1.Short- to medium-term safety and efficacy of the implantable Corheart 6 left ventricular assist system in patients with end-stage heart failure

Objective To investigate the efficacy and safety of the Corheart 6 left ventricular assist system in patients with end-stage heart failure. Methods A retrospective study was conducted on patients with end-stage heart failure who were treated with Corheart 6 left ventricular assist system from March 2022 to June 2024 in 4 hospitals in Jiangsu Province. The efficacy of the device was evaluated by comparing changes in clinical indicators at preoperative, discharge, 3-month postoperative, and 6-month postoperative timepoints, including the New York Heart Association (NYHA) functional classification, left ventricular ejection fraction (LVEF), and left ventricular end-diastolic diameter (LVEDD). The safety of the device was assessed by analyzing the intraoperative position and orientation of the blood pump inlet cannula, as well as the incidence of adverse events. Results In this study, 39 patients were collected, including 34 males and 5 females with a mean age of (56.4±12.5) years, ranging from 20 to 75 years. There was no operative death. There was no death in postoperative 3 months with a survival rate of 100.0%. There were 3 deaths in 6 months postoperatively, with a survival rate of 92.3%. All patients had a preoperative NYHA cardiac function classification of class Ⅳ. The NYHA cardiac function class of the patients improved (P<0.05) at discharge, 3 and 6 months after surgery when compared to the preoperative period. LVEF was significantly higher at 3 months after surgery than that during the preoperative period (P<0.05). LVEDD was significantly smaller at discharge, 3 and 6 months after surgery than that during the preoperative period (P<0.05). The safety evaluation's findings demonstrated that all 39 patients' intraoperative blood pump inlet tubes were oriented correctly, the artificial blood vessel suture sites were appropriate, there were no instances of device malfunction or pump thrombosis, or instances of bleeding or hemolysis, and the rate of the remaining adverse events was low. Conclusion With a low rate of adverse events and an excellent safety profile, the Corheart 6 left ventricular assist system can efficiently enhance cardiac function in patients with end-stage heart failure. It also has considerable clinical uses.
2.Practice and exploration of horizontal scientific research contract information management based on risk prevention and control

Objective:This study aims to improve the management level and quality of horizontal scientific research contracts in hospitals, reduce the risk of contract management, and explore an efficient mode of information management of horizontal scientific research contract management.Methods:Taking a Tertiary Grade A hospital as an example, this study described and summarized the management status of horizontal scientific research contracts from 2018 to 2022, sorted out the potential risks and prevention and control nodes of the review, signing, performance, and archiving of horizontal scientific research contracts in the whole cycle management process, and designed an information management model according to the relevant elements of risk prevention and control and the standardized requirements of contract management, thereby developing information platform related functions.Results:The horizontal research contract management function of the clinical research platform based on the integration concept was available to meet the requirements of risk prevention and control of horizontal research contracts, and the contract review and signing cycle of the information management mode was shortened by 33.33% and 31.95% respectively compared with the traditional management mode. The platform realized a standardized whole process management of contract submission, review, repair and revision, signing, performance, and filing.Conclusions:Information management can effectively reduce the risk of horizontal scientific research contract management. It is necessary to strengthen the docking and interaction function with other information platforms and further explore the information auxiliary function to improve the refined management effect of horizontal scientific research contracts.

Cardiac arrest (CA) is a critical condition in the field of cardiovascular medicine. Despite successful resuscitation, patients continue to have a high mortality rate, largely due to post CA syndrome (PCAS). However, the injury and pathophysiological mechanisms underlying PCAS remain unclear. Experimental animal models are valuable tools for exploring the etiology, pathogenesis, and potential interventions for CA and PCAS. Current CA animal models include electrical induction of ventricular fibrillation (VF), myocardial infarction, high potassium, asphyxia, and hemorrhagic shock. Although these models do not fully replicate the complexity of clinical CA, the mechanistic insights they provide remain highly relevant, including post-CA brain injury (PCABI), post-CA myocardial dysfunction (PAMD), systemic ischaemia/reperfusion injury (IRI), and the persistent precipitating pathology. Summarizing the methods of establishing CA models, the challenges encountered in the modeling process, and the mechanisms of PCAS can provide a foundation for developing standardized CA modeling protocols.

