1.Association between key air pollutant combinations and respiratory disease hospitalizations in Hefei from 2019 to 2024

Background Air pollution is a major environmental factor threatening respiratory health. Different pollutants exhibit varying degrees of lag effects on respiratory diseases, and synergistic effects may exist among multiple pollutants. There is an urgent need to identify the key air pollutants influencing respiratory diseases and their interactive effects at specific lags. Objective To identify key pollutants affecting hospital admissions for respiratory diseases, to analyze their lag effect characteristics, and to quantify the impact of multi-pollutant synergistic effects on respiratory disease admissions. Methods Daily air pollution data, meteorological data, and respiratory disease hospitalization records were collected from multiple national monitoring stations in Hefei City from 2019 to 2024. A two-stage analytical framework was employed. First, a distributed lag model (DLM) was used to construct pollutant lag matrices, followed by least absolute shrinkage and selection operator (LASSO) regression to select key variables among fine particulate matter (PM2.5), sulfur dioxide (SO2), nitrogen dioxide (NO2), carbon monoxide (CO), and ozone (O3). Second, a generalized additive model (GAM) was established, incorporating product interaction terms and excess relative risk (ERI) to quantitatively assess synergistic effects among the selected pollutants. Results Through LASSO regression, 24 pollutant lag terms with non-zero coefficients were identified, among which NO2, PM2.5, and SO2 accounted for 66.7% of the total positive effects and exhibited distinct lag patterns. Exposure to NO2 showed acute risk, with a relative risk of 1.040 (95%CI: 1.023, 1.057) at lag0. Objective To evaluate the reliability of Vitek 2 AST-N335 card for determining the susceptibility of Acinetobacter bauman-nii(AB)to cefoperazone/sulbactam.Methods A total of 318 non-repeated clinical isolates of AB collected in 2023 were tested for antimicrobial susceptibility to cefoperazone/sulbactam using broth microdilution(BMD),the AST-N335 card,and the Kirby-Bauer(K-B)disk diffusion method.Using BMD as the reference method,the reliability of AST-N335 card was assessed,and the accuracy of K-B disk diffusion method as the confirmatory test was validated.Results Compared with BMD,the susceptibility testing of 318 AB strains to cefoperazone/sulbactam using the AST-N335 card showed categorical agreement(CA)of 87.8%(279/318),very major er-ror(VME)of 6.0%(19/318),major error(ME)of 0%(0/318),and minor error(mE)of 1.9%(6/318),which fall outside of the acceptable error range.In contrast,the K-B method achieved CA of 99.4%(316/318),VME of 0%,ME of 0.3%(1/318),and mE of 0.3%(1/318),all within acceptable limits.Of these,the errors with AST-N335 card occurred within the minimum inhibitory concentration(MIC)range of 8-32 μg/mL.Using BMD as the reference method,further analysis was performed on the 171 AB strains with AST-N335 card MIC values of 8-32 μg/mL for cefoperazone/sulbactam.It was revealed that at MIC of 32 μg/mL,the CA was 0%;at MIC of 16 μg/mL,CA was 5.3%(1/19)and VME rate was 84.2%(16/19),both of which substantially exceeded accepta-ble error ranges.At MIC of 8 μg/mL,the CA was 94.9%(131/138)and VME was 2.2%(3/138),both approaching the acceptable ranges.Conclusion The results obtained with Vitek 2 AST-N335 card in determining for cefoperazone/sulbactam are unreliable when the MIC values fall within the range of 8-32 μg/mL,which leads to an underestimation of the resistance rate to cefoperazone/sulbac-tam.This issue requires urgent attention in both laboratories and clinical practice.The K-B disk diffusion method could serve as a sup-plementary verification approach in routine laboratories.

