ObjectiveTaking Aurantii Fructus Immaturus juice(AFI)-processed Atractylodis Macrocephalae Rhizoma(AMR) as an example, this study aims to systematically compare the volatile and non-volatile components of AMR and its processed products, investigate the key differential components, evaluate their anti-inflammatory activities, and elucidate the synergistic mechanism of processing. MethodsThe chemical compositions of volatile and non-volatile components in AMR and AFI-processed AMR were systematically characterized using gas chromatography-mass spectrometry(GC-MS) and ultra-performance liquid chromatography-quadrupole-time-of-flight mass spectrometry(UPLC-Q-TOF-MS), with relative mass fractions and response values determined separately. Volatile components were identified through searches in the National Institute of Standards and Technology(NIST)17 database, comparison with retention index(RI) and fragmentation pattern matching. Non-volatile components were identified by searching Waters Traditional Chinese Medicine (TCM) spectral library, in conjunction with PubChem and MassBank, characteristic fragmentation patterns and response values were also used to support identification. Differential components were screened using principal component analysis(PCA), orthogonal partial least squares-discriminant analysis(OPLS-DA), with variable importance in the projection(VIP) value >1. Components with high log2fold change(FC) among major differential groups were selected as those exhibiting significant changes before and after processing. The anti-inflammatory activity of the differential compounds was evaluated by assessing their effects on nitric oxide(NO) production in a lipopolysaccharide(LPS)-induced RAW264.7 macrophage model. Enzyme-linked immunosorbent assay(ELISA) was used to detect the effects of the differential components on tumor necrosis factor(TNF)-α, interleukin(IL)-1β, IL-6, and monocyte chemotactic protein(MCP)-1 levels, and immunofluorescence(IF) was employed to assess their effects on nuclear transcription factor(NF)-κB p65 translocation, thereby elucidating the underlying molecular mechanisms. ResultsA total of 36 compounds were identified in the volatile components of AMR and AFI-processed AMR, among which, sesquiterpenes and monoterpenes were significantly increased after processing. In the non-volatile components, 36 compounds were identified, and the main differential components were flavonoids, sesquiterpenoids, and triterpenoids. Flavonoids were the primary differential components distinguishing AMR from its processed products, representing compounds directly introduced during processing. Five compounds, including atractylenolide Ⅲ, tangeritin, nobiletin, hesperidin and narirutin, were selected as representatives of three classes based on their most prominent differential expression among different compound types for subsequent anti-inflammatory activity studies. The results showed that 100 μmol·L-1 tangerine and narirutin could significantly inhibit LPS-induced NO production(P<0.01) in a concentration-dependent manner. Tangeritin was able to significantly inhibit the levels of TNF-α and MCP-1 secreted by RAW264.7(P<0.05), while narirutin significantly inhibited the levels of TNF-α, IL-1β, MCP-1 and IL-6(P<0.01). IF revealed that both tangeritin and narirutin significantly blocked the translocation of NF-κB p65 from the cytoplasm to the nucleus. ConclusionAFI-processed AMR significantly alters the chemical composition profile of AMR, and the newly introduced flavonoid components during processing may be key to its enhanced anti-inflammatory effects.

Objective To investigate the dynamic balance ability of healthy young adults under different obstacle-crossing strategies, thereby providing a theoretical basis for fall prevention training and public facility design. Methods Twenty healthy young adults participated in the experiment using F-scan plantar pressure analysis insoles. The subjects were required to cross three obstacles with different combinations of height and width. With their dominant foot serving as the leading foot and the non-dominant foot as the trailing foot, the subjects performed both lateral and forward crossing maneuvers, and their plantar pressure data were collected. Results Different crossing strategies significantly affected the adjustment speed of the leading foot's center of pressure in the medial-lateral direction(COP_ML),the area of the 95%confidence circle,ML amplitude, and anterior-posterior(AP)amplitude(P<0.05).These strategies also significantly impacted the trailing foot's COP_ML adjustment speed, the area of the 95%confidence circle, and the range between the maximum and minimum swings(P<0.05).For the leading foot, during lateral and forward crossing, the balance parameter values under different heights and widths were statistically significant(P<0.05),increasing as the height and width increased. For the trailing foot, during forward crossing, the balance parameter values under different heights were statistically significant(P<0.05),increasing with height, while during lateral crossing, the differences in balance parameter values were not statistically significant(P>0.05).Conclusions Healthy young adults demonstrate better balance ability with the leading foot during forward obstacle crossing, which aligns with the movement habits of the dominant foot and daily activity patterns. The trailing foot exhibits a more stable plantar pressure distribution during lateral obstacle crossing, likely due to a larger contact area and more even center of gravity distribution.

