1.Construction and evaluation of a predictive model for the degree of coronary artery occlusion based on adaptive weighted multi-modal fusion of traditional Chinese and western medicine data

Objective:
To develop a non-invasive predictive model for coronary artery stenosis severity based on adaptive multi-modal integration of traditional Chinese and western medicine data.
Methods:
Clinical indicators, echocardiographic data, traditional Chinese medicine (TCM) tongue manifestations, and facial features were collected from patients who underwent coronary computed tomography angiography (CTA) in the Cardiac Care Unit (CCU) of Shanghai Tenth People's Hospital between May 1, 2023 and May 1, 2024. An adaptive weighted multi-modal data fusion (AWMDF) model based on deep learning was constructed to predict the severity of coronary artery stenosis. The model was evaluated using metrics including accuracy, precision, recall, F1 score, and the area under the receiver operating characteristic (ROC) curve (AUC). Further performance assessment was conducted through comparisons with six ensemble machine learning methods, data ablation, model component ablation, and various decision-level fusion strategies.
Results:
A total of 158 patients were included in the study. The AWMDF model achieved excellent predictive performance (AUC = 0.973, accuracy = 0.937, precision = 0.937, recall = 0.929, and F1 score = 0.933). Compared with model ablation, data ablation experiments, and various traditional machine learning models, the AWMDF model demonstrated superior performance. Moreover, the adaptive weighting strategy outperformed alternative approaches, including simple weighting, averaging, voting, and fixed-weight schemes.
Conclusion
The AWMDF model demonstrates potential clinical value in the non-invasive prediction of coronary artery disease and could serve as a tool for clinical decision support.
2.Study on Tongue Manifestations of Patients with Different Syndromes in Non-Small Cell Lung Cancer and Their Correlation with Laboratory Indicators

Objective To study the tongue manifestation of patients with different syndromes in non-small cell lung cancer(NSCLC)and the correlation between tongue characteristics of different syndromes and tumor markers and coagulation indicators.Methods Totally 497 patients with NSCLC were grouped according to syndrome differentiation,and the differences in tongue characteristics of different syndromes were compared.Bivariate correlation analysis was used to study the correlation between tongue characteristics and serum tumor markers and coagulation indicators in patients with NSCLC of different syndromes.Results Compared with healthy people of different syndromes,in TB-a,yin deficiency and phlegm-heat syndrome>healthy group>qi-yin deficiency syndrome>spleen deficiency and phlegm-dampness syndrome>lung stagnation and phlegm-stasis syndrome(P<0.001).In TB-L,healthy group>spleen deficiency and phlegm-dampness syndrome>qi-yin deficiency syndrome>lung stagnation and phlegm-stasis syndrome>yin deficiency and phlegm-heat syndrome(P<0.001).In TB-b,yin deficiency and phlegm-heat syndrome>qi-yin deficiency syndrome>spleen deficiency and phlegm-dampness syndrome>healthy group>lung stagnation and phlegm-stasis syndrome(P<0.001).Yin deficiency and phlegm-heat syndrome had the highest TB-a and the lowest Per-all.Spleen deficiency and phlegm-dampness syndrome had the highest TB-L and Per-all.Lung stagnation and phlegm-stasis syndrome had lower TB-b and TC-b than other groups,lower TB-a than the healthy group,and a high Per-all index(P<0.05).In terms of tumor markers,Per-all in spleen deficiency and phlegm-dampness syndrome was positively correlated with Ca199,Ca50 and Ca242(P<0.05).In terms of coagulation indicators,the tongue texture index of lung stagnation and phlegm-stasis syndrome had a high correlation with the coagulation indicator Fg(P<0.01).Conclusion Different TCM syndromes of NSCLC have their own typical tongue characteristics.Tongue manifestations of different syndromes are correlated with tumor markers and coagulation indicators,respectively,which can reflect changes in clinical status.
3.Study on Chromaticity Characteristics of Gastrointestinal Tumors and Construction of Auxiliary Diagnostic Models

