OBJECTIVE To build a new workflow of pharmacy intravenous admixture services (PIVAS), effectively connect intelligent equipment, and promote the intelligent development of PIVAS. METHODS Based on intelligent auxiliary equipment, PIVAS workflow was optimized, and a process-oriented model was established. This model integrated intelligent prescription review (automatic prescription review+manual intervention mode), intelligent labeling, intelligent allocation, intelligent sorting, and finished infusion quality inspection system. Furthermore, an assessment was conducted to examine unreasonable medical order rate of intelligent prescription review, the working efficiency and error rate of intelligent labeling machine and intelligent sorting machine, and the dispensing efficiency and accuracy of intelligent dispensing robot. RESULTS Under the intelligent prescription review mode, the rate of unreasonable medical orders decreased from 0.157% to 0.050% (P<0.05); automatic labeling efficiency reached 21.7 sheets/min, surpassing the manual labeling efficiency of 13.8 sheets/min (P<0.05), and the daily labeling error rate decreased from 6.1‰ to 2.5‰ (P<0.05). Simultaneously operating two dispensing robots significantly improved the efficiency of batch dispensing and reduced the residual amount of liquid medicine (P<0.05); additionally, a quality testing system for finished infusion was established, involving appearance, Tyndall effect, insoluble particles, turbidity, absorbance, pH and osmotic pressure, to ensure the quality of finished infusion and reduce the risk of infusion. CONCLUSIONS The new process of PIVAS connected with intelligent devices in our hospital can improve work efficiency, reduce dispensing errors, ensure the quality of finished infusion, and improve the level of pharmaceutical care.

[Objective] To investigate the factors associated with alanine aminotransferase (ALT) abnormalities in multi-ethnic blood donors across five Zang autonomous prefectures in the plateau regions of Qinghai Province, and to provide evidence for ensuring blood safety and formulating screening strategies. [Methods] A retrospective analysis was performed on the ALT abnormal test results of blood donors in the Zang autonomous prefectures of Qinghai from 2022 to 2024. The correlations between ALT levels and factors including gender, age, altitude, and infectious markers were investigated. [Results] The overall ALT unqualified rate among blood donors in this region was 9.01%. Significant differences in ALT levels were observed across genders and age groups (P<0.05). Variations in ALT abnormality rates were also noted among different plateau regions (P<0.05). Overall, ALT values exhibited an increasing trend with rising altitude. The average ALT unqualified rates were 11.19% in Zang donors, 7.96% in Han donors, and 4.79% in donors from other ethnic groups (P<0.05). No statistically significant association was observed between ALT abnormality and the presence of HBV/HCV infectious markers (P>0.05). [Conclusion] In the plateau areas of Qinghai, multi-ethnic blood donors have a relatively high ALT levels and ALT unqualified rates, showing distinct regional characteristics. ALT elevation in voluntary blood donors is related to non-pathological factors such as gender, age, and dietary habits, but not to infectious indicators.

OBJECTIVE To analyze the non-genetic risk factors for severe cutaneous adverse reactions (SCARs) related to antiepileptic drugs (AEDs) in HLA-B*15:02 negative patients, and provide a basis for clinical precision medication. METHODS A retrospective case-control design was used to include patients who underwent HLA-B*15:02 testing at our hospital from January 2022 to December 2024. Patients were divided into SCARs group (15 cases who were HLA-B*15:02 negative and diagnosed with SCARs) and control group (38 cases who were HLA-B*15:02 negative and used AEDs). Risk factors were evaluated using univariate analysis and a multivariable Firth penalty likelihood logistic regression model (Firth regression), and Benjamin-Hochberg false discovery rate (FDR) and Firth regression were used for correction, and sensitivity analysis was used to quantify the impact of potential biases in carbamazepine exposure rates in the control group on the results. RESULTS Univariate analysis showed that age≥50 years, use of carbamazepine, and combination use of antibiotics/antiviral drugs were risk factors for developing AEDs- related SCARs (OR=18.15, 7.54, 13.46, 95%CI of 4.13-79.84, 1.89-30.08, 1.36-133.18, all P<0.05), while taking lamotrigine was a protective factor [OR=0.10, 95%CI of 0.02-0.39, P<0.05]. After FDR correction, the above factors still maintained statistical significance (P<0.05). The results of multivariate analysis showed that age≥50 years [adjusted OR=16.27, 95%CI of 3.98-66.55, P<0.001] and taking carbamazepine [adjusted OR=7.11, 95%CI of 1.82-27.85, P=0.005] were independent risk factors for the occurrence of SCARs-related AEDs. The results of sensitivity analysis showed that the adjusted risk OR range for taking carbamazepine was between 14.2 and 28.4. CONCLUSIONS Age≥50 years and use of carbamazepine are independent non- genetic risk factors for the development of SCARs-related AEDs in HLA-B*15:02 negative patients. It is recommended that elderly patients should prioritize the use of AEDs other than carbamazepine.

