1.Hepatitis B cirrhosis comorbid with Staphylococcus lugdunensis bloodstream infection: A case report

As a coagulase-negative staphylococcus, Staphylococcus lugdunensis has stronger virulence and pathogenicity than other Staphylococcus bacteria and is comparable to Staphylococcus aureus. In recent years, there has been a gradual increase in the number of related case reports, A patient with hepatitis B cirrhosis and Staphylococcus lugdunensis bloodstream infection was admitted to our department and was followed up for more than 2 years, and the patient was analyzed in terms of clinical features, pathogenicity, invasion, and the application of related antibiotics.
2.Correlation between vitamin D level and immune dysfunction in children with recurrent respiratory tract infection

Objective:To investigate the characteristics of vitamin D levels in children in Gansu Province,and to analyze the correlation between vitamin D levels and immune disorders in children with recurrent respiratory tract infections(RRTIs).Methods:From January 2020 to December 2021,9 790 cases of children aged 0~6 years who received vitamin D detection in pediatric depart-ment of maternal and Child health care hospitals and tertiary general hospitals in 6 cities and prefectures of Gansu Province were retro-spectively selected,characteristics of vitamin D in 5 000 children were analyzed.Ninty children with RRTIs in 5 000 children were taken as study group,and 80 healthy children were taken as control group.Correlation between vitamin D levels and immune function indexes(IgA,IgG,IgM,complement C3 and complement C4)between the two groups was compared to analyze the diagnostic value of vitamin D for children RRTIs.Results:After analyzing vitamin D data of 5 000 children in Gansu Province,it was found that vita-min D deficiency rate,deficiency rate and adequate rate were 11.58%,41.38%and 47.04%respectively,and no vitamin D overdose or poisoning were found.Vitamin D level of children was affected by age and season.Vitamin D deficiency of children aged 3 to 4 years was more serious,and vitamin D level of children in winter was the lowest,and vitamin D level was related to the growth and develop-ment of children and disease.25(OH)D level and immune function indexes in study group were lower than those in control group(P<0.05).Level of 25(OH)D was related to the age,allergic history,passive tobacco exposure and susceptible season of RRTIs(P<0.05).Immune function indexes of children with adequate vitamin D were higher than children with insufficient and deficient vitamin D(P<0.05).Level of vitamin D in children with RRTIs was positively correlated with their immune function(P<0.05).Low birth weight,premature delivery,preference for food,daily amount of fruits and vegetables,outdoor activity time,calcium,iron,zinc,25(OH)D,IgA,IgG,IgM,complement C3 and C4 were all independent risk factors for RRTIs in children(P<0.05).Vitamin D had a high diagnostic value for RRTIs in children(P<0.05).Conclusion:Vitamin D level of children in Gansu Province is related to age and season.Lower vitamin D level in children's RRTIs is related to the immune disorders of children,which can be used for the diag-nosis of this disease.
3.Short- to medium-term safety and efficacy of the implantable Corheart 6 left ventricular assist system in patients with end-stage heart failure

Objective To investigate the efficacy and safety of the Corheart 6 left ventricular assist system in patients with end-stage heart failure. Methods A retrospective study was conducted on patients with end-stage heart failure who were treated with Corheart 6 left ventricular assist system from March 2022 to June 2024 in 4 hospitals in Jiangsu Province. The efficacy of the device was evaluated by comparing changes in clinical indicators at preoperative, discharge, 3-month postoperative, and 6-month postoperative timepoints, including the New York Heart Association (NYHA) functional classification, left ventricular ejection fraction (LVEF), and left ventricular end-diastolic diameter (LVEDD). The safety of the device was assessed by analyzing the intraoperative position and orientation of the blood pump inlet cannula, as well as the incidence of adverse events. Results In this study, 39 patients were collected, including 34 males and 5 females with a mean age of (56.4±12.5) years, ranging from 20 to 75 years. There was no operative death. There was no death in postoperative 3 months with a survival rate of 100.0%. There were 3 deaths in 6 months postoperatively, with a survival rate of 92.3%. All patients had a preoperative NYHA cardiac function classification of class Ⅳ. The NYHA cardiac function class of the patients improved (P<0.05) at discharge, 3 and 6 months after surgery when compared to the preoperative period. LVEF was significantly higher at 3 months after surgery than that during the preoperative period (P<0.05). LVEDD was significantly smaller at discharge, 3 and 6 months after surgery than that during the preoperative period (P<0.05). The safety evaluation's findings demonstrated that all 39 patients' intraoperative blood pump inlet tubes were oriented correctly, the artificial blood vessel suture sites were appropriate, there were no instances of device malfunction or pump thrombosis, or instances of bleeding or hemolysis, and the rate of the remaining adverse events was low. Conclusion With a low rate of adverse events and an excellent safety profile, the Corheart 6 left ventricular assist system can efficiently enhance cardiac function in patients with end-stage heart failure. It also has considerable clinical uses.
4.Effect of the Otago Exercise Program on Activities of Daily Living in Older Adults With Sarcopenia

