1.Clinical effect of meropenem combined with cefotaxime sodium on treatment of interstitial pneumonia patients complicated with pulmonary infection

OBJECTIVE To explore the clinical effect of meropenem combined with cefotaxime sodium on treatment of interstitial pneumonia(IP)patients complicated with pulmonary infection and observe the influence on vascular endothelial growth factor(VEGF),Toll-like receptor 2(TLR2)and transforming growth factor β1(TGF-β1)so as to provide guidance for treatment of the IP patients complicated with pulmonary infection and assessment of curative effect.METHODS A total of 102 IP patients complicated with pulmonary infection who were treated in Guiyang Second People's Hospital from Feb,2022 to Feb,2024 were recruited as the research subjects and were randomly divided into the CTX group and the combination group,with 51 cases in each group.The CTX group was treated with cefotaxime sodium,and the combination group was given meropenem combined with cefotaxime sodium.The curative effect,lung function indexes,blood gas indexes,T lymphocyte subsets,peripheral blood VEGF,TLR2 and TGF-β1 were observed and compared between the two groups.RESULTS There was significant difference in the effective rate of treatment between the combination group and the CTX group(Z=7.229,P=0.007).The lung function of the combination group was better than that of the CTX group after the treatment for 7 days(P<0.05);the levels of helper T cell type 1(Th1),Th1/Th2 and CD4+Foxp3+Treg of the combination group were higher than those of the CTX group(P<0.05),while the Th2 level of the combination group was low-er than that of the CTX group(P<0.05).The levels of VEGF,TLR2 mRNA and TGF-β1 of the combination group were respectively(32.14±8.77)pg/ml,(1.23±0.30)mRNA and(17.25±4.56)pg/ml,lower than those of the CTX group(P<0.05).There were no significant differences in the drug-induced adverse reactions between the two groups of patients.CONCLUSIONS Meropenem combined with cefotaxime sodium can boost the curative effect on IP patients complicated with pulmonary infection,improve the lung function and blood gas indexes,reg-ulate the immune response,intensify the immune function,reduce the expression levels of VEGF,TLR2 mRNA and TGF-β1,and promote the rehabilitation of the patients.

The current classification methods for traditional Chinese medicine (TCM) visceral syndrome differentiation suffer from excessive generalization, which hinders their clinical application. Based on the analysis of the pattern of "one syndrome corresponding to multiple formulas", this paper focused on the principle of syndrome-formula correspondence, and proposed that formula-syndromes are the smallest units for refining visceral syndromes. By establishing the correspondence between formula-syndromes and visceral syndromes, this study aims to further clarify the refined categories of syndromes and their treatment patterns, providing a new perspective for the standardization and objectification of TCM syndromes.

4.Efficacy of online pulmonary rehabilitation management among community-dwelling patients with stable chronic obstructive pulmonary disease

Objective:To explore the efficacy of online pulmonary rehabilitation (PR) management among community-dwelling patients with stable chronic obstructive pulmonary disease (COPD).Methods:This study was a single-center randomized controlled trail with an unblinded design. A total of 130 patients with stable COPD who visited Zhuanqiao Community Health Service Center in Shanghai Minhang District from October 2020 to March 2022 were randomly divided into study group and control group with 65 cases in each group. Both groups received conventional treatment, while patients in study group attended online rehabilitation management, including face-to-face rehabilitation instruction and multiple online guidance. Pulmonary ventilation function including forced vital capacity (FVC), forced expiratory volume in the first second (FEV 1) and percentage of forced expiratory volume in the first second to forced expiratoty volume (FEV 1%pred), modified British Medical Research Council Dyspnea Scale (mMRC), chronic obstructive pulmonary disease assessment test (CAT), score of 6 minutes walking distance (6MWD) and DOSE (dyspnea, degree of airflow obstruction, smoking status, the number of exacerbation) index were measured at baseline and after 8 weeks of rehabilitation, and compared between two groups. Results:The baseline data of the two groups were comparable. After 8 weeks of management, FVC, FEV 1, FEV 1%pred, mMRC, CAT, 6MWD and DOSE index of both groups were improved compared with the baseline level(control group: t=-7.799, -7.581, -9.010, 3.565, 9.887, -16.677, 3.795; study group: t=-12.623, -13.914, -17.644, 7.404, 22.457, -26.826, 7.968; all P<0.05). The FEV 1%pred, CAT and 6MWD in the study group were better than those in the control group ( t=-2.939, 2.277,-2.130, all P<0.05); while there were no significant differences in FVC, FEV 1, mMRC and DOSE index between the two groups( t=-0.162, -1.280, 0.925, 1.939,all P>0.05). Conclusions:The online pulmonary rehabilitation management can better improve lung function, dyspnea symptoms and exercise tolerance of patients with stable COPD, which can be used for rehabilitation training and management of community-dwelling patients.
5.Influencing factors of self-management behavior in cancer patients based on a theoretical domain framework.

