1.A Preliminary Study on the Characteristics of Hot Flushes in Bilateral Ovariectomy Rats and the Regulating Effect of Erxian Decoction

OBJECTIVE To explore the characteristics of hot flushes and the effect of Erxian Decoction in rats after bilateral oo-phorectomy.METHODS Forty-eight female SD rats were randomly divided into 4 groups:sham operation group,bilateral ovariecto-my group,17β-estradiol treatment group and Erxian Decoction treatment group,with 12 rats in each group.After bilateral ovariecto-my,rats were gavaged with 17β-estradiol(0.1 mg·kg-1·d-1)and Erxian Decoction(8 g·kg-1·d-1)for 6 weeks.On the 21st and 42nd days of modeling,the body weight,rectal temperature,infrared thermography of the body surface and other characteristic informa-tion of the rats were detected.Serum E2 content was detected by ELISA.The pathological changes of the uterus and foot pad sweat glands of the rats were observed by HE staining.The hypothalamic estrogen receptor(ER)and thermoreceptor transient receptor poten-tial vanilloid 3(TRPV3)and transient receptor potential vanilloid 4(TRPV4)were detected by immunohistochemistry.The protein ex-pression of ERβ,PAC1R and PKA was detected by Western blot.RESULTS Compared with the sham operation group during the same period,the body weight of the rats in the bilateral ovariectomy group increased significantly(P<0.01),while the body weight of the rats in the 17β-estradiol group and Erxian Decoction group increased slowly.Compared with the sham operation group,the rectal temperature of the bilateral ovariectomy group and the 17β-estradiol group increased significantly on the 21st day after modeling(P<0.01),and decreased significantly on the 42nd day after modeling(P<0.05);on the 21st and 42nd days after modeling,the maxi-mum body surface temperature of the rats in the bilateral ovariectomy group,the 17β-estradiol group and the Erxian Decoction group decreased significantly(P<0.01),and on the 42nd day after modeling,the tail/body temperature ratio of the rats in the bilateral ovari-ectomy group increased significantly(P<0.05);the number of sweat points in the bilateral ovariectomy group increased significantly(P<0.01);the uterine index of the bilateral ovariectomy group,the 17β-estradiol group and the Erxian Decoction group decreased sig-nificantly(P<0.01);the serum E2 in the bilateral ovariectomy group decreased significantly(P<0.05);the number of ER-positive cells in the hypothalamus of the rats in the bilateral ovariectomy group decreased significantly(P<0.01),while the number of TRPV3 positive cells increased significantly(P<0.05);the number of ER and TRPV3 positive cells decreased in the 17β-estradiol group(P<0.05),and the number of TRPV4 positive cells increased(P<0.01);17β-estradiol inhibited the expression of PAC1R and PKA pro-teins(P<0.05),and Erxian Decoction significantly inhibited the expression of PAC1R,PKA and ERβ proteins(P<0.01).Compared with the bilateral ovariectomy group,the rectal temperature of the Erxian Decoction group decreased significantly on the 21st day(P<0.01);the number of sweat spots in the 17β-estradiol group and the Erxian Decoction group decreased significantly(P<0.01);the E2 level in the Erxian Decoction group increased significantly(P<0.05),the number of ER positive cells in the hypothalamus increased significantly(P<0.01),and the expression of ERβ and PAC1R proteins decreased significantly(P<0.01).CONCLUSION Bilater-al oophorectomy rat model is associated with hot flashes,and its symptom characteristics are yin deficiency and internal heat,as well as insufficient yang qi,resulting in a yin-yang imbalance;Erxian Decoction affects the sensitivity of temperature changes by promoting es-trogen secretion and its receptor expression.

3.Clinical characteristics, treatment strategy, and clinical outcomes in type 2 intestinal failure