Objective To explore the effects of the Otago Exercise Program(OEP)on activities of daily living,muscle strength,balance,and physical function in older adults with sarcopenia,to compare OEP with conventional exercise training,and to provide a basis for clinical rehabilitation programs for older adults with sarcopenia.Methods In this randomized controlled trial,120 older adults clinically diagnosed with sarcopenia were enrolled.The participants were randomly assigned to the OEP intervention group(experimental group)and the conventional exercise intervention group(control group),with 60 in each group.The experimental group underwent 12 weeks of OEP training,three times a week,with each session lasting 45 minutes.The control group underwent conventional exercise training following the same schedule.The Modified Barthel Index was used as the primary outcome measure to assess activities of daily living.Secondary outcome measures included muscle strength,gait stability,dynamic balance,and physical function status,evaluated using grip strength,6-meter walking speed,the Timed Up and Go Test(TUGT),and the Short Physical Performance Battery(SPPB).Results A total of 120 older adults with sarcopenia were included.The mean age of the participants was(80.17±8.48)years.Baseline data before treatment showed no statistically significant differences between the two groups.Both groups completed the treatment within 12 weeks without experiencing any adverse events.The baseline data for the experimental group were as follows,MBI at(67.00±22.76)points,hand grip strength at(15.29±4.94)kg,gait speed at(0.61±0.26)m/s,TUGT time at(15.05±6.74)s,and SPPB score at(6.17±1.40)points,while the corresponding post-intervention findings were as follows,(78.72±15.83)points,(17.67±5.83)kg,(0.77±0.28)m/s,(13.49±6.16)s,and(9.25±1.71)points,respectively.The experimental group showed improvements in all measures from baseline to post-intervention(P<0.05 for all measures).As for the control group,the baseline data for the corresponding measures were as follows,(67.20±22.12)points,(15.00±5.35)kg,(0.58±0.23)m/s,(17.29±6.90)s,and(6.00±1.24)points,respectively.The post-intervention findings increased to(71.13±20.28)points,(15.47±5.72)kg,(0.64±0.28)m/s,(16.50±6.99)s,and(6.73±1.61)points,respectively,but the changes were not statistically significant(P>0.05).Furthermore,an intergroup comparison of intervention effects(post-intervention minus preintervention)revealed significant differences in mean changes from baseline.The experimental group demonstrated improvements of(+11.72±6.32)points in modified Barthel Index,(+11.72±6.32)kg in grip strength,(+0.16±0.09)m/s in gait speed,(—1.56±1.32)s in TUGT time,and(—1.56±1.32)points in SPPB score.In contrast,the control group showed smaller changes of(+3.93±5.65)points,(+0.47±1.37)kg,(+0.06±0.07)m/s,(—0.79±1.54)s,and(+0.73±1.12)points,respectively(all P<0.05).Intergroup comparisons revealed superior outcomes in the experimental group across all measures.Conclusion OEP significantly enhances activities of daily living,improves muscle strength,balance,and physical function in older adults,and is more effective than conventional rehabilitation exercise programs,making it suitable for extensive clinical application.

Objective To observe ultrasonic manifestations of ovarian cysts in neonates and infants.Methods Totally 40 neonates or infants with large or complicated ovarian cysts were retrospectively enrolled,and ultrasonic manifestations,surgical operation findings or follow-up data were analyzed.Results Among 40 cases,thin wall and echoless unicellular cysts were found in 20 cases,among which 9 were accompanied by"ascus",and these cases were classified into simple cyst group.Thick wall cysts with spot-like,flocculent or heterogeneous solid components were observed in 19 cases,including 11 cases with"double wall sign",8 with solid-liquid stratification and 1 case with"ascus",and these cases were taken as complex cyst group.Atrophic solid cyst was found in 1 case.The long diameter,anteroposterior diameter and short diameter of simple cyst group were all larger than those of complex cyst group(all P<0.05).Ultrasound correctly diagnosed ovarian origin tumors in 14 cases(14/40,35.00%).Hemorrhage combined with torsion necrosis was detected in 18 cases of complicated ovarian cysts in surgical operation.Thirty-eight cases were confirmed by surgical pathology,while 2 cases were confirmed by follow-up,including 1 case of simple ovarian cyst spontaneously subsided and 1 case of complicated ovarian cyst fell off and migrated to right lower liver margin.Conclusion Ovarian cysts in neonates and infants could be roughly divided into simple and complex and atrophic solid ovarian cysts,with ultrasonic manifestations having certain characteristics,which were helpful for diagnosis.

