Objective To explore the mechanism of the long non-coding RNA(lncRNA)small nucleolar RNA host gene 1(SNHG1)/microRNA(miR)-340-5p/pentraxin 3(PTX3)signaling pathway in osimertinib resistance of lung cancer. Methods The expression levels of SNHG1,miR-340-5p and PTX3 in lung cancer tissues, adjacent tissues and osimertinib-sensitive and resistant cell lines were detected by real-time fluores-cence quantitative PCR.SNHG1 was knockdown using siRNA to detect its effects on cell proliferation,apopto-sis and osimertinib sensitivity. The dual-luciferase reporter assay verified the binding relationship between SNHG1 and miR-340-5p,as well as between miR-340-5p and PTX3.Western blotting was used to analyze the expression changes of PTX3 protein. Results SNHG1 was highly expressed in lung cancer tissues and osimer-tinib-resistant cells, while the expression of miR-340-5p was downregulated.SNHG1 inhibits the function of miR-340-5p by directly binding to it and releases the negative regulation of miR-340-5p on PTX3,resulting in the high expression of PTX3 in lung cancer drug-resistant cells.Knockdown of SNHG1 can increase the apop-tosis rate of drug-resistant cells, inhibit the ability of colony formation, and enhance the sensitivity of cells to osimertinib. The miR-340-5p inhibitor upregulated the expression of PTX3 in lung cancer sensitive cells.Con-clusion SNHG1 as competitive endogenous RNA inhibition of miR-340-5p,thus raising PTX3 expression, the drug resistance of lung cancer cells,SNHG1/miR-340-5p/PTX3 shaft may provide potential targets in the treatment of lung cancer drug resistance.

Neuropathic pain (NP) is a chronic and intractable pain syndrome triggered by lesions or diseases of the somatosensory nervous system. It has a high incidence in the general population and currently lacks effective treatment methods, which seriously reduces the life quality of patients. Peripheral sensitization and central sensitization are the key mechanisms for the occurrence and development of NP. A variety of immune cells are involved in the processes of peripheral sensitization and central sensitization. After peripheral nerve injury, immune cells such as macrophages, astrocytes and Schwann cells infiltrate and release chemical substances. Through regulating inflammatory reactions and ion channels, they initiate and amplify pain signals, increasing the sensitivity of nociceptors to incoming signals and forming peripheral sensitization. Immune cells in the central nervous system, represented by microglia and astrocytes, are stimulated by pro-nociceptive substances released by primary afferent nerve fibers, enhancing their excitability. Subsequently, they synthesize and release pro-inflammatory cytokines, leading to the enhancement and remodeling of neuronal connections in the spinal cord and brain, resulting in central sensitization. This article reviews the role of immune cells and peripheral sensitization and central pain sensitization, providing a theoretical basis and practical guidance for the understanding and treatment of NP.

A 2-year-old boy was admitted to Shanghai Children's Medical Center, Shanghai Jiao Tong University School of Medicine in Nov 30th, 2018, due to polydipsia, polyphagia, polyuria accompanied with increased glucose levels for more than 2 weeks. He presented with symmetrical short stature [height 81 cm (-2.2 SD), weight 9.8 kg (-2.1 SD), body mass index 14.94 kg/m2 (P10-P15)], and with no special facial or physical features. Laboratory results showed that the glycated hemoglobin A1c was 14%, the fasting C-peptide was 0.3 ng/mL, and the islet autoantibodies were all negative. Oral glucose tolerance test showed significant increases in both fasting and postprandial glucose, but partial islet functions remained (post-load C-peptide increased 1.43 times compared to baseline). A heterozygous variant c.1366C>T (p.R456C) was detected in GATA6 gene, thereby the boy was diagnosed with a specific type of diabetes mellitus. The boy had congenital heart disease and suffered from transient hyperosmolar hyperglycemia after a patent ductus arteriosus surgery at 11 months of age. Insulin replacement therapy was prescribed, but without regular follow-up thereafter. The latest follow-up was about 3.5 years after the diagnosis of diabetes when the child was 5 years and 11 months old, with the fasting blood glucose of 6.0-10.0 mmol/L, and the 2 h postprandial glucose of 17.0-20.0 mmol/L.