4.Transvaginal ultrasound parameters combined with clinical features for predicting success pregnancy or not of polycystic ovary syndrome patients with infertility after in vitro fertilization/intracytoplasmic sperm injection

Objective To explore the value of transvaginal ultrasound parameters combined with clinical features for predicting success pregnancy or not of polycystic ovary syndrome(PCOS)patients with infertility after in vitro fertilization/intracytoplasmic sperm injection(IVF/ICSI).Methods Totally 348 PCOS patients with infertility were retrospectively analyzed.Patients with presence of a gestational sac on ultrasound and did not experience miscarriage within 4 months after IVF/ICSI were classified as success pregnancy(pregnancy group,n=178),while the rest 170 patients were enrolled in non-pregnancy group.Univariable and multivariable logistic regression analyses were employed to assess clinical data,IVF/ICSI and transvaginal ultrasound parameters to identify the independent predictors of non-pregnancy in PCOS patients with infertility after IVF/ICSI,and a regression equation was constructed.The predictive efficacy,calibration and clinical value of this model were evaluated.Results Patients'age,duration of infertility,ovulation induction protocol(long protocol),duration of gonadotropin treatment,numbers of antral follicles and dominant follicles,total ovarian area,ovarian medullary area and endometrial thickness showed on transvaginal ultrasound before IVF/ICSI,as well as IVF with 2 pronuclei(IVF 2PN)and ICSI with 2 pronuclei(ICSI 2PN)or not were all independent predictors of success pregnancy or not in PCOS patients with infertility after IVF/ICSI(all P<0.05).The area under the curve of the regression model was 0.771,and its predicted probabilities correlated with actual probabilities.Decision curve analysis indicated that the regression model had good clinical benefit.Conclusion Transvaginal ultrasound parameters combined with clinical features could effectively predict success pregnancy or not in PCOS patients with infertility after IVF/ICSI.
5.Effects of hepatitis B virus genotypes B and C on hepatic stellate cells under different alanine aminotransferase statuses

Objective To explore the effects of hepatitis B virus genotypes B and C on hepatic stellate cells(HSC)proliferation,cell cycle,and secretion under different alanine aminotransferase(ALT)statuses.Methods A total of 270 patients diagnosed with chronic hepatitis B in the First Hospital of Lanzhou University between January 2023 to December 2023 were selected by stratified random sam-pling.In the group with normal ALT,there were 42 patients with genotype B and 48 patients with genotype C.In the group with ALT ≥ the upper normal limit(UNL)and<2 times the UNL(2 UNL),there were 48 patients with genotype B and 45 patients with genotype C.In the group with ALT≥2 UNL,there were 45 patients with genotype B and 42 patients with genotype C.Sera from these patients was used as the interfering sera.After HSC were treated with the interfering serum,the proliferation of HSC was detected by the MTT method,the cell cycle was detected by flow cytometry,the mRNA expressions of TGF-β1,Smad3,Smad7,α-SMA,Collagen Ⅰ,and Collagen Ⅲ were detected by real-time PCR,and the expressions of the corresponding proteins were detected by Western blotting.Results When the ALT level was normal,hepatitis B virus genotypes B and C had no obvious effect on HSCs.When the ALT level was abnormal,particularly in the group with ALT ≥ 2 UNL,the virus with genotype C was more likely to promote the proliferation of HSC,reduce the number of cells in the G0/G1 phase,increase the number of cells in the S phase and G2/M phase,enhance the mRNA and protein expressions of TGF-β1,Smad3,Collagen Ⅰ,Collagen Ⅲ,and α-SMA,and decrease the mRNA and protein expressions of Smad7,with significant differences(P<0.05).Conclusion The effects of hepatitis B virus genotypes B and C on HSC differ varying ALT statuses.During abnormal ALT elevation,genotype C hepatitis B virus demonstrates more pronounced effects on HSC proliferation,cell cycle progression,and secretory function compared to genotype B.
6.Association Between Obstructive Sleep Apnea and Behavioral Problems in Children

