1.Effect of arthroscopic double locking ring suture technique on shoulder joint function in small and medium-sized patients with rotator cuff tear

Objective To explore the effect of arthroscopic double locking ring suture technique on shoulder joint function in small and medium-sized patients with rotator cuff tear.Methods From January 2020 to June 2022,54 small and medium-sized patients with rotator cuff tear were selected and randomly divided into control group and treatment group with 27 cases in each group.The control group was repaired by conventional single-row modified Mason-Allen technique,while the treatment group was repaired by arthroscopic double locking ring suture technique.The operation time,blood loss,total hospitalization time,the clinical efficacy and recurrence rate 6 months after operation of the two groups were compared.The pain at different time points,abduction of shoulder joint,University of California at Los Angeles(UCLA)scale,American Shoulder and Elbow Surgeons(ASES)scale and Constant-Murley scale before and 3 months after operation were compared.The incidence of joint stiffness was followed up for 6 months.Results The operation time in the treatment group was significantly shorter than that in the control group,with statistical significance(P<0.05).After 6 months'follow-up,the total excellent and good rate in the treatment group was significantly higher than that in the control group,with statistical significance(P<0.05).At the 1,3 and 6 months after operation,the visual analogue scale(VAS)of the two groups decreased compared with that before operation,and the treatment group was lower than that of the control group,with statistical significance(P<0.05).Three months after operation,the UCLA scale,ASES scale and Constant-Murley scale of the two groups were higher than those before operation,and the treatment group was higher than that of the control group,and the differences were statistically significant(P<0.05).At the 1,3 and 6 months after operation,the abduction range of the patients in the two groups increased compared with that before operation,and the differences were statistically significant(P<0.05).During the follow-up of 6 months,the incidence of joint stiffness in the treatment group was 3.70%,which was significantly lower than that in the control group(29.63%),and the differences were statistically significant(P<0.05).After 6 months'follow-up,the recurrence rate in the treatment group was 0.00%,which was significantly lower than that in the control group(18.52%),with statistical significance(P<0.05).Conclusion Small and medium-sized patients with rotator cuff tears can be treated by arthroscopic double locking ring suture technology,which can shorten the operation time,effectively relieve the pain of patients,improve shoulder joint mobility,improve shoulder joint function,reduce the incidence and recurrence rate of joint stiffness,and improve clinical efficacy.
2.The accuracy of artificial intelligence-assisted design and 3D printed personalized osteotomy guide plates in assisting total knee arthroplasty

Objective:To observe the accuracy of artificial intelligence-assisted design and 3D printed personalized osteotomy guide plate-assisted total knee arthroplasty (TKA).Methods:A total of 112 patients with knee osteoarthritis scheduled for TKA admitted to the Second People′s Hospital of Guangxi Zhuang Autonomous Region from February 2022 to February 2024 were prospectively included. They were divided into the intelligent group and the traditional group by lottery, with 56 cases in each group. The traditional group used conventional osteotomy guides, while the intelligent group used artificial intelligence-assisted design and 3D printed personalized osteotomy guides. The surgical conditions, postoperative imaging indicators, deviation range of hip-knee-ankle angle (HKA) and clinical efficacy of the two groups were observed.Results:There was no statistically significant difference in incision length between the two groups ( P>0.05). The operation time and blood loss in the intelligent group were both less than those in the traditional group (all P<0.05). The differences in postoperative HKA, sagittal tibial prosthesis angle (LTC), and sagittal femoral prosthesis angle (LFC) in the intelligent group were all smaller than those in the traditional group (all P<0.05), but there were no statistically significant differences in coronal femoral prosthesis angle (FFC) and coronal tibial prosthesis angle (FTC) between the intelligent group and the traditional group ( P>0.05). There was a statistically significant difference in the deviation range grade of HKA between the two groups ( P<0.05), and the proportion of HKA deviation ≤1° in the intelligent group was higher than that in the traditional group ( P<0.05). After the operation, the Visual Analogue Scale (VAS) scores for pain in both groups were lower than those before the operation (all P<0.05), and the range of motion (ROM) of the knee joint and the American Knee Society Score (KSS) were both increased compared with those before the operation (all P<0.05). However, there were no statistically significant differences in the VAS scores, ROM, and KSS of pain after the operation between the two groups (all P>0.05). Conclusions:Artificial intelligence-assisted design and 3D printed personalized osteotomy guide plates can improve the accuracy of TKA surgery, shorten the operation time and reduce the amount of blood loss.

5.Bone density and bone metabolism in plasmapheresis donors

【Objective】 To investigate the level of serum bone metabolism and biochemical markers and bone density of plasmapheresis donors, and to provide scientific basis for ensuring the health and safety of plasmapheresis donors in China. 【Methods】 A total of 437 plasmapheresis donors from Linwu plasmapheresis station in Hunan Province from July 1 to September 30, 2022 were recruited to determine the levels of total serum calcium, albumin, serum 25-hydroxyvitamin D (25OHD), serum type I procollagen N-terminal propeptide (P1NP), and collagen type 1 crosslinked carboxyl-terminal peptide (β-CTX). Dual-energy X-ray method was used to measure the bone density of the anteroposterior lumbar spine (L1-L4) and bilateral femoral neck bone density of plasmapheresis donors. Plasmapheresis donors were grouped according to the type of plasma donation (first-time and repeat plasmapheresis donors) and the total number of plasma donations to assess the differences in bone density and serum bone metabolism biochemical markers between groups. The dose-response relationship between the total number of plasmapheresis donations and biochemical indexes was analyzed by limiting cubic spline, and the influencing factors of different indexes were explored by multiple linear regression. 【Results】 A total of 437 plasmapheresis donors were included in this study, including 187 first-time plasmapheresis donors and 250 repeat plasmapheresis donors. There were no significant differences in bone density and prevalence of osteoporosis between first-time donors and repeat donors (P>0.05). There was also no significant difference in bone density levels between groups of total number of plasmapheresis donations. The levels of albumin and 25OHD decreased with the increase of the total number of plasma donations, while the serum P1NP level was positively correlated with the total number of plasma donations. The results of the restriction cubic spline showed that the total number of plasmapheresis donations had a nonlinear dose-response relationship with 25OH and P1NP (P<0.05). The results of multiple linear regression showed that the frequency of plasmapheresis donation was the influencing factor of 25OHD, and the total number of plasmapheresis donation was the influencing factor of P1NP. 【Conclusion】 Plasmapheresis donation does not affect the bone health of donors and increase the risk of osteoporosis due to the use of long-term anticoagulants, but it will increase the osteogenic activity of plasmapheresis donors. It is recommended that middle-aged and elderly plasmapheresis donors supplement vitamin D appropriately.