Objective To analyze the characteristics of facial and tongue chromaticity parameters in patients with gastrointestinal tumors by setting the inspection image characteristics of patients with gastrointestinal tumors as the main research content;To establish an auxiliary diagnostic model for gastrointestinal tumors.Methods One-way ANOVA,t-test,Mann-whitney U test,canonical correlation analysis and Spearman statistical methods were used to analyze the characteristics of inspection image indexes and correlation of tumor markers of the 391 cases in the control group and 359 patients with gastrointestinal tumors.Machine learning methods such as SVM,Random Forest,KNN,Naive Bayes,XG Boost and Ada Boost were used to establish an auxiliary diagnostic model for gastrointestinal tumors.Results In terms of facial indicators,there were differences in F-R,F-G and F-B indicators among the control group,early-stage gastrointestinal cancer patients,and mid-to late-stage gastrointestinal cancer patients,in the comparison of tongue features among,TC-L,TB-L and TB-a of the control group,patients with early gastrointestinal tumors,and patients with intermediate and advanced gastrointestinal tumors showed a gradual downward trend;the AUC of the auxiliary diagnosis model of gastrointestinal tumor disease based on the chromaticity parameters of face tongue image constructed by Ada Boost algorithm was 0.930.Conclusion The auxiliary diagnostic model of gastrointestinal diseases constructed by facial and tongue images has good diagnostic effect,which can provide objective data support for in-depth exploration of the complex relationship between diagnosis and disease.
4.Research Progress in Multi-Region Inspection for Assessing Blood Stasis Syndrome

As a common syndrome type in TCM,blood stasis syndrome is diagnosed based on the four diagnostic methods of TCM,with inspection providing especially intuitive information.In recent years,with the advancement of objectification studies on the four diagnostic methods,inspection techniques for blood stasis syndrome have gradually transitioned from qualitative to quantitative analysis.This article reviewed recent progress in modern research on multi-region inspection for blood stasis syndrome,including facial complexion,tongue characteristics,sublingual collateral vessels,and microcirculation signs.Multi-region inspection technology has progressively established standardized acquisition protocols integrated with artificial intelligence technology,achieving a transition from qualitative to quantitative analysis.These region-specific data demonstrate clear associations with cardiovascular and metabolic diseases,supporting diagnostic objectification of blood stasis syndrome.However,further efforts remain necessary to expand clinical samples,integrate macro-micro data,standardize quantitative criteria,and establish collaborative diagnostic criteria for precise syndrome differentiation.
5.Expression of serum miR-423-5p and ferritin in elderly osteoporosis and their relationship with hip fragility fracture

Objective To investigate the expression of serum micro ribonucleic acid(miR)-423-5p and fer-ritin in elderly osteoporosis(OP)and their relationship with hip fragility fracture.Methods A total of 79 eld-erly OP patients admitted to the hospital from March 2020 to March 2024 were selected as the observation group,and 62 healthy elderly patients with complete physical examination records and reports were selected as the control group during the same period.Patients in the observation group were divided into fracture group(42 cases)and non-fracture group(37 cases)according to the occurrence of hip fragility fracture.Serum miR-423-5p and ferritin levels were compared between the observation group and the control group and between the fracture group and the non-fracture group.The difference of bone mineral density(BMD)at different sites between the fracture group and the non-fracture group was compared.The correlation between serum miR-423-5p and ferritin and BMD at different sites was analyzed by Pearson,and the predictive value of serum miR-423-5p and ferritin in elderly hip fragility fractures was analyzed by receiver operating characteristic(ROC)curve.Results Serum miR-423-5p and ferritin in observation group were higher than those in control group,the differences were statistically significant(P<0.05).The levels of serum miR-423-5p and ferritin in frac-ture group were higher than those in non-fracture group,and the differences were statistically significant(P<0.05).Greater trochanteric BMD and total hip BMD in the non-fracture group were higher than those in the fracture group,and the differences were statistically significant(P<0.05).According to Pearson correlation analysis,serum miR-423-5p and ferritin levels in elderly OP patients were negatively correlated with BMD de-tected in lumbar spine,femoral neck,greater trochanter,intertrochanter and total hip(P<0.05).ROC curve analysis results showed that the sensitivity,specificity and the area under the curve(AUC)of serum miR-423-5p combined with ferritin were 0.786,0.892 and 0.818(95%CI:0.715-0.912),which were higher than the AUC predicted by the two alone and had higher predictive value(P<0.05).Conclusion The combination of serum miR-423-5p and ferritin levels can predict the risk of hip fragility fracture in elderly OP patients.
6.Application of cognitive interview in cross-cultural adjustment of Questionnaire about Gynaecological and Pelvic Pain Symptoms

