Neurosyphilis is a group of clinical syndromes caused by treponema pallidum invading the central nervous system and causing varying degrees of damage to the brain parenchyma, brain (spinal) membrane or spinal cord. Cerebral syphilitic gumma is the rarest form of neurosyphilis, and its clinical manifestations lack specificity, which is easily confused with other intracranial space occupying lesions on imaging. In this article, a patient with cognitive dysfunction, intracranial hypertension, and imaging findings similar to multicentric glioma was diagnosed as cerebral syphilitic gumma by serology, cerebrospinal fluid test and brain histopathology. The symptoms basically disappeared after treatment with standard penicillin. This report aims to raise awareness of cerebral syphilitic gumma in order to reduce missed diagnosis and misdiagnosis.

3.Recent advances and challenges of CpG ODN adjuvants
Chinese Journal of Biologicals 2025;38(06):746-752
Adjuvants are an important part of vaccines. In order to overcome the weaknesses of traditional adjuvants, it is essential to develop new adjuvants. At present, CpG ODN has been used as vaccine adjuvants in clinical and preclinical studies of preventive antiviral, anti-parasitic and therapeutic tumor vaccines. Hepatitis B vaccine and COVID-19 vaccine with CpG ODN 1018 as adjuvant have been approved for marketing. In addition to single application, it is one of the most important research directions to develop new delivery systems of anti-tumor vaccines(such as nanoparticles and liposomes)with CpG ODN as adjuvant, and the delivery efficiency and targeting of CpG ODN need to be improved. In addition to the traditional subcutaneous and intramuscular immunization routes, CpG ODN is also being applied to the development of intranasal mucosal immune adjuvants, and its efficacy and mechanism as mucosal immune adjuvants have not been fully clarified.CpG ODN is an agonist of Toll-like receptor 9(TLR9), which can activate natural immunity and further mediate adaptive immune response. Its characteristic is that it can cause strong Th1 immune response. Although many clinical studies showed that CpG ODN adjuvants only cause slight and short-term adverse reactions, some studies also showed that CpG ODN may cause or aggravate autoimmune diseases and inflammatory syndrome by promoting Th1 response. How to overcome this shortcoming of CpG ODN adjuvants may be a challenge for the development of CpG ODN adjuvants in the future. In this paper, the latest research progress and challenges of CpG ODN adjuvants are reviewed.

Objective:To evaluate the efficacy and safety of Neuroform Atlas stent-assisted coil embolization in patients with middle cerebral artery bifurcation aneurysms. Methods:A retrospective analysis was performed; the clinical data of 46 patients with middle cerebral artery bifurcation aneurysms accepted Neuroform Atlas stent-assisted coil embolization in First Affiliated Hospital of Zhengzhou University, Beijing Tiantan Hospital Affiliated to Capital Medical University, First Affiliated Hospital of Harbin Medical University, Zhujiang Hospital of Southern Medical University and First Affiliated Hospital of Chongqing Medical University from January 2022 to March 2024 were collected. There were 28 ruptured aneurysms (60.87%) and 18 unruptured aneurysms (39.13%). Follow-up was performed for more than 3 months; Raymond-Roy grading was used to evaluate the aneurysm embolization immediately after embolization and during follow-up; perioperative hemorrhagic or ischemic complications were recorded; modified Rankin Scale (mRS) was used to evaluate the prognosis of the patients at discharge and during follow-up (mRS score≤2: good prognosis, and mRS score>2: poor prognosis). Results:Coil embolization was successful in all 46 patients. DSA immediately after embolization showed that 41 patients (89.13%) had completely occluded aneurysms (Raymond-Roy grading I), 2 patients (4.35%) had residual aneurysm neck (Raymond-Roy grading Ⅱ) and 3 patients (6.52%) had partially occluded aneurysms (Raymond-Roy grading Ⅲ). Perioperative complications occurred in 5 patients, including 2 with postoperative cerebral infarction, 1 with hydrocephalus, 1 with postoperative pneumonia leading to respiratory failure, and 1 with stent thrombosis during embolization. Both at discharge and 3 months after embolization, 43 patients (93.48%) had good prognosis and 3 patients (6.52%) had poor prognosis. No obvious ischemic complications (such as stent restenosis) or hemorrhagic complications (such as re-rupture of the aneurysms) were found in all patients. Thirty patients (65.22%) had imaging follow-up for 6-12 months: 26 (86.67%) had Raymond-Roy grading I, 3 (10.00%) had Raymond-Roy grading II, and 1 (3.33%) had Raymond-Roy grading III.Conclusion:Neuroform Atlas stent-assisted coil embolization has good short-term efficacy and high safety in middle cerebral artery bifurcation aneurysms, but long-term follow-up observation is still needed to verify its efficacy.