Objective To investigate the effect of obstructive sleep apnea(OSA)on behavioral problems in children and the association between them.Methods A simple random sampling method was used to select 100 children aged 4 to 12 years for the case group.All of them were diagnosed with OSA through overnight polysomnography at the Sleep Medicine Center,West China Fourth Hospital,Sichuan University between October 2022 and October 2023.An additional 100 children without snoring symptoms and clinically evaluated and confirmed as not having OSA were enrolled as the control group.General demographic data of the participants were collected.The Caregiver Report Form of the Achenbach Child Behavior Checklist(CBCL)was used for behavioral problem assessment,and polysomnography data were collected.The chi-square/t test was used to analyze the inter-group differences in general data,the total score of behavioral problems,and scores for each dimension.Linear regression was performed to analyze the relationship between OSA and the total score for children's behavioral problems and those for the different dimensions.Logistic regression was applied to analyze the relationship between the obstructive apnoea-hypopnea index(OAHI)and behavioral problems in children with OSA.A logistic regression model integrating the OAHI×sex interaction term was constructed to evaluate the moderating effect of sex on the association between OAHI and behavioral problems.Results No significant differences were observed in general demographic data between the case and control groups.The total score for behavioral problems and those for each dimension were higher in the case group than those in the control group,with the total score of the case group being 24.60±1.55 and that of the control group being 8.85±0.75(P<0.001).The results of the linear regression analysis showed a positive association between OSA and both the total score for behavioral problems(b=16.01;95%CI,12.56-19.47)and those for each dimension.The results of the logistic regression analysis showed that,after adjusting for covariates,OAHI was a risk factor for behavioral problems in children with OSA(odds ratio[OR]=1.17;95%CI,1.04-1.31).After stratification by sex and adjustment for covariates,the OR value of the effect of OAHI on behavioral problems was slightly higher in female participants(1.57)than that in male participants(1.21).The interaction effect analysis showed that sex moderated the association between OAHI and behavioral problems(OR=1.64;95%CI,1.02-2.64;P=0.04).Conclusion Children with OSA are prone to developing behavioral problems.OAHI is a risk factor for behavioral problems in children with OSA,with a potentially greater effect observed in girls.
7.Associations between traditional Chinese medicine constitution and depression/anxiety/sleep disturbances in patients with chronic pain: A cross-sectional study

ObjectiveTo investigate the relationship between the composition of traditional Chinese medicine (TCM) and depression/anxiety/sleep disturbances (D/A/S) in patients with chronic pain.MethodsThis cross-sectional study was conducted at 13 tertiary hospitals across China, enrolling patients who experienced chronic pain between November 2023 and May 2024. The Patient Health Questionnaire-9, Generalized Anxiety Disorder-7, Pittsburgh Sleep Quality Index, and TCM constitution categories were used to assess the patients. The association between the TCM constitution and the D/A/S ratio was analyzed using multivariable logistic regression.ResultsA total of 1107 patients (63.2% women) were analyzed. Compared with those with a balanced constitution, patients who had qi-deficiency and yin-deficiency were at a higher risk of depression. Qi-deficiency and yin-deficiency were associated with anxiety. Sleep disturbances were common in patients with qi-deficiency constitution (odds ratio [OR]: 2.32, 95% confidence interval [CI]: 1.42–3.81), yang-deficiency constitution (OR: 1.94, 95% CI: 1.26–2.98), yin-deficiency constitution (OR: 2.03, 95% CI: 1.24–3.32), blood stasis constitution (OR: 2.07, 95% CI: 1.01–4.22), and qi-stagnation constitution (OR: 2.66, 95% CI: 1.35–5.25).ConclusionIn patients with chronic pain, specific TCM constitutions may be associated with D/A/S. Further longitudinal studies are needed to clarify the potential causal relationships between TCM constitution types and these conditions.
8.Screening of molecular markers related to lupus nephritis based on bioinformatics technology