4.A multicenter retrospective histopathological analysis of testicular torsion-related orchiectomy specimens

Objective This study aims to explore the correlation between the grade of testicular tissue damage and clinical features which is used to evaluate the viability of testicular tissue after the orchidopexy for testicular torsion.Methods We conducted a retrospective,multicenter analysis of pathological specimens obtained from patients who under-went unilateral orchiectomy due to testicular torsion.Tissue viability was reassessed by the Mikuz grading scale.The corre-lations among tissue viability,duration of torsion symptoms,the angle of torsion,preoperative Doppler ultrasound image and intraoperative testicular resuscitation methods were analyzed.Results Eighty-five pathological specimens were en-rolled from 4 medical centers.The age range was 18 years(IQR 14-33),with a median torsion angle of 480°(IQR 180°-1080°).The interval from the torsion onset to surgical intervention was 18 hours(IQR 9-84).In the subgroup analysis of different torsion durations,the distribution of Grade I injuries was as follows.The subgroup within 12 hours ac-counted for 37.5%.The subgroup between 12 to 24 hours accounted for 14.28%.And the subgroup between 24 to 48 hours ac-counted for 6.45%.The subgroup exceeding 48 hours only accounted for 3.13%.Four patients received orchiectomy due to pre-operative ultrasound image indicating necrosis,correlating with Mikuz grade 3 tissue damage.The decision was made based on the failure of color recovery of affected testis in 57 cases,revealing 12.28%with grade 1 and 35.89%with grade Ⅱ damage.Twenty-four cases were identified as an Arda score of 3,with 4.16%showing grade 1 and 20.83%showing grade Ⅱ Mikuz damage.The method of Arda's grading could be used to determine the vitality of testicular tissue more accurately compared to the post-resusci-tation color observation method(P=0.032).Conclusion Pathological reassessment provides a precise determination of tis-sue viability in the affected testis.In some instances,the testicular tissue damage can be found potentially reversible.
5.Diagnostic Value of Ultrasound for Residual Pregnancy Products Combined with Uterine Arteriovenous Fistula after Early Pregnancy Miscarriage and Pre-dictive Value for Major Bleeding

Objective:To explore the diagnostic value of ultrasound for retained products of conceptions com-plicated with uterine arteriovenous fistula(UAVF)after early pregnancy miscarriage and the predictive value for massive bleeding by constructing a risk model for bleeding.Methods:A total of 90 patients who were diagnosed by ultrasound in West China Second Hospital of Sichuan University as having retained products of conceptions complicated with UAVF after early pregnancy miscarriage from July 2013 to September 2021 were selected,among which 37 cases underwent digital subtraction angiography(DSA).Clinical and ultrasound data were collected and statistical analysis was conducted on the ultrasound parameters of whether DSA examination was accompanied by UAVF and whether there was major bleeding(spontaneous and intraoperative).We performed multivariate Lo-gistic regression analysis on parameters with statistical differences,plotted receiver operating characteristic(ROC)curve for predicting major bleeding(spontaneous and intraoperative),and calculated the area under the curve(AUC).Results:① After DSA examination,14 out of 37 patients(37.8%)were diagnosed with UAVF based on ultrasound diagnosis of pregnancy residue combined with UAVF.Univariate and multivariate Logistic an-alyses showed that the expanding inner diameter of blood vessels was an independent risk factor for residual pregnancy products combined with UAVF after early pregnancy miscarriage(OR 37.099,P=0.027).ROC curve analysis revealed that dilated vessel diameter had an AUC of 0.78,sensitivity of 64%,specificity of 91%,and an optimal cut-off value of 0.85 cm for the diagnosis of UAVF.②Out of 90 patients,44(48.9%)experienced sponta-neous massive bleeding.Univariate and multivariate Logistic analyses showed that the presence of vascular pools or dilated vessels was an independent risk factor for spontaneous massive bleeding(OR 5.163,P=0.004).③A-mong70 patients undergoing surgical treatment,7 experienced intraoperative bleeding(11.7%).Univariate and multivariate Logistic analyses showed that the enlargement of the maximum diameter of the lesion was an inde-pendent risk factor for intraoperative massive bleeding(OR 7.278,P=0.007).ROC curve analysis revealed that the AUC of the maximum diameter of the lesion for predicting the risk of intraoperative bleeding was 0.92,with a sensitivity of 100%,specificity of 82%,and an optimal threshold of 3.95 cm.Conclusions:Dilated vessel diameter in ultrasound examination parameters has high diagnostic value for residual pregnancy products combined with UAVF after early pregnancy miscarriage.Patients with vascular pools or dilated vessels and large lesions have a higher risk of spontaneous and intraoperative bleeding,and clinical prediction models can be used to assess risk of bleeding.
6.Construction and application effect of"internet+"Tibetan-language medication service platform