8.Effects of polydatin on inflammatory response in dry eye disease rats and its mechanism

OBJECTIVE To investigate the effects of polydatin (PD) on the inflammatory response in dry eye disease (DED) rats and its potential mechanism based on protein kinase A (PKA)/cyclic adenosine monophosphate response element binding protein (CREB) signaling pathway. METHODS Male SD rats were randomly divided into blank control group (normal group), model group (simple DED group), 0.05%PD group, 0.5%PD group and 0.5%PD+PKA inhibitor H-89 group (0.5%PD+H-89 group), with 15 rats in each group. Except for the normal group, the rats in other groups were prepared with a DED model by injecting scopolamine hydrobromide 12.5 mg/d onto the surface of eyeball. At the same time, each drug group was given corresponding liquid medicine (0.5% or 0.05% PD, eye drip, 3 times a day; 1 mg/kg H-89, intraperitoneal injection, once a day) for 7 d in total. The tear secretion, corneal fluorescein staining score and conjunctival goblet cell density of rats were detected in each group; the pathological changes of corneal tissues were observed, and the levels of inflammatory factors (tumor necrosis factor-α, interleukin-1β, interleukin-6) and the expression of PKA/CREB signaling pathway-related proteins were detected E-mail:rwei@tmu.edu.cn in corneal tissues. R ESULTS Compared with normal group, the corneal epithelium of rats in the simple DED group was thickened, stromal layer cells were disordered and partially absent, nuclear spacing was larger, and a large number of inflammatory cells infiltrated; the tear secretion, conjunctival goblet cell density and the phosphorylation levels of PKA and CREB in corneal tissues were reduced significantly, while the corneal fluorescein staining score and the levels of inflammatory factors in corneal tissue were increased significantly (P<0.05). Compared with the simple DED group, the pathological injuries of corneal tissues of rats in 0.05%PD and 0.5%PD groups were alleviated, each quantitative index was significantly improved, and the improvement effect of 0.5%PD group was more obvious (P<0.05). H-89 could reverse the improvement effect of PD on each index significantly (P<0.05). CONCLUSIONS PD can increase tear secretion and the conjunctival goblet cell density, and reduce the inflammatory response and pathological injury of corneal tissue in DED rats. The above effects are related to the activation of PKA/CREB signaling pathway.

10.Effect of inhalation of sevoflurane during cardiopulmonary bypass on early postoperative brain injury in patients undergoing cardiac valve replacement

Objective:To evaluate the effect of inhalation of sevoflurane during cardiopulmonary bypass (CPB) on early postoperative brain injury in the patients undergoing cardiac valve replacement.Methods:Forty-two American Society of Anesthesiaologists physical status Ⅱ or Ⅲ patients of either sex, aged 40-70 yr, weighing 47-86 kg, scheduled for elective single valve replacement under CPB, were divided into 3 groups ( n=14 each) using a random number table method: control group (group C), combined intravenous-inhalational anesthesia group (group CA) and sevoflurane group (group S). During CPB, propofol 4-6 mg·kg -1·h -1 was intravenously infused in group C, propofol 2-3 mg·kg -1·h -1 was intravenously infused, and 0.5 MAC sevoflurane was inhaled via the membrane oxygenator in group CA, and 1.0-1.5 MAC sevoflurane was inhaled via the membrane oxygenator in group S. The anesthesia and sedation index values were maintained at 40-60 during operation in the three groups.Blood samples were taken from arteries before anesthesia induction (T 1), at 30 min and 6 and 24 h after termination of CPB (T 2-4) for determination of plasma concentrations of neuron-specific enolase (NSE) and Tau protein. Results:Compared with group C, the plasma concentration of NSE was significantly decreased at T 2, 3, and plasma concentration of Tau protein was decreased at T 2-4 in group S, and the plasma concentration of Tau protein was decreased at T 2 in group CA ( P<0.05). Compared with group CA, the plasma concentration of NSE was significantly decreased at T 2, 3, and the plasma concentration of Tau protein was decreased at T 2-4 in group S ( P<0.05). Conclusions:Inhalation of sevoflurane during CPB can reduce early postoperative brain injury to a certain extent in the patients undergoing cardiac valve replacement.