Objective:To evaluate the characteristics, clinical management and clinical outcomes of type 2 intestinal failure (IF).Methods:A descriptive case-control study was carried out. The inclusion criteria were as follows: (1) the diagnosis of IF was performed according to the European Society for Parenteral and Enteral Nutrition (ESPEN) consensus statement. (2) using a requirement for parenteral nutrition (PN) of 28 days or more as surrogate marker. (3) a multidisciplinary team (MDT) included surgeons, nutritionist, pharmacist, stoma therapists, and critical care physicians. (4) complete laboratory data. Patients with type 1 and type 3 IF and those who do not cooperate with follow-up. All the data of 67 type II IF were collected from the database in Sir Run Run Shaw Hospital from Jan 2016 to Dec 2023. The pathophysiology, clinical management, and outcomes of type II IF were analyzed.Results:A total of 67 type II IF were included. The median age was 54 (15-83) with 43 males and 24 females. The body mass index was (17.5±3.8) kg/m 2, the incidence of malnutrition was 67.2% (45/67), the incidence of sarcopenia was 74.6% (50/67), the median number of previous surgeries was 2.0 (1-13), and the median duration time of PN was 2.1 (1-12) months. The underlying disease of type 2 IF included 36 Crohn`s disease, 2 ulcerative colitis, 3 radiation enteritis, 2 intestinal Behcet's disease, 4 mesenteric infarction, 1 aggressive fibromatosis, 5 abdominal cocoon syndrome, 5 gastrointestinal perforation, 1 hernia, 4 intestinal dysmotility, and 4 other reasons (gastrointestinal tumor, trauma, and non-Hodgkin's lymphoma). According to the pathophysiology of IF, there were 33 intestinal fistula, 12 intestinal dysmotility, 6 mechanical obstruction, 13 short bowel syndrome, and 3 extensive small bowel mucosal disease. After treatment with MDT, 67 patients with type 2 IF received nutritional support therapy for intestinal rehabilitation treatment, of which 36 patients recovered with oral diet or enteral nutrition, 31 patients underwent reconstructive surgery after intestinal rehabilitation treatment failure. The median duration time of reconstructive surgery was 2.7 (1-9) months. 24 patients recovered intestinal autonomy after surgery, with 7 deaths, including 6 deaths due to abdominal infections and 1 case of intestinal dysmotility with abiotrophy and liver failure. Conclusion:Standardized multidisciplinary treatment plays an important role in type II intestinal failure, and it promotes patients with intestinal failure regain enteral autonomy.

5.Vibro-acoustography based on large-angle dual-frequency focused transducer

To enhance the spatial resolution of vibro-acoustography,an improved vibro-acoustography system is constructed using a large-angle dual-frequency focused transducer.Based on this system,mimicking phantoms are used as the experimental objects to explore the spatial resolution of vibro-acoustography,and test the vibro-acoustography results of mimicking phantoms with different acoustic characteristics.Additionally,the effects of the position of hydrophone and difference frequency on the vibro-acoustography results are studied,and the effects of transducer component distribution on vibro-acoustography results are analyzed with a simulation model established with k-Wave.The research results indicate that the vibro-acoustography system with a large-angle dual-frequency focused transducer has higher spatial resolution,contrast and specificity in imaging mimicking phantoms with different acoustic characteristics.The choice of difference frequency is critical for the imaging quality,but the position of the hydrophone and transducer component distribution have trivial effects on vibro-acoustography results.The study is expected to provide new ideas for the development of vibro-acoustography technology,and to promote its further practical application.
6.Establishment and exploration of a subcutaneous transplanted tumor model of hepatocellular carcinoma in mice with Qi stagnation and blood stasis syndrome

