Objective: To characterize perioperative dynamic changes in immune-cell phenotypes and inflammatory cytokines in patients undergoing CPB (cardiopulmonary bypass) cardiac surgery, and to explore their associations with postoperative outcomes. Methods: In this prospective cohort study, 120 adult patients who underwent elective cardiac surgery under CPB at West China Hospital from May 2022 to March 2023 were enrolled. Perioperative immune-cell phenotypes and concentrations of 40 inflammation-related cytokines were measured. The primary outcomes were the sequential organ failure assessment (SOFA) score at 24 h after surgery and ΔSOFA (the peak SOFA score within 48 h after surgery minus the preoperative SOFA score). Secondary outcomes included major adverse cardiovascular events (MACE), acute kidney injury (AKI), respiratory failure, severe liver injury, and infection. Results: The mean age of enrolled patients was 57±10 years. Of these, 52% (62/120) were male and 90% (108/120) underwent valve surgery. During the rewarming to the end of CPB, neutrophil counts rapidly increased (7.39×10
/L vs preoperative 3.07×10
/L, P<0.001), with significant upregulation of CD11b (7.30×10
/L vs preoperative 3.05×10
/L, P<0.001) and CD54 (7.15×10
/L vs preoperative 2.99×10
/L, P<0.001). Lymphocyte counts increased at the end of CPB (1.75×10
/L vs preoperative 1.12×10
/L, P<0.001) but decreased significantly at 24 h after surgery (0.59×10
/L vs preoperative 1.12×10
/L, P<0.001). Plasma analysis showed that multiple pro-inflammatory cytokines increased during CPB and remained elevated up to 24 h after surgery; five chemokines and the anti-inflammatory cytokine IL-10 peaked at the end of CPB. The SOFA score increased from 1 (1, 2) preoperatively to 7 (5, 10) at 24 h after surgery, with a ΔSOFA of 6 (4, 8). Within 30 days after surgery, 48 patients (40.0%) developed AKI, 17 (14.2%) developed infection, 4 (3.3%) developed severe liver injury, 3 (2.5%) developed respiratory failure, and 3 (2.5%) experienced MACE. During the 2-year follow-up, 8 patients (6.7%) experienced MACE and 5 (4.2%) died. Conclusion: Multi-organ dysfunction is common after cardiac surgery under CPB (median ΔSOFA, 6), accompanied by perioperative activation of multiple immune-cell subsets and upregulation of pro-inflammatory, anti-inflammatory, and chemotactic mediators. This study provides data-driven evidence and research clues for further investigation of the associations between CPB-related immune perturbations and postoperative organ dysfunction and clinical outcomes.

Journal of Sun Yat-sen University(Medical Sciences) 2026;47(4):739-746
ObjectiveTo analyze the clinical manifestations, diagnostic approaches, treatment strategies, and prognostic outcomes of uterine arteriovenous fistula (UAVF). MethodsWe retrospectively analyzed 6 patients with confirmed UAVF admitted to the Department of Gynecology, the Eighth Affiliated Hospital of Sun Yat-sen University, between January 2010 and December 2023. We summarized the clinical data of the cohort, including age, gravidity, chief complaints, treatment regimens, and postoperative follow-up outcomes, and explored the clinical efficacy and safety of ultrasound-guided microwave ablation for UAVF in conjunction with a literature review. ResultsThe patients were aged 26-44 years, with a mean age of (34.33±6.12) years and a median age of 33.5 years. All patients were initially screened via color Doppler ultrasound and definitively diagnosed by imaging modalities including uterine arteriography, computed tomography, and magnetic resonance imaging. The median gravidity was 4(4,6), with 5 patients having a history of ≥2 pregnancies. Five patients had a history of intrauterine instrumentation, among whom 4 had undergone ≥2 procedures. Regarding clinical presentations, 3 patients presented with abnormal vaginal bleeding (2 with massive hemorrhage), 2 were asymptomatic with abnormalities detected only on imaging, and 1 reported lower abdominal pain. In terms of management, 1 patient with recurrence after uterine artery embolization (UAE) underwent ultrasound-guided microwave ablation; 1 received UAE alone; 2 patients with concomitant retained products of conception underwent hysteroscopic mechanical morcellation (1 developed massive hemorrhage 10 hours postoperatively, which was successfully controlled with uterine balloon tamponade); 1 underwent focal lesion resection; and 1 underwent total hysterectomy. All 6 patients achieved favorable therapeutic outcomes. ConclusionAbnormal vaginal bleeding is the most typical clinical manifestation of UAVF, and digital subtraction angiography of the uterine arteries is recognized as the gold standard for definitive diagnosis. Emergency UAE is indicated for patients presenting with acute massive hemorrhage and hemodynamic instability. For patients with failed UAE or post-UAE recurrence who desire uterine and fertility preservation, ultrasound-guided microwave ablation represents a safe, effective, and novel minimally invasive therapeutic option.