OBJECTIVE To build a Tibetan-language medication service platform based on"internet+"and evaluate its effect on improving medication compliance and safety of Tibetan patients with chronic disease.METHODS Medication guidance contents of commonly used drugs in the outpatient department were summarized,translated and recorded in Tibetan-language or video to form a"text-audio-video"multi-dimensional"internet+"Tibetan-language medication service platform.A total of 387 Tibetan outpatients with chronic disease in our hospital after the implementation of"internet+"Tibetan-language medication service platform(from January 2024 to June 2024)in our hospital were selected as the intervention group,and 387 Tibetan outpatients before the implementation(from January 2023 to June 2023)were selected as the control group.Patients in the control group received conventional window-based Chinese-language medication services,while patients in the intervention group received both conventional window-based Chinese-language medication service and"internet+"Tibetan-language medication service.The medication compliance of patients was evaluated using the 12-item Medication Compliance Scale.A six-level causality assessment was conducted as the principles for analyzing adverse drug reactions(ADR)set by the National Center for ADR Monitoring.Additionally,statistics were compiled on the occurrence of ADR that were assessed as"definite""probable"or"possible"in the causality assessment.RESULTS The proportion(31.0%)of patients with good medication compliance and compliance scores[39.0(37.0,42.0)]of patients in the intervention group were significantly better than control group[7.0%,21.0(19.0,23.0)](P<0.05).There were no statistically significant differences in the incidence of various types of ADR or the overall incidence between the two groups(P>0.05).CONCLUSIONS The"internet+"Tibetan-language medication service platform is constructed successfully;the service can effectively improve the medication compliance of Tibetan-language patients,but its effect on improving the medication safety of patients is limited.

9.Disease costs in inpatients with schizophrenia,major depressive disorder,and bipolar disorder

Objective:To evaluate direct and indirect costs for schizophrenia,major depressive disorder(MDD)and bipolar disorder,and to compare their differences of cost composition,and to explore the drivers of the total costs.Methods:A total of 3 175 inpatients with schizophrenia,MDD,and bipolar disorder were recruited.In-patient's self-report total direct of medical costs outpatient and inpatient,out-of-pocket costs,and direct non-medical costs were regarded as direct costs.Productivity loss and other loss caused by damaging properties were defined as indirect costs.The perspectives of this study included individual and societal levels.Multivariate regression analysis was applied for detecting the factors influencing disease costs.Results:The total cost of schizophrenia was higher than those of MDD and bipolar disorder at individual and societal levels.The indirect costs of three mental disorders were higher than the direct costs,and the indirect cost ratio of bipolar disorder was higher than those of schizophre-nia and MDD.Age,gender,working condition and marital status(P<0.05)were the important drivers of total costs.Conclusion:The economic burden of the three mental disorders is relatively heavy.Schizophrenia has heaviest disease burden,and the productivity loss due to mental disorders is the driving force of the soaring disease cost
10.Comparison of clinical characteristics between first-episode and relapse of major depressive disorder

Objective:To describe demographic,clinical and physiological characteristics,treatment between first-episode major depressive disorder(MDD)and relapse MDD,and to explore characteristics of relapse MDD.Methods:Totally 858 patients who met the diagnostic criteria for depression of the Diagnostic and Statistical Manual of Mental Disorders,Fifth Edition(DSM-5),were included by using the Mini International Neuropsychiatric Interview(MINI),Clinician-Rated Dimensions of Psychosis Symptom Severity,and Hamilton Depression Scale etc.Among them,529(58.6%)were first-episode depression and 329(36.0%)were relapsed.The differences of demographic characteristics,clinical and physiological characteristics,treatment were compared byx2test and Kruskal-Wallis rank sum test.Multivariate logistic regression was used to explore the characteristics of MDD recur-rence.Results:Compared to first-episode MDD,relapse MDD had more comorbidity(OR=2.11,95%CI:1.00-4.44),more days out of role(OR=1.26,95%CI:1.01-1.56),more history of using psychiatric drug more than one month(OR=1.41,95%CI:1.02-1.97)and electroconvulsive therapy(OR=3.23,95%CI:1.42-7.36),and higher waist-hip ratio(OR=33.88,95%CI:2.88-399.32).Conclusion:Relapse MDD has positive as-sociation with comorbidity of mental disorders,out of role,and higher waist-hip ratio.