BACKGROUND:Oriented polycaprolactone/silk fibroin blend electrospinning scaffolds can repair peripheral nerve damage and promote nerve regeneration, but the repair effect is limited, and it is necessary to further combine bioactive materials, growth factors, and other factors to prepare bionic neural scaffolds.Calcium titanate nanoparticles have the ability to promote adhesion between Schwann cells and have been widely used in the field of biomedical materials.OBJECTIVE:To prepare polycaprolactone/silk fibroin-calcium titanate functionalized scaffolds by electrospinning technology, and to further explore the effect of the scaffold on Schwann cell viability and adhesion.METHODS:Polycaprolactone/silk fibroin scaffolds were prepared by electrospinning technology. The optimal parameters were selected by adjusting the syringe flow rate, receiving device speed and spinning time during the preparation process for subsequent experiments.Under the optimal parameters, randomly oriented polycaprolactone/silk fibroin scaffolds(denoted as PCL/SF-R),oriented polycaprolactone/silk fibroin scaffolds(denoted as CTO-0),and oriented polycaprolactone/silk fibroin scaffolds containing 0.1,0.5,and 1.0 mg/mL polydopamine modified calcium titanate(denoted as CTO-1,CTO-5,and CTO-10,respectively)were prepared by electrospinning technology. The physical properties of the five groups of scaffolds were characterized. The extracts of the five groups of scaffolds were co-cultured with rat fibroblast cell line(L929).Cell proliferation was detected by MTT assay.Rat Schwann cell line(RSC96)was inoculated on the five groups of scaffolds.Cell viability, cytoskeleton staining, and cytoskeleton and cell adhesion related genes were detected.RESULTS AND CONCLUSION:(1)The optimal parameters for preparing electrospinning scaffolds were as follows:syringe flow rate 15 μL/min,receiving device speed 1 800 r/min,and spinning time 1 hour.(2)The PCL/SF-R group had the largest water contact angle and the smallest elastic modulus, while the CTO-0 group had the smallest water contact angle and the largest elastic modulus. The degradation rate of the scaffolds in the CTO-0 group was lower than that in the CTO-1 group, and the degradation rate of the scaffolds decreased with the increase of the mass concentration of calcium titanate in the oriented scaffolds.(3)The scaffolds in the five groups had no toxic effects on L929 cells and had good biocompatibility.CCK-8 assay results showed that CTO-10 could improve the viability of RSC96 cells.Cytoskeleton staining results showed that the aspect ratio and cell protrusion length of cells on the oriented scaffolds were higher than those on the randomly oriented scaffolds.qPCR test results showed that the mRNA expressions of YAP and Cntn2 in the CTO-5 and CTO-0 groups were higher than those in the PCL/SF-R group(P<0.05).(4)The results indicate that the oriented polycaprolactone/silk fibroin-calcium titanate scaffold has good biocompatibility, mechanical properties, and hydrophilicity, which can enhance the viability and migration ability of Schwann cells and promote peripheral nerve regeneration.