Chinese Journal of Trauma 2021;37(10):865-875
Traumatic rib fractures are the most common injury in thoracic trauma. Previously, the patients with traumatic rib fractures were mostly treated non-surgically,of which 50%,especially those combined with flail chest presented chronic pain or chest wall deformities and over 30% had long-term disabilities, being unable to retain a full-time job. In the past two decades, thanks to the development of internal fixation material technology, the surgical treatment of rib fractures has achieved good outcomes. However, there are still some problems in clinical treatment, including inconsistency in surgical treatment and quality control in medical services. The current consensuses on the management of regional traumatic rib fractures published at home and abroad mainly focus on the guidance of the overall treatment decisions and plans, and relevant clinical guidelines abroad lacks progress in surgical treatment of rib fractures in recent years. Therefore, the Chinese Society of Traumatology affiliated to Chinese Medical Association and Chinese College of Trauma Surgeons affiliated to Chinese Medical Doctor Association,in conjunction with national multidisciplinary experts, formulate the Chinese Consensus for Surgical Treatment of Traumatic Rib Fractures(2021)following the principle of evidence-based medicine, scientific nature and practicality. This expert consensus puts forward some clear, applicable, and graded recommendations from aspects of preoperative imaging evaluation, surgical indications, timing of surgery, surgical methods, rib fracture sites for surgical fixation, internal fixation methods and material selections, treatment of combined injuries in rib fractures,in order to provide references for surgical treatment of traumatic rib fractures.

Objective:To understand knowledge, belief, practice level of insulin injection and influencing factors of behavior of clinical nurses in tertiary general hospitals, so as to provide the basis for formulating targeted training programs for insulin injection. Methods:Using the convenient sampling method, a total of 169 clinical nurses in the adult ward of a tertiary general hospital were selected as the research objects in May 2020. The self-made basic information questionnaire and Insulin Injection Knowledge, Belief and Practice Questionnaire were used for investigation. Binary Logistic regression analysis was used to investigate the influencing factors of insulin injection behavior of clinical nurses. Results:Among 169 clinical nurses, the awareness rate of guidelines was 82.84% (140/169) , 95.86% (162/169) of nurses had patient education experience, the incidence rate of insulin injection needle stab injury was 14.79% (25/169) , and 90.53% (153/169) of nurses had the need to receive insulin injection system training. The total scores of knowledge, belief and practice dimensions of the Insulin Injection Knowledge, Belief and Practice Questionnaire for clinical nurses were respectively 60.00 (45.00, 70.00) , 50.00 (49.00, 50.00) and 85.00 (82.00, 85.00) , and the total score of the questionnaire was 189.00 (178.50, 200.50) . The results of binary Logistic regression analysis showed that knowing the guidelines, receiving insulin injection operation training and insulin injection needle stick experience were the influencing factors of insulin injection behavior ( P<0.05) . Conclusions:Clinical nurses in tertiary general hospitals have a high level of insulin injection beliefs and behaviors and there are still deficiencies in insulin injection knowledge. In the training work related to clinical insulin injection, attention should be paid to strengthening and maintaining the belief and behavior of standardized insulin injection, emphasizing the safety of insulin injection, and combining with the technical guidelines and specifications of insulin injection, improving the knowledge reserve and application ability of insulin injection of clinical nurses.

Objective To explore the anti-tumor effect of 17XL strains of Plasmodium yoelii(P.y)infection on melanoma in mice. Methods B16F10 tumor cells were axillarilly injected into the right flank of 20 C57BL/6 mice to establish tumor-bearing mouse models. The next day,the mice were randomly divided into a P.y infection group and control group,10 mice each group. Each mouse of the P.y infection group was intraperitoneally injected with 1×106 red blood cells including 20% P.y infection red blood cells, and each one of the control group were intraperitoneally injected with 1×106 normal red blood cells of C57BL/6 mice. The time of tumor formation of the mice in the two groups was observed and the tumor volumes were measured. Results The time of tumor formation in the P.y infection group[(11.30 ± 0.21)d]was significantly later than that in the control group [(10.40 ± 0.22)d](P < 0.05). From the tumors could be accurately measured to the study end point, both the tumors of mice in the two groups were growing, and the tumor volumes of mice in the P.y infection group were significantly less than those in the control group at each time point(all P < 0.05). The growth rate of tumors in the P.y infection group[(71.10 ± 6.29)mm3/d]was significantly slower than that in the control group[(302.80 ± 49.94)mm3/d](P < 0.05),and the growth rates of tumors every day in the P.y infection group were significantly slower than those in the control group(all P < 0.05). Conclusion The P.y in-fection can delay the occurrence of tumor and inhibit the growth of melanoma.