OBJECTIVE To report a case of hepatitis B virus (HBV) reactivation induced by iparomlimab and tuvonralimab, summarize the clinical characteristics and potential mechanisms of such adverse reactions induced by immune-checkpoint inhibitors (ICIs), and provide references for clinical application. METHODS From the perspective of a clinical pharmacist, a retrospective analysis was conducted on the treatment course of a patient with metastatic cervical cancer who experienced HBV reactivation after receiving iparomlimab and tuvonralimab. Additionally, an analysis of the correlation with adverse reactions was performed, and the clinical characteristics, risk factors, potential mechanisms, key points of treatment approaches and pharmaceutical care associated with HBV reactivation induced by ICIs were summarized. RESULTS & CONCLUSIONS The patient developed HBV reactivation and severe liver injury after using iparomlimab and tuvonralimab. The condition improved following drug discontinuation, and symptomatic treatment such as glucocorticoids. According to Naranjo’s Assessment Scale and China’s Measures for the Reporting and Monitoring of Adverse Drug Reactions, the association between iparomlimab and tuvonralimab and HBV reactivation was judged as “highly probable”, and it was identified as a new adverse reaction; the correlation between iparomlimab and tuvonralimab, paclitaxel and liver injury was “highly probable”. HBV reactivation in hepatitis B patients receiving standardized antiviral therapy is very rare after ICIs treatment; HBV reactivation is related to the overactivation of the immune system and disruption of immune balance induced by ICIs. For such patients, glucocorticoids should be administered for treatment, accompanied by pharmaceutical care, including pre- medication risk assessment and monitoring of relevant indicators during treatment.

ObjectiveTo investigate the genotyping and drug resistance trends of 461 strains of diarrheagenic Escherichia coli (DEC) isolated and identified in Suzhou, Jiangsu Province from 2019 to 2023. MethodsDEC detected in Suzhou in the past 5 years was used as the research subject, and the virulence genotyping was tested by real-time fluorescence quantitative polymerase chain reaction (PCR). The microbroth dilution method was used to perform drug susceptibility test, and the corresponding susceptibility (S), intermediate (I) and resistance (R) results were obtained based on the minimum inhibitory concentration (MIC) values, according to the criteria of United States Clinical and Laboratory Standardization Committee (CLSI) 2017. Differences of DEC drug resistance among different virulence genotypes were compared by χ2 test or Fisher exact probability method. ResultsA total of 461 DEC strains were detected in Suzhou from 2019 to 2023, of which the highest proportion was enterotoxigenic Escherichia coli (ETEC) accounting for 45.77% (211/461), followed by enteropathogenic Escherichia coli (EPEC) accounting for 32.32% (149/461) and enteroaggregative Escherichia coli (EAEC) accounting for 20.39% (94/461), while enterohemor-rhagic Escherichia coli (EHEC) and enteroinvasive Escherichia coli (EIEC) were individually distributed. The antimicrobial drug with the highest resistance rate was ampicillin (61.61%), followed by cefazolin (49.89%) and nalidixic acid (44.47%). There were statistically significant differences in drug resistence rates of the three major virulence genotypes of DEC (ETEC, EPEC and EAEC) to ampicillin (AMP), ampicillin/sulbactam (AMS), amoxicillin/clavulanic acid (AMC), cefoxetine (CFX), gentamicin (GEN), streptomycin (STR), tetracycline (TET), nalidixic acid (NAL), and chloramphenicol (CHL), and methotrexate/sulfamethoxazole (SXT). The multi-drug resistance (MDR) rate of DEC was 59.87% (276/461), and the MDR rate of each genotype, from high to low, was EIEC (75.00%), EAEC (71.28%), EHEC (66.66%), EPEC (61.74%) and ETEC (52.86%). ConclusionETEC, EPEC and EAEC are the main genotypes prevalent in DEC in Suzhou in recent years. The drug resistance strains and MDR are still serious, which should arouse wide public health concern and take targeted prevention and control measures.