Objective:To analyze the research trends and hotspots of bupivacaine liposomes from 1994 to 2023 using bibliometrics. Methods:A comprehensive search was conducted for the literature related to bupivacaine liposomes in the Web of Science Core Collection from 1994 to 2023. The CiteSpace software was used to conduct an in-depth analysis of the included literature data, including publication year, country, institution, author, journal, cited references, keywords, etc. Results:A total of 875 papers related to bupivacaine liposomes were included. The research and development of bupivacaine liposomes were divided into 3 stages: slow development (1994-2011), a sharp rise (2011-2021), and stabilization (2021-2023). The United States was in a leading position in terms of the number of publications, centrality, and author cooperation, and Harvard University had the largest number of publications. de Paula E had the most publications, Bramlett K had the highest number of citations, and Boogaerts J had the highest centrality of publications. Journals such as Anesthesia and Analgesia made significant contributions to this field. The most cited references focused on the infiltration of wounds and the periprosthetic injection of bupivacaine liposomes. The keyword analysis showed that local anesthetics, postoperative pain, etc. were commonly used keywords, and enhanced recovery after surgery was an emerging hotspot. Conclusions:Bupivacaine liposomes show good application prospects in the field of peripheral nerve block due to their unique pharmacological properties and safety characteristics and are expected to prolong the duration of postoperative analgesia. However, there is a difference between the actual effect and the expectation, and more clinical trials are needed to evaluate the curative effect, providing a more solid and reliable theoretical basis and practical guidance for clinical practice.

Objective:To explore the effect of neck muscle group exercise on setup errors in postoperative radiotherapy for head and neck cancers. Methods:A total of 126 head and neck cancer patients scheduled for radiotherapy at the First Affiliated Hospital, Air Force Medical University from February 2021 to October 2022 were prospectively enrolled. Among these patients, four patients discontinued treatment due to personal reasons, and the remaining 126 patients were randomly divided into an experimental group and a control group, with 61 patients in each group. The experimental group received neck muscle group exercise, while the control group received routine treatment without intervention. Cone-beam CT (CBCT) scans were performed weekly to measure setup errors at the levels of the clivus and the 4 th and 7 th cervical vertebrae (C4 and C7, respectively). Three-dimensional displacement, systematic errors, and random errors were calculated for each level. The appropriate margins of planning target volumes (PTVs) were determined using the Van Herk formula. Results:Baseline characteristics were well-balanced between the two groups, with no statistically significant differences ( P > 0.05). Compared to the control group, the experimental group showed significantly smaller setup errors in the left-right ( x) and anterior-posterior ( z) directions at the clivus level, as well as in the z direction at the C4 and C7 vertebral levels ( t = 2.30, 5.29, 4.07, 2.40, P < 0.05). The required PTV margin in the z direction increased to 4.0 mm at C7 from 2.4 mm and 2.8 mm at the clivus in the experimental and control groups, respectively. Correlation analysis revealed a strong negative correlation between the x-direction at the clivus and C4 vertebral levels and the couch angle (RTN) among all patients ( r = -0.548, -0.452, P < 0.001). A moderate negative correlation was observed between the inferior-superior ( y) direction and the z-direction at the C4 and C7 vertebral levels ( r = -0.160, -0.222, P < 0.001). Conclusions:Neck muscle group exercise can reduce setup errors and PTV margins in the anterior-posterior direction in postoperative radiotherapy for head and neck cancer patients.