Objective:Cognitive interviews were used to explore the target population's understanding and cognition of the Questionnaire about Gynaecological and Pelvic Pain Symptoms (ENDOPAIN-4D) in the Chinese context. The consistency of language expression and concept between the Chinese version ENDOPAIN-4D and the original scale was verified.Methods:Qualitative research methods were employed. Objective sampling method was used to select the patients with endometriosis/adenomyosis in the gynecological ward of Chinese Academy of Medical Sciences & Peking Union Medical College Hospital from October to December 2023 as the interview subjects. Two rounds of cognitive interviews were conducted, and the interview data were coded and analyzed using the Question Appraisal System. Based on the interview results and the recommendations of the expert group, the scale items were revised to form the Chinese version of ENDOPAIN-4D, and reliability and validity tests were conducted.Results:This study ultimately included 24 patients with endometriosis/adenomyosis, aged 24-47 years. The results of the first round of interviews showed that some interviewees had ambiguous understandings of the 16 items in the questionnaire, most of the item questions were concentrated in the category of "3 clarity, 4 hypotheses", and a small number of questions appeared in the category of "2 guidance, 5 knowledge/memory, 7 options", and were revised. The results of the second round of interviews showed that respondents could correctly understand the scale items. The Cronbach α coefficient of the total scale was 0.874, and the retest reliability was 0.952. The correlation coefficient of the Chinese version of the Global Pain Scale as the reference index of compatibility validity was 0.764 ( P<0.01), and the exploratory factor analysis extracted a total of 4 dimensions, with a cumulative variance contribution rate of 61.169%. The final formal questionnaire included 21 items in 4 dimensions including spontaneous pelvic pain, pain during sexual intercourse, intestinal pain and/or symptoms, and other symptoms. Conclusions:Cognitive interviews were used to identify cognitive biases in the target population during the completion of the scale and to address them appropriately, reducing the differences in the target population's understanding of the ENDOPAIN-4D scale and improving its scientific validity and reliability.

8.Qualitative study on nursing undergraduates' experiences with a blended teaching model in midwifery courses

Objective:To explore the experiences of nursing undergraduates with the blended teaching model in midwifery courses.Methods:This qualitative study employed purposive sampling to select 17 nursing undergraduates from the 2021 cohort who participated in the "Midwifery Theory and Skills 2" course in December 2023. Face-to-face semi-structured interviews were conducted, and the data were analyzed using Colaizzi's seven-step method.Results:Three main themes and eight subthemes were identified. First, the blended teaching model enhanced professional knowledge and skills in midwifery: online resources expanded professional knowledge; online demonstration videos improved operational skills; offline group collaboration strengthened communication skills. Second, it developed professional awareness and emotions in midwifery: systematic course learning enhanced professional identity; offline role-playing cultivated emotional perception; clinical experience sharing by teachers fostered a sense of responsibility in midwifery. Third, it optimized blended teaching resources: the user experience on online platforms was improved, and offline classroom settings were better designed.Conclusions:The blended teaching model aids in enhancing students' comprehensive knowledge and skills in midwifery and fosters professional emotions related to the field. It is recommended to further optimize blended teaching resources to improve the effectiveness of this teaching model in midwifery courses.
