1.Transient Gastric Pressure Elevation Synergizing With Impaired Esophagogastric Junction Barrier Function Plays a Pivotal Role in the Refractory Gastroesophageal Reflux Disease

Background/Aims:
The pathophysiology of refractory gastroesophageal reflux disease (RGERD), which differs from proton pump inhibitor dependent gastroesophageal reflux disease (DGERD), remains incompletely elucidated. This study aims to compare esophageal motility patterns, transdiaphragmatic pressure gradients (TPG), and reflux profiles between RGERD and DGERD patients, and to delineate dynamic pressure gradient-esophagogastric junction (EGJ) interactions in these patients.
Methods:
In this retrospective study, 274 patients who underwent 24-hour impedance-pH monitoring and high-resolution manometry, along with an assessment of proton pump inhibitor responsiveness, were classified as RGERD (32.5%), DGERD (54.4%), or non-GERD (13.1%). Clinical characteristics, TPG, esophageal motility, and reflux metrics were compared between RGERD and DGERD patients. Subgroup analysis excluding hiatal hernia (HH) was conducted to investigate the pathophysiology of RGERD.
Results:
The RGERD group exhibited a significantly higher proportion of chest pain compared to the DGERD group. Regarding reflux profiles, RGERD patients without HH (RGERDHH- group) experienced increased weakly acidic reflux (P < 0.001) and prolonged bolus exposure (P = 0.006) compared to their counterparts (DGERDHH- group). Mechanistically, the RGERDHH- group showed reduced lower esophageal sphincter basal pressure (P = 0.010) and EGJ contractile integral (P = 0.005). Notably, following a wet-swallow, the RGERDHH- group experienced the significant elevation in gastric pressure and TPG. Correlation analyses revealed weakly acidic reflux and bolus exposure were positively correlated with gastric pressure variation, and inversely correlated with lower esophageal sphincter basal pressure.
Conclusions
Transient gastric pressure elevation and compromised EGJ barrier function drive weakly acidic reflux and esophageal bolus exposure. This pressure gradient-barrier mismatch underpins the refractoriness of RGERD.
2.Clinical Analysis of 30 Cases of Uterine Arteriovenous Fistula Diagnosis Ultra-sonography

Objective:Analyze the clinical manifestations,diagnosis,treatment,and prognosis of uterine arterio-venous fistula(UAVF).Methods:A retrospective analysis was conducted on the clinical data of 30 patients diag-nosed with UAVF by ultrasonography at The First Affiliated Hospital of Zhengzhou University from January 1,2019,to December 31,2022.Results:Among the 30 patients,the median age was 33 years.All patients had a his-tory of uterine surgery,and 3 cases(10.0%)were complicated by gestational trophoblastic disease.Vaginal bleeding was the presenting symptom in 26 cases(86.7%).All patients were diagnosed using color Doppler ul-trasonography.Additionally,22 patients(73.3%)underwent digital subtraction angiography(DSA)for definitive diagnosis.The treatments administered were as follows:8 patients underwent uterine artery embolization(UAE)to hysteroscopic lesion resection,10 patients received UAE only,4 patients underwent hysteroscopic lesion resec-tion only,4 patients required UAE due to massive bleeding during hysteroscopic lesion resection,1 patient under-went direct hysterectomy,3 patients received conservative management.Among the 22 patients who underwent UAE,19 cases(86.4%)experienced no recurrence of vaginal bleeding.Among the 26 patients who underwent fertility-preserving treatment,18(69.2%)regained normal menstruation.Of the 10 patients within this group who desired pregnancy,4 subsequently had successful deliveries.Conclusions:UAVF primarily manifests as irregular vaginal bleeding.Color Doppler ultrasonography can serve as the initial diagnostic tool,while DSA provides a de-finitive diagnosis.UAE and hysteroscopic lesion resection can be considered preferred treatment options for pa-tients desiring fertility preservation.
3.Predictive value of DCE-MRI quantitative parameters for lymph node metastasis in breast cancer

Objective:To investigate the predictive value of quantitative parameters of dynamic contrast enhanced magnetic resonance imaging (DCE-MRI) in lymph node metastasis of breast cancer.Methods:A retrospective study was conducted on 98 breast cancer patients admitted to our hospital from Jan. 2021 to Dec. 2022. All patients underwent DCE-MRI before surgery and were followed up for 2 years. Finally, 16 cases of lymph node metastasis were confirmed and included in the metastasis group, and the remaining 82 cases without metastasis were included in the non- metastasis group. The clinical data, preoperative routine MRI features and DCE-MRI quantitative parameters of the two groups were analyzed and compared, and the predictive value of DCE-MRI quantitative parameters was analyzed by receiver operating characteristic curve (ROC) .Results:There was no statistically significant difference in age, course of disease and pathological type between the two groups ( t=0.65, 1.43, χ2=1.33, P>0.05) ; The proportion of patients with middle or high pathological grade in metastasis group was higher than that in non-metastasis group ( χ2=11.29, P<0.05). The maximum diameter, shape, internal enhancement pattern and time intensity curve type showed no statistically significant difference ( t=0.54, χ2=3.51, 0.67, 0.42, P>0.05) ; The proportion of patients with blurred edge features in metastasis group was higher than that in the other group ( χ2=4.53, P<0.05). The volume transfer constant (K trans) and reflux rate constant (Kep) in the metastasis group were higher than those in the other group ( t=2.97, 3.22, P<0.05), and extracellular space volume ratio (Ve) showed no statistically significant difference ( t=0.42, P>0.05). Receiver operating characteristic curve (ROC) results showed that the area under the curve (AUC) of K trans, Kep and their combined prediction of breast cancer lymph node metastasis was 0.758, 0.710 and 0.764, respectively, and the sensitivity was 56.25%, 42.75% and 62.50%, respectively. The specificity was 86.59%, 89.02% and 86.59% (all P<0.05) . Conclusions:Quantitative parameters of DCE-MRI can be used to predict lymph node metastasis of breast cancer, among which K trans, Kep and their combination are more effective in diagnosis.