Objective:To investigate the relationship of serum interleukin(IL)-17, IL-21 and IL-22 levels with the severity and prognosis of acute ischemic stroke(AIS).Methods:One hundred patients with AIS and treated in Handan Central Hospital from April 2021 to April 2023 (case group) and 60 patients with high-risk stroke who participated in physical examination during the same period (control group) were retrospectively selected as the study objects. The severity of AIS patients was assessed by the National Institutes of Health Stroke Scale (NIHSS), and 18-month follow-up was completed. They were divided into good prognosis group and poor prognosis group according to recurrence or death. The levels of serum IL-17, IL-21 and IL-22 in all groups were compared, and the correlation between serum IL-17, IL-21 and IL-22 levels and the severity and prognosis of AIS was analyzed by Spearman test. The risk factors of poor prognosis in AIS patients were screened by Logistic regression. Results:The levels of serum IL-17, IL-21 and IL-22 in the case group were higher than those in the control group: (28.77 ± 4.20) ng/L vs. (22.39 ± 3.34) ng/L, (26.53 ± 4.19) ng/L vs. (21.07 ± 3.26) ng/L, (7.21 ± 1.12) ng/L vs. (6.18 ± 0.84) ng/L, there were statistical differences ( P<0.05). There were statistical differences in levels of serum IL-17, IL-21 and IL-22 in AIS patients with different severity ( P<0.05). The levels of serum IL-17, IL-21 and IL-22 in the good prognosis group were lower than those in the poor prognosis group: (27.76 ± 4.06) ng/L vs. (31.37 ± 3.39) ng/L, (25.36 ± 3.34) ng/L vs. (29.52 ± 4.71) ng/L, (6.78 ± 0.77) ng/L vs.(8.35 ± 1.12) ng/L, there were statistical differences ( P<0.05). Spearman test results showed that the levels serum IL-17, IL-21 and IL-22 in AIS patients had positively correlated with the severity of the disease ( r = 0.453, 0.526, 0.470, P<0.05), and had positively correlated with poor prognosis ( r = 0.408, 0.397, 0.574). P<0.05). Logistic regression analysis showed that high levels of IL-17 ( OR = 1.562), IL-21 ( OR = 1.434) and IL-2 ( OR = 15.192) were independent risk factors for poor prognosis in AIS patients ( P<0.05). Conclusions:The levels of serum IL-17, IL-21 and IL-22 are closely related to the severity and prognosis of AIS, and are independent risk factors for poor prognosis of patients, which can be used to evaluate the prognosis.

Objective:To investigate the alterations in the topological properties of gray matter and white matter dynamic and static functional brain networks in patients with major depressive disorder (MDD) and bipolar depression (BDD) using graph theory analysis, and to evaluate the potential of their combination as biomarkers for differential diagnosis between unipolar and bipolar depression. Methods:From March 2021 to April 2024, inpatients were recruited from the Department of Psychosomatic Medicine, Zhongda Hospital, Southeast University, including 132 patients with MDD, 84 patients with BDD, and 91 healthy controls (HCs). Resting-state structural and functional MRI data were collected, and dynamic and static functional brain networks of gray matter and white matter were constructed. Graph theory analysis was applied to calculate global and nodal network properties, differences in topological attributes among the three groups were compared by One-way analysis of covariance, and Turkey′s post hoc test was used for further pairwise comparison. The network topology attribute indicators with statistically significant inter-group differences were selected using the Least Absolute Shrinkage and Selection Operator regression (LASSO) for feature classification. The diagnostic performance of combined gray and white matter network features for distinguishing MDD from BDD was assessed using receiver operating characteristic (ROC) curves and a random forest model. Results:In the analysis of the static gray matter functional network, both MDD and BDD patients showed abnormal local topological properties. Compared with HCs, the MDD group exhibited abnormal betweenness centrality (BC) in the left inferior frontal gyrus, left precuneus, left ventromedial occipital cortex, right ventromedial occipital cortex, and right anterior thalamus ( t=-3.95-3.62, all P<0.05). The degree centrality (DC) of the left and right anterior thalamus was also abnormal in the MDD group ( t=3.78,4.14, both P<0.001), as was the nodal efficiency (Ne) of the left precuneus and bilateral anterior thalamus ( t=2.37, 3.61, 3.82, all P<0.05). Compared with HCs, the BDD group showed abnormalities in DC and Ne of the left precuneus ( t=-2.76, P=0.014; t=-3.01, P=0.007). In the analysis of the dynamic white matter functional network, both MDD and BDD patients demonstrated abnormal temporal variability of local topological properties. Compared with HCs, the MDD and BDD groups showed reduced BC temporal variability in the left superior corona radiata ( t=-2.39, P=0.047; t=-4.28, P<0.001), and there were significant differences in DC temporal variability in the right posterior limb of the internal capsule and lentiform nucleus ( t=2.65, P=0.021; t=3.49, P=0.001) in MDD group compared with HCs and BBD. The differential diagnosis model combining gray and white matter dynamic and static network topological features achieved an area under the ROC curve of 0.80. Conclusion:Both MDD and BDD exhibit altered topological properties in static gray matter functional networks and dynamic white matter functional networks. The combination of these features may aid in the differential diagnosis of MDD and BDD.
