Objective To analyze the incidence of prehypertension (PHTN) and hypertension of male workers in a coal mine and investigate the influencing factors. Methods A total of 10 155 male workers in a coal mine in Shanxi Province were selected as the study subjects using the judgment sampling method. Occupational medical examination was carried out to examine the prevalence of abnormal blood pressure (including PHTN and hypertension) and its influencing factors. Results The prevalence of abnormal blood pressure was 69.9%, with a PHTN prevalence of 44.2% and a hypertension prevalence of 25.7%. The results of multinomial logistic regression analysis showed that the risk of PHTN and hypertension increased with age, drinking frequency and body mass index (all P<0.05). Workers with a pulse rate≥100 times/min had a higher risk of PHTN and hypertension than those with a pulse rate of 60-100 times/min (all P<0.05). The risk of PHTN and hypertension in workers exposed to noise, silica dust or other dust was higher than those without such occupational hazard exposure (all P<0.05). The risk of hypertension in workers with abnormal blood glucose was higher than that in workers with normal blood glucose (P<0.05). Exposure to organic solvents was associated with a higher risk of hypertension (P<0.05). Conclusion The prevalence of PHTN and hypertension among male coal miners in this mine is relatively high. The individual factors such as age, alcohol consumption, pulse rate, and body mass index, along with occupational exposure factors such as noise, silica dust, and other dusts, are influencing factors for PHTN and hypertension.
3.The case and thinking for signal interference in resolver of carousel on Varian VitalBeam medical linear accelerator

In this article, a case for random signal interference in resolver (or encoder) of carousel on Varian VitalBeam medical linear accelerator was illustrated. This case was considered as a rare failure of the technology platform (including TrueBeam) by Chinese, European and American technical experts in Varian. In this article, the differences of the carousel in the structure and control system between VitalBeam and traditional accelerator were comparatively analyzed to enable the peers to have a deeper understanding of resolver. The logic and methods to solve the fault were summarized, providing reference experience for analyzing the problems related to carousel. It is found that the interlocking system is imperfect, which provides a factual and technological basis for Varian to optimize the system.
4.Development of a reference substance for live bacterial count of Streptococcicosis live vaccines.

This study attempts to develop a reference substance for the live bacteria count of Streptococcicosis live vaccines in order to evaluate the validity of live bacterial count in inspection and testing. We prepared a batch of live Streptococcus suis reference substance for live bacterial count, tested their physical property, purity, vacuum degree, remaining moisture, and determined their homogeneity, thermal stability and transportation stability. Moreover, we organized collaborative calibration to assign count values to the reference substance and determine the shelf life of the reference substance in 12 months. The results showed that the physical property, the purity, the remaining moisture and the vacuum degree of the reference substance were all in compliance with the requirements of the Chinese Veterinary Pharmacopoeia. The homogeneity test showed that the coefficient of variation of the count of the reference substance was less than 10%, indicating a good homogeneity. Transportation stability test showed that the reference substance remained active after 72 h transportation in summer and winter with the package of styrofoam boxes and ice packs. Thermal stability test showed that the reference substance could be stored for up to 3 months at -20 °C, or up to 21 days at 4 °C. According to the collaborative calibration, the reference vaccine was assigned a count value range of (8.5-12.1)×107 CFU/ampoule. The shelf life test showed that the reference substance was stable for 12 months when stored at -70 °C. The reference substance could provide a reference for the live bacterial count of Streptococcicosis live vaccines. Moreover, it could also be used as a reference to evaluate the quality of corresponding agar media.

5.Principle and troubleshooting for the irregular case of UDRS interlock for Varian CLINAC-IX linear accelerator

The UDRS interlocking fault is a common dose interlocking of Varian high-energy linear accelerator. Due to the complexity of the system, this fault has brought challenges to the engineers for a long time. Hence, an effective solution is urgently required. In this article, the circuit and principle for UFRS interlocking fault for Varian CLINAC-IX linear accelerator were explicitly analyzed and the maintenance methods were summarized. The management methods of such rare fault were shared, aiming to provide reference for subsequent management of similar interlocking faults.
6.Application of evacuated blood collection combined with non sealing technique in venous blood collection of phase I clinical drug test

Objective:To explore the feasibility and effect of evacuated blood collection combined with non sealing technique in the collection of pharmacokinetic blood samples in phase I clinical trials.Methods:Using convenient sampling method, 60 patients in phase I clinical trial who were admitted between November 2017 and October 2019 were selected as the research objects, and were divided into control group and improvement group according to the admission sequence. Among them, 31 patients collected from November 2017 to October 2018 were taken as the control group, and blood samples were collected in 390 tubes by syringe in open indwelling needle; from November 2018 to October 2019, 29 patients were selected as the improvement group, and 388 blood samples were collected by evacuated blood collection combined with non sealing technique in the open indwelling needle. The incidence of indwelling needle plugging, hemolysis of blood samples and the duration of single blood sampling were compared between the two groups.Results:In the incidence of indwelling needle plugging, the improvement group was 3.09% (12/388) , the control group was 2.05% (8/390) , there was no significant difference between the two groups (χ 2=0.800, P>0.05) ; in the incidence of hemolysis of blood samples, the improvement group was 2.66% (10/376) , the control group was 7.33% (28/382) , the difference between the two groups was statistically significant (χ 2=7.857, P<0.01) ; the improvement group was (29.40±1.99) seconds in the duration of single blood sampling, the control group was (40.74±0.88) seconds, the difference between the two groups was statistically significant ( t=101.15, P<0.01) . Conclusions:In the collection of pharmacokinetic blood samples of phase I clinical trials of new drugs, evacuated blood collection combined with non sealing technique can not only protect the blood vessels of the subjects, but also control the time of intensive blood collection within 12 hours, save the operation time of blood collection and guarantee the quality of blood samples. It is worth popularizing to ensure the scientific and standard test.
7.Dynamic changes of chest CT imaging in patients with COVID-19.