Objective To identify the key differentially expressed genes in the patients with lupus nephritis(LN)using bioinformatics methods and discover potential diagnostic biomarkers of LN for exploring the pathogenic mechanisms of LN.Methods The GSE99339 dataset related to chronic kidney disease was obtained from the Gene Expression Omnibus(GEO)database.Weighted gene co-expres-sion network analysis(WGCNA)was performed to identify the core modules highly correlated with LN.The differentially expressed genes(DEGs)between the two groups(32 LN patients and 14 healthy controls)from GSE32591 dataset were identified using Limma package.The intersection of DEGs and the LN-related module genes was obtained.The Gene Ontology(GO)function annotation and Kyoto Encyclopedia of Genes and Genomes(KEGG)pathway enrichment analyses of intersection genes were conducted using DAVID online tool to construct protein-protein interaction(PPI)network,and the key genes were screened using Cytoscape software.Boxplots and receiver operating characteristic(ROC)curves were generated to evaluate the diagnostic efficiency of the key genes based on the the gene expression data of GSE112943 validation dataset.Results The greenyellow module(102 genes)was strongly correlated with LN(r=0.85,P<0.01).A total of 361 DEGs were identified from GSE32591 dataset,among which 53 genes were found to intersect with the green-yellow module genes.The LN-related genes were mainly enriched in the biological processes and pathways which related to antiviral response,innate immunity,hepatitis C,and influenza A virus.The top five key genes in the PPI network were IFIT3,MX1,OAS1,RSAD2,and OAS3.The expression levels of 1FIT3,MX1,OAS1 and RSAD2 in LN were significantly different from con-trol groups(P<0.05).ROC curve analysis showed that the AUC values for the four genes in predicting LN were all greater than 0.8,indicating high diagnostic efficiency.Conclusion IFIT3,MX1,OAS1 and RSAD2 may be the key differentially expressed genes in LN and may be potential diagnostic biomarkers and therapeutic targets for LN.
9.Correlation between vitamin D level and immune dysfunction in children with recurrent respiratory tract infection

Objective:To investigate the characteristics of vitamin D levels in children in Gansu Province,and to analyze the correlation between vitamin D levels and immune disorders in children with recurrent respiratory tract infections(RRTIs).Methods:From January 2020 to December 2021,9 790 cases of children aged 0~6 years who received vitamin D detection in pediatric depart-ment of maternal and Child health care hospitals and tertiary general hospitals in 6 cities and prefectures of Gansu Province were retro-spectively selected,characteristics of vitamin D in 5 000 children were analyzed.Ninty children with RRTIs in 5 000 children were taken as study group,and 80 healthy children were taken as control group.Correlation between vitamin D levels and immune function indexes(IgA,IgG,IgM,complement C3 and complement C4)between the two groups was compared to analyze the diagnostic value of vitamin D for children RRTIs.Results:After analyzing vitamin D data of 5 000 children in Gansu Province,it was found that vita-min D deficiency rate,deficiency rate and adequate rate were 11.58%,41.38%and 47.04%respectively,and no vitamin D overdose or poisoning were found.Vitamin D level of children was affected by age and season.Vitamin D deficiency of children aged 3 to 4 years was more serious,and vitamin D level of children in winter was the lowest,and vitamin D level was related to the growth and develop-ment of children and disease.25(OH)D level and immune function indexes in study group were lower than those in control group(P<0.05).Level of 25(OH)D was related to the age,allergic history,passive tobacco exposure and susceptible season of RRTIs(P<0.05).Immune function indexes of children with adequate vitamin D were higher than children with insufficient and deficient vitamin D(P<0.05).Level of vitamin D in children with RRTIs was positively correlated with their immune function(P<0.05).Low birth weight,premature delivery,preference for food,daily amount of fruits and vegetables,outdoor activity time,calcium,iron,zinc,25(OH)D,IgA,IgG,IgM,complement C3 and C4 were all independent risk factors for RRTIs in children(P<0.05).Vitamin D had a high diagnostic value for RRTIs in children(P<0.05).Conclusion:Vitamin D level of children in Gansu Province is related to age and season.Lower vitamin D level in children's RRTIs is related to the immune disorders of children,which can be used for the diag-nosis of this disease.