Objective To explore the establishment of a subcutaneously transplanted tumor model of hepatocellular carcinoma in mice with Qi stagnation and blood stasis syndrome.Methods Forty male C57BL/6 mice were randomly divided into 4 groups:NC group,QZXY group,Tumor group,and QZXY+Tumor group.They were categorized based on the modeling of Qi stagnation and blood stasis syndrome(7 days)combined with the modeling of subcutaneous transplantation of hepatocellular carcinoma tumor(20 days).Observations were conducted of the syndrome manifestations as well as the tumor size and weight of the mice after modeling.Results(1)Body weight:on the 7th day of modeling,the weights of the QZXY group and QZXY+Tumor group were significantly lower than that of the NC group(P<0.05).(2)Body temperature:on the 7th day of modeling,body temperature significantly decreased in the QZXY group(P<0.05),while it increased in the Tumor group(P<0.05)compared with the NC group.On the 27th day of modeling,the temperature of the QZXY+Tumor group was significantly lower than that of the NC group(P<0.05).(3)Syndrome manifestations:according to the syndrome scoring table,mice in both the QZXY group and QZXY+Tumor group exhibited Qi stagnation and blood stasis syndrome on the 7th day of modeling(P<0.05).As modeling time extended,the score of mice in the Tumor group increased with the formation of the tumor,and the score of mice in the QZXY+Tumor group was significantly higher than that of the other three groups(P<0.05).(4)Claw petechiae:the number of claw petechiae significantly increased in all three groups of modeled mice compared with the NC group(P<0.05),with the QZXY+Tumor group showing the highest number.(5)Claw r value:the r value of the claw was significantly lower in all three groups of modeled mice than that in the NC group(P<0.05).Additionally,the r value of the claw in the QZXY+Tumor group was consistently lower than that of the other three groups.(6)Open field activity:the vertical and horizontal activity of mice in the QZXY+Tumor group decreased significantly compared with that of the NC group(P<0.05).(7)Coagulation indexes:APTT,TT,and FIB were significantly increased in the QZXY+Tumor group(P<0.05 or P<0.01)compared with those in the NC group.(8)Tumor size and weight:compared with the Tumor group,the QZXY+Tumor group showed significantly increased tumor size and weight(P<0.05).Conclusions This study successfully established a subcutaneous transplanted tumor model of hepatocellular carcinoma in mice with Qi stagnation and blood stasis syndrome.The findings indicated that Qi stagnation and blood statsis syndrome may occur during the course of live cancer.Besides,the causes inducing the Qi stagnation and blood stasis syndrome will further accelerate the progression of liver cancer.

8.Evaluation of hot-flash model rats induced by bilateral oophorectomy and tamoxifen administration

Objective To explore the animal model of syndrome used in the study of hot flash phenomenon in women.Methods Twenty-four female SD rats were divided randomly into three groups:Con group,Ovx group,and tamoxifen group(n = 8 rats per group).Hot flashes were induced by bilateral oophorectomy and intragastric tamoxifen 10 mg/(kg·d),respectively.Open-field activity,anal temperature,and body surface infrared thermograms were detected on model days 14 and 28.The rats were then killed on day 29 and their uteruses were removed,weighed,and sectioned.Blood estradiol and catecholamine levels were determined by enzyme-linked immunosorbent assay.Gene expression levels of adrenal sex hormone synthetases(Star,Cyp11a1,Cyp17a1,Cyp19a1,Por,Hsd3b2,Hsd17b1)and catecholamine synthetases(Th,Ddc,Dbh,Pnmt)in the adrenal medulla were detected by reverse transcription-polymerase chain reaction.Results Rat body weight was significantly higher in the Ovx group compared with Con group(P<0.01),while body weight increased slowly in the tamoxifen group.The maximum body surface temperature was significantly decreased on day 28 in the Ovx group(P<0.01),the difference between the maximum and minimum abdominal temperatures was significantly increased on day 14(P<0.05),the difference between the maximum and minimum temperatures on the back was significantly increased on day 28(P<0.01),and the open-field activity was decreased(P<0.01).Compared with the sham operation group,the maximum body surface temperature in the tamoxifen group was significantly decreased(P<0.01)but the open-field activity was increased(P<0.01).The uterine index was significantly decreased in both models(P<0.01).Estradiol levels were significantly decreased(P<0.01)and NE and epinephrine were also significantly decreased in the Ovx group compared with Con group(P<0.05),and β-EP was also significantly decreased in Ovx group(P<0.05).Adrenal Cyp11a1 gene expression was significantly increased(P<0.05)while Cyp17a1 and Hsd17b1 gene expression levels were significantly decreased(P<0.05)in bilateral ovariectomized rats compared with Con group.Compared with Con group,gene expression levels of Star and Por were significantly increased(P<0.01)while Cyp17a1 gene expression was significantly decreased(P<0.01)in the tamoxifen group,and Pnmt gene expression was significantly down-regulated in Ovx group(P<0.01).Conclusions Bilateral ovariectomized rats can be used for the study of perimenopausal hot flashes,which resemble kidney Yang and Yin deficiency in traditional Chinese medicine.
9.Quality of life and bowel function after laparoscopic proctocolectomy and ileal pouch anal anastomosis in patients with ulcerative colitis