OBJECTIVE To provide references for medical institutions’ outpatient pharmacies to increase the scanning rate, scanning efficiency and scanning accuracy of drug traceability codes.METHODS A total of 4 157 unscanned traceability code records from the first two‑week period of June 2025 were selected from the outpatient pharmacy of our hospital. Pareto chart was adopted to analyze the reasons for unscanned codes, and the Objective-Rules-Training-Check-Culture (ORTCC) model was utilized to carry out fine management. The traceability code scanning rate, pharmacists’ work efficiency (number of prescriptions dispensed per pharmacist per hour, number of drug boxes dispensed per pharmacist per hour), patients’ medication-waiting time and dispensing errors were compared among four periods: before scanning implementation (June 2024), before management intervention (June 2025), the first phase after management (September 2025), and the second phase after management (December 2025), so as to evaluate the management effects.RESULTS The traceability code scanning rates in the first phase and second phase after management were remarkably higher than those before management ( P <0.05), and the scanning rate in the second phase after management was obviously higher than that in the first phase after management ( P <0.05). Compared with the period before scanning implementation, the number of prescriptions dispensed per pharmacist per hour, the number of drug boxes dispensed per pharmacist per hour, decreased significantly and patients’ medication-waiting time prolonged markedly before management ( P <0.05). In the second phase after management, both the number of prescriptions dispensed per pharmacist per hour and the number of drug boxes dispensed per pharmacist per hour increased significantly, and patients’ medication-waiting time shortened greatly in contrast with the status before management ( P <0.05). Besides, no statistically significant differences were found in the number of drug boxes dispensed per pharmacist per hour and patients’ medication-waiting time between the second phase after management and the period before scanning implementation ( P >0.05).After the implementation of traceability-code scanning management, dispensing errors presented a gradually declining trend in each stage.CONCLUSIONS The fine management system for outpatient drug traceability codes constructed based on Pareto chart and ORTCC model can improve the traceability code scanning rate and pharmacists’ work efficiency of outpatient pharmacies, and reduce drug dispensing errors.

Body weight abnormalities, including overweight, obesity, and underweight, have become a dual public health challenge in Chinese adults: overweight and obesity lead to a variety of chronic complications, while underweight increases the risks of malnutrition, sarcopenia, and organ dysfunction. To systematically address these issues, multidisciplinary experts in endocrinology, sports science, nutrition, and psychiatry from various regions have held multiple weight management seminars. Based on the latest epidemiological data and clinical evidence, they expanded the guideline to include assessment and intervention strategies for underweight, in addition to the core content of obesity management. This guideline outlines the etiological mechanisms, evaluation methods, and multidimensional management strategies for overweight and obesity, covering key areas such as diagnosis and assessment, medical nutrition therapy, exercise prescription, pharmacological intervention, and psychological support. It is intended to provide a scientific and standardized approach to weight management across the adult population, aiming to curb the rising prevalence of obesity, mitigate complications associated with abnormal body weight, and improve nutritional status and overall quality of life.

Objective:To explore the relationship between mindfulness, optimism, and subjective well-being in post-lung transplantation patients, so as to provide a basis for nursing staff to improve patients' subjective well-being.Methods:This study was a cross-sectional survey. Convenience sampling was used to select 205 post-lung transplantation patients admitted to the First Affiliated Hospital of Guangzhou Medical University from October 2022 to November 2023 for the study. General Information Questionnaire, World Health Organization-Five Well-Being Scale, Mindful Attention Awareness Scale (MAAS) and Life Orientation Test-R (LOT-R) were used to investigate the study participants. Pearson correlation was used to analyze the correlation between subjective well-being and mindfulness and optimism in patients after lung transplantation. AMOS 24.0 software was used to construct a mediating model to analyze the path relationship between mindfulness, optimism and subjective well-being.Results:A total of 205 questionnaires were distributed and 202 valid questionnaires were recovered, with an effective recovery rate of 98.54% (202/205). In 202 patients after lung transplantation, the WHO-5 Well-Being Scale total score, MAAS total score, and LOT-R score were (16.31±4.73), (56.75±9.44), and (18.49±3.85), respectively. Lung transplantation patients' subjective well-being was positively correlated with mindfulness and optimism ( r=0.570, 0.600, both P<0.01). Optimism partially mediated the relationship between mindfulness and subjective well-being, with an effect value of 0.290 and an effect proportion of 52.35% (0.290/0.554) . Conclusions:Mindfulness and optimism are both positively correlated with subjective well-being in post-lung transplantation patients, and mindfulness could also influence subjective well-being through the mediating effect of optimism. Healthcare professionals should fully explore and cultivate positive psychological resources, such as mindfulness, in lung transplant patients, by increasing optimism as the target of intervention, which in turn improves patients' subjective well-being.

Clinical data of two patients with X-linked adrenoleukodystrophy (X-ALD) initially presenting as Addison′s disease were collected from the Department of Endocrinology, First Medical Center of Chinese PLA General Hospital. Relevant medical history, clinical features, laboratory tests, and genetic results were analyzed. The two male patients, aged 7 years (case 1) and 15 years (case 2), initially presented with generalized skin hyperpigmentation, without any family history of similar disorders. Both had normal growth and development, and adrenal CT and brain MRI revealed no significant abnormalities. Elevated very long-chain fatty acid (VLCFA) levels were detected. Genetic analyses identified a maternally inherited missense mutation (c.830G>A, p.Gly277Glu) in the ATP-binding cassette subfamily D member 1 (ABCD1) gene in case 1, and a missense mutation (c.1499G>T, p.Gly500Val) in case 2. Protein structural predictions indicated both mutations as potentially damaging or damaging, and both were classified as likely pathogenic according to American College of Medical Genetics and Genomics (ACMG) criteria (PM1/PM2/PP3_Moderate and PM2/PP3_Moderate/PM6, respectively), supporting their correlation with the clinical phenotype. Clinicians should maintain vigilance for X-ALD in male patients presenting with Addison′s disease, and combined VLCFA and genetic testing can effectively prevent misdiagnosis or delayed diagnosis.