Acta Universitatis Medicinalis Anhui 2023;58(6):896-901
Objective:
To investigate the effect of berberine (BBR) on the proliferation and autophagy of mesangial cells in high glucose (HG) environment and the specific molecular mechanism.
Methods:
Mesangium cells at exponential growth stage were divided into the following groups : normal group, high glucose group, high glucose + BBR treatment group (30,60 and 90 μmol / L) ,high glucose + BBR (90 μmol / L) + AMPK inhibitor Compound C group ( CC group) ; the number of mesangial cells was calculated by high content cell imager. The expressions of type Ⅳ collagen ( Col-Ⅳ) ,fibronectin (FN) and microtubule-associated protein 1 light chain 3B (LC3B) in mesangial cells were detected by immunofluorescence assay. The protein expression levels of LC3B,Beclin-1, p62,Col-Ⅳ , FN and silencing regulatory factor 1 (SIRT1) / adenylate activated protein kinase (AMPK) signaling pathway were detected by Western blot.
Results:
Compared with the normal group ,high content cell imaging showed abnormal proliferation of mesangial cells in the hyperglycemic group. The results of immunofluorescence and Western blot showed that the expression levels of Col-Ⅳ and FN deposited in mesangial extracellular matrix increased in the high glucose group. The results of Western blot showed that the protein expressions of SIRT1,p- AMPK,LC3B and Beclin-1 decreased, while the protein expressions of p-p65 and p62 increased.BBR inhibited the abnormal proliferation of mesangial cells induced by high glucose.BBR could reduce the expression levels of Col-Ⅳ and FN deposited in mesangial extracellular matrix. BBR could increase the expressions of SIRT1 ,p- AMPK,LC3B and Beclin-1 proteins in mesangial cells, while decrease the expressions of p-p65 and p62 proteins. CC group weakened the inhibition of mesangial cell proliferation and autophagy by high dose BBR.
Conclusion
Berberine can effectively inhibit the proliferation of mesangial cells induced by high glucose and increase the level of autophagy, which may be related to SIRT1 / AMPK signaling pathway.

Objective :
To investigate the regulatory effects and partial mechanisms of berberine(BBR) on podocyte epithelial mesenchymal transformation(EMT) and apoptosis induced by high glucose(HG).
Methods :
Podocytes at exponential growth stage were divided into normal group, high glucose group and berberine group(30, 60 and 90 μmol/L). The rate of podocyte apoptosis was detected by flow cytometry. Transwell and scratch assay were used to detect podocyte migration. Western blot was used to detect the expression of apoptosis protein Bim, podocyte EMT-associated proteins nephrin, α-smooth muscle actin(α-SMA), vimentin and tyrosine kinase 2(JAK2)/signal transductor and transcriptional activator 3(STAT3) pathway-related proteins. Laser confocal method was used to detect the expression of EMT-related proteins nephrin, α-SMA and vimentin in podocytes.
Results :
Flow cytometry results showed that BBR(30, 60 and 90 μmol/L) could reduce the rate of podocyte apoptosis in high glucose environment. Transwell and scratch test results showed that BBR(30, 60 and 90 μmol/L) could inhibit abnormal podocytes migration in high glucose environment. Western blot results showed that BBR(30, 60 and 90 μmol/L) could decrease the protein expression of p-JAK2, p-STAT3, α-SMA, vimentin and Bim, and increase the protein expression of nephrin in podocytes under high glucose condition. Laser confocal results showed that the BBR(30, 60, and 90 μmol/L) could decrease the expression of α-SMA and vimentin in podocyte cytoplasm, increase the expression of nephrin in podocyte nucleus and cytoplasm.
Conclusion
BBR may alleviate the EMT and apoptosis of podocytes induced by high glucose through JAK2/STAT3 signaling pathway.

Background::Understanding the characteristics of newly diagnosed primary human deficiency virus-1 (HIV-1) infection in the context of the post-antiretroviral therapy era and HIV drug prophylaxis is essential for achieving the new target of 95-95-95-95 by 2025. This study reported the characteristics of newly diagnosed primary HIV-1 infection in Shenzhen.Methods::This is a real-world retrospective study. Eighty-seven newly diagnosed primary HIV-1-infected patients were recruited from January 2021 to March 2022 at the Third People’s Hospital of Shenzhen. Demographic, epidemiological, diagnostic, drug resistance, and medical data were described and analyzed.Results::Overall, 96.6% (84/87) of the newly identified primary HIV-1-infected patients were male, including 88.5% (77/87) men have sex with men (MSM), with a median age of 29.0 years (Q 1-Q 3: 24.0-34.0 years); of these, 85.1% (74/87) reported high-risk sexual behaviors with casual partners. The rate of condom usage was only 28.7% (25/87). The overall rate of pre-exposure prophylaxis (PrEP) and post-exposure prophylaxis (PEP) was 8.0% (7/87, including 4 PrEP and 3 PEP cases) around the potential exposure, although 41.4% of the patients had prior awareness of such interventions. Moreover, only 19.5% (17/87) had previously used PrEP or PEP. Of those, 58.8% (10/17) of the patients obtained drugs from the internet, and only 35.3% (6/17) reported good compliance. A total of 54.0% (47/87) of subjects were diagnosed by the HIV nucleic acid test. Acute retroviral syndrome appeared in 54.0% (47/87) of patients. The prevalence of transmitted drug resistance (TDR) mutation was 33.9% (19/56), including 6 (10.7%) against nucleoside reverse transcriptase inhibitor (NRTI) plus non-nucleoside reverse transcriptase inhibitor (NNRTI), 8 (14.3%) against NNRTI, and 5 (8.9%) against protease inhibitor (PI) only. Conclusions::Owing to the low utilization rate and incorrect usage of PrEP and PEP, massive efforts are needed to promote HIV-preventive strategies in the MSM population. The extremely high prevalence of TDR mutation in this population implies the need for future pretreatment drug resistance surveillance.