OBJECTIVE:
To analyze the dynamic changes of chest CT images of patients with coronavirus disease 2019 (COVID-19).
METHODS:
Fifty-two cases of COVID-19 were admitted in the First Affiliated Hospital of Zhejiang University School of Medicine. The consecutive chest CT scans were followed up for all patients with an average of 4 scans performed per patient during the hospitalization. The shortest interval between each scan was 2 days and the longest was 7 days. The shape, number and distribution of lung shadows, as well as the characteristics of the lesions on the CT images were reviewed.
RESULTS:
The obvious shadows infiltrating the lungs were shown on CT images in 50 cases, for other 2 cases there was no abnormal changes in the lungs during the first CT examination. Ground-glass opacities (GGO) were found in 48 cases (92.3%), and 19 cases (36.5%) had patchy consolidation and sub-consolidation, which were accompanied with air bronchi sign in 17 cases (32.7%). Forty one cases (78.8%) showed a thickened leaflet interval, 4 cases (7.6%) had a small number of fibrous stripes. During hospitalization, GGO lesions in COVID-19 patients gradually became rare,the fibrous strip shadows increased and it became the most common imaging manifestation. The lesions rapidly progressed in 39 cases (75.0%) within 6-9 days after admission. On days 10-14 of admission, the lesions distinctly resolved in 40 cases (76.9%).
CONCLUSIONS
The chest CT images of patients with COVID-19 have certain characteristics with dynamic changes, which are of value for monitoring disease progress and clinical treatment.

9.Analysis of clinicopathological characteristics and prognosis in 101 patients with pancreatic neuroendocrine neoplasms

Objective The aim of our study was to describe the clinicopathological characteristics and to evaluate the prognostic factors of postoperative PNENs.Methods The clinicopathological data of 101 postoperative PNENs patients were collected and analyzed retrospectively.We analyzed the clinicopathological characteristics and prognostic factors.The survival analysis was conducted by Kaplan-Meier method and survival analysis was performed by Cox model.Results 101 patients with PNENs were enrolled,including 46.5% male(n=47) and 53.5% female(n=54),the median age was 51 years old (ranged 19-80).94 patients(93.1%) with neuroendocrine tumors(NETs),7 patients(6.9%) with neuroendocrine carcinomas(NECs).The mean size of tumor was 3.3 cm(ranged 0.5-15).Immunohistochemistry staining positive rates for chromogranin A (CgA) and synaptophysin (Syn) were 89.0% and 98.0% respectively.Patients were 66 cases (65.3%),23 cases (22.8%) and 12 cases (11.9%) in TNM stage I,II and IV respectively.34 cases(33.7%) were F-PNENs,and 67 cases(66.3%) were NF-PNENs.Our study demonstrated that there were statistically significant differences between F-PNENs and NF-PNENs in stage,grade,neural and vascular invasion,CgA positive rate(χ2=10.372,18.162,9.699,5.547,4.043,P<0.05).Meanwhile,significant differences were found between NETs and NECs in stage,neural and vascular invasion,CgA positive rate(χ2=11.642,5.254,14.458,5.254,P<0.05).PNENs had better prognosis,with 5-year survival rate of 84.3%.The independent prognostic factor for survival was grade.Conclusion PNENs are a family of tumors with diversity clinical presentation and prognosis.Surgery is the main treatment method,grade of PNENs is the independent prognostic factor.
10.Analysis of predisposing factors of postoperative complications of 706 patients and the differential expression of key genes in early and advanced stages of esophageal cancer

Objective To study the predisposing factors of postoperative complications of esophageal cancer,and analyze the different genetic expression of mRNA,DNA methylation and mutations with different esophageal cancer staging.Methods A total of 706 patients with esophageal cancer from the First Affiliated Hospital of Nanjing Medical University and Jiangsu People's Hospital from 2010 to 2013 and 2015 to 2016 were involved in this study.Data of genetic information from 107 patients with esophageal cancer were obtained from the Cancer Genome Atlas (TCGA) database.Results The analysis showed that age was correlated with SEP (P =0.011),and pTNM staging was correlated with the occurrence of AF (P =0.010) and CL (P =0.030).The postoperative nutritional pattern was also significantly associated with the occurrence of AF (P =0.041).There was a significant difference in the expression of EPHA3 mRNA in patients with esophageal cancer in different TNM stage (P =0.006),DNA methylation level of NOTCH1 (P =0.027),and mutations genes such as ZNF750 (P =0.037) and MALAT1 (P =0.044).Conclusion Postoperative complications of esophageal cancer could be associated with patients'age,TNM staging and postoperative nutritional patterns.The expression of EPHA 3 mRNA and the DNA methylation of NOTCH 1 show significant difference in different TNM stages of esophageal cancer,and both of them have higher expression in advanced patients.Mutation of ZNF750 and MALAT1 are negatively correlated with the staging of esophageal carcinoma.
