.Necessity of Setting Up Security System of Assistive Technology in China

10.Effect of methylprednisolone stosstherapy on CD4+CD25+ regulatory T cells and CD95 levels in peripheral blood of patients with myasthenia gravis

Objective To analyze the effect ofmethylprednisolone stosstherapy on CD4+CD25+ regulatory T cell (Treg) and apoptosis factor CD95 expressions in the external peripheral blood of myasthenia gravis (MG) patients and explore the influencing factors of different clinical efficacy.Methods Thirty-one patients with MG,admitted to our hospital from January 2013 to September 2015,were included in this study;and 27 normal subjects were selected as control group.Methylprednisolone stosstherapy was given to the patient group.Expressions of Treg and CD95 in peripheral blood were detected by flow cytometry before and after treatment.Results Treg percentage of patients whose clinical absolute scale scores less or equal than 3 points and relative scale scores higher than 0.5 after methylprednisolonestosstherapy (13.39%+2.71%) was significantly higher than that ofhealthy controls (8.35%+1.87%,P<0.05);Treg percentage of patients whose clinical absolute scale scores greater than 3 as well as relative scale scores less than 0.5 after methylprednisolone stosstherapy (8.17%+1.31%) was slightly lower than that of healthy controls (P>0.05).CD95 percentage of patients whose clinical absolute scale scores less or equal than 3 points and relative scale scores higher than 0.5 after methylprednisolone stosstherapy (36.47%±5.32%) was not statistically different as compared with that of healthy controls (35.28%±5.58%,P>0.05),CD95 percentage of patients whose clinical absolute scale scores greater than 3 as well as relative scale scores less than 0.5 after methylprednisolone stosstherapy (34.97%±5.12%) was not statistically different as compared with that of healthy controls (P>0.05).Conclusion Treg ratio increases,CD95 change is not obvious in patients whose clinical curative effect is improved markedly after methylprednisolone stosstherapy.