OBJECTIVES:
To explore the factors that influence self-management behavior in cancer patients based on the theoretical domain framework.
METHODS:
Studies in Chinese and English about factors influencing self-management behavior in cancer patients were searched from Wanfang database, CNKI, VIP, SinoMed, PubMed, Embase, CINAHL, Web of Science Core Collection, Cochrane library and Medline from inception to June 2022. Two investigators independently identified, extracted data, and collected characteristics and methodology of the studies. Factors were analyzed with Nvivo12, and the theoretical domain framework was mapped to the theoretical domain. Then the secondary node was generalized by theme analysis. Finally, the specific influencing factors were summarized and analyzed.
RESULTS:
Thirty-four studies were included for analysis. A total of 194 factors were mapped to 13 theoretical domains, and 31 secondary nodes were summarized. Theoretical domains environmental context and resources, social/professional role and identity, and beliefs about consequences were the most common factors. Knowledge, age, self-efficacy, disease stage, social support, gender, economic status and physical status were the most influential factors for self-management in cancer patients.
CONCLUSIONS
The influencing factors of self-management of cancer patients involve most of the theoretical domains, are intersectional, multi-source and complex.

6.Application of peritoneal dialysis routine examination in reducing peritonitis associated with peritoneal dialysis

Objective:To explore the application of peritoneal dialysis routine examination in reducing the incidence of peritonitis associated with peritoneal dialysis.Methods:From July 2018 to June 2019, 191 patients with peritoneal dialysis who were followed up regularly in the nephrology department were selected as the study subjects.Using convenient sampling method, outpatient follow-up on Tuesday were selected as control group(95 cases) and fixed outpatient follow-up on Thursday were selected as observation group(96 cases). Routine follow-up was performed in the control group, and routine examination of peritonitis was performed in the observation group during the follow-up.Intervention was given immediately when the problems were found to the naked eye and the examination results were abnormal.The incidence of peritonitis in the two groups was compared.Results:Peritonitis occurred in 24 cases in the control group(25.26%), and 9 cases in the observation group(9.38%). The incidence of peritonitis in the observation group was lower than that in the control group, and the difference was statistically significant(χ 2=5.972, P<0.05). Conclusion:The occurrence of peritoneal dialysis-related peritonitis is related to a variety of factors.Paying attention to the routine examination of peritonitis and observing the color of peritonitis, strengthening the aseptic fluid exchange operation, and following up the patients can effectively reduce the occurrence of peritonitis and improve the quality of dialysis.
7.Effect of methotrexate on regulation for the number of regulatory T cells and expression of Foxp3 in psoriasis.

To explore the role of methotrexate (MTX) in regulating the number of regulatory T cells (Treg) and the mRNA expression of transcription factor Foxp3.
Methods: 1) We analyzed the number of Treg and the mRNA expression of Foxp3 by flow cytometry (FCM) and quantitative real-time PCR (qRT-PCR) respectively in patients with psoriasis vulgaris, patients with psoriasis vulgaris after the 8-week treatment of MTX, and healthy people. 2) BALB/c female mice were smeared with imiquimod (IMQ) cream for 6 days. We recorded the change of the lesion in mice every day. The morphological changes of lesion in mice were evaluated by the psoriasis area and severity index (PASI) and HE staining. 3) The mouse model was randomly divided into a control group and an MTX group. The MTX group was treated with different doses of MTX (38.5 and 77.0 nmol/L) on the third day of this experiment. The morphological changes of lesion in mice were evaluated by PASI and HE staining. We tested the number of Treg and the expression level of Foxp3 mRNA in splenic lymphocytes.
Results: 1) The number of Treg and the expression level of Foxp3 mRNA were lower in psoriasis vulgaris patients than those in the healthy control group (P<0.05). After 8-week treatment of MTX, the number of Treg was increased (P<0.05) and Foxp3 mRNA level was up-regulated (P<0.01). 2) Typical psoriasis-like skin lesions, such as red scaly skin plaque were found after topical application of IMQ. Both the number of Treg in the splenic lymphocytes of mice and the Foxp3 mRNA level of Treg were reduced by IMQ (P<0.01 and P<0.05). 3) Different doses of MTX for mice showed the ability to improve skin lesion, increase the number of Treg in the spleen of mice and Foxp3 mRNA level in psoriatic dermatitis of mice (P<0.05).
Conclusion: MTX is able to regulate the number of Treg and Foxp3 mRNA expression in psoriasis.