Chinese Journal of Integrated Traditional and Western Medicine in Intensive and Critical Care 2025;32(4):508-512
Heat stroke is a life-threatening emergency characterized by a rapid elevation in core body temperature(typically>40℃)and central nervous system dysfunction(e.g.,delirium,seizures or coma),often accompanied by systemic inflammatory response, coagulation disorders, and multiple organ failure, with an extremely high mortality rate.Among the multiple organ injuries caused by heat stroke, the liver,as the core organ for metabolism and detoxification in the body,is one of the targets that are vulnerable to high temperatures. If not intervened promptly and effectively, acute liver injury(ALI)can rapidly evolve into acute liver failure(ALF),significantly increasing the complexity of clinical treatment and the risk of patient death. In recent years, with the frequent occurrence of heat wave events worldwide, the pathological mechanisms and prevention strategies of heat stroke and its related liver injuries have received increasingly widespread attention.Although basic and clinical studies have made certain progress, revealing key pathological changes such as hepatocyte necrosis, excessive inflammatory response and coagulopathy, there is still a lack of unified and standardized consensus guidelines for the clinical evaluation, diagnosis and systematic treatment of heat stroke combined with liver failure. Therefore, this article aims to systematically review and summarize the pathogenesis, clinical presentation, diagnostic approaches, and treatment strategies of current liver failure related to heat stroke,in order to provide clear diagnostic and treatment ideas and practical references for clinical workers, and thereby provide theoretical basis and clinical guidance for improving the prognosis of critically ill patients with heat stroke liver failure.

Objective To identify the enhancer landscape marked by histone H3K27ac modifications in diffuse-type gastric cancer(DGC)tissues, and to elucidate the epigenetic remodeling mechanisms by which active enhancers regulate cachexia-related genes. Methods Gastric mucosal tissue samples were collected from Department of Gastroenterology of Army Medical Center of PLA during January 2022 to March 2023,including 10 normal gastric mucosa tissues(Normal group),10 DGC tissues diagnosed with cachexia(DGC group),and 10 organoids derived from DGC tissues(Organoid group).Using H3K27ac chromatin targeting cleavage and tagmentation(CUT&Tag)technology, genomic modification regions were captured to screen specific active enhancers and their potential target genes in DGC tissues.CRISPR-dCas9 gene editing technology was used to intervene with the enhancers, and the expression of target genes was detected with Western blotting and qRT-PCR.Sixteen female SPF-grade BALB/c Nude mice(6~8 weeks old, weighing 18~21 g)were utilized to establish an orthotopic xenograft tumor model using the human diffuse-type gastric cancer cell line MKN45.Cachexia-related phenotypes were evaluated in 3 groups:normal group(n=4),silencing group(n=6),and control group(n=6).Results Significant differential enhancer regions were identified between DGC and normal gastric mucosa tissues.DGC tissues exhibited a marked increase in enhancer abundance(P<0.05)and signal intensity when compared with the normal counterparts.Integrated analysis of transcriptome data revealed that some of these active enhancers up-regulated the expression of GDF15,a cachexia-associated target gene in DGC.Targeted silencing of the active enhancer of GDF15 using CRISPR/dCas9-KRAB plasmid technology resulted in a significant reduction in GDF15 expression at both mRNA levels(P<0.05)and protein. Results from orthotopic transplantation experiments of DGC demonstrated that silencing of active enhancers alleviated the cachexia phenotype in nude mice(P<0.05).Conclusion DGC exhibits enhancer remodeling, which regulates the expression of the cachexia-associated gene GDF15,and thereby contributes to the pathogenesis and progression of cancer cachexia.

Objective To investigate the correlation between early clinical outcomes following endoscopic surgery for unilateral lumbar disc herniation and variations in the cross-sectional area of the sciatic nerve. Methods Fifty-two patients(n=52)with unilateral lumbar disc herniation were recruited from June 2023 to June 2024 at the Pain Department of the First Affiliated Hospital of Nanchang University. The cross-sectional area(CSA)values of bilateral sciatic nerves were measured at the gluteal sciatic tuberosity-great femoral rotor level using ultrasound before and on the 4th day after surgery. The change in nerve cross-sectional area ΔCSA and sciatic nerve swelling rate(SR-SN)were calculated.Patient BMI(Body Mass Index),visual analog scale(VAS),Japanese Orthopaedic Association score(JOA),Oswestry Disability Index(ODI),and Treatment Improvement Rate(TIR)were collected preoperatively and on postoperative day 4.Changes in sciatic nerve cross-sectional area in unilateral lumbar disc herniation and its relationship to clinical outcomes were analyzed using the scoring scales as controls. Results Compared to the preoperative period, there was a significant decrease in VAS and ODI scores,as well as an increase in JOA score on the 4th postoperative day(P<0.01).Furthermore,surgical intervention led to substantial improve-ment in clinical outcomes for the patients.Prior to surgery, the CSA value of the affected side of the sciatic nerve was greater than that of the healthy side(P<0.01).Although CSA value decreased on the affected side after surgery,it remained higher than that of the healthy side(P<0.01).Additionally,there was a significant difference between SR-SN values on both sides with higher values observed on the affected side(P<0.01).Notably,△CSA on the affected side exhibited a negative correlation with postoperative VAS score,JOA score, and treatment improvement rate(P<0.05);The SR-SN on the affected side exhibited a positive correlation with BMI and negative correlations with postoperative VAS score,JOA score, and treatment improvement rate(P<0.05).However,no significant correlations were observed between △CSA,SR-SN,and ODI score on the affected side.(P>0.05).Conclusions In patients undergoing endoscopic surgery for lumbar disc herniation,a greater change in the cross-sectional area of the affected sciatic nerve before and after the operation is associated with more pronounced improvement in early symptoms. However, further investigation is required to explore the relationship between this change and recovery of both lumbar spine and lower limb function.