Based on the implementation of GMP, comprehensive improvement of quality control measures, and consideration of the 3Rs principle for experimental animals, Several years ago, WHO, the European Pharmacopoeia and the FDA gradually abolished the testing for abnormal toxicity of biological products, and the Chinese Pharmacopoeia 2025 Edition(Volume Ⅲ)also revised the testing for abnormal toxicity of biological products. In order to help users of the Chinese Pharmacopoeia(Volume Ⅲ) better understand and implement this pharmacopoeia, this article provides a detailed review of the changes in regulatory concepts for abnormal toxicity test in various countries and the process of gradually phasing out abnormal toxicity test, as well as the actual situation of China’s pharmaceutical industry. It also interprets the ideas and considerations for revising the Chinese Pharmacopoeia 2025 Edition(Volume Ⅲ) on abnormal toxicity test for biological products.

Objective:To establish a modified one-stage clotting assay (mOSA) based on the STA-R Evolution coagulation analyzer for quantifying emicizumab (EMI) concentration and to preliminarily evaluate its analytical performance; meanwhile to explore the clinical utility of the standard one-stage clotting assay (sOSA) in indirectly predicting EMI levels through surrogate factor Ⅷ (FⅧ) activity.Methods:A total of 30 pediatric patients with hemophilia A (HA) treated with EMI in the Hemophilia Treatment Center of Shenzhen Children′s Hospital from January 2023 to March 2025 were enrolled, and 48 post-treatment plasma samples were collected. EMI standards (2.5~100 μg/ml) were prepared using FⅧ-deficient plasma to establish the mOSA detection system. The linearity, accuracy, and precision of the method were evaluated. Surrogate FⅧ activity was measured by sOSA to estimate EMI concentrations, and its correlation with mOSA-derived EMI concentrations was analyzed using Spearman correlation analysis. The equivalent FⅧ activity in patient plasma samples was measured using a human chromogenic substrate assay-based FⅧ activity detection reagent, and Spearman correlation analysis was employed to evaluate its correlations with both the EMI concentrations measured by the mOSA method and estimated by the sOSA method respectively.Results:The established mOSA method for EMI detection showed excellent linearity in the range of 2.5?100 μg/ml ( Y=1.047 X?1.033, R 2=0.995, P<0.001). Average spike recovery rates at 25, 50, and 75 μg/ml were 101.55%(25.39/25.00), 105.31%(52.66/50.00), and 98.20%(73.65/75.00), respectively. Coefficients of variations of within-and inter-batch were 3.47%?4.80% and 6.30%?8.96%, respectively. A prediction model for EMI concentration was established as follows: estimated EMI concentration (μg/ml)=0.095×[alternative FⅧ activity (%) measured by sOSA]+2.652 ( R2=0.999, P<0.001). Validation demonstrated a strong correlation between the EMI concentration measured by the mOSA method and the EMI concentration estimated by the sOSA method ( r=0.989, P<0.001), with good consistency ( Y=1.014 X+0.684, R2=0.972, P<0.001). Both the EMI concentration measured by the mOSA method and the EMI concentration estimated by the sOSA method showed extremely strong correlations with the equivalent FⅧ activity ( r=0.986 and 0.987, respectively; P<0.001 for both). Conclusions:The mOSA system established on the STA-R Evolution analyzer demonstrates robust linearity, accuracy, and reproducibility, fulfilling clinical requirements for therapeutic drug monitoring of EMI. The sOSA method provides reliable indirect estimation of EMI concentrations through surrogate FⅧ activity, offering critical support for emergency decision-making.
10.Investigation on status quo of indwelling needle use in 77 hospitals of Chongqing city

Objective To investigate the status quo of indwelling needles use in the hospitals at all lev-els in Chongqing city,and to evaluate the normalization and safety of indwelling needles use by referring to Nursing Practice Standards for Intravenous Therapy.Methods A self-designed questionnaire was used in July 2023 to conduct a cross-sectional survey on the use of indwelling needles in the hospitals at all levels in Chongqing city.Results A total of 30 807 patients in 77 hospitals were included,among them 62.67%used indwelling needles.The closed type indwelling needles accounted for 88.87%and the safety type indwelling needles accounted for 41.74%.Non-needle infusion connectors accounted for 37.78%,and steel needle con-nected with heparin caps accounted for 51.69%.95.66%of puncture site was in the upper limb,the non-cor-rosive drug infusion accounted for 87.57%,and the complications occurrence rate was 2.46%.Among the 6 419 surveyed nurses,75.51%removed indwelling needles based on clinical indications.There were statistically significant differences in the usage rates of closed type indwelling needles and non-needle infusion connectors among hospitals at different levels(P<0.05).There were also statistically significant differences in the inci-dence rates of complication and phlebitis(P<0.05).Conclusion The overall application of indwelling needles in the hospitals at all levels in Chongqing city is generally standardized,but the use of safety type indwelling needles and needle-free infusion connectors still needs to be improved.