Objectives:To assess bowel function and quality of life in patients with ulcerative colitis (UC)after ileal pouch anal anastomosis (IPAA).Methods:Clinical data of 37 UC patients after IPAA between 2014 and 2017 were retrospectively analyzed at Sir Run Run Shaw Hospital School of Medicine Zhejiang University. The IBDQ and Bowel Function questionnaire were used for analyzing correlation between clinical variables and quality of life or bowel function.Results:Laparoscopic operation was performed in 12 cases at stage 2 and 25 cases at stage 3. Postoperative defecation of stage 3 patients were better than that of stage 2 ( t=6.72, P<0.05). The number of daily defecation in age >45-year-old group was more than that in <45-year-old ( t=3.49, P<0.05), and the rate of evening stool seepage in the older group was higher than in the younger group( t=5.28, P<0.05). The total score of intestinal symptoms of IBDQ in patients of pouchitis was lower than that without pouchitis ( r=0.330, P<0.05). The total score in age >45 in terms of systemic symptoms ( r=0.349, P<0.05) and emotional function ( r=0.379, P<0.05) was higher than age <45. Conclusions:Outcomes of UC patients after IPAA are satisfactory, bowel function and quality of life is related with age, and stage of IPAA affect postoperative defecation.

Objective To evaluate the effects of turmeric volatile oil (TVO) combined with cisplatin on the proliferation and apoptosis of a human cutaneous squamous cell carcinoma cell line A431,and to explore their mechanisms.Methods Some cultured A431 cells at exponential growth phase were divided into several groups to be treated with 5,10,20,40 and 80 mg/L TVO,as well as high-glucose Dulbecco's modified Eagle's medium (DMEM) containing 1% dimethyl sulfoxide (DMSO,control group),respectively.After 24-hour treatment,cell counting kit 8 (CCK8) assay was performed to estimate the proliferative activity of A431 cells in the above groups.Some other A431 cells were divided into 4 groups:control group treated with high-glucose DMEM containing 1% DMSO,TVO group treated with 40 mg/LTVO,cisplatin group treated with 10 mg/L cisplatin,and TVO + cisplatin group treated with 40 mg/L TVO and 10 mg/L cisplatin.After 24-hour treatment,CCK8 assay was performed to estimate the cellular proliferative activity,inverted microscopy to observe changes in cell morphology,fluorescence microscopy to detect cell apoptosis after acridine orange (AO)/ethidium bromide (EB) double-staining,colorimetry to evaluate the activity of Caspase-3 and Caspase-9,and Western blot analysis to determine the protein expression of Caspase-3 and p-glycoprotein.Results After 24-hour treatment with 5,10,20,40 and 80 mg/L TVO,the cell proliferation rates were inhibited by (12.83 ± 6.4)%,(16.27 ± 11.4)%,(21.61 ± 9.1)%,(33.11 ± 2.0)% and (46.00 ± 3.3)% respectively,and the inhibition rates were all significantly higher in these groups than in the control group (4.03% ± 1.4%,all P < 0.05).The 50% inhibitory concentration (IC50) of TVO at 24 hours was (61.66 ± 1.03) mg/L.Compared with the control group,the proliferation inhibition rates significantly increased in the TVO group,cisplatin group and TVO + cisplatin group (all P < 0.05),suggesting that the combination of TVO and cisplatin showed synergistic inhibitory effects with a combination index of 1.366.Moreover,A431 cells turned round to different extents and became apoptotic in the TVO group and cisplatin group,and the TVO + cisplatin group showed obviously decreased number of cells and a large number of cell debris.The TVO + cisplatin group also showed significantly increased activity of Caspase-3 (1.520 ± 0.115) and Caspase-9 (2.760 ± 0.297) as well as protein expression of Caspase-3 (1.482 ± 0.016) compared with the TVO group (Caspase-3 activity:1.117 ± 0.095;Caspase-9 activity:1.259 ± 0.059;Caspase-3 protein expression:1.156 ± 0.006,all P < 0.01) and cisplatin group (Caspase-3 activity:1.381 ± 0.089;Caspase-9 activity:1.829 ± 0.171;Caspase-3 protein expression:1.296 ± 0.021,all P < 0.01),but significantly decreased p-glycoprotein expression (0.528 ± 0.014) compared with the TVO group (1.311 ± 0.011,P < 0.01) and cisplatin group (1.169 ± 0.012,P < 0.01).Conclusion TVO combined with cisplatin can synergistically inhibit the proliferation of A431 cells and induce cell apoptosis,which may be associated with activation of the caspase system and decreased expression of pglycoprotein.