8.Noninvasive assessment of hepatic fibrosis staging with MR elastography versus T1ρ imaging

Objective To compare the diagnostic values of magnetic resonance elastography (MRE) and T1ρ imaging in staging hepatic fibrosis (HF) in a rabbit model.Methods The institutional animal care and use committee approved all experiments.Sixty healthy rabbits were divided into HF group (n=44) and control group (n=16).Each eight rabbits in the HF group and 4 rabbits in the control group were randomly selected at the 4th,5th,6th week and the remaining rabbits at the 10th week after subcutaneous injection with 0.1 ml 50% CCl4 oily solution per kilogram of body respectively,to undergo liver MR scan including axial liver MRE and T1ρ imaging.The values of liver stiffness (LS) and T1ρ were measured.Masson trichrome staining of liver tissue was used.According to the Scheuer scoring system,rabbits were classified into F0 to F4 group based on the percentage of hepatic fibrosis.The difference of LS values and Tip values among stage F0 to F4 were compared by the one-way ANOVA analysis.The correlations between pathological staging and LS,T1ρ values were performed by the Spearman correlation analysis.ROC curve analysis was performed to compare the value of MRE with T1ρ imaging.Results Forty three rabbits were included,there were 10,8,8,8,9 rabbits in F0,F1,F2,F3 and F4 stage,respectively.LS values were (1.051±0.155),(1.335±0.235),(1.401±0.163),(2.001±0.499) and (2.981±0.714) kPa in F0,F1,F2,F3 and F4,respectively,while T1 p values were (23.20±4.02),(24.28±2.93),(25.40± 1.82),(24.69± 1.85) and (31.54±3.39) ms (all P<0.05).The correlation of LS values with hepatic fibrosis staging measured on MRE was stronger than T19 values (r values were 0.916 and 0.608,all P<0.01).Area under ROC curve of LS value for differentiating hepatic fibrosis stage were 0.938 to 0.989,while the areas of T1ρ were 0.771 to 0.954.Conclusion MR elastography is an accurate technique for quantitatively staging hepatic fibrosis and superior to T1ρ imaging.
9.Analysis of the features of orthopaedic diseases in the South Pacific region in overseas medical service

Objective To analyze medical care delivered to orthopaedic cases during " Mission Harmony-2014" and explore features of orthopaedic diseases in the South Pacific region.Methods General medical data of orthopaedic patients from Tonga,Fiji,Vanuatu and Papua New Guinea in the South Pacific region were classified and summarized,by using the hospital ship information system,and general medical conditions and features of those orthopaedic patients from the 4 island countries were analyzed statistically.Results During "Mission Harmony-2014",there were 3 872 person-times for orthopaedic patients accounting for 17.3% of all the patients,of whom 2 211 person-times were male patients accounting for 57.1%,and 1 661 person-times were female patients accounting for 42.3%.Of all the orthopaedic cases,there were 1 932 person-times delivered to the patients due to osteoarthritis (accounting for 49.9%) and 1 153 person-times delivered to the patients due to spinal disorders (accounting for 29.8%).Also during this medical mission,CT and radiologic scanning was delivered to 798 cases,of which 680 were positive cases,with the positive rate of detection accounting for 85.2%.Conclusions There is a great demand for medical service in orthopaedics in South Pacific region.The high rate of osteoarthritis is the significant feature in the distribution of orthopaedic diseases in this region,which might be closely associated with the overweight of local residents.
